Objectives: The study was aimed at assessment of exposure to endotoxins, (1→3)-β-D-glucans and mite, cockroach, cat, dog allergens present in settled dust in premises of children as agents which may be significantly correlated with the occurrence of allergic symptoms and diseases in children. Materials and Methods: The study covered 50 homes of one- or two-year-old children in Poland. Samples of settled dust were taken from the floor and the child's bed. The levels of (1→3)-β-D-glucans (floor), endotoxins (floor) and allergens of mite, cat, dog and cockroach (floor and bed) were analyzed. Results: Average geometric concentrations (geometric standard deviation) of endotoxins, (1→3)-β-D-glucans, Der p1, Fel d1, Can f1 and Bla g1 in children homes were on the floor 42 166.0 EU/g (3.2), 20 478.4 ng/g (2.38), 93.9 ng/g (6.58), 119.8 ng/g (13.0), 288.9 ng/g (3.4), 0.72 U/g (4.4) and in their beds (only allergens) 597.8 ng/g (14.2), 54.1 ng/g (4.4), 158.6 ng/g (3.1) 0.6 U/g (2.9), respectively. When the floor was covered with the carpet, higher concentrations of endotoxins, (1→3)-β-D-glucans and allergens (each type) were found in the settled dust (p < 0.05). The trend was opposite in case of allergens (except dog) analyzed from bed dust and significantly higher concentrations were found in the rooms with smooth floor (p < 0.05). Conclusions: Among the analyzed factors only the type of floor significantly modified both the level of biological indicators and allergens. The results of this study could be the base for verifying a hypothesis that carpeting may have a protective role against high levels of cockroach, dog and cat allergens.

Parents actively share information about their children on Facebook, but little research has explored the extent of this issue. The goal of this paper is to theorize a new type of parents’ online activities concerning their children, especially the problem of sharenting, which is increasingly common in contexts where social media such as Facebook play a significant role in relationships and interactions. This paper explores what kind of baby pictures parents share on Facebook and what are the likely causes of doing it. The presented research was conducted with the use of social media ethnography among 168 Polish parents using Facebook. The findings have shown that the phenomenon of sharenting is common practice among parents.

Opis:
Indoor air quality (IAQ) in schools is an important public health concern. Particular attention should be paid to children, because they are more vulnerable to air pollution than adults. Although carbon dioxide is not considered a toxic air pollutant, its concentrations inside buildings may have a negative impact on users. The most commonly described health problems include tiredness and its effects (headaches, bad mood, poor attentiveness). Carbon dioxide concentrations are often used as a surrogate for the rate of outside supply air per occupant. Permissible concentration of carbon dioxide in confined spaces is 1,000 ppm. This minimum sanitary requirement is recommended by the European Office of the WHO and ASHRAE. The Polish Committee for Standardization has established general guidelines concerning the quality of air inside non-residential buildings. This standard was drawn up based on EU directives, through classification of IAQ, as it is influenced by the CO2 concentrations above the outdoor level (ΔCO2). Based on the above regulations, the increase in indoor CO2 concentrations in relation to CO2 concentrations in outdoor air (CO2) were measured along with physical parameters (temperature and relative humidity) to evaluate IAQ in the classrooms of two primary schools built in the nineteenth century in Poland. More than 100-year-old buildings built of bricks before World War II are a frequent occurrence in rural schools in the area of Upper Silesia, Poland. One of the school buildings underwent thermal effectiveness improvement while the other has not been modernized in these terms yet. The aim of the research was to check the extent to which thermal modernization of an old public building influences indoor air quality in the context of concentrations of carbon dioxide. The measurement period was chosen for late spring in order to avoid the impact of municipal emissions (so-called low emissions) common in rural areas. The average concentration of carbon dioxide in school rooms after thermal modernization (WTM) were higher than the concentrations observed in the school not subjected to thermal modernization (NTM). The CO2 concentrations ranged from 1,287±333 ppm to 2,146±386 ppm and from 936±196 ppm to 1,878±567 ppm in WTM and NTM school buildings, respectively. The results clearly indicate a higher occurrence of poor air quality (up to 70% of time during the school days) in the thermally improved school building compared to the one without thermal improvement (up to 52% of the time). The measurements also showed that the exposure of school children to elevated carbon dioxide concentrations during classes was higher in thermally improved primary school buildings, which in turn may represent a health concern.
Pomimo iż ditlenek węgla nie jest uważany za trujące zanieczyszczenie powietrza, poziomy jego stężeń wewnątrz budynków mogą skutkować negatywnym wpływem na użytkowników danych pomieszczeń. Wśród najczęściej opisywanych problemów zdrowotnych wymienia się uczucie zmęczenia i jego konsekwencje (bóle głowy, złe samopoczucie, kłopoty z koncentracją). W celu oceny potencjalnych skutków zdrowotnych dokonano pomiarów stężeń ditlenku węgla wraz z parametrami fizycznymi (temperaturą i wilgotnością względną) w pomieszczeniach dwóch szkół podstawowych zbudowanych w XIX wieku. Budynek jednej z nich został poddany termomodernizacji, w drugim jej nie przeprowadzono. Uzyskane wyniki wskazują na częstsze występowanie niskiej jakości powietrza w szkole poddanej termomodernizacji (do 70% czasu w ciągu dnia nauki) w porównaniu do szkoły bez termomodernizacji (do 52% czasu). Przeprowadzone badania wskazują, iż uczniowie budynku szkoły podstawowej poddanego termomodernizacji są częściej narażeni na podwyższone stężenia ditlenku węgla w czasie lekcji aniżeli uczniowie budynku szkoły niepoddanego termomodernizacji, co w konsekwencji może dla nich stanowić zagrożenie zdrowotne.

