Consumer and system-wide gains remain limited by an outmoded policy regime. With scientific innovation running far ahead of public policy, physicians, researchers and patients are not receiving full advantage of the latest developments. European health systems require a seamless and rapid flow of digital information, including genomic, clinical outcome, and claims data. Research derived from clinical care must feed back into assessment, in order to advance care quality for consumers. National health systems are heterogeneous; the solutions and required fundamental approaches differ between the European member states and are not entirely portable and scalable. To date, this applies not only to general systemic aspects but particularly to cross-border reimbursement issues and the exchange of treatment and patient data. To answer those needs, an international consortium was established to implement the project “Interregional coordination for a fast and deep uptake of personalised health”: Regions4PerMed. A cycle of international events, such as conferences, in situ visits and workshops, has been planned. Interdisciplinary groups of experts will exchange thoughts and experiences to design solutions that could be implemented in the various healthcare systems. Regions4PerMed aims to coordinate regional policies and innovation programmes in personalised medicine and personalised health to accelerate the deployment of personalised health for patients. Key Action 2 is dedicated to health technologies and smart and integrated care.

The Analytical Hierarchy Process (AHP) is one of the multi-criteria methods with tools that are effective for decision-making in complementary or alternative medicine (CAM). This approach, in combination with other available methods, such as the Utrecht Method, allows medical professionals and patients themselves to take a balanced choice of special treatment and health control. This is particularly important for pregnant women who suffer from nausea and vomiting during pregnancy (NVP). The mechanisms of toxicosis occur, and the way of treatment have certain uncertainties. The decision-making process for therapeutic care must consider several factors, the determination of which is the result of collaboration between the patient and the healthcare provider. This paper presents the results of a decision-making study regarding the use of the ginger medicinal plant in helping a woman with NVP symptoms. The research was conducted using the original questionnaire according to the methodology proposed by the developers of the Utrecht Method. The chosen respondents were healthcare professionals with experience in treating NVP and pregnant women. The Analytical Hierarchy Process (AHP) capabilities in decisionmaking were shown in an example of analyzing the benefits, risks, and side effects of using ginger during pregnancy. The combination of two methodological approaches allowed to successfully connect the proven facts of using CAM therapy with the subjective assessment of all stakeholders. The study demonstrates that this approach successfully uses decision-making by structuring hierarchical decision elements and weighing the criteria involved in the decision-making issue. The article examines the practicality and effectiveness of using AHP when making decisions regarding the treatment and improvement of the condition of pregnant women with NVP, and summarizes the quantitative and qualitative indicators of the effectiveness of this method. The presented combination of two methodological approaches in decision-making allows for choosing an optimized medical strategy for supporting pregnant women through CAM therapy.

