In this paper, we introduce the notion of fuzzy n-fold integral filter in BL-algebras and we state and prove several properties of fuzzy n-fold integral filters. Using a level subset of a fuzzy set in a BL-algebra, we give a characterization of fuzzy n-fold integral filters. Also, we prove that the homomorphic image and preimage of fuzzy n-fold integral filters are also fuzzy n-fold integral filters. Finally, we study the relationship among fuzzy n-fold obstinate filters, fuzzy n-fold integral filters and fuzzy n-fold fantastic filters

In this paper, the concepts of n-fold implicative ideals and n-fold obstinate ideals in BL-algebras are introduced. With respect to this concepts, some related results are given. In particular, it is proved that an ideal is an n-fold implicative ideal if and only if is an n-fold Boolean ideal. Also, it is shown that a BL-algebra is an n-fold integral BL-algebra if and only if trivial ideal {0} is an n-fold obstinate ideal. Moreover, the relation between n-fold obstinate ideals and n-fold (integral) obstinate filters in BL-algebras are studied by using the set of complement elements. Finally, it is proved that ideal I of BL-algebra L is an n-fold obstinate ideal if and only if \(\frac{L}{I}\) is an n-fold obstinate BL-algebra.

Laryngeal reinnervation is the method of treatment in unilateral vocal fold paralysis. During the surgery an anastomosis between the main branch of ansa cervicalis and recurrent laryngeal nerve is formed. Regeneration of the nerve’s functional abilities is a long process, lasting from 4 to 6 months, up to a year. A 35 year-old male was admitted to the Department of Otolaryngology at Warsaw Medical University in order to undergo surgical removal of reoccurrence of Schwannoma in left cerebellopontine angle by translabyrinthine approach. Post-operative period was complicated with vagal nerve paralysis. The patient reported hoarseness, dysphagia and choking. Physical examination revealed left vocal fold immobilization in abducted position, saliva in piriform fossa and left-sided soft palate paralysis yet the patient was discharged home. 4 weeks after the surgery, the patient was admitted to the hospital again due to developing aspiration pneumonia. Augmentation of the left vocal fold with calcium hydroxyapatite was performed 2 months later. After this intervention temporary vocal improvement and choking frequency reduction was observed. Finally, the patient underwent laryngeal reinnervation with formation of ansa cervicalis main branch to recurrent laryngeal nerve anastomosis. Left vocal fold regained its functional abilities and the quality of the voice improved. Laryngovideostroboscopy performed 6 months later revealed mobilization of left vocal fold sufficient for proper glottis closure. The voice measures normalized and the swallowing improved. Conclusions This case report and analyzed data confirm that laryngeal reinnervation surgery in unilateral vocal fold paralysis helps achieve satisfactory effects in voice function and life quality.

Opis:
Vocal cord palsy as a result of tonsillectomy in local anesthesia is a very rare complication. In literature, there is only few publication describing this side effect of tonsillectomy. We present a case of 26 years old men who was classified for surgical removing of tonsils in local anesthesia using 1% lignocaine with 1:100 000 epinephrine. During the procedure respiratory failure with stridor was developed, examination using indirect laryngoscopy show bilateral vocal chord palsy. The authors present probably pathomechanism of this complication.

Opis:
Narrow band imaging (NBI) by enhancing the contrast between the mucosal epithelium and submucosal vessels facilitates diagnosis of precancerous and cancerous lesions, as well as hypertrophic lesions such as laryngeal papillomatosis. Narrow band imaging (NBI) is an optical technique based on the modification of white light by the use of special optical filters. Every change in the microvascular architecture of the mucosa is classified according to Ni’s classification (2011). The use of NBI improves sensitivity and specificity of assessment of laryngeal lesions and allows more precise assessment of the status of surgical margins of early-stage and locally-advanced laryngeal cancers managed in endoscopic laser cordectomy.

