Introduction and aim. In this study, we evaluated parameters that might be associated with hospitalization in patients admitted to the emergency department (ED) with an acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Material and methods. Patients with COPD who presented to ED due to AECOPD between January 1, 2020 and December 31, 2021 were included in the study. Patient data were obtained from the hospital database. Univariable and multivariable logistic regression methods were used to identify the relationship between hospitalization and clinical parameters. Results. The study included 237 patients divided into two groups: inpatients (n=124) and outpatients (n=113). We found significant differences between the two groups in terms of temperature, oxygen saturation, respiratory rate, C-reactive protein, white blood cell count, procalcitonin, albumin, arterial blood pH, pCO2, and non-invasive mechanical ventilation (NIMV) requirement. Multivariable logistic regression analysis showed that body temperature [odds ratio (OR):1.62;95% confidence interval (CI):1.21–4.91; p<0.001], oxygen saturation (OR:0.73, 95% CI:0.39-0.94, p<0.001), respiratory rate (OR:1.96; 95% CI: 1.07–6.14; p<0.001), albumin (OR:0.71; 95% CI:0.41–0.93; p=0.042), procalcitonin (OR:2.93; 95% CI:1.22–4.84; p<0.001), arterial blood pH (OR:0.78; 95% CI:0.29-0.91; p=0.038), pCO2 (OR:2.45; 95% CI:1.24–4.65; p<0.001), and NIMV requirement (OR:2.31; 95% CI:1.41–5.13; p<0.001) were the independent predictors of hospitalization. Conclusion. Our findings may help identify patients who will require hospitalization at an early stage.

INTRODUCTION: Chronic Obstructive Pulmonary Disease (COPD) is a progressive disease that develops due to inflammation in the airways. The aim of this study is to demonstrate the effectiveness of c-reactive protein/albumin ratio (CAR) as a 30-day mortality indicator in COPD patients admitted to ICU. MATERIAL AND METHODS: A total of 235 COPD patients with available data between January 2018 and December 2018 were included in this retrospective cohort study. Demographics, APACHE II, Charlson comorbidity index (CCI), SOFA score, CAR and outcomes were evaluated. RESULTS: 87 (37%) of the cases were female and 148 (63%) were male. Their ages ranged from 26 to 95 years, with an average of 70.9± 11.4. The non-survivors had significantly higher APACHE II, CCI, SOFA score, procalcitonin, creatinine, mechanical ventilation (MV) time, WBC, CRP and CAR compared to the survivors (p˂0.05). Albumin and prealbumin were significantly lower (p˂0.05) in non-survivors. In the univariate model; age, sepsis, inotropic support, APACHE II score, CCI, SOFA, procalcitonin, creatinine, MV time, WBC, CRP, albumin, prealbumin and CAR were observed to be significantly effective (p˂0.05) in predicting 30-day mortality. In the multivariate reduced model; inotropic support, SOFA, WBC and prealbumin value exhibited significant independent (p<0.05) effectiveness in predicting 30-day mortality. Albumin, CRP, CAR, APACHE II, SOFA and CCI value were observed to be significant in predicting mortality (p=0.000). CONCLUSIONS: In the study, the predictive power of APACHE II score, CCI, SOFA score, albumin and prealbumin values alone was found to be significantly higher than that of the CAR.

