The Haddon matrix is a potential tool for recognizing hazards in any operating engineering system. This paper presents a case study of operational hazards at a large construction site. The fish bone structure helps to visualize and relate the chain of events, which led to the failure of the system. The two-tier Haddon matrix approach helps to analyze the problem and subsequently prescribes preventive steps. The cybernetic approach has been undertaken to establish the relationship among event variables and to identify the ones with most potential. Those event variables in this case study, based on the cybernetic concepts like control responsiveness and controllability salience, are (a) uncontrolled swing of sheet contributing to energy, (b) slippage of sheet from anchor, (c) restricted longitudinal and transverse swing or rotation about the suspension, (d) guilt or uncertainty of the crane driver, (e) safe working practices and environment.

This paper deals with the phase-shift fault analysis of stream cipher Grain v1. We assume that the attacker is able to desynchronize the linear and nonlinear registers of the cipher during the keystream generation phase by either forcing one of the registers to clock one more time, while the other register is not clocked, or by preventing one of the registers from clocking, while the other register is clocked. Using this technique, we are able to obtain the full inner state of the cipher in reasonable time (under 12 hours on a single PC) by using 150 bits of unfaulted keystream, 600 bits of faulted keystreams and by correctly guessing 28 bits of the linear register.

Risk analysis is a key phase of the process of water supply safety management. The aim of this paper is to present new propose of methods for risk of the first type (associated with quantity of supplied water) and the risk of the second type (associated with quality of supplied water) analyses in water supply system. The paper contains the description of matrix methods for risk analyses and methods describing cause-and-effect relationship of failure events (fault tree analysis).

The Solokha gas field is one of the most valuable hydrocarbon fields in the central axial part of the Dnieper-Donets Basin (DDB). Every new exploration well there has to be highly validated in order to reduce possible drilling risks. In this study, the characteristics of faults, as possible seals, are analysed, based on the latest well and 3D seismic data. Evaluating fault seal risk is a constitutive factor in hydrocarbon exploration and production. For this reason, to reduce uncertainty in the faults’ leaking facilities, we used a couple of known approaches, such as juxtaposition analysis, with building Allan diagrams, to determine the relationship between sand-sand juxtaposition and the occurrence of hydrocarbons-bearing sands. We used the Shale Gouge Ratio (SGR) calculation with prediction of clay content distribution along the faults, pressure data analysis, seismic at tributes modelling and self-organized map clustering analysis for accurate delineation of sand bodies shape. This represents a complex research method of integration of high-quality depth processing with seismic structural and stratigraphic interpretation and, also, geological modeling. In this paper, we assessed the Jurassic and Visean faults of the Solokha gas field, according to their fluid cross-flow facilities. This research will provide valuable information about the presence of prospective places for hydrocarbon accumulation and will likely impact the well staking process for the upcoming drilling.

The development of fault pattern in the Silesian Nappe (Central Carpathian Depression) in the eastern part of Polish Outer Carpathians is outlined, from field observations and interpretation of air photos and radar images. Fault slip analysis and palaeostress reconstruction was applied to determine different systems of strike-slip, reverse and normal faults and the relative age of each system was determined. The results show a consistent evolution of fault systems which occurred as several episodes. Some of these episodes are local but others represent a regional pattern of faulting across the whole Polish Eastern Outer Carpathians. The beginning of fault evolution took place in Late Oligocene to Late Miocene times. The oldest phase is represented by reverse and thrust faults of system R1 with SW-NE compression; a younger phase involves origination of strike-slip faults belonging to system S1 (with the same direction of compression). Reverse (system R2) and strike-slip (system S2) faults were formed locally during a N-S compressional event. Dextral strike-slip faults of system S3 with simultanous opening of a dextral set of fault system S1 originated subsequently. The youngest events are represented by normal faults N1, N2, and N3 systems during NW-SE, and SW-NE to N-S extension.

Technological progress, as well as the development of the sciences on occupational health and safety, increased safety and improved working conditions in enterprises of even the most onerous industries. To maintain the required level of safety, preventive actions are taken to identify all hazards in the workplace. In practice, many methods are used to identify hazards depending on the type of technological processes and the types of workstations analyzed. The article presents the advantages of the fault tree analysis method as a tool to support the process safety management in the enterprise. The article presents features of the fault tree analysis method as a tool for better security management in an enterprise. Cause and effect relationships between events in the fault tree schema on selected examples were examined. Indirect and direct causes of accidents and failures in the enterprise have been identified, including human errors (human factor). The use of the fault tree analysis method to support accident and failure risk assessment in an industrial plant was also analyzed.