Objective: Spa treatment arouses a lot of controversy and conflicting opinions. The aim of this study was to evaluate the clinical efficacy and safety profile of spa treatment. Assessment was based on a review of available publications. Methods: The Cochrane Library, MEDLINE, EMBASE and the Centre for Reviews and Dissemination (CRD) databases were searched for health technology assessment (HTA) reports and systematic reviews. All publications which met the predesigned inclusion criteria were included. The data search was concluded on 27st May 2011. Additionally Acta Balneologica, Rehabilitacja Medyczna and Fizjoterapia Polska were searched for both clinical trials and reviews.Results: During the search 872 publications were identified of which 41 articles were selected for further analysis, out of which 8 systematic reviews met the inclusion criteria upon the basis of full text analysis. Additionally 11 reports which had been published in Acta Balneologica and 3 in Fizjoterapia Polska were included in the study. Based on the performed review of the available studies, spa treatment appears to be an effective method in the treatment of patients suffering from osteoarthritis, fibromyalgia, chronic low back pain, psoriatic arthritis, ankylosing spondylitis and rheumatoid arthritis. Spa treatment is associated with no serious side effects.Conclusions: Spa treatment is effective in the relief of symptoms of rheumatic diseases and musculoskeletal disorders. However, existing research is not sufficiently strong to draw firm conclusions. Well-designed controlled clinical trials are needed for the assessment of the effects of spa treatment.
Założenia: Leczenie uzdrowiskowe budzi wiele kontrowersji oraz skrajnych opinii odnośnie skuteczności i bezpieczeństwa tej formy terapii, a co za tym idzie finansowania leczenia w uzdrowisku ze środków publicznych. Cele: Celem pracy jest ocena efektywności klinicznej leczenia uzdrowiskowego przeprowadzona na podstawie analizy wyników doniesień naukowych odnalezionych w ramach przeglądu literatury. Metodyka: Poszukiwano wiarygodnych opracowań wtórnych dostępnych w bazach informacji medycznej: The Cochrane Library, bazie MEDLINE, EMBASE oraz Centre for Reviews and Dissemination (CRD). Włączano wszystkie prace spełniające kryteria włączenia (przeglądy systematyczne oraz niezależne raporty oceny technologii medycznej – raporty HTA), które zostały opublikowane do dnia 27 maja 2011 roku. Dodatkowo przeszukano artykuły opublikowane w polskich czasopismach: „Acta Balneologica”, „Rehabilitacja Medyczna” oraz „Fizjoterapia Polska” (do analizy włączano zarówno pierwotne badania kliniczne, jak również opracowania wtórne). Wyniki: W wyniku przeszukania elektronicznych baz informacji medycznej odnaleziono 872 pozycje bibliograficzne, z których do dalszej analizy włączono 41 doniesień naukowych. Spośród 41 doniesień naukowych, wstępnie zakwalifikowanych do analizy, na etapie weryfikacji prac na podstawie pełnych tekstów do analizy włączono 8 prac. Dodatkowo do analizy włączono 11 doniesień naukowych opublikowanych w czasopiśmie „Acta Balneologica” oraz 3 prace opublikowane w „Fizjoterapii Polskiej”. Na podstawie wyników odnalezionych doniesień naukowych ocenia się, że lecznictwo uzdrowiskowe istotnie zmniejsza nasilenie bólu oraz poprawia jakość życia pacjentów z chorobą zwyrodnieniową stawów, fibromialgią, przewlekłym bólem odcinka lędźwiowo-krzyżowego kręgosłupa, łuszczycowym zapaleniem stawów, zesztywniającym zapaleniem stawów kręgosłupa oraz reumatoidalnym zapaleniem stawów. Nie raportowano żadnych poważnych działań niepożądanych związanych z leczeniem w uzdrowisku. Wnioski: Leczenie uzdrowiskowe jest skuteczne w zmniejszaniu nasilenia objawów niektórych chorób układu mięśniowo-szkieletowego. Niezbędne są dalsze, dobrze zaprojektowane, kliniczne badania z grupą kontrolną, oceniające skuteczność leczenia uzdrowiskowego.

Introduction: Georgia faces great challenges at the beginning of the new millennium as a developing country. There are high risk factors for various diseases, often associated with environmental pollution and the significant lack of green eco-cover. The purpose of this research is to study the factors affecting environmental pollution and health deterioration in Georgia. Materials and Methods: Within the quantitative study in-depth survey of respondents and experts were conducted using a pre-structured questionnaire and focus group method. Results: 93% (n=582) of respondents believe that air or arable lands contamination with hazardous waste affects their health. Nevertheless, 46.4% (n=289) of respondents maintain a positive attitude towards future technologies, thinking that modern advances can change the environment so that people's lives become more comfortable and productive. According to experts, their place of residence is contaminated. The majority of respondents (73.4%, n=367) report that in Georgia they are most concerned about the incidence of diseases caused by unhealthy essentials (food, clothing and living environment) and unhealthy conditions. 63.7% (n=319) of the respondents think that agricultural products grown by chemical methods are a major threat, while 69.3% (n=347) of the respondents think that various foodstuffs (sweets, preserves, dairy products, etc.), containing the hazardous substances are not useful, though the alternative is less found in the country. Within the framework of the study, a model of city arrangement as well as the shelves for growing useful plants and micro-greens waves in the rooms of the apartments have been developed. Conclusion: It is advisable to encourage eco-projects by the state and to give people long-term loan at a low interest, that will allow people to live in ecological cities. The content project of application developed, which detects pesticides in foodstuffs and determines how harmful or healthy the goods are, will help people to protect themselves from the risks of negligence.