Background: Measurement of fractional exhaled nitric oxide (FeNO) is a useful technique for detection of eosinophilic airway inflammation and assessment of efficiency of corticosteroid treatment in patents with respiratory disease. Generally studies agree that measurement of FeNO is a useful non-invasive biomarker in patients with chronic obstructive pulmonary disease (COPD), however, there are reports that do not confirm such a relationship between FeNO and COPD. Aim of the study: The main objective of this study was to investigate FeNO levels in Polish patients with COPD compared to healthy controls. As a secondary objective, we assessed the influence of smoking on FeNO levels in healthy patients, and patients with COPD. Material and methods: FeNO concentration was measured using an electrochemical analyzer in healthy nonsmokers (n=21), healthy smokers (n=25), and former smokers with COPD (n=30) and smokers with COPD (n=38). General characteristics, hematological variables and serum biochemical parameters were also obtained and analyzed using the Kruskal-Wallis test. Results: FeNO measurement revealed significantly reduced NO levels in healthy smokers compared to healthy non-smokers, former smokers with COPD and smokers with COPD (median [range]: 14 [6–17] vs. 21 [15–29], 25 [15–53], and 19 [11–32] ppb, respectively, p<0.001). Moreover, we found increased FeNO levels in ex-smokers with COPD compared with smokers with COPD (p<0.05). No associations between FeNO and other analyzed parameters were found. Conclusions: Levels of FeNO, measured by with an electrochemical analyzer, were elevated among patients with COPD compared to healthy non-smoking controls. Moreover, our study confirms that smoking results in a reduction in FeNO concentration in both healthy patients and patients with COPD.

Background. Osteoporosis is a significant problem in patients with advanced Chronic obstructive pulmonary disease. The etiology for the bone loss includes smoking, vitamin D deficiency,concomitant endocrine diseases, sedentary lifestyle, use of glucocorticoids. Osteoporosis is the pathology of calcium metabolism, thus the peculiarities of calcium homeostasis in Chronic obstructive pulmonary disease patients, depending on their bone mineral density were researched in the article. Material and methods. It was examined 86 patients with stage 3 Chronic obstructive pulmonary disease. Bone densitometry was performed on lumbar vertebrae using dual energy X-ray absorption scanner “Lunar”. Results. It was not find a significant difference in the duration of disease among patients suffering from 3rd stage CO PD with or without bone mass deficiency. All females with low bone mineral density had concomitant endocrine pathologies. It was established the increase of calcium content in sputum of patients with CO PD. Osteoporosis in patients with chronic obstructive pulmonary disease develops under influence of numerous factors, among which are: lack of physical activity, increase in calcium excretion with urine and sputum, low amount of calcium in diet, reduced exposure to sunlight, concomitant endocrine diseases in females and exposure to stressful situations, smoking, vitamin D deficiency. Conclusions. Osteoporosis in patients with chronic obstructive pulmonary disease develops under influence of numerous factors, among which are: lack of physical activity, increase in calcium excretion with urine and sputum, low amount of calcium in diet, reduced exposure to sunlight, concomitant endocrine diseases in females and exposure to stressful situations.
Wprowadzenie. Osteoporoza stanowi istotny problem wśród pacjentów z zaawansowaną postacią przewlekłej obturacyjnej choroby płuc. Do głównych przyczyn utraty masy kostnej należą: palenie wyrobów tytoniowych, niedobór witaminy D, współistniejące zaburzenia hormonalne, siedzący tryb życia oraz przyjmowanie glikokortykosteroidów. Osteoporoza to zaburzenie metabolizmu wapnia, dlatego też w tym artykule podjęto próbę zbadania cech szczególnych homeostazy wapnia u pacjentów leczonych na przewlekłą obturacyjną chorobę płuc w zależności od ich gęstości mineralnej kości. Materiał i metody. Badaniem objęto 86 pacjentów w III stadium przewlekłej obturacyjnej choroby płuc. Pomiar gęstości kości przeprowadzono w odcinku lędźwiowym kręgosłupa za pomocą aparatu Lunar. Wyniki. Nie zauważono znacznej różnicy czasu trwania choroby pomiędzy pacjentami w III stadium przewlekłej obturacyjnej choroby płuc, u których stwierdzono niedobór masy kostnej a tymi, u których owego niedoboru nie stwierdzono. U wszystkich kobiet z niską gęstością mineralną kości stwierdzono współistniejące zaburzenia hormonalne. Stwierdzono również wzrost zawartości wapnia w plwocinie pacjentów cierpiących na przewlekłą obturacyjną chorobę płuc. Na rozwój osteoporozy u tychże pacjentów wpływ mają liczne czynniki, m.in. brak aktywności fizycznej, wzrost wydalania wapnia z moczem i plwociną, mała zawartość wapnia w diecie, zmniejszona ekspozycja na światło słoneczne, współistniejące zaburzenia hormonalne u kobiet oraz ekspozycja na czynniki stresogenne, palenie wyrobów tytoniowych oraz niedobór witaminy D. Wnioski. Na rozwój osteoporozy u pacjentów z zaawansowaną postacią przewlekłej obturacyjnej choroby płuc wpływ mają liczne czynniki, m.in. brak aktywności fizycznej, wzrost wydalania wapnia z moczem i plwociną, mała zawartość wapnia w diecie, zmniejszona ekspozycja na światło słoneczne, współistniejące zaburzenia hormonalne u kobiet oraz ekspozycja na czynniki stresogenne.