Many lives and aircrafts have been lost due to human errors associated with mental workload overload (MWLOL). Human errors are successfully considered in existing Fault Tree Analysis (FTA) methods. However, MWLOL is considered through Performance Shaping Factors indirectly and its information is hidden in FT construction, which is not conducive to analyze the root causes of human errors and risks. To overcome this difficulty, we develop a risk analysis method where Multiple Resources Model (MRM) is incorporated into FTA methods. MRM analyzes mental workload by estimating the resources used during performing concurrent tasks, probably including abnormal situation handling tasks introduced by basic events in FT. Such basic events may cause MWLOL and then trigger corresponding human error events. A MWLOL gate is proposed to describe MWLOL explicitly and add these new relationships to traditional FT. This new method extends previous FTA methods and provides a more in-depth risk analysis. An accident, a helicopter crash in Maryland, is analyzed by the proposed method.

Due to the unique geographic location, complex navigation environment and intense vessel traffic, a considerable number of maritime accidents occurred in estuarine waters during recent years which caused serious loss of life, property and environmental contamination. Based on the historical data of maritime accidents from 2003 to 2012, which is collected from Shenzhen Maritime Safety Administration, this paper conducted a risk analysis of maritime accidents by applying Bayesian network and fault tree analysis. First a Bayesian network model was introduced to describe the consequence of accidents based on the accident investigation report. Then fault tree analysis was applied to estimate the probability on the basis of accident statistics and ship traffic flow. Finally the risk of maritime accidents in Shenzhen Waters was depicted through the consequence multiplied by the probability of an accident.

The mechanical interface has the characteristics of low shock and vibration, and is emphasized in the aerospace and ocean engineering fields. In this paper, a mechanical interface based on coupled cylindrical cam mechanisms is designed. It can achieve the expected functions, but there exist faults in some times. The fault modes and causes of the interface are firstly analyzed. Then a design approach based on Monte Carlo simulation is presented for analyzing and optimizing its reliability. According to the fault modes, the performance functions of the interface are established for obtaining the optimal scheme. A case is given to illustrate the proposed method. The simulation results and the prototype experiments prove that the optimization scheme effectively improves the reliability of the interface, and has better performance than the original one.

The article presents an application of a method based on fault tree analysis and the Monte Carlo simulation in the assessment of reliability and availability of the rail means of transport. The primary target of the presented method is a cause and effect assessment of the occurrence of undesirable events, the determination of selected reliability indices and identification of the weakest components of rail vehicle that affect the downtime and technical availability most strongly. To illustrate the application of the presented method, the results of a project involving a 6Dg diesel locomotive, carried out in cooperation with the biggest Polish rail carrier, are shown. The assessment of availability and reliability was based on real operation data of a selected sample of seventy-five locomotives. Based on the collected data from the operation of the 6Dg locomotives, the times-to-failure and the times-to repair models were determined. A fault tree model of the locomotive was developed to assess the influence of the faults of the components on the reliability of the vehicle. A discrete simulation process allows to obtain a chosen characteristics and values of the selected measures, which – according to the authors – may be applied to assess the reliability and availability of the rail vehicles. Specialist software including Weibull++, BlockSim and MiniTab aided calculations were performed. The software includes and advanced solutions in the range of the reliability and availability simulations. The test results indicate that the proposed solution has a wide applicability potential.

