Objective: The number of lowland dwellers traveling at high altitudes has greatly increased in recent decades. Business travelers flying to La Paz or Lhasa are at risk of altitude illnesses, similarly to miners and soldiers transported to high altitudes. Traveling to high altitude requires adaptation and if this process fails due to too rapid an ascent rate or susceptibility of the climber, acute mountain sickness (AMS) may result. Doctors and nurses in travel clinics, health centers and occupational health care clinics may face patients asking advice on how to plan their trips or manage AMS, or the health care personnel may take part in a travel to high altitude environment. Methods: Two patients, aged 29 and 47 years, who fulfilled the criteria for AMS were studied. The clinical findings and treatment in the field are described including the review of the current recommendations for prevention and treatment of AMS. Results: Both patients developed a severe AMS due to too rapid ascent and their denial of the symptoms. Conclusions: Prevention is the safest and the most efficient method in the care concerning AMS. Realizing the risk of mountain sickness, active inquiry about the symptoms and correctly timed reaction to them, in other words interrupting the ascent or descending, help to reduce and even to prevent the development of serious problems.

The aim of the paper was to investigate health aspects of high altitude trekking such as preparation for the physical exertion during trekking at high altitude, the impact of mountaineering on the daily life before and after the expedition, the effect of high-mountain conditions on health and well-being. It was found that in the pre-departure period trekkers commonly train to ensure that they are physically fit for the expedition. They train alone or under the supervision of a trainer. Self-prepared workouts may turn out to be insufficient due to the lack of appropriate training plans. The most challenging aspects of high altitude trekking for the body include carrying too heavy equipment, dealing with illegibly marked routes, wearing inappropriate clothing, having an unbalanced diet, not having enough water, which can lead to dehydration and infections. Misconduct by other people poses a risk. The specific type of effort involved in mountaineering requires balanced nutrition in terms of both micro- and macro-elements. To find the right combination, one has to either experiment or seek advice from a dietitian. However, relatively few people consult a nutrition coach. Among sanitary problems, the most serious one is inappropriate human waste disposal, the resulting lack of drinkable water. Some of the observed problems result from insufficient regulations regarding the conduct in the mountains and from trekkers’ lack of awareness regarding good practices in such extreme conditions.

Opis:
Portable hyperbaric chambers are a therapeutic option for altitude disease in the mountains. By an increase of the oxygen partial pressure the patient’s symptoms ameliorate significantly. Portable hyperbaric chambers may be used as ‘stand alone therapy’ as well as in combination with other therapeutic options. Here we present the recommendation how to use the device and how to avoid problems. The recommendation has been agreed by all members of the medical commission of the world umbrella organization Union Internationale des Associations d’Alpinisme (UIAA MedCom).

Opis:
Visual loss in the wilderness setting is potentially fatal. Firstly it may be a warning sign of a serious systemic problem and secondly the patient may lose their functional independence and ability to respond to objective danger. The issues discussed in this paper fall broadly into two categories, those that are unique to the high altitude setting and those that could happen anywhere but require treatment to protect vision then standard ophthalmological care is unavailable. The aims are to provide practical knowledge on how to manage simple eye problems and also how to recognize the warning signs when evacuation may be required. In keeping with all wilderness medicine, preparation and prevention are essential to avoid eye problems in the mountains. This paper is intended for physicians, interested non-medical people and expedition operators as a practical guide to the treatment and prevention of eye problems on expeditions.

In 2021, the Space Systems Laboratory of the University of Pisa (UniPi) started a student-oriented high-altitude ballooning programme intended to provide an opportunity for hands-on experience in support of the teaching of scientific and technical courses. The programme provides mentoring on scientific, technical and management issues, along with financial support and assistance with integration and launch on a sounding balloon platform. The goal is to conduct flying experiments in the stratosphere, retrieve them after landing and process the results; by doing so, the students experience all phases of a scientific mission project, from conceptual design to realisation, operations and post-flight analysis. Following a call for proposal open to all students of the UniPi across all study areas, three experiments were selected featuring multidisciplinary teams. This paper summarises the features of the programme’s first edition and presents the main lessons learned.

Introduction: The Borg Scale for perceived exertion is well established in science and sport to keep an appropriate level of workload or to rate physical strain. Although it is also often used at moderate and high altitude, it was never validated for hypoxic conditions. Since pulse rate and minute breathing volume at rest are increased at altitude it may be expected that the rating of the same workload is higher at altitude compared to sea level.Material and methods: 16 mountaineers were included in a prospective randomized design trial. Standardized workload (ergometry) and rating of the perceived exertion (RPE) were performed at sea level, at 3,000 m, and at 4,560 m. For validation of the scale Maloney-Rastogi-test and Bland-Altmann-Plots were used to compare the Borg ratings at each intensity level at the three altitudes; p < 0.05 was defined as significant.Results: In Bland-Altmann-Plots more than 95% of all Borg ratings were within the interval of 1.96 x standard deviation. There was no significant deviation of the ratings at moderate or high altitude. The correlation between RPE and workload or oxygen uptake was weak.Conclusion: The Borg Scale for perceived exertion gives valid results at moderate and high altitude – at least up to about 5,000 m. Therefore it may be used at altitude without any modification. The weak correlation of RPE and workload or oxygen uptake indicates that there should be other factors indicating strain to the body. What is really measured by Borg’s Scale should be investigated by a specific study.

