The paper discusses the mathematical model of thermal comfort in the hyperbaric facility. Based on human thermal balance and thermal comfort conditions the comfort equation for the hyperbaric environment was derived. The comfort equation enables to calculate all those combinations of the diver’s activity, clothing (thermal insulation, moisture permeation factor) and environmental variables (temperature, pressure, composition, relative humidity and velocity of the breathing gas and mean radiant temperature), which would create thermal comfort in the hyperbaric environment. The paper presents also the solution of thermal comfort equation for dives up to the depth of g = 590 m, with helium-oxygen and hydrogen-oxygen breathing mixture, as well as the comfort diagrams.

In this paper the authors would like to present a correct procedure in both surgical and hyperbaric treatment of patients with gas gangrene admitted to a surgical department during ER. Gas gangrene is not very common these days, but when it comes to dealing with gangrenous infection in the emergency it is quite likely to make errors in both diagnostic and therapeutic manners. When there is a gas gangrene in a patient at the emergency time plays crucial role and the proper application of procedures is vital for the patient’s survival. 10 cases made the study group here, all of them were patients diagnosed and treated surgically due to gas gangrene. As shown here, It is important to perform a revision of surgical wounds after few hours since primary surgery and to begin hyperbaric treatment as quickly as possible. The findings and suggestions included in this study are supported by own experience of The 2nd Department of General and Gastrointestinal Surgery of Medical University in Białystok, Poland.

The Sweedish project codenamed DykSMART was carried out in 2013–2015. The results of this project created the base for for international project #R024 Diving with State Maritime Resources in the Baltic DiveSMART‐Baltic of Interreg Baltic Sea Region launched in 2016. The Interreg Baltic Sea Region Programme 2014–2020 supports the integrated territorial development and cooperation for a more innovative, better accessible and sustainable Baltic Sea region. Partners from the countries around the Baltic Sea work together in transnational projects on common key challenges and opportunities. DiveSMART project is dedicated to improved coordination concerning the organizations with emergency divers, in order to be able to handle an international emergency situation: to be aware of which diving competences are available, and also relevant diving equipment; common international exercises; knowledge of education and definitions of various levels of competence; research & development. The project DiveSMART Baltic has received Flagship status. The article is the first in the planned cycle of articles referring to the tasks realised in the Naval Academy within the framework of DiveSMART project. This article presents a review of pollutants of the hyperbaric breathing agent in connection with their toxic action. This subject is linked to the work package four of the DiveSMART project Risk reduction — develop and enhance methods for life support systems of distressed persons in both physical and psychological areas.
W latach 2013–2015 w Szwecji prowadzony był krajowy projekt pod akronimem DykSMART. Rezultaty tego projektu tworzą bazę pod międzynarodowy projekt #R024 Nurkowanie z wykorzystaniem służb państwowych rejonu Bałtyku DiveSMART‐Baltic programu Interreg Baltic Sea Region uruchomionego w 2016 roku. Program Interreg Baltic Sea Region w perspektywie 2014–2020 wspiera zintegrowany rozwój terytorialny i współpracę na rzecz bardziej innowacyjnego, lepiej dostępnego i trwałego rozwoju regionu Morza Bałtyckiego. Partnerzy z krajów nadbałtyckich współpracują w międzynarodowych projektach dotyczących wspólnych kluczowych wyzwań i możliwości regionu. Projekt DiveSMART służy polepszeniu koordynacji w sytuacji kryzysowej podczas międzynarodowej organizacji akcji z wykorzystaniem nurków służb państwowych poprzez: określenie dostępnych kompetencji i wyposażenia nurkowego; organizację ćwiczeń międzynarodowych; określenie systemów szkoleń narodowych prowadzących do różnych poziomów umiejętności nurkowych; badania naukowe i rozwój. Projekt DiveSMART uzyskał status projektu flagowego. Artykuł jest pierwszym z cyklu referujących jego postępy w ramach zadań zlecanych Akademii Marynarki Wojennej. W artykule zaprezentowano przegląd zanieczyszczeń hiperbarycznej atmosfery oddechowej z uwzględnieniem ich toksycznych właściwości. Zadanie to dotyczy czwartego pakietu roboczego projektu DiveSMART Zmniejszenie ryzyka — opracowywanie i ulepszanie systemów zachowania życia osób narażonych na niekorzystne oddziaływania fizyczne i psychologiczne.

