Objectives: The study presents data concerning occupational exposures among the staff of 5 hospitals in the Małopolska province in 2008–2012, taking into account the frequency and circumstances of exposure formation, occupational groups of hospital workers, as well as diversification of the reported rates in subsequent years between the hospitals and in each of them. An additional objective of the analysis was to assess the practical usefulness of the reported data for planning and evaluation of the effectiveness of procedures serving to minimize the risk of healthcare workers’ exposure to pathogens transmitted through blood. Material and Methods: Data were derived from occupational exposure registries kept by 5 hospitals of varying sizes and operational profiles from the Małopolska province from the years 2008–2012. Results: Seven hundred and seventy-five cases of exposure were found in a group of 3165 potentially exposed workers in the analyzed period. Most cases were observed in nurses (68%) and these were mainly various types of needlestick injuries (78%). Exposure rates with respect to all workers ranged from 2.6% to 8.3% in individual hospitals, but the differences in their values registered in the hospitals in subsequent years did not bear any statistical significance, in a way similar to the rates calculated separately for each occupational group. Conclusions: There was no upward or downward trend in the number of reported cases of exposure to bloodborne pathogens in the studied period in any of the hospitals. Statistically significant differences in the percentages of exposures were reported between individual hospitals in some years of the analyzed period, which confirms the need for registries in individual units in order to plan and evaluate the effectiveness of preventative measures.

Italy is one of the most important destination countries for Romanians. At the same time, the Italian care sector relies mainly on migrant labour, most of whom are Romanian women. Historically, Italy is considered one of the landmark countries for the southern or Mediterranean welfare state, characterised by its fragmented labour market, underdeveloped social protection system, informal economy and unpaid care work, usually done by the women in the family. Italy has one of the highest rates in Europe of both the elderly population and life expectancy at birth. In the last 20 years, the care work was gradually redistributed to migrant care workers, most of them women from former socialist countries, who often live in the household where they work. Migration from Eastern Europe, particularly Romania, has been facilitated, on the one hand, by rising unemployment and low-paid job opportunities in migrants’ countries of origin in the context of the deindustrialisation of state industry and, on the other, by the Italian elderly public-support system which is based on cash benefits granted to the family which can be redistributed to employ migrant care workers. In this paper we analyse three specific types of care work migration from Romania to Italy and the main challenges which they face, taking into account the specifics of the work and the type of migration chosen. The methodology is qualitative, based on 20 semi-structured online interviews with Romanian care workers and two interviews with stakeholders.

ObjectivesHealth care services in Poland are delivered by public and private providers. The aims of this study were to assess the attitudes towards private and public health care services in Poland and to identify differences between them, in the opinion of physicians.Material and MethodsA questionnaire-based survey was carried out among physicians attending mandatory courses delivered at the School of Public Health, the Centre of Postgraduate Medical Education in Warsaw, Poland. The questionnaire included 29 questions concerning private and public health care services.ResultsCompleted questionnaires were obtained from 502 physicians (67.7% females), aged 42.1±10.8 years, with a response rate of 77.2%. In the opinion of the surveyed doctors, the major advantages of private health care units, in comparison with public ones, are short waiting times for an appointment (88.2%), an efficient on-site service (78.6%) and convenient appointment times (75.7%). The respondents gave high scores to items such as relations with patients (p < 0.001), superiors (p < 0.001) and colleagues (p = 0.03) when working in private, rather than public, institutions. In the opinion of physicians, public health care institutions guarantee better employment conditions (44.4% vs. 13%; p < 0.001) and security (29.1% vs. 11.1%; p < 0.001) than private ones. The respondents did not observe any differences (p > 0.05) between public and private facilities in terms of the involvement of medical staff and infrastructure. There were significant differences (p < 0.001) in the perception of working conditions in public and private health care institutions depending on the medical education level and the place of primary employment. Conclusions: Among physicians in Poland, private medical institutions are perceived as better organized and granting faster as well as more comprehensive access to health care services when compared to public ones. Closing the gaps between working conditions in public and private units could encourage physicians to practice in the public health care sector.

