Introduction. Endoprosthetics is the most effective method of returning to a normal way of life without pain. The key element is the post-operative rehabilitation program where a patient has to comply with certain rules and principles. The rehabilitation also includes the family of the patient. Each person, after any surgery requires an individual approach, and no patient can be commonly treated. Aim. The aim of the article is to compare the effects of an early rehabilitation after the implantation of an artificial hip joint in two patient cases. Description of the cases. This research was conducted at Bodden-Klinken GmbH. An individual case study was used, based on an example of two patients at the age of 59 who had implantation of an artificial hip joint. The carried out rehabilitation of the patients was conducted without any disruptions. Patients were able to freely walk on crutches nine days after the procedure. Conclusions. In the cases analyzed, the rehabilitation course was described until the ninth day after implantation of an artificial hip joint. High results in terms of patient rehabilitation were achieved. Furthermore, in order to continue their therapy, they were referred to the sanatorium.

The contemporary international pandemic proved that a flexible approach towards work, trade and healthcare is not only favorable but a must. Hence, the devices enabling home‐rehabilitation became one of the urgent needs of the medical market. The following overview is a part of an R&D project aimed at designing an exoskeleton and developing methods enabling effective home rehabilitation. It contains a comparison of current devices in terms of their kinematics, applications, weights, sizes, and integration with selected ICT technologies. The data is analyzed regarding conclusions from qualitative research, based on in‐depth interviews with physiotherapists and questionnaires organized beforehand. The investigation assesses whether commercial and developed devices enable feedback from a patient by all possible means; hence, if they could allow effective telerehabilitation. Moreover, their capabilities of increasing engagement and accelerating improvements by supervising techniques and measuring biomechanical parameters are evaluated. These outcomes are a base to set the constraints and requirements before designing an exoskeleton dedicated to home treatment.

Introduction. The Constitution of 1997 of the Republic of Poland guarantees all citizens the equal right to healthcare benefits financed from public funds. The National Health Fund (NFZ), being the main payer in the system, is responsible for contracts with both public and private healthcare providers. Patients with healthcare insurance are entitled to guaranteed healthcare benefits in accordance with the current medical knowledge within the limits of NFZ funds available. The aim of the thesis is to analyze the availability of the guaranteed healthcare benefits in medical rehabilitation in the Podkarpacie province. Materials and methods. The research includes information and data on amounts of money for healthcare contracts related to medical rehabilitation in the Podkarpacie province from July 1, 2014 to June 30,2017. The information is posted on the website of the Rzeszow Podkarpacie Branch of The National Health Fund. Results. The analysis conducted indicates that in the Podkarpacie province there are significant statistical differences in the distribution of funds for ambulatory physiotherapy and medical rehabilitation care in individual districts. Conclusions. It is necessary to increase investment funds and a change in the distribution, increasing access to rehabilitation to those in need

Thermal waters belong to the greatest treasures of Podhale. They are used not only in heat engineering but also in recreation, medicine and rehabilitation (Kępińska 2004, 2009, Tomaszewska 2009). It is also known that thermal waters are treated and used for drinking or commercial purposes (Bujakowski & Tomaszewska 2007, Tomaszewska 2009, 2011, Tomaszewska & Bodzek 2013). Thermal waters in Bańska Niżna are exploited by PEC Geotermia Podhalańska S.A. Company. Commercial use of these waters began in 1993, when the system based on geothermal doublet, supplied heat to several houses (Ślimak & Wartak 2009). Currently, the exploitation is made by using three wells: Bańska IG-1 well, Bańska PGP-1 well and Bańska PGP-3 well. Hydrogeochemical characteristic of thermal waters in Bańska Niżna was based on researches concerning the state of those waters, which was being carried out every two weeks between December 2013 and August 2015. Field research was based on measurements of non-stable parameters – pH, electrical conductivity γ 25 and the temperature on the flow. The results of field measurements were supplemented with archival data: water temperature at the well head, pressure and well discharge. The samples for laboratory tests were taken by the author of this work in accordance with the standard ISO 5667-11:2004. The chemical composition analysis of thermal waters was carried out by accredited Hydrogeochemical Laboratory of the Department of Hydrogeology and Engineering Geology AGH in Kraków (certificate of Polish Centre of Accreditation No. AB 1050). This analysis included measurements of concentration of 45 indicators. On the basis of the results of the research, the hydrogeochemical type of thermal waters in Bańska Niżna was defined as sulfate-chloride-sodium-calcium type (according to the classification by Shchukarev–Priklonskiy (Prikłoński & Łaptiew 1955)). The medium values of the basic parameters of analyzed wells, estimated on the basis of data from 2013–2015, are as follows: - Bańska IG-1 well: pH 6.63, γ 25 3.48 mS/cm, temperature at the well head 78.5°C, pressure 11.8 ba, well discharge 58.1 m 3 /h, mineralization 2522.8 mg/dm 3 ; - Bańska PGP-1: pH 6.69, γ 25 3,39 mS/cm, temperature at the well head 85.9°C, pressure 14.4 ba, well discharge 317.3 m 3 /h, mineralization 2533.6 mg/dm 3 ; - Bańska PGP-3: pH 6.72, γ 25 3.43 mS/cm, temperature at the well head 85.0°C, pressure 14.0 ba, well discharge 221.3 m 3 /h, mineralization 2514.3 mg/dm 3. The database from the years 2013–2015 was supplemented by archival results of chemical composition analyzes. On that basis, the assessment of the thermal waters chemical composition stability in Bańska Niżna was prepared, according to the scheme presented in work (Kmiecik & Korzec 2015). It is especially important because physical parameters and chemical composition of geothermal waters can be change during the long-term exploitation. According to Polish law, confirm stability of the physico-chemical parameters is necessary because using these waters in medicine (Dz.U. 2011, nr 163, poz. 981 art. 5). Moreover, the hydrogeochemical model of formation of those waters was defined.