Background. Cigarette smoking is a major public health problem, which leads to the formation of chronic obstructive pulmonary disease (COPD) and is one of the main causes of avoidable death and disability worldwide. Objective. The aim of study was analysis and comparison of the visceral fat in the body of the three groups of subjects (non-smokers, smokers and COPD patients) by Tanita Viscan 140. Material and Methods. The control group was composed of: (1) non-smokers (n=30), consisted of 13 males (43.4%) and 17 women (56.6%) - the average age was 52 ± 6.51 years and (2) smokers (n=30), consisted of 12 men (40%) and 18 women (60%) – the average age 46.53 ± 9.22 years. Study group consisted of patients with COPD (n=60), which consisted of 48 men (80%) and 12 women (20%). Mean age was 69.25 ± 9.90 years. The measurement of visceral fat by Tanita Viscan device 140, which uses bioelectrical impedance analysis to measure fat in the abdomen of the patient in the supine position. Results. High levels of visceral fat (women from 36.9% to 52.3% and more, men from 27.1% to 40.3% or more) were observed in 19 patients (3 women and 16 men), with 19 smokers (10 women and 9 men) and non-smokers in 22 subjects (10 women and 12 men). The average value of waist circumference measured with a Tanita Viscan 140 was in the group of patients 96.38 ± 12.27 cm, in the group of smokers 95.23 ± 10.12 cm and in group of non-smokers 96.86 ± 10.88 cm. Conclusions. The results of our work are of great importance for the health assessment not only among patients with COPD but also in the group of smokers. Therefore it would be appropriate to remind the general public, eg. by campaign for chronic obstructive pulmonary disease and its serious complications and reduce the life quality of these patients and thus help protect human health and in particular young people from the harmful effects of tobacco products.

Palenie papierosów jest poważnym problemem zdrowotnym, który prowadzi do powstawania przewlekłej obturacyjnej choroby płuc (POChP) i jest jednym z głównych przyczyn śmierci i niepełnosprawności na całym świecie. Cel badań. Dokonano pomiaru zawartości w organizmie tłuszczu trzewnego za pomocą urządzenia Tanita Viscan 140, który wykorzystuje metodę bioelektrycznej impedancji do pomiaru tkanki tłuszczowej w jamie brzusznej pacjenta w pozycji leżącej. Materiał i metody. Grupa kontrolna składała się z osób niepalących (n = 30), w tym z 13 mężczyzn (43,4%) i 17 kobiet (56,6%) - średni wiek wynosił 52 ± 6,51 roku. Grupa palaczy papierosów (n = 30) składała się z 12 mężczyzn (40%) i 18 kobiet (60%) - średni wiek 46,53 ± 9,22 roku. Badaniem objęto również grupę pacjentów z POChP (n = 60), która składała się z 48 mężczyzn (80%) i 12 kobiet (20%). Średni wiek wynosił 69,25 ± 9,90 roku. Dokonano pomiaru w organizmie zawartości tłuszczu trzewnego za pomocą urządzenia Tanita Viscan 140, który wykorzystuje metodę bioelektrycznej impedancji do pomiaru tkanki tłuszczowej w jamie brzusznej pacjenta w pozycji leżącej. Wyniki. Wykazano wysoki poziom tłuszczu trzewnego (w zakresie od 36,9% do 52,3% u kobiet i w zakresie 27,1% do 40,3% u mężczyzn) u 19 chorych (3 kobiety i 16 mężczyzn) na przewlekłą obturacyjną chorobę płuc zaobserwowano, u 19 palaczy (10 kobiet i 9 mężczyzn ) i w grupie 22 niepalących osób (10 kobiet i 12 mężczyzn). Średnia wartość obwodu talii mierzona za pomocą urządzenia Tanita Viscan 140 wynosiła: w grupie pacjentów z POChP 96,38 ± 12,27 cm, w grupie palaczy 95,23 ± 10,12 cm, a w grupie niepalących 96.86 ± 10,88 cm. Wnioski. Wyniki pracy mają istotne znaczenie dla oceny stanu zdrowia nie tylko wśród pacjentów z POChP, ale także w grupie osób palących. Wskazana byłaby edukacja społeczeństwa poprzez organizowanie kampanii na temat poważnych powikłań zdrowotnych i obniżeniu jakości życia pacjentów z POChP, aby w ten sposób chronić zdrowie ludzi, szczególnie młodych, przed szkodliwym działaniem palenia produktów tytoniowych.

