Opis:
One of the most important trends in engineering biomechanics is experimental and numerical analysis of the stress and strain state existing in organs subject to a heavy load; e.g., the knee and hip joints. Clinical tests experimental and numerical studies on design of implants replacing damaged organs are also of crucial importance in development of biomechanics. The research is focused on determination of loads, experimental techniques as well as on simplifications assumed in measurements and applied models. The conducted investigations were concerned with estimation of mechanical behaviour of hip and knee implants after arthoplasty. The experimental tests on both the real objects and models were performed. Numerical simulation was made by using the finite element method. The tests have proved that from the biomechanical point of view proper selection of endoprothesis is of crucial importance for achieving positive long-term results of alloplasty.
Jednym z najważniejszych kierunków badań w dziedzinie biomechaniki inżynierskiej jest doświadczalna i numeryczna analiza stanu odkształceń i naprężeń w organach ludzkich poddanych znacznym obciążeniom, jak chociażby staw biodrowy i kolanowy. Badania kliniczne, doświadczalne i numeryczne poświęcone konstrukcji elementów zastępujących uszkodzone elementy ciała ludzkiego stanowią jeden z najważniejszych kierunków biomechaniki. Dyskusje na tym tle skoncentrowane są na problemach beterminacji występującego stanu obciążenia, stosowanych technik pomiarowych oraz uproszczeń warunków pomiaru i przyjmowanych modeli. Przeprowadzone badania poświęcone zostały ocenie mechanicznej współpracy sztucznych elementów stawu biodrowego i kolanowego w warunkach modelowych, jak i metodą symulacji numerycznej, metodą elementów skończonych. Badania udowodniły duże znaczenie prawidłowej selekcji endoprotez z punktu widzenia biomechaniki układu na długoterminowe powodzenie alloplastyk.

Introduction. Endoprosthetics is the most effective method of returning to a normal way of life without pain. The key element is the post-operative rehabilitation program where a patient has to comply with certain rules and principles. The rehabilitation also includes the family of the patient. Each person, after any surgery requires an individual approach, and no patient can be commonly treated. Aim. The aim of the article is to compare the effects of an early rehabilitation after the implantation of an artificial hip joint in two patient cases. Description of the cases. This research was conducted at Bodden-Klinken GmbH. An individual case study was used, based on an example of two patients at the age of 59 who had implantation of an artificial hip joint. The carried out rehabilitation of the patients was conducted without any disruptions. Patients were able to freely walk on crutches nine days after the procedure. Conclusions. In the cases analyzed, the rehabilitation course was described until the ninth day after implantation of an artificial hip joint. High results in terms of patient rehabilitation were achieved. Furthermore, in order to continue their therapy, they were referred to the sanatorium.

Opis:
Background. Developmental dysplasia of the hip (DDH) is a developmental disorder which is reported to be associated with hip instability. When untreated, it can lead to irreversible joint damage. DDH is known to be a multifactorial disease involving genetic, mechanical and environmental factors. The greatest causative potential is attributed to the genetic component. Growth Differentiation Factor 5 (GDF5) is among the most studied genes associated with processes of regeneration and maintenance of joints. The aim of this work was to analyse the association of SNP rs143383 in the GDF5 gene and the occurrence of DDH, along with association with various contributing factors in the Caucasian population. Material and methods. A total of 118 samples were analysed for the presence of the mutation. DNA was isolated from all individuals from peripheral blood. SNP rs143383 in the GDF5 gene was genotyped using the TaqMan assay. A standard chi-square test was used to compare allele and genotype distributions in patients and healthy controls. Results. The association analysis of genotypes of DDH and rs143383 revealed a significant association. Also, the association of GDF5 and selected contributing factors was statistically significant in female gender (p=0.002), family history (p<0.001), count of pregnancy (p=0.009), laterality of hip involvement and initial US examination. Conclusions. 1. The results indicate an important effect of rs143383 polymorphism in the GDF5 gene on DDH development. 2. However, our results also suggest that rs143383 is not the only contributing factor in the genetic component of DDH.

Opis:
Estimation of hip joint loading is fundamental for understanding joint function, injury and disease. To predict patientspecific hip loading, a musculoskeletal model must be adapted to the patient’s unique geometry. By far the most common and cost effective clinical images are whole pelvis plain radiographs. This study compared the accuracy of anisotropic and isotropic scaling of musculoskeletal model to hip joint force prediction by taking patient-specific bone geometry from standard anteroposterior radiograms. Methods: 356 hips from 250 radiograms of adult human pelvis were analyzed. A musculoskeletal model was constructed from sequential images of the Visible Human Male. The common body position of one-legged stance was substituted for the midstance phase of walking. Three scaling methods were applied: a) anisotropic scaling by interhip separation, ilium height, ilium width, and lateral and inferior position of the greater trochanter, b) isotropic scaling by pelvic width and c) isotropic scaling by interhip separation. Hip joint force in one-legged stance was estimated by inverse static model. Results: Isotropic scaling affects all proportions equally, what results in small difference in hip joint reaction force among patients. Anisotropic hip scaling increases variation in hip joint force among patients considerably. The difference in hip joint force estimated by isotropic and anisotropic scaling may surpass patient’s body weight. Conclusions: Hip joint force estimated by isotropic scaling depends mostly on reference musculoskeletal geometry. Individual’s hip joint reaction force estimation could be improved by including additional bone geometrical parameters in the scaling method.