Objectives: The risk of transmission of Mycobacterium tuberculosis from patients with tuberculosis to health care workers (HCWs) has been well documented but little is known about the prevalence of latent tuberculosis infection (LTBI) in Iranian HCWs. The aim of this study was to determine the prevalence of LTBI among HCWs by using IFNgamma-release assay and compare the results with those of tuberculin skin test (TST). Methods: Two hundred HCWs were evaluated with both TST and QuantiFERON-TB Gold In Tube test (QFT-GIT). The obtained data were analyzed by SPSS v.16 Software. Results: The participants were 73 males and 127 females with the mean age of 34.36±8.26 years. TST was positive in 105 cases (52.5%) and the QFT results were positive in 17 cases (8.5%). There was poor agreement between the two tests (53%, к = 0.115). Induration diameter of TST ≥ 10 mm and working duration ≥ 10 years were independent predictors for positive QFT (p = 0.004). Conclusions: Due to the fact that BCG vaccination has been administered routinely to all HCWs in Iran, specifi c tests should be introduced for high risk groups. QFT thus seems to be more effective for LTBI diagnosis than TST among HCWs with BCG immunization history.

Objectives: Good ventilation is more important in hospitals than in many other buildings. The objective of this study was to evaluate the effect of the condition, performance and modernity of ventilation systems on the perceived indoor air quality (IAQ) and the indoor air-related symptoms of hospital personnel. Materials and Methods: An inspection and evaluation graded on a 1 to 3 scale of the condition, performance, and modernity of the ventilation systems was carried out in ten central hospitals in Finland. The perceived IAQ and the related symptoms were collected by means of an indoor air questionnaire survey among the workers in these hospitals. Results: The condition, performance and modernity of the ventilation systems were good in 40% of the hospitals included in the research and poor in another 40% of them. In hospitals where the majority of the ventilation systems were assessed to be good, the prevalence of the indoor air-related complaints and symptoms was lower than in hospitals where the majority of the ventilation systems were assessed as needing extensive repairs. Conclusions: The condition and performance of the ventilation systems in hospitals had a significant impact on the perceived IAQ and the symptoms observed among the employees in Finnish hospitals. Therefore, it is important that hospital ventilation systems are maintained properly and regularly. Furthermore, they should be renovated, at the latest when their technical lifespan expires.

Objectives: The association between depressive symptoms and psycho‑organisational work environment has been established in the literature. Some studies have evaluated depressive symptoms in healthcare workers, but little research has been carried out among nurse managers. The aim of the study is to evaluate the depressive symptoms prevalence among nurse managers' population and work environment factors. Material and Methods: A descriptive correlational research design was used. Data were collected from 296 nurse managers in five hospitals in the eastern area of France between 2007 and 2008. Health outcomes were evaluated by measuring depressive symptoms (CES-D scale), the exposure data by assessing psycho‑organisational work environment with effort-reward imbalance-model of Siegrist. Multiple logistic regressions were used to describe the strength of the association between depressive symptoms and effort-reward imbalance adjusted for personal and occupational characteristics of the nurse managers. Results: Among the nurse managers, a third had depressive symptoms, and 18% presented an effort-reward imbalance (ratio: ≥ 1). A significant association was found between depressive symptoms and effort-reward imbalance (OR = 10.81, 95% CI: 5.1-23, p < $10^{-3}$), and with esteem as a reward (OR = 3.21, 95% CI: 1.6-6.3, p < $10^{-2}$). Conclusion: In view of the hierarchical situation of nurse managers and their primary roles in hospitals, it is necessaryto take prevention measures to improve their work environment and health.

Occupational health practitioners deal with the questions of confidentiality, relation between the right to work and the right to health, between individual freedom and the risks other can run. An audit of pre-employment health assessment of health care workers, in the United Kingdom and in France, discussed the efficiency of pre-employment screening. Screening tests and medical examinations should not be used as a pretext to avoid implementing effective preventive measures. The pre-employment examination has mostly to be used for education of the future employees and for collection of baseline data. Appropriate procedures such as developing preventive policies, health promotion, and control of hazards in the work environment are efficient for the promotion of equal employment rights for women, older workers, or people with disabilities.