Introduction and objective. COPD is a medical state characterized by chronically poor airflow, and typically worsens over time. Farmers have an increased risk of COPD because of being exposed to ammonia, hydrogen sulfide, inorganic dust, and organic dust. The quality of life of the ill depends on biomedical as well as psychosocial factors, the impact of which has not been a frequent subject of studies among COPD patients. The aim of the study was to indicate the factors that have negative and positive influence on the quality of life of farmers suffering from COPD. Materials and method. The study was conducted among 84 farmers treated for COPD in the Department of Pneumology, Oncology and Allergology of the Medical University in Lublin, Poland. The differences between the farmers concerned: severity of the disease, level of education and income, frequency of smoking tobacco and drinking alcohol, kinds of support they receive from their families, and the level of depression and anxiety experienced by the patients. Results. The study revealed that most patients suffered from depressive and anxiety disorders, and the level of depression higher among the patients who smoked more. Lack of family support had significant influence on exacerbation of the patient’s depressive and anxiety symptoms. Patients who had recently experienced a critical situation presented with more severe COPD symptoms, lower quality of life and a higher level of depression. Higher income of the patients had positive influence on their quality of life. Farmers addicted to alcohol suffered from a higher level of anxiety.

Background To improve the effectiveness of lung cancer screening using low-dose computed tomography (LDCT), the presence of smoking-related comorbidities that may significantly affect mortality in this group should be taken into account. Material and methods A questionnaire survey and spirometry tests were conducted in a group of 730 respondents as part of a lung cancer screening study between 2016 and 2018. People diagnosed with COPD underwent a three-year follow-up to assess the incidence of medical events. Results Our study confirmed that cardiovascular diseases (CVDs) were the most common comorbidities in patients who were diagnosed with COPD and participated in LDCT lung cancer screening. Among the CVDs, the most common were arterial hypertension (45.8%) and coronary artery disease (12.5%). Tobacco-related diseases (e.g. CVD, lung cancer, and exacerbations of COPD) were the leading causes of emergency department visits and hospitalizations. The number of visits due to COPD in specialized clinics more than doubled in the observed period. Conclusions Properly planned screening tests allow not only for the detection of the disease for which they were designed but also for the assessment of comorbidities. In patients undergoing lung cancer screening, it is justified to extend the diagnostics to include spirometry.

It has all started 10 years ago, at my first WON CA Europe conference in Istanbul in 2008, where I became amazed by so many general practitioners, who performed and presented their scientific work there. However it took me 7 years since the idea to start my PhD studies until it´s final completion in 2017. My PhD journey wasn´t straight, but rather twisty, with 2 interruptions. In 2011 I started to earn my basic research skills at 2 courses. The first one was the European General Practice Research Network (EGPRN) course in Nice and the second University of Crete´s research workshop in Slovakia lead by Professor Christos Lionis. The easiest part of my PhD studies was the clinical one – administering questionnaire and performing spirometry with my patients at my rural general practice. I also enjoyed teaching medical students at undergraduate as well as postgraduate level. I gave lectures at national conferences and published articles about general medicine in Slovak scientific journals, focusing on prevention, patient safety and respiratory diseases, especially COPD. I also contributed to two medical text books. My research was presented as posters or oral presentations at 3 WONCA Europe conferences, where I found a great space for sharing research ideas and results. Final results of my PhD thesis are going to be presented at Krakow conference. Even though I was a fruitful author of publications, reaching the goal of an international publication was the most difficult part for me, not achievable without a help of experienced colleague, Austrian general practitioner, Professor Gustav Kamenski.