Hip osteoarthritis is a serious clinical and social problem. The number of patients who suffer from degenerative changes in the hip joints and require endoprosthesis-plasty is constantly increasing. This paper presents physiotherapeutic activities based on Proprioceptive Neuromuscular Facilitation (PNF) which optimize a patient’s mobilization using the reserves in their body fully to make improvements in movement and to regain lost functions for achieving beneficial therapeutic effects. The aim of the study was to evaluate the influence of PNF therapy on changes in muscle strength, mobility, and gait pattern in patients after Total Hip Arthroplasty. The case described here regards a 63-year-old woman diagnosed with left hip osteoarthritis who had Total Hip Arthroplasty. The patient was examined twice before and after PNF therapy. The range of mobility of hip joints, level of pain, muscular strength and gait were assessed. Applied PNF therapy, including dynamic (eccentric, concentric) and static muscle training, post-isometric relaxation, stabilization and control in the stance phase, resulted in improved hip joint mobility, muscle strength, gait pattern and pain reduction in the patient. The case study demonstrates that a short (two-week) but intensive (over two hours per day) PNF therapy positively influenced selected motor functions after Total Hip Arthroplasty.

The subject of the article is the evaluation of the strength of revision implants made of titanium or tantalum alloy, used during bone reconstruction of a hip joint while potentially using additional stabilizing screws, necessary due to significant bone loss. The article provides a preliminary strength analysis of implants, indispensable for further evaluation of strength limitations due to the risk of implant damage depending on the structure and number of additional screw holes. In the human locomotor system, the hip joint is the joint with the most load, hence the main problem is to establish an adequate load model which ought to be assumed for the needs of implant strength analysis. It is found necessary to perform a short, analytical review of the existing hip joint load models from the point of view of choosing the proper one, considering evaluation of implant strength by means of numerical studies using FEM. Differences in the implant load distribution depending on the used material are shown.

Since many of people in their functional activities mostly place their knee joint in flexed position, the hamstring muscles tend to be shortened. On the other hand, shortness of these muscles affect the knee joint directly and the Hip Joint Flexibility indirectly. The purpose of this study is to compare the effect of static and PNF stretching on hip joint flexibility of un-training female students. Twenty-four 18–30 years old un-training female students without any history of pathology in hip, knee or back were selected. They were divided into three groups with 8 women in each group (static stretch, PNF stretch and control). The two stretch groups received stretching program three days every week for six weeks, while the control group did not. The result shows that range of hip joint flexibility of both groups of static and PNF stretching increased (P < 0.05). However, there was no significant difference between these two groups (P > 0.05). The while it remained unchanged in control subjects (P > 0.05). Employing both methods (static and PNF stretching) increase the hip joint flexibility. The findings of this study suggest that optimal flexibility is achieved with a combination of these two methods. Thus, we recommend the combined training method to athletes who require very high flexibility.

The paper presents the results of a preliminary study on the structural analysis of the hip joint, taking into account changes in the mechanical properties of the articular cartilage of the joint. Studies have been made due to the need to determine the tension distribution occurring in the cartilage of the human hip. These distribution are the starting point for designing custom made human hip prosthesis. Basic anatomy, biomechanical analysis of the hip joint and articular cartilage are introduced. The mechanical analysis of the hip joint model is conducted. Final results of analysis are presented. Main conclusions of the study are: the capability of absorbing loads by articular cartilage of the hip joint is preliminary determined as decreasing with increasing degenerations of the cartilage and with age of a patient. Without further information on changes of cartilage’s mechanical parameters in time it is hard to determine the nature of relation between mentioned capability and these parameters.

Objective. The aim of the study was to assess the effectiveness of a combined therapy (TENS+UD) and postisometric relaxation of muscles in a 3-week therapeutic programme directed to seniors with diagnosed degenerative disease of the hip joint (OA). Materials and method. The study involved patients with diagnosed degenerative disease of the hip joint. A total of 60 people – 11 men and 49 women (mean age – 68 years) were examined. The therapy was applied for a period of 3 weeks. Seniors were randomly divided into 2 equal groups. In Group I, the treatment programme included a combined therapy (TENS+UD) and individual exercises, i.e. postisometric relaxation of muscles (PIR) at the hip joint; in Group II, seniors were treated with individual exercises, i.e. postisometric relaxation of muscles (PIR). Results. The study showed that the applied therapeutic programme had a significant effect. In both groups an improvement in mobility after the therapy was shown in all planes of movement. It was also found that the therapy proved effective in fighting pain syndromes. However, it should be noted that after assessing pain twice (VAS, Laitinen Scale) in Group I, the therapy proved to be more effective in fighting pain in comparison to Group II. Conclusions. In non-invasive treatment, in order to increase the analgesic activity in patients with degenerative changes of the hip joint, it is also recommended to include a physical stimulus in the form of combined therapy in the physiotherapeutic procedure, comprising techniques of energising muscles.