Background Harmful chemicals are used in various forms from different sources in hospital settings. The standard gold method in risk control studies still determines exposure by personal or ambient measurements. In the absence of trained personnel, resources, or sufficient time, qualitative methods should be used to assess exposure. This study aims to compare quantitative and qualitative results of chemical risk exposure. Material and Methods Both qualitative (perceptions without monitoring data of the workers and experts) and quantitative perceptions (perceptions with monitoring data) were recorded. Two experts were asked to evaluate exposure intensity in pathology department workers, secretary workers, and cleaning workers. Occupational hygiene measurements were taken based on the occupational health and safety department risk assessment results, expert job analysis, and pilot study measurements. Results While most workers reported feeling highly exposed to chemical risks, the majority of experts reported medium-risk exposures and high-risk exposures. Three occupational hygiene measurements (3/45: 6.6%) exceeded the permissible time-weighted average, and the other results were within the acceptable range. Conclusions There was a significant difference between the estimated exposure and the measured exposure in hospital settings. A correlation was not found between workers’ perceptions of chemical risk exposure and the chemical risk levels measured in this study.

Objectives To analyze the number, epidemiology and circumstances of needlestick and sharps injuries (NSSI) and exposures to body fluids and to identify further preventive measures to improve the occupational safety of health care workers (HCW). Material and Methods Setting: German university tertiary-care referral center. Retrospective study based on injury documentation sheets of the hospital’s staff and faculty health service and, if given, on reports by continuity doctors and by the accident and emergency department in January 2014–June 2016. Results Altogether, 567 injuries were registered with a significant decrease of cases over the study period. The majority of accidents occurred in the operating theater (35%). Stress, time pressure, overstrain, carelessness and distraction were found to be the main reasons for injuries. At least 30% of the cases were preventable, mainly by wearing personal protective equipment (PPE), by proper disposal of an item and by early replacement of overfilled sharps containers (SC). In 20% of the cases involving an item, the injury was caused by a safety-engineered device (SED). Almost one-third of these injuries were attributable to an improper use of the SED. Conclusions Despite many efforts made to reduce their number, NSSI still occur. Health care workers and students should be offered regular trainings to be sensitized to this topic and to learn the appropriate use of SED. Moreover, organizational measures must be taken, such as the provision of suitable PPE and safe SC. Strategies need to be established to improve the working conditions and reduce the stress level of HCW.

Objectives Health care workers, including dentists, are at the front line for acquiring blood-borne virus infections. This study aimed to investigate the prevalence of hepatitis B, hepatitis C and human immunodeficiency viruses among Iranian dentists. Material and Methods The survey included 1628 dental health care workers who attended the 51st annual Congress of the Iranian Dental Association. Data on the risk of blood-borne virus transmission during health care and infection control practices were gathered from self-administered questionnaires. Sera were screened serologically by enzyme-linked immunosorbent assay (ELISA). The positive samples were examined by polymerase chain reaction (PRC) followed by direct sequencing. Results Six (0.36%) and 81 (5.0%) were positive for hepatitis B surface antigen (HBsAg) and (anti-hepatitis B virus core antigen (anti-HBc), respectively. Only 1 (0.061%) was positive for anti-hepatitis C virus (anti-HCV). No case was positive for anti-human immunodeficiency virus (anti-HIV). One case was diagnosed as being occult hepatitis B virus (HBV) infection. One thousand five hundred thirty-five (94.3%) of participants had received at least 1 dose of HBV vaccine. One thousand three hundred fifty-nine (88.5%) contained hepatitis B surface antibody (anti-HBs) > 10 IU/ml, of whom 55 (4.0%) were anti-HBc positive, suggesting that they had been infected with HBV in the past. Anti-HBc positive cases had past histories of hepatitis, either their own or their spouses’. Individuals with inadequate anti-HBs levels (< 10 IU/ml) were significantly more prevalent among anti-HBc positive cases (p < 0.001). Conclusions The prevalence of blood-borne viruses among dental HCWs found in this study was lower than past reports from Iranian dentists and general population. The implementation of HBV vaccination together with improvement in infection control procedures has reduced the potential for risk infection among Iranian dentists.