Objectives: Chronic obstructive pulmonary disease (COPD) may be work-related. It has been estimated that 15% of the population burden of COPD is attributable to occupational exposure. However, in Poland COPD is rarely recognized as an occupational disease. The aim of the study has been to analyze the causes of the low prevalence of work-related COPD in the context of the existing criteria as well as to analyze which part of the assessment – clinical or hygienic one – is responsible for such a low rate of occupational COPD recognitions. Material and Methods: The study group included 150 patients hospitalized with a suspicion of occupational COPD. Each patient underwent a clinical examination, spirometry and reversibility test using bronchodilator. Moreover, hygienic evaluation of work conditions was performed in all the considered cases. Results: In the case of the patients who fulfilled the criteria for COPD diagnosis in accordance with the Global Initiative for Chronic Obstructive Lung Disease (GOLD) occupational origins of the disease, the disease was not recognized because 24.1% of the individuals did not meet spirometric criteria included in a definition of COPD in the Polish list of occupational diseases, while 27.8% of the individuals did not fulfill the criterion of a documented exposure to dusts and irritant gases. None of these criteria was fulfilled by 42.6% of the patients. Conclusions: In our country, both clinical and hygienic criteria result in limitations in recognition of occupational COPD. There is the need to establish new guidelines for the recognition of COPD as a compensable disease in Poland. Int J Occup Med Environ Health 2018;31(2):139–150

Opis:
Purpose: This study aims to determine the correlation between death anxiety and anxiety in elderly patients with chronic obstructive pulmonary disease. Materials and methods: This study has been conducted on the individuals with chronic obstructive pulmonary disease (COPD) who were over 65 years. They received outpatient and inpatient treatment at a hospital. 171 elderly persons that were determined via power analysis were included in the research. Data was collected using a survey form, Death Anxiety Scale, State Anxiety Scale, and Trait anxiety scale. We used the following test statistics: t-test, ANOVA and Kruskal-Wallis Variance Analysis, and Pearson Correlation Test analysis. Results: 66.1 percent (113) of the participants are male and 33.9%(58) female; mean age is 726.83. The mean death anxiety score of the participants is 9.044.02. The mean State Anxiety score of elderly individuals with chronic obstructive pulmonary disease is 44.81.29; the trait anxiety mean score is 48.851.13. In those chronic obstructive pulmonary disease patients, we found a significant (p<0.001) correlation between state and trait anxiety, and that the latter two are high in cases where death anxiety is high. Conclusion: We found that death anxiety and anxiety are high in patients with COPD.