The aim of the study was to determine the effect of the application of hyperbaric oxygen therapy on the course of an infection with group A type T-3 hemolytic β streptococcus. Experiments were carried out on Porton white mice and in vitro blood plates. General and local infections with streptococci were induced in animals. The infected animals were treated with hyperbaric oxygenation. The lethal effect of infection was significantly inhibited using hyperbaric oxygenation on the first and second day following the infection.

Introduction: New effective treatments are sought to eliminate COVID-19-related hypoxia. Hyperbaric oxygen therapy is an effective method in the treatment of many diseases accompanied by hypoxia. Aim: The aim of this study was to analyse the literature on the use of hyperbaric oxygen in the treatment of COVID-19 and to evaluate the effectiveness and safety of this method. Materials and methods: Medical databases (Medline and PBL) and websites were reviewed using the terms hyperbaric oxygen and COVID-19. 25 works were qualified for the analysis. Conclusions: The analysed literature shows that hyperbaric oxygen therapy is an effective and safe method of treating patients with COVID-19. Due to the limited amount of scientific evidence assessing the use of HBOT in COVID-19, there is a need for further research to confirm the effectiveness and safety of this method.
Wstęp: Do eliminowania hipoksji związanej z COVID-19 poszukiwane są nowe sposoby skutecznego leczenia. Tlenoteriapia hiperbaryczna jest skuteczną metodą w leczeniu wielu chorób z towarzyszącą hipoksją. Cel: Celem niniejszej pracy było przeanalizowanie literatury dotyczącej zastosowania hiperbarii tlenowej w leczeniu COVID-19 oraz ocena skuteczności i bezpieczeństwa tej metody. Materiały i metody: Dokonano przeglądu medycznych baz danych (Medline i PBL) oraz stron internetowych za pomocą haseł: hiperbaria tlenowa i COVID-19. Do analizy zakwalifikowano 25 prac. Wnioski: Z przeanalizowanej literatury wynika, że tlenoterapia hiperbaryczna jest skuteczną i bezpieczną metodą leczenia pacjentów z COVID-19. Ze względu na ograniczoną liczbę dowodów naukowych oceniających stosowanie HBOT w COVID-19, istnieje potrzeba przeprowadzania dalszych badań potwierdzających skuteczność i bezpieczeństwo tej metody.

This two-part article reviews the current legal situation of professional personnel implementing hyperbaric procedures other than those used for diving. Numerous inconsistencies between the existing legal acts and the lack of legal regulations concerning procedures not financed from the State budget are shown. The first significant problem was the lack of mutual correlation of various regulations of the Minister of Health concerning hyperbaric diseases, as well as incoherence with medical and nursing specialisation programs in force in Poland. The second problem is the lack of requirements in the documents of the Ministry of Health for medical staff other than doctors and nurses, including technical staff, necessary for the implementation of a safe oxybarotherapy procedure. The situation is clearer with respect to technical personnel than in relation to medical personnel. There are provisions which strictly define the qualifications of such staff for a very narrow group of technical personnel. Although they deal with issues related to the use of hyperbaric chambers in diving, to date no other separate regulations have been developed for medical applications of hyperbaric therapy. Unfortunately, both in centres financed by the National Health Fund and in private centres, no-one observes these regulations because there is no such formal requirement. The same applies to occupational research (occupational medicine) for all groups of personnel involved in hyperbaric oxygen therapy treatments. It was also found that medical hyperbaric centres not seeking funding from the budget (the National Health Fund) do not have even minimum qualification requirements for the medical staff working there. Furthermore, there is no knowledge of the requirements set out in other legal acts other than those concerning medicine.

