Opis:
The publication of the HL7-FHIR standard offers new possibilities for integrated applications in healthcare. Although trial implementations have only recently started, the application of FHIR in context of a Personal Health Monitoring solution is worth investigating. Most of the existing telemonitoring solutions in healthcare rely on guidelines defined by the Continua Health Alliance (CHA). This paper compares the requirements of CHA and HL7-FHIR with respect to data traffic between client devices and server side applications. Therefore an existing CHA-compliant telemonitoring solution is extended towards supporting HL7-FHIR. Both approaches were simultaneously evaluated in a live system with 68 participants. The results of the evaluation show that the FHIR approach offers the possibility of reducing data traffic in comparison to the CHA solution.

This study assessed the quality of referral letters to the Glaucoma Outpatient Clinic and patients’ first consultation results. A retrospective study included 196 patients newly referred to the clinic. Referrals were mainly issued by ophthalmologists (156; 79.6%). Regarding their urgency level, the planned referrals (184; 93.9%) were dominant. The majority of referral diagnoses (147; 75%) were confirmed as glaucoma in the clinic, whereas 17 (8.2%) glaucoma diagnoses were rejected. 64 (32.7%) patients did not require further ophthalmological review. The assessment of the quality of referral letters revealed that important clinical data was missing. There is a need to improve the referral system to specialist glaucoma clinics in Poland.

ObjectivesThe study aimed to investigate the direct and indirect – mediated through insomnia – effect of coronavirus anxiety on exhaustion, from the perspective of Hobfoll’s theory of conservation of resources (COR). According to the COR theory, critical events (e.g., the coronavirus epidemic) make people fearful of losing their valuable resources. A prolonged state of anxiety may lead to sleeping troubles, which over time results in an increase in exhaustion.Material and MethodsData were collected from 440 Polish healthcare providers, including nurses and midwives, doctors, paramedics, medical assistance workers, and wardens. Three measures were used: the Coronavirus Anxiety Scale, the Copenhagen Psychosocial Questionnaire (the sleeping trouble subscale) and the Oldenburg Burnout Inventory (the exhaustion subscale). Hypotheses were tested using structural equation modeling.ResultsThe obtained results fully support the hypotheses. Both the direct and indirect relationships between coronavirus anxiety and exhaustion were observed. Specifically, high coronavirus anxiety increased insomnia, which in turn contributed to the development of exhaustion.ConclusionsThe results are consistent with the COR theory. Prolonged coronavirus anxiety and sleeping problems depleted healthcare providers’ resources and made them feel exhausted. Exhaustion among these workers can have serious consequences not only for themselves but also for the health of their patients. Therefore, research into effective ways to deal with coronavirus anxiety is needed.

Goal – the aim of the article is to present the Bulgarian economic situation and development for the last 20 years. The observed trends reasons are indicated, the factors are derived and the socio‑economic consequences for the country are determined by statistical data analysis on manufacturing, labor resources, education, healthcare, employment, unemployment and income of the Bulgarian population. Research methodology – standard research methods are used for the article’s purposes such as: analysis and synthesis, critical analysis, comparative analysis, inductive and deductive method. Score/results – as a result of the analysis of the main macroeconomic indicators for Bulgaria, conclusions are reached about the vulnerability of the small economy and the existing risks in its development. At the same time, the available internal reserves are presented, which can be used to balance risks and increase the potential for economic growth. Originality/value – the article presents the author’s vision for the economic development of Bulgaria along with the related problems and opportunities, based on the official statistical information.

Opis:
Purpose: The paper presents an application of the Theory of Constraints (TOC) aimed at defining and maximising the potential of hyperbaric chamber as a key resource in a specialist hospital. Design/methodology/approach: The authors developed their own TOC based method of process improvement, supported by tools of Lean Management. Various constraints including an external one were taken into consideration. As a follow-up a project using the method was carried out in a hospital’s Hyperbaric Oxygen Therapy laboratory. Findings: The developed and implemented practical solution has improved patients’ access to the service at about 40% per day, shortened patients’ waiting time at about 90% and helped to overcome the threat of financial underperformance. Moreover, in combination with strict safety rules, the solution has proven its high resistance against difficulties caused by the COVID-19 pandemic. Research limitations/implications: A hyperbaric therapy is a narrow branch of medicine, therefore application of the method in other healthcare domains will require an appropriate adaptation. Practical implications: Despite the narrow subject of the study, the described approach is universal and can be used to maximise the capacity utilisation of various critical resources in hospitals and other healthcare providers. Originality/value: This is the first known to the authors application of a TOC/Lean based systematic hybrid approach to improve performance of a healthcare provider, which is taking into account several constraints including an external one. Additionally, this is the first paper on TOC in healthcare published in Poland.

Opis:
The article analyses systems of electronic information resources (registers and databases) in the field of the healthcare in different countries. Not only do such systems provide information to support the treatment of patients, but they also accumulate large amounts of statistics, thus enabling their qualitative operational analysis. Electronic health systems are particularly popular in the United States, and as regards Europe, in Denmark and Norway. These countries created good conditions for the efficient introduction of new electronic information resources that would store all the accumulated information about the COVID- 19 pandemic. The aim of the article is to summarise information on the use of electronic registers and databases to create an information base for the study of the COVID-19 pandemic and its consequences in different countries, and to formulate proposals for the improvement of electronic health systems in Ukraine. On the basis of the results of their research, the authors propose a list of electronic registers that can significantly improve the analysis of both the course and the consequences of the coronavirus disease. The list includes: the register of immunisations, a specialised register of complications after the recovery from COVID-19, a register of tests for the presence of this disease or the recovery, etc., as well as a register of hospitalisation cases.