The article focuses on the reorganization of teaching and educational space, which would be based on the principles of innovation, and its integration into European and world educational space. It is ascertained that one of the priority methods for developing national education is to expand additional free opportunities for physical culture and sports classes in the system of educational institutions; to develop a network of free institutions of physical culture and sports for children and young people outside the system of formal education. The efficiency of recreational technologies in the teaching and educational process in educational institutions which improve the quality of sports and recreational activities and ensure their availability and performance has been proved. It is shown that using elements of floorball sports in the physical education preschool system improves the performance of physical development, physical fitness and psycho-emotional sphere of children of 5 and 6 years of age.
Artykuł dotyczy głównie reorganizacji nauczania i przestrzeni edukacyjnej, w oparciu o innowacyjne zasady, oraz integracji z europejską i światową przestrzenią edukacyjną. Ustalono, że jedną z priorytetowych metod rozwoju edukacji narodowej jest poszerzenie dodatkowych, bezpłatnych możliwości korzystania z kultury fizycznej i zajęć sportowych w systemie placówek edukacyjnych; rozbudowa sieci bezpłatnych placówek kultury fizycznej i sportu dla dzieci i młodzieży niezależnych od systemu edukacji formalnej. Uznano efektywność technik rekreacyjnych w procesie nauczania i wychowania stosowanego w placówkach edukacyjnych, które poprawiają jakość aktywności sportowej i rekreacyjnej i zapewniają ich dostępność. Wykazano, że zastosowanie elementów unihokeja w wychowaniu fizycznym na poziomie przedszkolnym stymuluje poprawny rozwój fizyczny, sprawność fizyczną i sferę psychoemocjonalną dzieci w wieku 5 i 6 lat.

A quiet revolution underway is leading to changes in healthcare for many countries of the world. The sentinels of health have always been physicians; the medical model, representing the research, education, and experience of medical doctors, has worked well for treating acute conditions of injury and physical illness and been very successful in reducing disease-producing morbidity and mortality. But pressing contemporary issues, such as spiraling healthcare costs, population aging and the need to manage chronic conditions, and recognition that mental health care is necessary for societies to be physically healthy, are forcing a reevaluation of existing conceptualizations of care. In response, physicians are increasingly working in integrated practices with other specialists to broaden care coverage to include social and behavioral conditions empirically demonstrated to influence medical care outcomes. Aiding the development of these new, more inclusive models of healthcare are advances in communication technologies. Practitioners are now using synchronous and asynchronous forms of communication to deliver physical and mental health services beyond the boundaries of traditional brick-and-mortar practices, into communities where clients live and work. This brief review of the potential of telehealth to address disparities in healthcare has two purposes: 1) examine an innovative model of comprehensive, integrated biopsychosocial services useful in single- and multiple-location practices; and, 2) identify challenges of using synchronous, virtual technologies for client-centered, mental health care service provision in rural, underserved areas.

The increasingly more visible trend of population ageing necessitates creating technologies supporting the functioning of older adults. Consequently, more gerontechnologies emerge designed to help the older adults in their daily functioning, from devices monitoring the health to special trolleys improving the mobility or Virtual Reality devices for active learning. This article aimed to determine the most desired group of gerontechnologies among current and future users. It focused on individual assessments of the most desirable group of gerontechnologies based on various criteria. The investigation aimed to find the criterion rated the highest in the selected group of gerontechnologies. The gerontechnology group was assessed against seven groups of criteria distinguished by the authors, i.e., Technology Innovation, Technology Demand, Social and Ethical Criteria, Technology Usability, Technology Functionality, Technology Ease of Use and Technology Use Risk. The survey was conducted in the form of a questionnaire, using CAWI (Computer-Assisted Web Interview) and CATI (Computer-Assisted Telephone Interview) methods, with the research sample comprised of 1 152 residents of Poland. Thus far, no studies have been conducted to evaluate this group of technologies based on the above-mentioned criteria.

One of the main problems in the scope of public health is poor identification of the waterborne diseases and their etiology. One of the aims of the new directive concerning the quality of water intended for human consumption is to implement the principles of hazard assessment in water supply systems (WSS) in the member states, on the basis of water quality monitoring and analyses of the system functioning conditions. The implementation of risk management procedures in WSSs is to ensure an increase in the water supply safety and, in turn, protect the consumer’s health against the harmful effects of any potable water contamination. At present, the secondary legislation to the Polish acts implementing the Directive of the Council of Europe is the Regulation of the Minister of Health on the quality of water intended for human consumption, which also sets forth the scope of monitoring the quality of hot water for Legionella spp., which can be an etiological factor of legionellosis. Another act regulating the water quality in terms of Legionella spp. is the Regulation of the Minister of Health on water quality in swimming pools. These acts, referring to the parameter of Legionella spp., are complementary. This paper presents the results of evaluating the change in the sanitary status of hot water quality in terms of contamination with Legionella spp. bacteria in the selected swimming pool facilities in southern Poland (Silesian Voivodship). On the basis of the available water quality test from state inspections (Chief Sanitary Inspectorate), an analysis of the microbiological quality of hot water in swimming pools was conducted. The analyses included a spatial distribution of those facilities, which results from the regionalisation subject to the territorially competent public health authority exercising supervision over the water quality in the Silesian Voivodship. The methods were presented for determination of the health risk of swimming pool users as a result of contamination of swimming pool water with Legionella spp. in relation to the availability of a given recreation facility. The final result of these studies is the use of GIS (Geographic Information System) tools in spatial risk interpretation.