This paper deals with mechanical and hardware design of a robot, used for the rehabilitation of upper extremities. It has been called ARR-1 (Arm Rehabilitation Robot). The robot has a semi-exoskeleton structure. This means that some parts of the robot fit closely to the human arm (an orthosis), but the weight of the construction does not load patient’s body. The device is used for the whole arm rehabilitation, but active joints are only situated in glenohumeral and elbow joints. The robot is electrically actuated.
Artykuł opisuje mechanikę oraz część sprzętową systemu sterowania robota przeznaczonego do rehabilitacji kończyny górnej. Robot został nazwany Arm Rehabilitation Robot - ARR i charakteryzuje się strukturą semi-egzoszkieletalną. Oznacza to, że część aktywna przylega do ciała pacjenta, a jego struktura kinematyczna przypomina kończynę górną, ale waga robota nie obciąża pacjenta. Urządzenie może być używane do rehabilitacji całej kończyny górnej, ale aktywne stopnie swobody znajdują się w stawach barkowo-obojczykowym oraz łokciowym. Robot jest napędzany silnikami elektrycznymi.

At the end of the XX century significant changes in the rehabilitation practice took place. The traditional, medical model of rehabilitation was replaced with the social model. The development of rehabilitation practice required modifications in the approach to the problems of the handicapped. Therefore, the article discusses the results of research which defined life problems experienced currently by the handicapped. As a consequence of the conducted analysis, a proposition has been presented which suggests that the process of adjusting to disability should be interpreted as an individual and linear sequence of changes which lead to varied rehabilitation outcomes. On the basis of this model the article proposes specific principles of influencing psychologically the handicapped in order to increase their involvement in the rehabilitation process.

The aim of this paper is to analyse whether the medical rehabilitation segment is an important part of the entire Polish healthcare system, and if the medical rehabilitation services are provided with adequate levels of financing and management. The study reviews published literature and legal acts, and undertakes an analysis of data acquired from international and national health data repositories. In Poland there exists no coordination between medical, vocational and social rehabilitation or between the rehabilitation delivered by the health resort facilities. There is an observed lack of coordination among public fund payers. The described lack of coordination influences not only patient treatments (it is difficult to measure outputs and outcomes), but also makes summarizing the total expenditures on curative rehabilitation more difficult. Even though numerous countries spend a smaller or comparable amount of money on rehabilitation (per patient), funds allocated to rehabilitation in Poland (expressed in PPS) are over seven times lower than in France, about five times lower than in Austria and Belgium, and three times lower than in the Netherlands.

Objectives Legislators and policymakers have expressed strong interest in intervention programs to reduce dependence on social disability benefits. Hybrid: ambulatory followed by home-based cardiac telerehabilitation – hybrid cardiac rehabilitation (HCR) seems to be a novel alternative for standard cardiac rehabilitation for patients with cardiovascular diseases (CVD) as a form of pension prevention paid by the Social Insurance Institution (SII). The kind of professional status may bias the motivation to return to work after HCR. The aim of our study was to evaluate whether the professional status can affect the effects of HCR. Material and Methods One hundred fifty-two patients with CVD referred by the SII for a 5-week HCR were qualified for the study. Patients (87.7% males), aged 57.31±5.61 years, were divided into 2 subgroups: W) white-collar employees (N = 22) and B) blue-collar employees (N = 130). To evaluate functional capacity, an exercise test on a treadmill was used. Results The number of days of absence in the cardiac rehabilitation program did not differ between the groups (mean ± standard deviation – B: 1.09±3.10 days, W: 1.95±3.64 days). There were significant improvements (p < 0.05) in measured variables after HCR in both (W and B) groups (max workload: 8.21±2.88 METs (measured in metabolic equivalents) vs. 9.6±2.49 METs, 7.76±2.51 METs vs. 8.73±2.7 METs, resting heart rate (RHR): 77±16.22 bpm vs. 69.94±12.93 bpm, 79.59±14 bpm vs. 75.24±11.87 bpm; double product, i.e., product of heart rate and systolic BP (DP rest) 10 815.22±2968.24 vs. 9242.94±1923.08, 10 927.62±2508.47 vs. 9929.7±2304.94). In group B, a decrease in systolic blood pressure (BP syst. – 137.03±17.14 mm Hg vs. 131.82±21.13 mm Hg), heart rate recovery in the 1st minute after the end of peak exercise (HRR1) (99.38±19.25 vs. 93.9±19.48) and New York Heart Association (NYHA) class (1.22±0.53 vs. 1.11±0.36) was observed. In group W, a decrease in diastolic blood pressure (BP diast.) at rest was observed (88.28±9.79 mm Hg vs. 83.39±8.95 mm Hg). The decrease in resting HR was significantly greater in group W (69.94±12.93 vs. 75.24±11.87, p = 0.034). Conclusions Hybrid cardiac rehabilitation is feasible and safe with high adherence to the program regardless of the patient’s professional status. Professional status did not influence the beneficial effect of HCR on exercise tolerance.

