Penetrating traumas of the facial skeleton are relatively rare; however, they can be dangerous and even life-threatening. The epidemiology of facial skeleton trauma is diverse and depends on the affected region, age, and the kind of foreign body that caused the injury. In Poland, the most common cause of penetrating traumas are accidents and assaults, and only rarely accidents are related to improper use of firecrackers or fireworks. The aim of this study is to present a literature review and our own experience with penetrating traumas of the face and the facial part of the cranium. Six cases with severe wounds and fractures of the facial skeleton were treated between the years 2000 and 2012 in our department. The applied treatment methods depended mainly on the general condition of the patients. In all cases, we achieved acceptable functional and aesthetic outcomes.

Background: Childhood represents a risk period of susceptibility to sport-related injuries. Objectives: The aim of the study was to investigate the prevalence of sports-related injuries including orofacial traumas in children attending primary schools and participating in competitive sports, then to compare their situation with that of children engaging in sporting activities only at school and in their leisure time. The second aim of the study was concentrated on the use of sports mouthguards. Methods : Data was obtained by means of a questionnaire and statistically analysed by the basic statistic methods in Microsoft Office Excel and Pearson’s chi-square test , and a statistical significance level p < 0.05 was established. Results: 341 children (175 girls and 166 boys) completed the questionnaires and 204 (59.8%) of them were competitive athletes. 146 (42.8%) children reported sports-related injuries and 47 (13.8%) children registered trauma of the orofacial region. Only 10 (2.9%) children regularly use sports mouthguards. No statistically significant differences were detected in the occurrence of sport-related injuries including orofacial traumas between the competitive athletes and non-competitive children. Conclusion: Sport-related trauma represents a risk not only for children engaging in competitive sport activities but also for children doing sports at school and occasionally in their free time.

Number of deaths related with injuries suffered as a result of experienced traumas is increasing. Penetrating traumas of the facial skeleton occur relatively rarely and much more often concern rather children than adults. Epidemiology relating this kind of trauma differs depending on the region of the world. In Poland, gunshot injuries as well as traumas caused by explosions of firecrackers or fireworks amount only to a slight percentage among all facial skeleton traumas, and the most common reason for penetrating traumas lies in accidents or assault with the use of sharp, narrow and long objects that easily enter bones of the facial skeleton. The present study reported the case of 50-year-old man who suffered from trauma of the facial skeleton, which resulted from foreign body (pickaxe) penetration into the subtemporal area, zygomatic arch and the right orbital cavity. The surgical treatment method and final outcome was presented and discussed.

Joshua Pederson’s Sin Sick: Moral Injury in War and Literature proposes the use of moral injury – a psychological concept describing the affliction of those who break their moral code when committing despicable acts – as a framework through which war narratives of the American War on Terror can be productively read without resorting to the controversial idea of perpetrator trauma, which seems to excuse veterans as victims of the war. Pederson provides the reader with a clinical overview of the condition as well as a first literary theory of moral injury as a manifestation of various forms of excess through a genealogical reading that includes analyses of Dostoevsky’s Crime and Punishment and Camus’ The Fall. Sin Sick appears as a step in the right direction as it addresses in a timely manner a blind spot in trauma theory using a concept that more accurately describes a specific type of suffering. In the author’s mind the very term “moral injury” entails an acknowledgement of the soldiers’ wrongdoing, and this allows him to defend contemporary war writers from critics accusing them of using a narrow point of view that focuses on the pain of the American protagonists and hides the suffering of their victims. Ultimately, the study introduces readers to a fresh way to understand the psychological pain of perpetrators and seeks to inaugurate a new branch of studies that can run in parallel with trauma theory.

Penetrating neck traumas are traumas that pass through the platysma muscle, and they account for 5% to 10% of all neck traumas. Penetrating head traumas can damage vital structures such as the airways, esophagus, major nerves and vessels. Thus, patients with penetrating neck traumas should be treated in hospital, and doctors should be careful when deciding on the best treatment. Currently, the need for surgery can be established based on imaging tests, clinical status of the patient, medical history, physical examination, and laboratory tests. Exploratory surgery is not necessary particularly because it is associated with complications. We present two patients with penetrating neck traumas who were treated in our center: a 22-year-old patient with a foreign body that, for two years, mimicked a tumor, and a 45-year-old patient with a stab wound requiring immediate treatment. In the first patient, finding the foreign body earlier would have allowed an immediate removal, which would have prevented subsequent surgery and reduced the risks of leaving the foreign body in the neck. In the second patient, immediate contrast-enhanced computed tomography (CT) of the neck showed a foreign body, its size and position, which helped to plan surgery. Conclusions: In hemodynamically stable patients with penetrating neck traumas, doctors should choose the best treatment based on imaging, medical history, and physical examination.