We analyzed occupational exposure to potentially infectious body fluids among health care workers (HCWs). Nurses were the most common exposed category of HCWs. In 73.6% cases needle sticks had been the reason of exposure. Recapping a needle was the cause of exposure in 6.9% accidents. Among 189 registered HCWs, 66 (34.9%) performed invasive procedures without any personal protective equipment. Prophylaxis with antiretroviral drugs was necessary in 43 (22.8%) cases. As many as 60.3% of exposure incidents to potentially infectious material result from non-compliance with the relevant recommendations. Continuous education and training is critically needed to prevent occupational exposure to blood-borne infections among health care workers.

A significant proportion of migrant workers around the world have difficult access to social protection, especially long-term benefits such as pensions. Domestic care workers are a particularly vulnerable group in this regard. Analysing the example of Polish migration to Germany, the aim of this paper is to present the strategies that migrants aged 45 and over undertake in the context of pension plans and ensuring an adequate level of social protection while working abroad. Using empirical material collected during semi-structured interviews with migrant care workers, representatives of employment agencies and experts, the article sheds light on the diverse conditions that influence the strategies of individual workers. Depending on the general knowledge of their social rights, their migration strategy or their personal situation, migrants adopt three main strategies called (1) escape from problems instead of social security, (2) secure and legal work above all and (3) an informed and inquisitive insured worker.
Znaczna część pracowników migrujących na całym świecie ma utrudniony dostęp do ochrony socjalnej, zwłaszcza do świadczeń długoterminowych, takich jak emerytura. Szczególnie wrażliwą grupą w tym obszarze są pracownicy zatrudniani w gospodarstwach domowych. W artykule analizowane są przykłady migracji Polaków do Niemiec, a celem niniejszego opracowania jest przedstawienie strategii, jakie podejmują migranci w wieku 45 lat i więcej w kontekście planów emerytalnych i zapewnienia odpowiedniego poziomu zabezpieczenia społecznego podczas pracy za granicą. Wykorzystując materiał empiryczny zebrany podczas wywiadów z migrującymi pracownikami opieki domowej, przedstawicielami agencji zatrudnienia oraz ekspertami, artykuł rzuca światło na zróżnicowane uwarunkowania wpływające na strategie poszczególnych pracowników. W zależności od ogólnej wiedzy na temat ich praw socjalnych, strategii migracyjnej lub sytuacji osobistej migranci przyjmują trzy główne strategie, określone jako: (1) ucieczka od problemów zamiast zabezpieczenia społecznego, (2) przede wszystkim bezpieczna i legalna praca oraz (3) świadomy i dociekliwy ubezpieczony pracownik.