Background: Craniocerebral injuries are one of the most common causes of mortality and disability in Poland. The treatment of patients who are in an intensive care unit is based primarily on stabilizing the patient’s general condition as well as basic duties according to the patient’s functioning. Aim of the study: The aim of this study is to demonstrate the importance of early rehabilitation and the role of physiotherapy in recovery after craniocerebral trauma. Case report: The subject was an 18-year-old patient who suffered craniocerebral trauma as a result of a road accident. After losing consciousness, he was in the intensive care unit, where he was placed on a medical ventilator. A properly selected physiotherapeutic procedure was performed. Passive exercises, contracture correction and appropriate positioning were used. To prevent pressure sores, anti-bedsore prophylaxis was implemented. Respiratory therapy played a key role. The goal of respiratory physiotherapy was to improve respiratory function by maintaining proper lung ventilation, increasing chest and diaphragm mobility along with maintaining the efficiency of respiratory muscles, as well as stimulating effective coughing and evacuation of secretions. The NDT-Bobath concept was used as therapy for spastic tension. The goal of the therapy was to get rid of pathological movement patterns and replace them with physiological patterns. The PNF method, classical and lymphatic massage, polysensory stimulation and music therapy were also used. Conclusions: Early and comprehensive rehabilitation in a patient after craniocerebral trauma is extremely important and determines therapeutic effectiveness. Comprehensive therapy and care are able to prevent a number of complications that threaten the patient as a result of immobilization.

Introduction. Airway clearance techniques are an essential part of routine respiratory physiotherapy, enabling bronchial secretion clearance—the mucus overproduction and retaining results in lung function deterioration and disrupts effective pulmonary rehabilitation. Several mucus clearance methods are included in the physiotherapy daily routine of patients with chronic lung conditions; nevertheless, new techniques and approaches are continuously developed. Aim. Thus, this systematic review summarizes novel airway clearance techniques applied in patients with chronic pulmonary conditions. Material and methods. The PubMed, Cochrane Library, and PEDro databases were searched from 2010 to 2021, and studies were selected based on eligibility criteria. Analysis of the literature. 101 patients from five studies describing four different techniques were included. Novel techniques were non-invasive ventilation, intrapulmonary percussive ventilation, trachea vibration, and PEP-sound wave combination. Significant improvements were noted for ventilation homogeneity (NIV), saturation (NIV), respiratory rate (IPV), and diffusion capacity (VL), whereas cardiovascular function and exercise endurance did not change significantly. Conclusion. The presented methods are considered to have similar effectiveness as well-known airway clearance techniques. However, the systematic use of presented methods in routine pulmonary rehabilitation must be preceded by in-depth investigation to provide no-bias results.

Introduction. Post-traumatic epilepsy develops as a complication of a serious craniocerebral trauma, frequently an open head injury, resulting in neurological impairments. Aim. The study was designed to discuss problems associated with sensory integration dysfunction observed in a seven-year old girl with epilepsy which occurred as a result of craniocerebral injury. Description of the case. The case study is based on information gained from the girl’s medical records, an interview with her guardians and a sensory function questionnaire. Additional examinations assessed the girl’s balance control, and her functional performance according to Barthel scale. The girl received a three-week sensory integration therapy which included exercise involving tactile, vestibular and proprioceptive stimulation, as well as balance exercises and self-care training. Conclusions. In this particular case, application of sensory integration therapy produced beneficial results. The findings show improvement in balance, coordination as well as self-care.

Opis:
In the paper, the problems of devices supporting functional rehabilitation of a human wrist were addressed. A literature review and a description of selected devices together with an indication of their advantages and disadvantages were conducted. The biomechanical structure of a human wrist was analyzed. On this basis and after taking into consideration ranges of motion of the selected joints the concept of a new mechanism was developed. A 3D model of the device was built in the Autodesk Inventor system. For the purpose of simulations another model was developed in the MSC Adams system. Issues of drives and sensors selection, as well as requirements for the control system, were examined.