Opis:
Hydrogen compressors perform specific functions in the technical system of crude oil processing. The authors characterize consequences of wear margin loss of the compressor, present design solutions of hydrogen piston compressor and divide its construction into functional units. The compressor fault analysis is based on post repair documentation of compressors maintained according to their service life, and making use of technical diagnostics applied to 21 compressors, covering the last 20 years. The analysis distinguishes the loss of compressor wear margin due to loss of functional unit wear margin and due to damage to a compressor component. Faults typical of hydrogen compressor elements have been described. Besides, the authors estimate fault risks for selected elements and risks of wear margin loss of selected functional units. Statistical data are given in tables and bar charts. The analysis outcome indicates the need to implement methods and tools for diagnosing the cylinder unit, comprising several functional units.
Opisano zadania sprężarek wodoru w systemie technicznym przetwarzania ropy naftowej. Scharakteryzowano następstwa utraty potencjału eksploatacyjnego sprężarki. Przedstawiono rozwiązania konstrukcyjne tłokowych sprężarek wodoru użytkowanej w rafinerii. Analizę uszkodzeń sprężarek przeprowadzono w oparciu o badania dokumentacji poremontowej, obsługiwanych według resursu oraz z wykorzystaniem metod diagnostyki, 21 sprężarek z ostatnich 20 lat. W analizie uwzględniono fakt, że w przypadku sprężarek obsługiwanych według resursu podczas remontu po upływie z góry ustalonego czasu użytkowania wymieniane są elementy sprężarki, których potencjał eksploatacyjny nie został jeszcze wyczerpany. Stwierdzono około 500 przypadków utraty potencjału eksploatacyjnego analizowanych sprężarek. W analizie rozróżniono utratę potencjału eksploatacyjnego sprężarki z powodu utraty potencjału eksploatacyjnego danego zespołu funkcjonalnego sprężarki i z powodu uszkodzenia elementu sprężarki. W artykule zamieszczono opisy wybranych uszkodzeń wybranych elementów sprężarki. Oszacowano ryzyko uszkodzenia wybranych elementów i ryzyko utraty potencjału eksploatacyjnego wybranych zespołów funkcjonalnych. Dane statystyczne zestawiono w formie tabel i wykresów słupkowych. Stwierdzono, że: elementom można przypisać charakterystyczne dla sprężarek uszkodzenia; liczba przypadków utraty potencjału sprężarki zależy od składu sprężanego gazu; największy udział procentowy w uszkodzeniach elementów mają pierścienie uszczelniające zaworów oraz sprężyny dociskowe zaworów; największe ryzyko jest związane z uszkodzeniami pierścieni tłokowych jako elementów oraz łożysk (w tym wodzika) jako zespołów funkcjonalnych, z uwagi na poważne w skutkach uszkodzenia wtórne; największy udział procentowy mają zawory jako zespoły funkcjonalne, z największym ryzykiem związana jest utrata potencjału zespołu cylindra: tłok, pierścienie, tuleja cylindrowa, uszczelnienie gazowe, zawory robocze.

The introduction to the Fault Tree Analysis has been presented in this article. Selected publications on the Fault Tree Analysis have been listed. The usefulness of the method in marine engineering application has been indicated. Reliability testing facility has been presented, that is typical two-stroke, slow-speed ship crosshead Diesel engine intended to power the ship. The way of creating system model with the Fault Tree Analysis application has been presented. Reliability structure of tested engine has been modelled using Reliability Block Diagrams, as well as the Fault Tree Analysis. Main tree describing the object reliability structure analysis and the sub-tree structure modelling crank system, subsystem of the cam shaft, cylinder exemplary system and turbocharger have been built. Qualitative analysis of the tree has been conducted – the minimal cut sets of the system has been determined. The directions for further research have been indicated.

According to Polish nuclear law, newly emerging nuclear facilities require probabilistic safety assessment (PSA). This article is intended to present the PSA method and to present the error tree method by which the probability of unavailability of the gravity reactor cooling system (GDCS) of the ESBWR power plant designed by GE Hitachi was determined. This work includes creatiion process of a damage tree and performing a quantitative analysis in SAPHIRE tool and estimating uncertainty using the Monte Carlo method. As a part of the work, it was shown that in the probability of failure of a single GDCS P LINE-A line, the most important element are the basic events related in particular to the operation of service valves.

Merchant shipping has undergone a great transformation over the past three decades. The shipping market is highly competitive, which coupled with high crewing and fuel costs, leads to high operational costs. One of the paramount factor involved in vessel operation is the Maintenance cost and there is a dire need to keep this cost to a minimum. Fortunately the earlier policy of repair only maintenance in commercial shipping has been done away with, and was replaced by the policy of preventive maintenance. Planned Maintenance System was introduced by ship management companies in the early 90’s. Planned Maintenance offered benefits over the repair only policy, but has its own demerits. Many a time machinery equipment is opened up for routine maintenance after a specified time interval, irrespective of the need. This could lead to potential failures, which is explained by the fact that preventive maintenance resulted in meddling of a well set piece of machinery equipment, leading to its subsequent failure. This is where Condition based maintenance or CBM steps into prominence. CBM monitors the health of the machinery equipment, analyses the condition and helps you in decision making. The Main Propulsion system forms the heart of a vessel and we need to ensure its reliability, together with the reliability of its associated sub-systems. The entire system can be represented by reliability block diagrams, to show the interdependence of various components comprising the system. This helps in the decision making process of CBM whereby ship’s engineer may decide to stop the running machinery equipment, open and overhaul the same, else postpone the overhaul for a later safe date.