Migration is a constituent element of the European context and in this paper we will analyze the Italian case and the connections between the labor market of care and assistance to elderly people. This phenomenon has an international dimension (León 2014) because it intercepts the routes of migrant women from Eastern Europe and triggers the creation of new family structures (Degiuli 2016; Lyon 2006). From a perspective based on finding ways to avoid generating exclusion and privatization processes by socio-economic context (Skornia 2014), we will analyze in-depth a community learning process that affects the elderly and their families, the care-workers and the transnational dimension, the local contexts and socio-health services. The participatory learning approach aims to create community bonds and social cohesion and to support intergenerational and intercultural citizenship (Deluigi et al. eds 2015; Newman, Tonkens 2011). Beyond the stereotypical images of migrants, often associated with threat and danger, we will highlight the various aspects of a cooperative coexistence, considering living together in inclusive societies as a challenge and a goal (United Nations 2016). Multicultural environments can choose intercultural dialogue as “an engine” of active citizenship and community learning (Packham 2008; Zepke 2013a). Education could be a stimulator of recognition and promotion of best practices in local home-care as a way of reflection and action. The transition from a “welfarism” culture of care to an eco-systemic home-care culture accepts the challenge of an open learning society, attentive to stories of migration between identity, intergenerational alliances and bearers of cultures.
W obecnym czasie migracja jest jednym z istotnych zjawisk w europejskim kontekście. W niniejszym artykule ukazany jest związek pomiędzy rynkiem pracy a osobami starszymi. Migracja ma wymiar międzynarodowy (León 2014), zatrzymuje migrantów (głównie kobiety) z Europy Wschodniej i przyczynia się do tworzenia nowych struktur rodzinnych (Degiuli 2016; Lyon 2006). W artykule analizowany jest proces uczenia się społeczności, który dotyczy także osób starszych, ich rodzin, pracowników opieki, pracowników usług socjalno¬ zdrowotnych, zarówno w wymiarze lokalnym, jak i transnarodowym. Do analizy wykorzystana jest perspektywa włączania, polegająca na unikaniu procesów wykluczenia i prywatyzacji (Skornia 2014). Uczenie się ma na celu tworzenie więzi społecznych i wzmocnienie spójności już istniejących oraz wspieranie międzypokoleniowego i międzykulturowego obywatelstwa (Deluigi et al. eds 2015; Newman, Tonkens 2011). Poza stereotypowymi obrazami imigrantów, często związanymi z lękiem i poczuciem zagrożenia, warto zwrócić uwagę na pozostałe aspekty migracji, takie jak kooperatywne współistnienie, gdzie wspólne życie w społeczeństwie sprzyja integracji, ujmowanej zarówno jako wyzwanie, jak i cel (United Nations 2016). W środowiskach wielokulturowych dialog międzykulturowy możne mieć zastosowanie jako „motor” dla aktywnego obywatelstwa i uczenia się społeczności (Packham 2008, Zepke 2013). Edukacja może być stymulatorem uznania i promowania najlepszych praktyk w lokalnej opiece domowej oraz może stanowić podstawę do refleksji i działania. Zmiana kultury państwa opiekuńczego na kulturę państwa typu opieka eko-systemowa może spowodować większą uważność na historie migrantów jako sojuszników międzypokoleniowych i nosicieli kultur.

Labour migration between Poland and Germany poses challenges for migrant care workers as well as for their families. Migrant care workers have to develop strategies to compensate for their physical absence in the daily lives of their family members. Nevertheless, working in the domestic care, especially the so-called 24-hours-care offers several possibilities to deal with these challenges. The goal of this study was to find out how migrant care workers from Poland organize their family lives from a distance and to what extent do their employers or clients commit themselves for the cause. For the purpose of this study 26 problem-centred semi-structured interviews with mainly female and a few male commuter migrants from Poland were carried out. The data analysis identified two strategies of organizing family life during absence. One group fosters a so-called intimacy from distance or virtual intimacy. They use various media, for example the internet, mobile or landline phones, to almost continuously stay in contact with their families in Poland. The other group manages to create conditions at their workplace which allow them to bring along their family members (mainly small children) for a short period of time or even for the whole duration of the employment. This solution depends on an agreement and cooperation with their clients. It appeared that the presence of family members at the workplace brings both families (of migrant workers and of clients) closer and creates a fictive kinship. In some cases it leads to conflicts. The understanding on behalf of the employers and the possibilities to stay in an intense and regular contact with their family influence the migrants’ work satisfaction, increase their ability to cope with stress and encourages an extended sense of the term ‘family’.

