Research was conducted to determine the prevalence and severity of chronic venous disorders (CVD) among people working in prolonged sitting or static standing postures. Clinical examination and duplex Doppler sonography were performed on 126 employees working in a sitting (96 individuals) or a standing posture (30 individuals). Evidence of CVD was found in 59.4% of individuals working in a sitting posture and in 83.4% of those working in a standing posture, and was significantly higher in employees working in a standing posture (p = .015). Incompetent perforating veins and vena saphena magna valves, and bilateral changes were the more frequent signs of CVD. The investigation showed that prolonged standing and sitting at work increases risk of developing CVD. Further, people working in a standing posture are at a significantly greater risk for CVD than those working in a prolonged sitting posture. They should thus be the subject of specific prophylaxis interventions.

CRS symptoms in children can be mild and uncharacteristic. The disease is multifactorial, and therefore requires a multifaceted, multidisciplinary approach. Treatment of CRS in children is difficult, infections often recur, and treatment methods are often ineffective. In children, surgery is performed infrequently, only when other therapies have failed. The first stage of surgery is adenoidectomy. Functional endoscopic surgery of the paranasal sinuses may be considered if no improvement of the patient after exhausting all possibilities of conservative treatment.

Abdominal pain is one of the main symptoms of pancreatic diseases, including chronic pancreatitis (CP). Once the disease is in remission, pain management is a key element of treatment. Co-analgesics may also be used at any stage of analgesia, among which antiepileptic drugs such as pregabalin. The first data on the beneficial effect of pregabalin on pain in the course of CP was published in a clinical trial in 2011, and it has been known since 2015 that the analgesic effect of pregabalin on CP has a different mechanism of action than conventional analgesics. Currently, research is focused on combining pregabalin with antioxidants and on methods to predict the patient's response to pregabalin administration. Based on current research, pregabalin appears to be a very useful agent for the pharmacologic treatment of pain in the setting of CP.

The aim of the study was the assessment of the peculiarities of iron metabolism in pregnant women suffering from chronic pyelonephritis. There was shown that pregnant females have signs of iron deficiency anemia which is more severe in women suffering from chronic pyelonephritis. The iIncidence of chronic pyelonephritis in women with signs of syderopenic syndrome is 35.0 %. There is discussed the pathogenesis of iron deficiency related to the renal dysfunction.

Background. Chronic autoimmune inflammation in arterial wall may lead to atherosclerosis progression. Objective. The aim of this study was to investigate the association between Treg, Th17 and B1a cell blood frequencies as well as IgM autoantibodies to oxLDL and the abundance of carotid atherosclerosis. Material and methods. 18 patients with increased IMT (intima-media thickness) and 65 patients with different severity of carotid atherosclerotic plaques were included. Treg, Th17 and B1a cell blood frequencies were assessed via direct immunofluorescence staining and flow cytometry, oxLDL as well as IgM autoantibodies to oxLDL were measured with commercial kits. Results. We observed higher values of Treg in patients without carotid atherosclerosis. Patients with intact carotid arteries as compared to patients with mild atherosclerotic plaques had decreased Th1 levels. OxLDL IgM levels were higher in patients with intact carotid arteries. Patients who received statin treatment had higher levels of Treg. Immunophenotyping of B cells revealed two cases of monoclonal B-cell lymphocytosis and 1 case of B-CLL (B-cell chronic lymphocytic leukemia) in elderly patients with intact carotid arteries. Conclusion. We hypothesize that certain parameters of cell immunity may hamper atherosclerosis while protecting from lymphoproliferative disorders.