Introduction: The most common cause of vocal fold paralysis (VFP) is iatrogenic injury. Patients with symptoms of VFP present to the specialist after a couple of weeks or even months since the onset of symptoms. In the literature, the data regarding speech impairment during the first days after a iatrogenic injury is lacking. Aim: to evaluate the quality of voice during first days of vocal fold paralysis following a iatrogenic injury. Materials and methods: Twenty-five patients with iatrogenic vocal fold paralysis who presented for phoniatric consultation at the Department of Otolaryngology of the Medical University of Warsaw between May 2015 and December 2016 were enrolled in the study. The patients were examined 1-2 days since the onset of speech deterioration. In all patients, laryngeal videolaryngostroboscopy was performed, based on which the following were assessed: vocal fold mobility, mucosal wave, phonation closure, simultaneity and amplitude of vocal fold vibration. Acoustic analysis was performed, and the following acoustic parameters were evaluated: DSI, F0, Shimmer, Jitter, NHR. Also, the maximal phonation time of [a] sound (MPT a) was assessed and the voice perception analysis with the GRBAS scale was performed. The patients self-evaluated their voice using a 10-point VAS scale. Results: Based on the videolaryngostroboscopy, phonatory insufficiency and asymmetrical vocal fold vibration were observed. On perception assessment with GRBAS scale, we noticed slight to mild degree hoarseness, breathiness of sound and weakened voice. In majority of patients, the maximal phonation time of [a] sound was significantly reduced. All patients showed abnormal acoustic parameters. None of them rated their voice as perfectly normal on VAS scale.

Objective: The process of ageing begins after 60 years of age and is referred to as presbyphonia (Vox senium). The causes include functional or organic voice disorders, often coexisting with dry upper respiratory tract infection. Introduction: The aim of the study is the use of high-speed camera and acoustic voice analysis in diagnostics of the clinical form of presbyphonia. M aterials and methods: The study included a group of 50 men, non-smokers, age from 51 to 72, who do not use their voice professionally. High-Speed Digital Imaging and HS camera have been used, allowing evaluation of real vibrations of vocal folds, along with acoustic voice analysis using a software by DiagNova Technologies. Results: VHI questionnaire has been used for self-assessment of voice disability. Visualizations of the larynx enabled recognition of hypofunctional dysphonia or atrophy of vocal folds that cause voice disorders. This was confirmed by parameters of voice acoustic evaluation: F0, NHR, narrowband spectrography. The pathological value of NHR and the presence of nonharmonic components in the range of high frequency levels indicated glottal insufficiency, recorded with the visualization technique of the larynx by HS camera. A significant shortening of maximum phonation time in relation to the control group has also been recorded. Discussion: The objective examination of voice pathology is crucial in diagnosis and rehabilitation, however, subjective assessment of the patient is important in the scope of the procedure used. The patient’s subjective self-rating assessment (VHI) confirmed the sense of voice disorders in elderly men, indicating the need for rapid and accurate clinical diagnosis.

Aim: We assessed long-term outcomes of vocal fold augmentation with calcium hydroxylapatyte performed under local anesthesia. Material and methods: We enrolled 17 patients with glottic insuf ficiency due to unilateral laryngeal paralysis or insuf ficiency of internal laryngeal muscles (10 women, 17 men; mean age, 57.6±17.7 years; median age, 61 years; age range, 32-83 years). All patients underwent laryngeal augmentation under local anesthesia, through the oral cavity, with calcium hydroxylapatite (Radiesse) injected laterally to the vocal folds, unilaterally or bilaterally. We assessed voice quality before laryngeal augmentation and at 3 and 12 months. To that end, we performed videolaryngostroboscopy, perceptual assessment of voice, acoustic analyses, and aerodynamic assessments; moreover, participants completed the Voice-Related Quality of Life (VRQoL) questionnaire. Results: Af ter 3 months, we observed a statistically significant improvement on the perceptual assessment of voice with regard to the G and A parameters, and good outcomes were also observed at 12 months. On the acoustic analyses, MDVP_Jitt, MDVP_Shim, and MDVP_NHR improved to 2.5, 5, and 0.1, respectively, at 3 months, and to 1.9, 3.6, and 0.1, respectively, at 12 months. MPTa was prolonged to 12.2s and 11.9s at 3 and 12 months, respectively. Voice intensity improved from 67dB(A) before augmentation to 68dB(A) and 71dB(A) at 3 and 12 months, respectively. VRQoL scores improved to 19.5 and 20 at 3 and 12 months, respectively. Discussion: Laryngeal augmentation with calcium hydroxylapatite performed under local anesthesia is associated with good long-term outcomes.