This article contains a description of the basic issues related to anatomy, loading of hip joint and its endoprosthesis research methods. The methods of testing and simulating hip joint loads, factors that influence the selection of parameters during the design of prostheses, typical solutions to engineering problems related to this topic are presented. Artykuł zawiera opis podstawowych zagadnień związanych z anatomią, obciążeniami i metodami badawczymi dotyczących stawu biodrowego oraz jego endoprotezy. Przedstawione są sposoby badania i symulowania obciążeń w stawie biodrowym, czynniki wpływające na dobieranie parametrów podczas projektowania endoprotezy oraz typowe rozwiązania problemów inżynierskich związanych z tym tematem. Artykuł zakończony jest krótkim streszczeniem zastosowań metody elementów skończonych podczas projektowania endoprotezy stawu biodrowego.

The article has been devoted to issues connected with the alloplasty and hip joint endoprostheses, that elements are being developed, which is supported by strength, tribological tests on used biomaterials, incl. polyethylene or computer modelling based on e.g. finite element method (FEM). In this paper, the results of research on the impact of the material articulations of the system head – acetabular and friction conditions on strength parameters of polyethylene components in the hip joint endoprosthesis. Numerical analysis of this friction node was carried out, using the ADINA System computer program and the simulations were performed at various friction conditions for metal/ polyethylene and ceramic/ polyethylene articulations with various UHMWPE modifications. The simulations results have shown the influence of tested material associations and friction conditions on parameters related to the strength of polyethylene cups, i.e. their displacements, stresses and deformations.

One of the main priorities of stroke rehabilitation is regaining patients’ independence in basic everyday activities. This paper is aimed at assessing impact of early stroke rehabilitation on hip joint mobility of the affected leg in patients after cerebrovascular accidents. The study included 30 subjects (13 men and 17 women) aged on average ±66.1, with hemiparesis, who were treated at the stroke unit and participated in a 4-week rehabilitation program. Measurements of passive and active motion ranges were conducted with a goniometer in both limbs: healthy and affected ones by stroke. There were statistically significant differences in motion ranges between healthy and affected limbs. Examination I revealed that affected limbs amounted to only 40% of physiological range, while in Examination II, the result reached 73%. The most significant motion limitations were noted in terms of bending, adduction and internal rotation, while the least significant in terms of external rotation and abduction. Exercises used during early stroke rehabilitation of hemiparesis patients considerably increased active motion ranges and maintained passive motion ranges in the hip joint of the affected limb. Patients with right-sided hemiparesis experienced much better improvement in hip joint motion of the affected limb. The 4-week period was insufficient to mobilize patients and help them regain full active mobility in the hip joint of the affected limb. These patients required further physiotherapy, until they fully regained functionality.

Objectives The primary endpoints of the study were to assess the effectiveness of hip joint arthroscopy in the treatment of femoroacetabular impingement (FAI) in patients with joint gap stenosis and to determine if and how quickly patients were able to return to work and physical activity. Material and Methods The prospective study of patients undergoing hip joint arthroscopy due to pain in FAI has been conducted. They were divided into 2 groups depending on the degree of the radiological examination. The criterion was the width of the joint gap. The study group involved 47 patients with hip joint gap of 2–3 mm, identified by means of the standardized X-ray examination. The control group consisted of 45 patients with hip joint gap > 3 mm. The post-operative follow-up period of the patients lasted at least 2 years. In addition, the Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) questionnaire together with Harris Hip Score (HHS) were applied. The patients were also evaluated for the post-operative time period enabling return to work. Results All the patients after hip joint arthroscopy returned to normal physical activity within 12 weeks after operation, enabling their return to work. However, it should be noted that during the post-operative follow-up, pain sensations either recurred or did not regress in 37 patients in the study group and 12 patients in the control group after treatment. The nearly equal results of the WOMAC questionnaire and HHS before operation significantly vary between both groups in the last follow up. In the study group they did not change expressively. Conclusions Despite the little invasiveness, hip joint arthroscopy in patients with joint gap stenosis brings about the far from satisfactory results. This procedure is not worth considering. Despite unsatisfactory pain relief, patients decided to returned to work, due to their occupational position and for fear of losing the job due to long absenteeism.