Introduction. Fournier gangrene (FG) is life - threatening condition, defined as the necrotizing fascitis of perineum and can spread to the adjacent areas. It is rare disease and infection is caused by mixed bacterial flora, seldom by fungal infection. Risk factors are: male sex, diabetes, hypertension, malignant neoplasms, alcoholism, immunospression. Material and methods. The analysis of four group patients treateted for Fournier gangrene was made about diagnostic and therapeutic process, assessment of prognosis based on Fournier’s Gangrene Severity Index). Results. All patients were males. Average age at the moment of diagnosis was 60 years. All of them had comorbidities resulting with the higher risk of susceptibility to FG. Morbitity was 50%, despite of all of patients had less than 9 points in FGSI. Discussion. The FG, despite of better diagnostic tools and technological progres remaines the significant clinical issue because of the mortality - 80%. „The golden standard” is surgical excision of necrotic tissues, antibiotics support, equation of fluid, electrolytes and base – acid balance, level of glycemia is very important. The treating results were assessed on the base of FGSI. The significance has the moment of performing the surgical intervention – it is proven, that should be carried out during 24 hours. The hyperbaric oxygen therapy is controversial. Seem to be appropriate if the infection is caused by anaerobic bacteria. Conclusions. Fournier syndrome is significant clinical issue. Its treatment requires early surgical approach with exicision of necrotic tissues, antibiotics support and treatment of hyperbaric oxygen in some cases.

Hyperbaric oxygen therapy is a method supporting the treatment of many diseases. Oxygen therapy treatments are conducted in hyperbaric chambers, in which patients breathe pure, 100% oxygen with higher than atmospheric pressure. This allows to increase the amount of oxygen supplied to all cells of the body many times over. The treatment with hyperbaric oxygen therapy enables the patient to recover faster and be fully active, and also reduces the costs of standard treatment. The aim of the study was to summarize the possible applications of hyperbaric oxygen therapy. The available literature in the PUBMED database was reviewed in September 2022 with the use of the phrases 'hyperbaric oxygen therapy', 'therapeutic applications'. The indications for therapy in a hyperbaric chamber are all kinds of diseases, both acute and chronic. The method supports the nourishment and regeneration of cells and tissues of the organism, and also slows down the aging process. However, due to the possible side effects of such therapy, patients should be qualified for its use after a careful analysis of their clinical condition and coexisting diseases. To sum up: hyperbaric oxygen therapy is most often used in the treatment of skin diseases and injuries, burns, and peripheral vascular diseases.
Hiperbaryczna terapia tlenowa jest metodą wspomagającą leczenie wielu chorób. Zabiegi tlenoterapii prowadzone są w komorach hiperbarycznych, w których chorzy oddychają czystym, 100% tlenem o wyższym od atmosferycznego ciśnieniu. Pozwala to wielokrotnie zwiększyć ilość tlenu dostarczonego do wszystkich komórek organizmu. Połączenie klasycznego leczenia z hiperbaryczną terapią tlenową ułatwia choremu szybszy powrót do zdrowia i pełnej aktywności, obniża również koszty standardowego leczenia. Wskazaniem do terapii w komorze hiperbarycznej są wszelkiego rodzaju schorzenia, zarówno o przebiegu ostrym, jak i przewlekłym. Metoda wspomaga odżywienie i regenerację komórek i tkanek ustroju, wpływa również hamująco na proces starzenia. Ze względu jednak na możliwe działania uboczne takiej terapii, pacjenci powinni być kwalifikowani do jej zastosowania po przeprowadzeniu wnikliwej analizy ich stanu klinicznego i współistniejących schorzeń.

Degloving injury poses a severe therapeutic challenge concerning both trauma and plastic surgery. The injury involves separation of skin and subcutaneous tissue from fascia and muscles. Treatment is often long-lasting and brings unsatisfying results due to the extent of damage, risk of infection and massive blood loss. In this article, we present the management and therapeutic outcomes of a patient admitted due to the degloving injury of the lower extremity caused by workplace accident. We described the complexity of treatment including surgical intervention as well as additional treatment, which combined brought good esthetic outcome.