Bisphenol A (BPA) is used in the chemical industry as a monomer in the production of plastics. It belongs to a group of compounds that disturb some of the functions of human body, the endocrine system in particular. Extensive use of BPA in manufacturing products that come in contact with food increases the risk of exposure to this compound, mainly through the digestive tract. Literature data indicate that exposure to bisphenol A even at low doses may result in adverse health effects. The greatest exposure to BPA is estimated among infants, children and pregnant women. The aim of this review is to show potential sources of exposure to bisphenol A and the adverse health effects caused by exposure to this compound in the group of particular risk.

Lead is an environmental contaminant. The majority of epidemiological research on the health effects of lead has been focused on children, because they are more vulnerable to lead than adults. In children, an elevated blood lead (B-Pb) is associated with reduced Intelligence Quotient (IQ) score. This paper summarizes the current opinions on the assessment of the health risk connected with the children’s environmental exposure to lead. The B-Pb level of concern of 100 μg/l proposed by the US Centers of Disease Control in 1991 was for a long time accepted as the guideline value. In the meantime there has been a significant worldwide decrease of B-Pb levels in children and present geometric mean values in the European countries range from 20 to 30 μg/l. The recent analyses of the association of intelligence test scores and B-Pb levels have revealed that the steepest declines in IQ occur at blood levels < 100 μg/l and that no threshold below which lead does not cause neurodevelopmental toxicity can be defended. European Food Safety Authority (EFSA) concluded in 2010, on the basis of results of Benchmark Dose (BMD) analysis, that an increase in B-Pb of 12 μg/l (BMDL₀₁) could decrease the IQ score by one point. It seems that this value can be used as a “unit risk” to calculate the possible decrease of IQ and, consequently, influence of the low-level exposure to lead (< 100 μg/l) on the health and socioeconomic status of the exposed population.

The rising prevalence of overweight and obesity in children and teenagers is a major challenge for public health. Obesity is a complex and heterogeneous disorder, affected by many interacting genetic and non-genetic factors. The aim of this article was to focus on the environmental risk factors for childhood obesity. Among different factors contributing to an increase in BMI, we highlighted the role of exposure to cigarette smoke, DDT, bisphenol A, pesticides, and noise. The correlation between exposure to environmental toxins during prenatal period and obesity development in later life was underlined. According to obesogenic environment hypothesis, some features of distal and proximal neighbourhood also have a pivotal impact on children's behaviour and may contribute to increasing the risk for overweight. The area of residence (urban or rural) may affect access to sports facilities or other opportunities for physical activity. Therefore, for designing adequate prophylaxis, it is essential to take into account modifiable risk factors present in residential neighbourhood. Prevention of childhood obesity should integrate activities for both micro- and macro-environment surrounding the child.
Wzrastająca częstość występowania nadwagi lub otyłości u dzieci i nastolatków stanowi poważne wyzwanie dla zdrowia publicznego. Otyłość to złożone i heterogenne zaburzenie wynikające z wielu interakcji pomiędzy czynnikami genetycznymi i pozagenetycznymi. Celem artykułu było skupienie się na środowiskowych czynnikach ryzyka dziecięcej otyłości. Spośród różnorodnych czynników przyczyniających się do wzrostu BMI na- świetliliśmy rolę ekspozycji na dym tytoniowy, DDT, bisphenol A, pestycydy oraz hałas. Podkreślony został związek pomiędzy narażeniem na środowiskowe toksyny w okresie prenatalnym a rozwojem otyłości w późniejszym życiu. Zgodnie z hipotezą obesogennego środowiska niektóre cechy dalszego i bliższego sąsiedztwa mają bardzo ważny wpływ na zachowania dzieci i mogą przyczynić się do wzrostu ryzyka nadwagi. Miejsce zamieszkania (wieś lub miasto) może warunkować dostępność do obiektów sportowych czy innych możliwości wykonywania aktywności fizycznej. Dlatego też, niezwykle ważnym jest uwzględnienie przy projektowaniu odpowiedniej profilaktyki modyfikowalnych czynników ryzyka obecnych w lokalnym środowisku. Prewencja dziecięcej otyłości powinna integrować działania na rzecz mikro- i makrośrodowiska otaczającego dziecko.