Opis:
Basing on analysis of tectonic mesostructures, the structural evolution stages of the Gnieździska Syncline have been determined. The structure represents a typical example of folds occurring in the SW margin of the Holy Cross Mountains. The well-exposed syncline displays a wide variety of structures, including: shear and extension joints, stylolites, cleavage, strike-slip and dip-slip faults as well as master joints. Tectonic structures resulting from flexural slip indicate that the Gnieździska Syncline developed as a flexural-slip fold as a result of horizontal NE-SW compression. The subsequent deformation phase included mesostructures pointing to the increasing activity of a nearby Gnieździska-Wola Morawiecka dextral strike-slip fault of regional extent. In the terminal phase of the post-kinematic uplift of the Holy Cross Mountains, T joints and master joints appeared.

Medialization thyroplasty (type I) is surgical procedure performed on the thyroid cartilage. The major indication for this surgery is significant glottis insufficiency due to unilateral vocal fold paresis. However the proce¬dure is also performed after vocal fold resections during cordectomy. The aim: The evaluation of voice results in patients after medialisation throplasty. Material and methods: In Otolaryngology Department of Medical University of Warsaw there were performed so far 8 thyroplasty procedures under local anaesthesia with implantation of medical silicon protesis. 6 patients had unilat¬eral vocal fold paresis and the rest two underwent in the past laser cordectomy due to T1a vocal carcinoma. Results: There were no complications during and post the surgery. The follow up examination in 1st , 3rd, 6th i 12th months postoperatively revealed for all patients significant improvement of glottal closure in laryngeal videostrobos¬copy. The voice quality improved both in perceptual evaluation (GRBAS scale) and acoustic analysis (F0, jitter, shim¬mer, NHR) in both patients groups. However the rate of improvement was much more significant in group with uni¬lateral vocal fold paresis. In all patients the maximum phonation time (MPT) increased. The self-evaluation of voice quality with Voice Handicap Index questionnaire confirmed also individual improvement. Conclusions: The speech rehabilitations is not successful in each patient with glottis insufficiency. The medialisation thyroplasty remains the standard procedure for permanent improvement of voice quality in those cases.

This paper focuses on mineral veins hosted in small-scale fractures within the Tertiary sandstones of the Magura nappe (Outer Carpathians). Joints, faults and tension gashes record three successive stages of the nappe structural evolution: (1) synsedimentary folding and thrusting, (2) regional rotation and (3) late-orogenic collapse. The flow of mineral-bearing fluids was channelized by small-scale fractures resulting in calcite and quartz-calcite veins. Folding: columnar calcite formed the most common filling of early joints and the joints-related tension gashes and strike-slip faults. The mineralisation was restricted to sandstones containing primary carbonates. Rotation: precipitation of columnar and fibrous calcite was largely restricted to joints reactivated as strike-slip faults. Collapse: the collapse-related mineralisation is the most abundant. Wide-spread fluid flow was channelized by normal faults resulting in fibrous calcite and quartz-calcite along these faults whereas several blocky and drusy calcite generation and single high temperature quartz-calcite assemblage precipitated in numerous adjoining joints.

On a basis of tectonic mesostructures, the pre-late Tremadocian tectonic event (Sandomirian “phase”), believed to account for deformation of the Cambrian system in the Kielce Unit (Holy Cross Mts., Central Poland), has been subdivided into 3 successive tectonic episodes of different kinematics. During the first stage (D1), flat, westerly vergent thrusts originated, giving rise to gravitational sliding. Subsequently, during D2, the entire sequence was imbricated by the northerly vergent thrusts. In the terminal phase (D3), map-scale folds developed, changing inclinations of previously formed structures.Mode of deformation during the D2 stage is typical for thrust and fold belts. Absolute dating of these events remains speculative, but the pre-late Tremadocian age of the D1 and D2 phases is evident. The D3 is considered as a continuation of the D2, but alternative concepts are also acceptable in the light of the presented data.