Maintaining a stable oxygen content in the breathing loop of the diving apparatus with a semi-closed respiratory circuit is essential for the safety of underwater mine clearance operations. This article discusses the influence of modified methods of flushing the breathing apparatus on the quality of the breathing loop ventilation process. The problem of flushing an apparatus is presented in light of the Polish Naval Academy's change from statistical to deterministic decompression schedules, based on the model of apparatus ventilation, oxygen toxicity hazard and decompression. The examination and determination utilising the required accuracy of the method used for flushing the breathing loop of the device allowed to assess the stable oxygen content during the decompression phase. The assumptions were verified based on a system for measuring oxygen content in the breathing loop.

Pre-dive checks and dive logs are fundamental documentation for any diving operation and must be incorporated as mandatory ‘good operating practices’ in scientific diving (SD) projects. Data included in dive logs may vary in detail, however, there is basic information to provide based on global standards. Differently to several developed countries in Europe, North America and Australasia, there are countries with incipient, sometimes non-regulated, SD programmes. In this article the importance of documentation in SD is highlighted and record forms are provided as templates, including versions both in English and Spanish. The Diving Supervisor (DS) is the designated person to fill the ‘Daily SciDive Log’ and ‘SCUBA & surface-supplied LogSheet’ (Table 1, 2 and 3, respectively), whilst every diver is responsible for filing their own ‘SciDiver’s Digital LogBook’ (Table 4). General and specific considerations for all tables are described throughout the text. This effort was done to facilitate systematic data management and start developing the bases towards solid national/regional standards on scientific diving operations, particularly for those countries with incipient (scientific) diving programmes.

Objective: Paraoxonase1 (PON1) and Arylesterase (ARE) levels are associated with reduced risk of atherosclerosis. The functional status of high density lipoprotein (HDL) is closely related to the PON1/ARE enzyme activity. Functional changes in treatment of sudden sensorineural hearing loss (SSNHL) may be achieved by post-translational modification of lipid metabolism induced by hyperbaric oxygen therapy (HBOT). Methods: Men patients with SSNHL who met the research criteria were included in the study. HBOT was performed on average 30 sessions. Laboratory measurements were made at the beginning and end of HBOT for the same patients. Serum levels of PON1/ARE and routine lipid laboratory parameters were measured to determine possible changes in SSNHL after HBOT. Results: In this study, a reducing effect on PON1 enzyme amount of long-term HBOT was detected. The serum PON1 amount of patients with SSNHL was 19.7 ± 2.7 ng / mL (mean ±} SD) before HBOT, and the serum PON1 decreased to 17.0 ± 2.1 ng / mL (mean ±} SD) after 30 sessions of HBOT. This decrease in PON1 levels was statistically significant (p =0.035). There was also a statistically significant decrease in the enzyme activity of ARE in the SSNHL patients (p=0.024). Conclusion: This preliminary study showed a significant decrease in serum PON1/ARE enzyme content in SSNLH patients with HBOT. In fact, it can be assumed that HBOT has no adverse effect on HDL functionality. However, the decrease in PON1 level by HBOT with 30 or more sessions may be important for the antioxidant function of HDL.It may possibly cause post-translational changes in antioxidant defense mechanisms due to increased oxidative stress with HBOT. In conclusion, larger clinical studies are needed to determine the possible effects of HBOT on HDL-related PON1/ARE functionality in SSNHL.

Opis:
The author undertakes to systematise and compare various types of losses of the technical and functional properties of equipment related to diving with other areas of human activity in which technical equipment is used. The basis for this is the extensive literature describing, inter alia, the issues of wear and tear of machine parts, their damage, failures, etc. a phenomena occurring in the “life” of virtually every technical object. The specificity of diving techniques makes it a relatively little analysed segment of activity from the perspective of exploitation. Based on the analysis of publications and own materials, the author has reviewed and matched them in terms of similarity of loss of technical and functional properties of various elements of diving technology in relation to the divisions and criteria used in generally understood utilisation.