Phthalates are a family of industrial chemicals that have been used for a variety of purposes. As the potential consequences of human exposure to phthalates have raised concerns in the general population, they have been studied in susceptible subjects such as pregnant women, infants and children. This article aims at evaluating the impact of exposure to phthalates on reproductive outcomes and children health by reviewing most recent published literature. Epidemiological studies focusing on exposure to phthalates and pregnancy outcome, genital development, semen quality, precocious puberty, thyroid function, respiratory symptoms and neurodevelopment in children for the last ten years were identified by a search of the PubMed, Medline, Ebsco, Agricola and Toxnet literature bases. The results from the presented studies suggest that there are strong and rather consistent indications that phthalates increase the risk of allergy and asthma and have an adverse impact on children's neurodevelopment reflected by quality of alertness among girls, decreased (less masculine) composite score in boys and attention deficit hyperactivity disorder. Results of few studies demonstrate negative associations between phthalate levels commonly experienced by the public and impaired sperm quality (concentration, morphology, motility). Phthalates negatively impact also on gestational age and head circumference; however, the results of the studies were not consistent. In all the reviewed studies, exposure to phthalates adversely affected the level of reproductive hormones (luteinizing hormone, free testosterone, sex hormone-binding globulin), anogenital distance and thyroid function. The urinary levels of phthalates were significantly higher in the pubertal gynecomastia group, in serum in girls with premature thelarche and in girls with precocious puberty. Epidemiological studies, in spite of their limitations, suggest that phthalates may affect reproductive outcome and children health. Considering the suggested health effects, more epidemiologic data is urgently needed and, in the meantime, precautionary policies must be implemented.

Objectives Industrially contaminated sites (ICS) represent an important public health issue in European countries. The Silesian Voivodeship, the region located in the southern part of Poland, the historical center of industry, has many sites that meet ICS criteria. Most of them are landfills containing lead, cadmium and zinc. Material and Methods The aim of the study has been to evaluate determinants of lead and cadmium blood levels for preschool children living in the vicinity of selected environmental hazards in Piekary Śląskie, the city located in the region of Silesia. Data about exposure was obtained from the parental questionnaire and was verified by levels of biomarkers in children’s blood. Finally, 678 preschool children were studied. Results Obtained results have confirmed that the statistically significant higher level of lead in blood has been revealed in the case of younger children whose fathers have a higher level of education and whose mothers smoke cigarettes at home, as well as for those living in the vicinity of any environmental hazard. In the case of cadmium, a higher level of the biomarker has been reported for older children who rather do not live close to any environmental hazard. We have recognized 30% of children with blood lead level (PbB) exceeding 2 μg/dl and 0.8% of children with blood cadmium level (CdB) exceeding the reference value (0.5 μg/l). Conclusions Own observation has confirmed that preschool children living in Piekary Śląskie are still exposed to heavy metals existing in environment. The source of the higher level in blood is not only the hazardous vicinity in the place of residence but also well-known factors associated with the age of children, parental life style and educational background. The parental questionnaire could be useful to identify potential determinants of exposure to lead and cadmium, however exposure must be verified by measurement of a proper biomarker. Objectives: To describe blood lead (Pb-B), cadmium (Cd-B) and mercury (Hg-B) levels in children living in urban, industrial and rural areas in Fez city (north of Morocco) and to identify the determinants and some renal effects of exposure. Material and Methods: The study was conducted from June 2007 to January 2008 in 209 school children (113 girls, 96 boys), aged 6-12 years, from urban, industrial and rural areas in Fez city. Interview and questionnaires data were obtained. Blood and urinary samples were analyzed. Results: The mean of blood lead levels (Pb-B) in our population was 55.53 μg/l (range: 7.5-231.1 μg/l). Children from the urban area had higher blood lead levels (BLLs) mean (82.36 μg/l) than children from industrial and rural areas (48.23 and 35.99 μg/l, respectively); with no significant difference between boys and girls. BLLs were associated with traffic intensity, passive smoking and infancy in the urban area. The mean of blood cadmium levels (BCLs) was 0.22 μg/l (range: 0.06-0.68 μg/l), with no difference between various areas. Rural boys had higher BCLs mean than rural girls, but no gender influence was noticed in the other areas. BCLs were associated with the number of cigarettes smoked at children's homes. The blood mercury levels (BMLs) mean was 0.49 μg/l (range: 0.01-5.31 μg/l). The BMLs mean was higher in urban and industrial areas than in the rural area with no gender-related difference. BMLs were associated with amalgam fillings and infancy in the urban area. About 8% of the children had BLLs ≥ 100 μg/l particularly in the urban area, microalbuminuria and a decrease in height were noticed in girls from the inner city of Fez and that can be related to high BLLs (89.45 μg/l). Conclusions: There is a need to control and regulate potential sources of contamination by these trace elements in children; particularly for lead.

