Opis:
Lyme borreliosis is a multi-organ disease transmitted by ticks, whose etiological factor is an anaerobic bacterium Borrelia burgdorferi sensu lato. The incidence of this disease has risen each year for more than 10 years now in all parts of the world where ticks are present. Due to the multiplicity of the clinical symptoms, the disease is difficult to diagnose as it resembles other illnesses. Further, its pathomechanism through which Borrelia burgdorferi spirochetes attack joints, heart, nervous system and skin is not fully understood. This leads to many problems, both diagnostic and therapeutic. At present, there are two views on Lyme disease: one forwarded by the Polish Society of Epidemiology and Physicians of Infectious Diseases and the other one recommended by the International Lyme and Associated Diseases Society. The following article presents clinical diagnostic procedures as well as additional laboratory, serological, histological, microbiological and genetic analyzes. It is an attempt to provide the most reliable diagnostic methods although it should be noted that all of them encounter difficulties. Accordingly, the diagnosis of Lyme disease is far from perfect and requires further research and standardization.
Borelioza jest wieloukładową chorobą przenoszona przez kleszcze, której czynnikiem etiologicznym jest bakteria beztlenowa Borrelia burgdorferi sensu lato. Od ponad 10 lat z roku na rok wzrasta zachorowalność na tą chorobę we wszystkich miejscach świata, tam gdzie występują kleszcze. Ze względu na złożoność objawów klinicznych jest ona trudna do rozpoznania imitując inne jednostki chorobowe. Nie do końca poznany jest patomechanizm, w którym krętki Borrelia burgdorferi atakują stawy, serce, układ nerwowy i skórę. Rodzi to wiele problemów zarówno diagnostycznych jaki i terapeutycznych. Obecnie ścierają się dwa poglądy na boreliozę ten reprezentowany przez Polskie Towarzystwo Epidemiologów i Lekarzy Chorób Zakaźnych oraz ten sugerowany przez Międzynarodowe Towarzystwo ds. Boreliozy i Chorób z Nią Powiązanych. Artykuł przedstawia diagnostykę kliniczną oraz badania dodatkowe w postaci analiz laboratoryjnych, serologicznych, histologicznych, mikrobiologicznych i genetycznych. Jest on próbą przedstawienia najbardziej wiarygodnych metod diagnostycznych z zaznaczeniem jednak, że we wszystkich napotyka się trudności i wnioskiem, że diagnostyka boreliozy wciąż jest niedoskonała i wymaga jeszcze wielu badań i standaryzacji.

In order to mitigate the spread of COVID-19, in the early stages of the pandemic outbreak, postponing elective procedures was recommended all around the world. Outpatient care and dental care were limited to telephone advice and emergency services. Dental staff is particularly vulnerable to SARS-CoV-2 contraction, because of the inevitable contact with patients’ body fluids during aerosol-generating procedures. The implementation of diagnostic tests among ambulatory patients could improve the occupational safety among outpatient care personnel. The aim of this review was to introduce information regarding COVID-19 diagnostics with a particular focus on the methods which can be utilized in an outpatient and dental care setting. An online PubMed database review of articles on COVID-19 diagnostics, published on February 12–May 15, 2020, was conducted. Reverse transcription polymerase chain reaction is the gold standard in COVID-19 diagnostics, which determines if a person has an active infection. Unfortunately, its utilization in outpatient care is limited. Serological enzyme-linked immunosorbent assays identify people who were infected, including those who have had an asymptomatic infection, but they do not give sufficient information about the acute infection. Rapid serological assays developed to facilitate testing outside of laboratories, especially in dental offices, are not recommended by the World Health Organization to be used outside research settings, and they should not constitute the basis for clinical decision-making because of frequent false-negative results which may consequently contribute to personnel infections. Out of all available COVID-19 diagnostic methods, rapid serological assays seemed to be a method of choice in outpatient medical care. Unfortunately, their results turned out to be unreliable. The best methods to ensure the occupational safety of medical staff and to avoid cross-infections in outpatient care facilities include a thorough epidemiological interview, temperature measurement to rule out patients with an active infection, and the implementation of strict infection control procedures.

Q fever is an infectious disease of humans and animals caused by Gram-negative coccobacillus Coxiella burnetii, belonging to the Legionellales order, Coxiellaceae family. The presented study compares selected features of the bacteria genome, including chromosome and plasmids QpH1, QpRS, QpDG and QpDV. The pathomechanism of infection – starting from internalization of the bacteria to its release from infected cell are thoroughly described. The drugs of choice for the treatment of acute Q fever are tetracyclines, macrolides and quinolones. Some other antimicrobials are also active against C. burnetii, namely, telitromycines and tigecyclines (glicylcycline). Q-VAX vaccine induces strong and long-term immunity in humans. Coxevac vaccine for goat and sheep can reduce the number of infections and abortions, as well as decrease the environmental transmission of the pathogen. Using the microarrays technique, about 50 proteins has been identified which could be used in the future for the production of vaccine against Q fever. The routine method of C. burnetii culture is proliferation within cell lines; however, an artificial culture medium has recently been developed. The growth of bacteria in a reduced oxygen (2.5%) atmosphere was obtained after just 6 days. In serology, using the IF method as positive titers, the IgM antibody level >1:64 and IgG antibody level >1:256 (against II phase antigens) has been considered. In molecular diagnostics of C. burnetii infection, the most frequently used method is PCR and its modifications; namely, nested PCR and real time PCR which detect target sequences, such as htpAB and IS1111, chromosome genes (com1), genes specific for different types of plasmids and transposase genes. Although Q fever was diagnosed in Poland in 1956, the data about the occurrence of the disease are incomplete. Comprehensive studies on the current status of Q fever in Poland, with special focus on pathogen reservoirs and vectors, the sources of infection and molecular characteristics of bacteria should be conducted.

In the present paper results of modelling of wind interaction with a water layer surface and research of the vortical structures arising downstream the flow around bodies are described. Results of the use of the method of acoustic diagnostics of spatially - periodic vortical flows are presented. At physical modelling in laboratory conditions wind was created with a low-velocity wind tunnel. The structure of an air stream and convective cells on water surface was investigated with the help of visualization. Some pictures of structures on the shallow sea surface are also demonstrated and discussed.

Opis:
Modern way of machines’ exploitation, due to their high level of structural complication, requires proper level of supervising. That supervising is generally based on detection of pre-failure states and evaluation of machines’ single elements or components condition. In the frame of development of the research capacity of the Mechanical Faculty of Maritime Academy in Gdynia, has been developed the Exploitation Decision Aid System for marine engines exploitation. The system was based on existing test bed with the marine diesel engine Sulzer AL 25/30 as a core element. Modernization of the measurement equipment, significantly extended research capacity, what resulted with improvement of quality, extension of the span, and acceleration of conducted research and development works in the domain of safety of exploitation and diagnostics of marine power plants. Investments in modern measurement apparatus enables also an extension of the range of research and expertise related to engines’ failures and pollutants emission, in relation to broad spectrum of implemented fuels. The goal has been achieved in way of modernization of engine’s monitoring system and stands in Technical Diagnostic Laboratory.