Objectives: The contribution deals with a risk assessment in practical applications of the high-energy liquid jet technology from the point of view of the risk identifi cation, estimation and evaluation. Materials and Methods: Differences between three different types of workplaces are highlighted and analysed - the indoor, the outdoor and the research ones. Theoretical analyses are supported by particular application of the method for the risk assessment in the Laboratory of Liquid Jets at the VŠB - Technical University of Ostrava. This laboratory is primarily oriented to research. Nevertheless, the conclusions can be used also for predominantly commercial workplaces. Results: Some new considerations and evaluations concerning health and safety are presented. Conclusions: Failure Mode and Effect Analysis (FMEA) procedures were applied and their limitations in risk assessment of water jet-based technologies are explained.

That requires taking into account multiple criteria, of which two important ones are: cost and effectiveness of a technology (others being, e.g., prevalence, safety, ethical and social implications). Hence, health and wealth need to be traded off against each other, and hence the willingness-to-pay (WTP) has to be determined. Various approaches to setting WTP have been taken, yet the results differ substantially. In the present paper I claim that the proper approach is to treat WTP as a fuzzy concept (the decision maker may not be able to decidedly state that a given health-wealth trade-off coefficient is acceptable/unacceptable - an idea backed up by the survey presented in the paper). Previous research shows how this fuzzy approach can be embedded in defining the preference relation and pairwise comparisons. In the present paper I account for the fact that there are often more than two alternatives available. To avoid difficulties that might arise (e.g., incompleteness or intransitivity of preferences) I show how the fuzzy approach can be used to define a fuzzy choice function based on the axiomatic approach. Some properties are discussed (e.g., how the approach handles the dominance and extended dominance), and the directions of further research are hinted at.

Opis:
Croatian national health ICT implementation strategy is determined by Croatian national health strategy and plan, Croatian ICT development strategy for 21st century, and requirements specifications for the health information system. National health ICT implementation strategy components are accented: purpose of the ICT implementation strategy, information principles, needs and ICT enablement in domains of patients, healthcare professionals, policymakers and managers and public. Telemedicine and telecare positions and implementation steps are described. Based on the determinants, three organizational levels have been established - government, ministerial and project levels. General architecture of Croatian healthcare information system and respective pilot projects and results of pilot implementations as well as national ICT environmental accelerators for health ICT implementations are presented.

Opis:
Mobile solutions revolutionize healthcare systems as well as the way of management in this area. The complex and dynamic character of healthcare organizations requires a special kind of management in the areas of healthcare knowledge and electronic health records (EHRs). Mobile applications can be very useful and helpful in the terms of information providing, but their role is not limited to transferring it between the doctors, patients and managerial staff. In this paper the role of mobile applications in the work of health care professionals (HCPs) has been shown.This article highlights the role of mobile applications in facilitating HCPs’ work in the terms of time management and EHRs storage and management.
Rozwiązania mobilne są bardzo silnym trendem w systemach opieki zdrowotnej, jak i w zarządzania w tym obszarze. Złożony i dynamiczny charakter organizacji opieki zdrowotnej wymaga specjalnego rodzaju zarządzania w zakresie wiedzy medycznej i elektronicznych rejestrów medycznych. Aplikacje mobilne mogą być bardzo przydatne i pomocne w zakresie dostarczania informacji, ale ich rola nie ogranicza się do przenoszenia ich między lekarzy, pacjentami i personelem kierowniczym. W niniejszym artykule uwypuklono rolę aplikacji mobilnych w pracy pracowników służby zdrowia (HCPS), podkreślono rolę aplikacji mobilnych w ułatwianiu pracy pracownikom służby zdrowia w zakresie zarządzania czasem i EHRs przechowywania i zarządzania.