Objectives: The main objective of the study was to determine the levels of house dust mite (Der p1), dog (Can f1), cat (Fel d1) and cockroach (Bla g2) allergens in kindergartens localized in an urban agglomeration. Material and Methods: A quantitative analysis of allergens was carried out in settled dust samples collected by vacuuming the floor surface in three kindergartens (N = 84) and children's clothing (N = 36). The samples were collected in springsummer and autumn-winter periods as well as at the beginning and end of the week. The allergen dust concentration was determined by enzyme-linked immunoenzymatic assay (ELISA). Results: The mean geometric concentrations (±geometric standard deviations) of allergens Der p1, Can f1, Fel d1 and Bla g2 determined in kindergartens were: 0.02±3.21 μg/g of dust; 0.97±4.49 μg/g of dust; 0.30±4.43 μg/g of dust and 0.01±3.08 μg/g of dust, respectively. Younger classrooms (children aged from 3 to 4 years) were characterized by almost twice higher mean concentration of allergen Fel d1, as compared to older classrooms (children aged from 5 to 6 years) (p < 0.05). A significant impact of seasonality on the level of dog allergen Can f1 was found (p < 0.05). No significant weekly variation was found in average concentrations of the allergens. Children who had a dog and/or cat at home were characterized by high concentrations of allergens Can f1 and Fel d1 on their clothes (59.2±5.39 μg Can f1/g of dust; 3.63±1.47 μg Fel d1/g of dust), significantly higher than concentrations of allergens in children who did not have any pets (p < 0.001). Conclusions: Special attention should be paid to keeping the kindergarten rooms tidy and clean and to an appropriate choice of furnishings and fittings which would prevent the proliferation of the house dust mite and accumulation of allergens.

Opis:
Objectives: The main goal of the study was to assess possible association between fetal exposure to fi ne particulate matter ($\text{PM}_\text{2.5}$) and exhaled carbon monoxide (eCO) measured in non-asthmatic children. Material and Methods: The subjects include 118 children taking part in an ongoing population-based birth cohort study in Kraków. Personal samplers of $\text{PM}_\text{2.5}$ were used to measure fi ne particle mass in the fetal period and carbon monoxide (CO) in exhaled breath from a single exhalation effort at the age of 7. In the statistical analysis of the effect of prenatal $\text{PM}_\text{2.5}$ exposure on eCO, a set of potential confounders, such as environmental tobacco smoke (ETS), city residence area, sensitization to house dust allergens and the occurrence of respiratory symptoms monitored over the seven-year follow-up was considered. Results: The level of eCO did not correlate with the self-reported ETS exposure recorded over the follow-up, however, there was a positive signifi cant relationship with the prenatal $\text{PM}_\text{2.5}$ exposure (non-parametric trend p = 0.042). The eCO mean level was higher in atopic children (geometric mean = 2.06 ppm, 95% CI: 1.58–2.66 ppm) than in non-atopic ones (geometric mean = 1.57 ppm, 95% CI: 1.47–1.73 ppm) and the difference was statistically signifi cant (p = 0.036). As for the respiratory symptoms, eCO values were associated positively only with the cough severity score recorded in the follow-up (nonparametric trend p = 0.057). In the nested multivariable linear regression model, only the effects of prenatal $\text{PM}_\text{2.5}$ and cough severity recorded in the follow-up were related to eCO level. The prenatal $\text{PM}_\text{2.5}$ exposure represented 5.1%, while children’s cough represented only 2.6% of the eCO variability. Conclusion: Our study suggests that elevated eCO in non-asthmatic children may result from oxidative stress experienced in the fetal period and that heme oxygenase (HO) activity in body tissues may be programmed in the fetal period by the exposure to fi ne particulate matter.