The way of understanding trauma still remains a crucial issue in both theoretical and practical terms (in diagnosis and therapy). In this article, we discuss the controversies around the definition, the process of diagnosing, and the consequences of trauma. The subject of analysis is the current diagnostic criteria for trauma (the narrow and broad approaches to trauma), including those set out in the current classifications of mental disorders: ICD-10 and DSM-5. We discuss posttraumatic stress disorder syndromes as well as the impact of traumatic events on mental health and psychosocial functioning. The principles of support for people who have experienced trauma are also suggested.

Despite the 70 years that passed since the settlement of the warfare, collective and individual trauma of the World War II still has a vivid impact on Polish society. Research on psychological effects of incarceration in concentration camps was initiated by Antoni Kępiński i Stanisław Kłodziński. Department of Psychiatry in Cracow, under the leadership of Maria Orwid, continued the research taking care of the offspring of the concentration camp survivors (so-called “second generation”). Psychological trauma sustained by parents or grandparents is still covered by silence. Younger generation often don’t know the war history of their ancestry. Israeli research of transgenerational transmission of trauma (TTT) reported significantly more frequent anxiety, depressive and psychosomatic symptoms among the offspring of Holocaust survivors (OHS). Analysis of trauma-afflicted families describe disturbed mechanisms of their functioning. Particularly parentification and troubles with individuation/separation, what leads to traumatization of next generations. Collective trauma of Polish nation is also very significant. It’s a place of interests of sociologists and social psychologists. Authors present their reflections on presence of features of transgenerational trauma in young Poles (so-called “third generation”).
Mimo blisko 70 lat, które minęły od zakończenia działań wojennych, trauma indywidualna i kolektywna pozostawiona w Polakach przez okrucieństwa II wojny światowej wciąż pozostaje żywa. Badania skutków psychicznych, jakich doświadczyły osoby uwolnione z obozów koncentracyjnych, ocalałe z Holocaustu, rozpoczęli Antoni Kępiński i Stanisław Kłodziński. Ośrodek krakowski, pod kierunkiem Marii Orwid, kontynuował badania, a także rozpoczął opiekę terapeutyczną nad dziećmi ocalałych, nazwanymi potem „II pokoleniem”. Z obserwacji wynika, że urazy psychiczne doznane przez rodziców czy dziadków najczęściej były i nadal są objęte milczeniem. Młodsze pokolenia często nie znają wojennych losów swoich przodków. Izraelskie badania nad transgeneracyjną transmisją traumy (TTT) pokazują wyraźnie, że wśród potomstwa ocalałych z Holocaustu (offspring of Holocaust survivors, OHS) częściej niż w populacji ogólnej występują objawy depresyjne, lękowe oraz psychosomatyczne. Analizy funkcjonowania systemów rodzinnych dotkniętych traumą opisują patologiczne mechanizmy ich działania. Dotyczy to zwłaszcza zjawiska parentyfikacji oraz zaburzeń procesu indywiduacji–separacji, co powoduje traumatyzację kolejnych pokoleń. Nie bez znaczenia jest również kolektywna trauma Polaków, leżąca głównie w polu zainteresowań socjologów i psychologów społecznych. Autorzy w pracy przedstawiają rozważania na temat obecności cech traumy transgeneracyjnej wśród młodych Polaków („III pokolenie”).

Transgression is often seen as a negative term; to cross social or ethical boundaries. In this paper, it is defined as ‘blurring of the symbolic boundaries between grandparents and younger generations in terms of the WWII experience’, which leads to living memory of the war, but also to experiencing and re-living the trauma of war and dislocation. It occurs through the immersion of younger generations in family history narratives, memorabilia, diaries and photographs that become a family treasure, owned jointly by the family members. In this paper, intergenerational transgression is analysed as a softand symbolic phenomenon, which on one hand preserves the memory of past, but on the other, cascades the negative experiences onto children and grandchildren. If this is true for WWII survivors, then it should be considered in other cases of long-term conflict and dislocation, particularly in recent conflicts such as Syria, Iraq, and Afghanistan. Understanding the connection between intergenerational transgression of war trauma may aid the process of healing.

This paper aims to read Doris Lessing’s “The golden notebook” as a literary experiment, which struggles to find meaning amid fragmented narratives. Aiming to reflect the structure of the experience of mental breakdown, the novel is abundant with literary strategies meant to enhance the understanding of the Real (in Lacanian sense) experience of the main protagonist. Ultimately, all of the stylistic endeavours are doomed to failure, and the experience which cannot be directly communicated surfaces as traumatic: it escapes both chronology and understanding; it renders the protagonist helpless against reality, which she perceives as full of violence. The reason behind the breakdown is elusive, yet it seems to be grounded in historical reality of the twentieth century. Through its inability to convey a message in a conventional novelistic form, “The golden notebook” emerges as a witness to the traumatic nature of human experience of the modern era.