Introduction: During the COVID-19 pandemic, workload and physical and mental exhaustion became more extensive, especially among health care workers (HCWs). Material and Methods: This study involved 50 people aged 25 to 64, of whom 37 were women and 13 men. These HCWs were physiotherapists (48%), nurses (30%) and physicians (22%), all professionally active during the COVID-19 pandemic. The research was conducted between June and October 2020, using an on-line survey. The COPE, LBQ and ISI questionnaires were used. Results: Analysis of occupational burnout (LBQ) and its various aspects showed that the Deterioration of relations with clients (patients) was significantly higher among those with up to five years and between 15 and 24 years of experience than in the group with 25–35 years of experience. Sense of professional ineffectiveness and Disappointment were significantly higher (p %lt; 0.05) in the group with the most work experience. 30% of the medical workers surveyed obtained a result indicating subliminal insomnia (ISI) and 28% – clinical insomnia. Comparison of COPE and LBQ scores showed that Psychophysical exhaustion (LBQ) correlated negatively with Religious coping (p = 0.032). Deterioration of relations with clients (patients) (LBQ) correlated negatively with such strategies (COPE) as Active coping (p = 0.048), Planning (p=0.006), Positive reinterpretation and growth (p < 0.001). Sense of professional ineffectiveness (LBQ) correlated negatively with Acceptance strategies (p = 0.017) and Behavioral disengagement (p = 0.021). Disappointment correlated positively with the Positive reinterpretation and growth strategy (p = 0.024). Conclusions: The COVID-19 pandemic had an impact on the development of burnout amongst the HCWs surveyed, especially that related to psychophysical exhaustion, a sense of lack of effectiveness and a sense of disappointment. The sleep cycle was affected. Cognitive-behavioral oriented workshops are recommended when facing burnout.
Wprowadzenie: W trakcie trwania pandemii COVID-19 obciążenie pracą oraz fizyczne i psychiczne wyczerpanie były coraz większe, szczególnie wśród pracowników ochrony zdrowia (POZ). Materiał i metody: W badaniu wzięło udział 50 osób w wieku od 25 do 64 lat – 37 kobiet i 13 mężczyzn. Byli to: fizjoterapeuci (48%), pielęgniarki (30%), lekarze (22%), aktywni zawodowo w czasie pandemii COVID-19. Badanie przeprowadzono w okresie od czerwca do października 2020 r. w formie ankiety on-line. Wykorzystano kwestionariusze COPE, LBQ i ISI. Wyniki: Analiza wypalenia zawodowego (LBQ) i jego poszczególnych aspektów wykazała, że Brak zaangażowania w relacje z klientem (pacjentem) był istotnie wyższy w grupie ze stażem pracy do 5 lat oraz 15–24 lat niż w grupie ze stażem 25–35 lat. Poczucie braku skuteczności zawodowej i Rozczarowanie były istotnie wyższe (p %lt; 0.05) w grupie z najwyższym stażem pracy. 30% ankietowanych pracowników medycznych uzyskało wynik ISI wskazujący na obecność bezsenności podprogowej, a 28% bezsenności klinicznej. Analiza korelacji pomiędzy wynikami COPE a LBQ wykazała ujemną korelację pomiędzy Wyczerpaniem psychofizycznym (LBQ) a Zwrotem ku religii (p = 0,032). Brak zaangażowania w relacje z klientem (LBQ) ujemnie korelował z takimi strategiami (COPE) jak: Aktywne radzenie sobie (p = 0,048), Planowanie (p = 0,006), Pozytywne przewartościowanie i rozwój (p < 0.001). Poczucie braku skuteczności zawodowej (LBQ) było ujemnie skorelowane ze strategiami Akceptacji (p = 0,017) i Brakiem zaangażowania (p = 0,021). Rozczarowanie dodatnio korelowało ze strategią Pozytywne przewartościowanie i rozwój (p = 0,024). Wnioski: Pandemia COVID-19 miała wpływ na rozwój wypalenia zawodowego badanych pracowników ochrony zdrowia, zwłaszcza wypalenia związanego z wyczerpaniem psychofizycznym, poczuciem braku skuteczności i poczuciem rozczarowania. Wpłynęło to na cykl snu. W obliczu wypalenia sugeruje się warsztaty poznawczo-behawioralne.