Background. Chronic obstructive pulmonary disease (COPD) is one of the leading causes of morbidity and mortality in modern society and can lead to the development of comorbidities. Among the last, gastroesophageal reflux disease (GERD) is frequently present, but often gets a close attention from doctors in the treatment of pulmonary patients. The aim of the current study was to determine the characteristics of clinical and lung disorders in the pathogenesis of COPD with concomitant GERD. Material and methods. We examined 113 COPD patients with isolated COPD or with COPD and concomitant GERD. All the patients underwent spirography, endoscopy, radiological and pH-metric procedures. Results. Many patients (95%) with concomitant pathology complained of heartburn, dysphagia, especially after meals, burning tongue, hoarseness and a lump in the throat. Among COPD patients without concomitant GERD, both clinical pulmonary manifestations met with almost the same frequency but were less pronounced (p>0.05). During the lung examination, we determined the level of FEV1, VC, FVC, FEF25-75 and FEV1/FVC, which differed significantly in COPD patients (p<0.05) compared to predicted normal values in human of the same sex, age, height and body weight. In the COPD with concomitant GERD cohort, a sharp decrease in spirography indices was found compared to patients with isolated COPD (p<0.001). Conclusions. COPD patients with concomitant GERD had significantly greater extraesophageal manifestations and lung disorders compared with COPD patients without comorbidity.
Wprowadzenie. Przewlekła obturacyjna choroba płuc (POChP) jest jedną z głównych przyczyn umieralności i zachorowań we współczesnym społeczeństwie, co w konsekwencji prowadzi do rozwoju chorób współistniejących. Choroba refluksowa przełyku (GERD) występuje często, ale zazwyczaj jest dokładnie badana u pacjentów z chorobami płuc. Celem badania jest określenie zaburzeń klinicznych i chorób płuc w patogenezie POChP z towarzyszącym GERD. Materiał i metody. Przebadanych zostało 113 pacjentów z POChP z izolowaną POChP oraz z POChP ze współistniejącym GERD. Wszystkich poddano zabiegom, takim jak: spirografia, endoskopia, a także działaniom radiologicznym i pH-metrycznym. Wyniki. Większość pacjentów ( 95%) ze współistniejącą patologią skarżyło się na zgagę, dysfagię, zwłaszcza po spożyciu posiłków, pieczenie języka, chrypkę oraz występowanie guzka w gardle. W tym czasie wśród pacjentów z POChP bez współistniejącej GERD oba objawy kliniczne w płucach spotkały się z prawie taką samą częstością, ale były mniej wyraźne (p>0,05). Podczas badania płuc ustalono poziomy FEV1, VC, FVC, FEF25-75, FEV1/FVC, które znacznie różniły się u pacjentów z POChP (p<0,05) w porównaniu do normalnych wartości u osób tej samej płci, wieku, wzrostu i masy ciała. W grupie pacjentów z POChP z towarzyszącym GERD stwierdzono gwałtowny spadek wskaźników spirograficznych w porównaniu z pacjentami z izolowaną POChP (p<0,001). Wnioski. Pacjenci z POChP ze współistniejącym GERD mieli znacznie większe objawy pozaprzełykowe i choroby płuc w porównaniu z pacjentami z POChP, ale bez chorób współistniejących.

Opis:
Purpose: Respiratory diseases affect the lungs and other parts of the respiratory system. The respiratory disease affects hundreds of millions of humans, and premature death is observed in nearly four million people yearly. The major cause of the increase in this disease is the increased level of air pollution and higher tobacco usage in public places. Design/methodology/approach: We have used the search engines PubMed and Google Scholar for the keywords Respiratory diseases, Nanomaterials, diagnosis, Nanomedicine, and Target drug delivery; recent and relevant articles are selected for reviewing this paper. Findings: Nanomedicine is a recent field of research that deals with monitoring, repairing, theragnosis, and development of human biological systems at the sub-atomic level, where we utilize engineered nanodevices and nanostructures. The conventional therapeutic strategies designed for respiratory diseases have limited solubility and bioavailability. Moreover, the robust effect of the drugs led to adverse side effects due to their high dose requirement. The local delivery of therapeutic Nanoparticles (NPs) or drug-loaded nano vehicles to the lung is a safe technique for managing various respiratory tract-related diseases like chronic obstructive pulmonary diseases, cystic fibrosis, lung cancer, tuberculosis, asthma, and infection. To overcome the difficulties of conventional treatment with antibiotics and anti-inflammatory drugs, nano-enabled drug delivery, nanoformulations of drugs as well as drug nanoencapsulation have been used recently. In this mini-review, we will discuss the importance and application of nanomedicine for diagnosis, treatment and clinical research involved in the different types of respiratory diseases. Practical implications: Nanomedicine provides an alternative delivery of drugs with the help of various nanocarriers, which enhances controlled drug delivery at the pulmonary region and can be used for treating and diagnosing respiratory diseases in vivo and in vitro studies. Further experiments followed by clinical examination are warranted to prove the potential application of nanomedicine in treating respiratory disease. Originality/value: This mini-review will help the readers and budding scientists apply new methods for developing highly efficient drugs with low side effects and improved targeted sites of action.