A network of pediatric environmental health specialty units (PEHSUs) in the United States was formed in 1998 out of a recognized need for clinical expertise in children’s environmental health. Documented trends in a rise of pediatric diseases caused or exacerbated by environmental conditions, coupled with the failure of medical schools and residency programs to cover these issues in a significant way, leaves health care providers, parents, communities, and governments at a loss for this specialized knowledge. The PEHSUs fill this gap by providing: 1) medical education, 2) general outreach and communications, and 3) consultative services to communities and health care professionals. This paper presents examples of key situations where PEHSU involvement was instrumental in improved patient outcomes or advancing clinical expertise in children’s environmental health. Challenges and opportunities for future directions for the program are also discussed.
Sieć Pediatrycznych Specjalistycznych Poradni Medycyny Środowiskowej (PEHSU) w Stanach Zjednoczonych została utworzona w 1998 roku w wyniku uznania konieczności przeprowadzania ekspertyz w zakresie zdrowia środowiskowego dzieci. Udokumentowane trendy wzrostu chorób dzieci wywołanych lub zaostrzonych przez czynniki środowiskowe i nakładający się równocześnie niedostatek programów nauczania z tego zakresu w szkołach medycznych lub w czasie rezydentury spowodowały brak tej specjalistycznej wiedzy wśród pracowników opieki zdrowotnej, rodziców, społeczności i władz. PEHSU wypełniają ten brak wykonując: 1) edukację medyczną, 2) promocję zdrowia środowiskowego 3) porady konsultacyjne dla społeczności i dla pracowników ochrony zdrowia. W niniejszej pracy przedstawiono najważniejsze przykłady, kiedy udział PEHSU był instrumentem dla polepszenia stanu zdrowia albo postępu ekspertyzy klinicznej w zakresie zdrowia środowiskowego.

Opis:
Effects of environmental exposures in utero and in the first years of life on early life health and development is a growing research area with major public health implications. The main aim of this work has been to provide an overview of the next step of the Polish Mother and Child Cohort Study (REPRO_PL) covering exposure, health and neurodevelopment assessments of children at 7 years of age. Details regarding methodology of the follow-up of the children are crucial for cross-cohort collaboration and a full understanding of the future research questions. Phase III of the REPRO_PL cohort covers a follow-up of 900 children at the age of 7 years old. The questionnaire filled in by the mothers is composed of: socio-demographic, child exposure and home environment information, nutritional status and health data. In the case of 400 children, environmental (including collection of urine, saliva and buccal cells), health status and psychomotor assessments are performed. Health and development check consists of physical measurements, child health status assessment (including lung function tests, skin prick testing, an interview/examination by an allergist) and psychomotor development tests (the Strength and Difficulties Questionnaire and the Intelligence and Development Scales). The results of the study will become available within the next few years. Extension of the REPRO_PL cohort with examinations of children at the age of 7 years old may provide a better understanding of the relationship between environmental and lifestyle-related factors and children’s health and neurodevelopment; and may further strengthen scientific base for policies and interventions promoting healthy lifestyle. Objectives To investigate the effect of CO₂ during sleep on next-morning cognitive performance in young schoolchildren, the authors performed a double-blind fully balanced crossover placebo-controlled study. Material and Methods The authors included 36 children aged 10–12 years in the climate chamber. The children slept at 21°C in 6 groups each at 3 different conditions separated by 7 days in a random order. Conditions were as follows: high ventilation with CO₂ at 700 ppm, high ventilation with added pure CO₂ at 2000–3000 ppm, and reduced ventilation with CO₂ at 2–3000 ppm and bioeffluents. Children were subjected to a digital cognitive test battery (CANTAB) in the evening prior to sleep and on the next morning after breakfast. Sleep quality was monitored with wrist actigraphs. Results There were no significant exposure effects on cognitive performance. Sleep efficiency was significantly lower at high ventilation with CO₂ at 700 ppm which is considered to be a chance effect. No other effects were seen, and no relation between air quality during sleep and next-morning cognitive performance was observed in the children emitting an estimated 10 l CO₂ /h per child. Conclusions No effect of CO₂ during sleep was found on next day cognition. The children were awakened in the morning, and spent from 45–70 min in well-ventilated rooms before they were tested. Hence, it cannot be precluded that the children have benefitted from the good indoor air quality conditions before and during the testing period. The slightly better sleep efficiency during high CO₂ concentrations might be a chance finding. Hence, replication is needed in actual bedrooms controlling for other external factors before any generalizations can be made.