Objectives: Workers in the mining industry in altitude are subjected to several risk factors, e.g., airborne silica and low barometric pressure. The aim of this study has been to assess the risks for this work category, evaluating single risk factors as airborne silica, altitude and work shift, and relating them with cardiovascular and ventilatory parameters. Material and Methods: Healthy miners employed in a mining company, Chile, working at varied altitudes, and subjected to unusual work shifts, were evaluated. Cardiovascular and respiratory parameters were investigated. Exposure to airborne silica was evaluated and compared to currently binding exposure limits. Results: At varied altitudes and work shifts, alterations emerged in haemoglobin, ventilation and respiratory parameters, related to employment duration, due to compensatory mechanisms for hypoxia. Haemoglobin increased with altitude, saturation fell down under 90% in the highest mines. The multiple linear regression analysis showed a direct relationship, in the higher mine, between years of exposure to altitude and increased forced vital capacity percent (FVC%), and forced expiratory volume in 1 s (FEV1). An inverse relationship emerged between forced vital capacity (FVC) and years of exposure to airborne silica. In the workplace Mina Subterrànea (MT-3600), statistically significant inverse relationship emerged between the Tiffeneau index and body weight. Conclusions: The working conditions in the mining industry in altitude appeared to be potentially pathogenic; further investigations should be realized integrating risk assessment protocols even in consideration of their undeniable unconventionality. Over the last 20 years, we can observe growing interest in the field of unmanned high altitude aerial vehicles. Especially the light aircrafts with electric motors become popular. The use of electricity to drive enables application of non-conventional energy sources like solar energy. The propeller remains the best propulsion system for planes powered by electricity. High altitude planes powered by propellers are able to fly over the wide range of altitude: from 0 to 25 kilometres. Such wide range of altitude is connected with variability of propeller work conditions (for example the density of the air is changing 10 times) which remarkably complicate the process of matching propeller to motor. The paper presents optimization model of propeller destined to unmanned high altitude airplane. Computational procedure deals with searching optimal distribution of lift coefficient line along the blade span of the propeller to obtain maximal efficiency for selected altitudes of flight. Genetic algorithm was used during the searching procedure. The database of optimal solutions is created as a result of computation and can be later exploited for choosing the best solution able to meet the requirements. Because the same propeller is used for take-off, climb and high altitude flight, then the airfoils of the propeller blades must be capable of operating over an extremely different flow condition caused by large change in air density. Propeller blades airfoils are required to operate within a low Reynolds number (below 105) and high subsonic Mach number (up to 0.6) flow field during high altitude flight. At low Reynolds number, the airfoils generate lesser lift and higher drags. The performance of the airfoils and consequently of the whole propeller can decline significantly. The objective is to find airfoils with good performance in all condition, which can appear during the flight, and to find the best shape of lift coefficient curve along the blade span of the propeller. That is necessary in order to obtain acceptable efficiency of the propeller for all flight conditions.

Cauliflower is grown in all the agro climatic zones of India, accounting for an area of 3.9 lakh hectares with production of 73.4 lakh tones. Hence, studies are to be undertaken to identify genotype and analyze the physical characteristics among the cultivated genotypes (Namdhari Seeds 60 N, Namdhari Seeds 133, Pusa Meghna and Basant) grown under hills and plains of Kerala, India. In this study, significant differences were observed in yield of cauliflower which ranged from 21 – 26.4 kg /16 m2 in hills and 0.78 – 1.43 kg /16 m2 in plains. Among the cauliflower genotypes, NS 133 was found to be the best for hilly region in early yield and yield attributes. The cauliflower genotype NS 133 exhibited better performance in hills in terms of high curd yield (26.4 Kg /16 m2), curd weight (800 g) and curd height (13.93 cm). In general, harvest maturity was delayed by 39 – 45 days in the plains as compared to higher altitudes. The cauliflower genotypes which exhibited superior performance in hills and plains were selected for further sensory evaluation studies. In this study cauliflower genotype NS 60N recorded superior organoleptic qualities when grown in plains than hilly region. However in the case of genotype NS 133, the differences in sensory qualities between produce from hills and plains were not significant.