Introduction. The problem concerning the impact of pregnancy on diabetic complications is a matter for discussion as there is some evidence suggesting that pregnancy may trigger development or progression of diabetic chronic complications. However, currently available data concerning this issue is still controversial. Objective. The aim of the study was to evaluate the impact of obstetric history on the development of chronic microangiopatic and macroangiopatic complications in type 1 diabetic women. Materials and Method. The retrospective study comprised 226 white Caucasian type 1 diabetic women, including 190 parous and 36 nulliparous women. Anthropometric data, information concerning the course of the disease, including metabolic control and chronic complications, together with obstetric history, were registered. Results. Parous women were older (p<0.001), but did not differ significantly regarding metabolic control in the course of the disease (p>0.05) and diabetes duration (p>0.05) from nulliparous subjects. There were no significant differences in the incidence (p>0.05) nor onset (p>0.05) of chronic diabetes complications between the groups. The number of deliveries did not correlate with either the incidence nor the onset of chronic complications. Longer DM duration at the moment of first delivery was related to the higher incidence of retinopathy (p<0.01), nephropathy (p<0.05) and neuropathy (p<0.001). Conclusions. The incidence of chronic diabetic complications does not differ between parous women and the subjects that were never pregnant, and is not related to the number of pregnancies.

The aim of the study was to determine dimensions of temperament in adults with chronic stuttering in the light of Pavlov’s temperament typology. The test sample consists of a (1) clinical group—adults with chronic stuttering (N = 202); and (2) a criterion group—adults with normal speech fluency (N = 198). The Pavlovian Temperament Survey (PTS) by Strelau and Zawadzki and our own survey were administered. Stutterers, compared to subjects with normal speech fluency, are characterized by a lower level of the excitation process, mobility and balance of nervous processes; they also tend to exhibit a higher level of the inhibition process. The severity of stuttering, the age at which the first symptoms of speech disfluency occur, treatment, evaluation of its effectiveness and intention to undergo therapy do not differentiate stutterers in terms of the considered temperament traits. The temperamental factors of chronic stuttering as per Pavlov’s typology are: increased reactivity to stimuli, reduced ability to adapt to new situations, the ability to flexibly respond to changing conditions, a weak type of the nervous system (lower strength of the excitation process, weaker mobility of the nervous processes), which makes it difficult to cope with challenges encountered and increases susceptibility to mental disorders.

Background: Many patients with chronic pancreatitis are elected for surgery when endoscopic interventions are ineffective. Duodenum preserving pancreatic head resection introduced by Charles F. Frey is the most common procedure used for surgical treatment of chronic pancreatitis. However, technical aspects of this procedure have not been studied extensively. Goal Our prospective randomized study is aimed to compare usage of single-layer continuous (I group) and two-layer interrupted sutures (II group) in constructing pancreatojejunostomy after Frey procedure. Methods and materials: In a period between 2009 and 2016, a total of 103 patients, diagnosed with chronic pancreatitis and determined medical indications for surgical treatment were included into the study and randomized into group I (52 patients) and group II (51 patients). Preoperative, intraoperative patient characteristics and postoperative results were compared between both groups. Results: Mean duration of surgery was statistically shorter in group I – 210 min, while in group II – 240 min (p = 0.004). Pancreatojejunoanastomosis construction time was shorter in group I – 19 (±6) min versus 51 (±18) min in group II, p <0,001. No statistically relevant differences were observed in postoperative morbidity: group I – 51.9% and group II – 45,1% (p = 0.177) and mortality: group I – 3.8% and group II – 2% (p = 0.636). Conclusions: Frey procedure using single-layer continuous pancreatojejunostomy is safe, fast and less complex method in surgical treatment of chronic pancreatitis.