Work accidents are a subject of particularly tight control, and craniofacial injuries belong to the most difficult types of trauma in terms of their pre-hospital management. The aim of this paper is to present the case of a patient who experienced craniofacial injury during a work accident. Analysis of the case history showed that due to specific type of injuries, this patient required complex surgical treatment and multi disciplinary consultations. Due to long-term incapacity for work, craniofacial injuries constitute an organizational and economic challenge.

Objectives Professionals helping victims of violence trauma, in addition to a number of negative effects, may also experience positive changes in the form of vicarious posttraumatic growth (VPTG). Cognitive trauma processing and empathy seem to be important to VPTG. The aim was to determine the relationship between empathy, cognitive trauma processing and VPTG in female professionals helping violence victims. Material and Methods The study included 154 women representing 3 professional groups (therapists, social workers, probation officers). The subjects’ age ranged 26–67 years (M±SD 43.98±10.83). The study used 3 standard measurement tools, i.e. the Secondary Posttraumatic Growth Inventory, the Empathic Sensitiveness Scale, measuring 3 aspects of empathy, i.e. empathic concern, personal distress and perspective taking, the Cognitive Processing of Trauma Scale, allowing assessment of 5 remedial strategies (positive cognitive restructuring, downward comparison, resolution/acceptance, denial, regret) and a survey developed for the use of the research. Results Positive correlations were found between empathy, cognitive coping strategies and VPTG. Positive cognitive restructuring and resolution/acceptance strategies were found to mediate the relationship between empathy and VPTG. Conclusions Professionals helping trauma victims may experience positive effects in the form of VPTG. The occurrence of VPTG in female professionals is enhanced by the use of empathy and positive coping strategies.

Introduction: Trauma is the leading cause of mortality in people below the age of 45 years. Abdominal trauma constitutes one-fourth of the trauma burden. Scoring systems in trauma are necessary for grading the severity of the injury and prior mobilization of resources in anticipation. Aim: The aim of this study was to evaluate RTS, ISS, CASS and TRISS scoring systems in blunt trauma abdomen. Materials and methods: A prospective single-center study was conducted on 43 patients of blunt trauma abdomen. Revised trauma score (RTS), Injury Severity Score (ISS), Clinical Abdominal Scoring System (CASS) and Trauma and Injury Severity Score (TRISS) were calculated and compared with the outcomes such as need for surgical intervention, post-operative complications and mortality. Results: The majority of the study subjects were males (83.7%). The most common etiology for blunt trauma abdomen as per this study was road traffic accident (72.1%). Spleen was the most commonly injured organ as per the study. CASS and TRISS were significant in predicting the need for operative intervention. Only ISS significantly predicted post-operative complications. All scores except CASS significantly predicted mortality. Conclusions: Among the scoring systems studied CASS and TRISS predicted the need for operative intervention with good accuracy. For the prediction of post-operative complications, only the ISS score showed statistical significance. ISS, RTS and TRISS predicted mortality with good accuracy but the superiority of one score over the other could not be proved.

Background: Craniocerebral injuries are one of the most common causes of mortality and disability in Poland. The treatment of patients who are in an intensive care unit is based primarily on stabilizing the patient’s general condition as well as basic duties according to the patient’s functioning. Aim of the study: The aim of this study is to demonstrate the importance of early rehabilitation and the role of physiotherapy in recovery after craniocerebral trauma. Case report: The subject was an 18-year-old patient who suffered craniocerebral trauma as a result of a road accident. After losing consciousness, he was in the intensive care unit, where he was placed on a medical ventilator. A properly selected physiotherapeutic procedure was performed. Passive exercises, contracture correction and appropriate positioning were used. To prevent pressure sores, anti-bedsore prophylaxis was implemented. Respiratory therapy played a key role. The goal of respiratory physiotherapy was to improve respiratory function by maintaining proper lung ventilation, increasing chest and diaphragm mobility along with maintaining the efficiency of respiratory muscles, as well as stimulating effective coughing and evacuation of secretions. The NDT-Bobath concept was used as therapy for spastic tension. The goal of the therapy was to get rid of pathological movement patterns and replace them with physiological patterns. The PNF method, classical and lymphatic massage, polysensory stimulation and music therapy were also used. Conclusions: Early and comprehensive rehabilitation in a patient after craniocerebral trauma is extremely important and determines therapeutic effectiveness. Comprehensive therapy and care are able to prevent a number of complications that threaten the patient as a result of immobilization.