Objectives To investigate 4 loci of 3 HSP70 genes in caustic soda production plant former workers, who have been exposed to metallic mercury vapors for a long time, and including numerous cases of chronic mercury intoxication (CMI). Material and Methods Polymorphisms in HSP70 gene family members (HSP1A1 (+190G/C, rs1043618), HSPA1B (+1267A/G and +2074G/C, rs1061581) and HSP1AL (+2437T/C, rs2227956)) genes were studied among 120 male workers involved in caustic soda production by mercury electrolysis at 2 plants in Eastern Siberia. These subjects had been chronically exposed to metallic mercury vapors for > 5 years and divided into 3 groups based on the occurrence and time of the CMI diagnosis, or absence of this disease. The Group 1 consisted of individuals (N = 46), who had had contact with mercury but were not diagnosed with the CMI. The Group 2 included workers (N = 56), who were diagnosed with the CMI longer than 14 years ago. The Group 3 consisted of the subjects (N = 18), who had been diagnosed with the CMI 3–5 years ago. The logistic regression analysis was used for 3 genetic models with and without adjustment for age and duration of mercury vapor exposure. Results We found that genotypes СС-HSPA1A (+190G/C) and GG-HSPA1B (+1267A/G) had a high predictive risk of the CMI development (adjusted odds ratio ($\text{OR}_\text{adj}$) = 5.58, p = 0.026 and $\text{OR}_\text{adj}$ = 14.7, p = 0.0015, respectively). Twelve individuals with the CMI had a specific combination of СС-HSPA1A (+190G/C) and GG-HSPA1B (+1267A/G) genotypes, which strongly associated with the diagnosis ($\text{OR}_\text{adj}$ = 12.3, p = 0.0285). Moreover, significant association with the CMI was also obtained for the haplotype G-C of 1267A/G and 190G/C polymorphisms (OR = 2.1, p = 0.018). Conclusions The association of СС-HSPA1A (+190G/C) and GG-HSPA1B (+1267A/G) genotypes and their combination for the CMI individuals suggests the role for HSPA1 genes in mercury-dependent mechanisms of the CMI development and progression. Objectives. Post exercise proteinuria (PEP) is found in about 20–40% of sportsmen after intensive exercise. Urinary NGAL is a new marker of tubulointerstitial kidney damage. The relationship between PEP and uNGAL has not been defined yet. In presented study a resting uNGAL as a predictor of PEP was analyzed. The changes of albuminuria after exercise were monitored to estimate a frequency and range of PEP. Methods. 40 amateur healthy runners (mean age 36.65 ±10.61 years) participating in 10-km run took part in the study. Before and after the competition urine was collected. NGAL, albumin and creatinine were subsequently measured in urine. uNGAL to creatinine ratio (NCR) and albumin to creatinine ratio (ACR) were calculated. Results. 28 participants (mean age 37.9 ±11.46, 19 M, 9 F) with uNGAL below 15 ng/ml before competition were analyzed. The increase of ACR was observed in every case. Mean post-exercise ACR was 104.55 ±123.1 mg/g and was significantly higher than pre-exercise ACR 6.33 ±5.86 mg/g (p < 0.0005). The positive correlation was found between resting NCR and post-exercise ACR (r = 0.60, p < 0.05). Conclusions. Resting uNGAL positively correlated with PEP. The possible explanation of these findings is that persons with PEP had some early, occult tubulointersitial kidney damage. It is speculated that those runners have higher risk of chronic kidney disease.

In this research work, a deterministic mathematical model for schistosomiasis transmission dynamics is presented. The model consists of five non-liniar ordinary differential equations incorporating the acute and chronic infectious compartments. The basic reproductive number, (the number of secondary infections when a single infectious individual is introduced into a population where everyone is susceptible) was obtained. Furthermore, we gained and analyzed for stability, the disease-free and endemic equilibrium. The qualitative feature of the model shows that the long-term behavior of the model is independent of initial conditions. Numerical simulation of the various state variables were obtained using matlab software.

Introduction. The renin-angiotensin-aldosterone system (RAAS) plays an important role in the pathogenesis of hypertension, cardiovascular diseases (CVDs) and chronic kidney disease (CKD). Drugs affecting the RAAS system, such as angiotensinconverting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs), are commonly used in the treatment of hypertension and heart failure. These drugs are also effective in reducing proteinuria and may, at least, delay end-stage renal disease in both diabetic and non-diabetic proteinuric CKD. However, novel drugs are needed to more effectively suppress the RAAS system and the progression of CKD. Aliskiren is the first direct renin inhibitor which has been approved for the treatment of hypertension. Additionally, a number of clinical studies have shown the antiproteinuric effect of aliskiren. Objective. This review focuses on the antihypertensive and antiproteinuric effects of aliskiren in patients with CKD. The pharmacology of aliskiren in these patients is also provided. Conclusions. Aliskiren-based hard endpoint large trials in non-diabetic nephropathy are needed in order to clarify the use of aliskiren in CKD patients.
