Opis:
Five families with the Lesch-Nyhan syndrome (LNS) and two families with the Kelley-Seegmiller syndrome (KSS) were studied. Seven different mutations were identified. Two transitions, C526→ T (Prol76Ser) and G481→A (Ala161Thr), in patients with a milder form of hypoxanthine-guanine phosphoribo-syltransferase (HPRT) deficiency were detected. In patients with the Lesch-Nyhan syndrome two transitions, G569→A (Glyl90Glu) and C508→T (Arg170Ter), two transversions, C222→A (Phe74Leu) and C482→A (Ala161Glu), and a deletion of seven nucleotides (from A394 to G400) were observed. All except two of the identified mutations are novel. The C222→A substitution in exon III is located within one of the clusters of hot spots of the HPRT gene and has been previously described in four unrelated patients. The other recurrent mutation C508→T in exon VII has been reported in eight families.

Opis:
The aim of the article is to present unmet expectations and needs of patients concerning the scope and quality of medical care. The complaints submitted with the Patients Ombudsman at the Ministry of Health have been deemed as exemplary, since this is a central authority of governmental administration relevant in cases regarding the protection of patients specified in the act, which encompasses the entire country in the scope of its powers. This is also the authority which has most often appeared in the mass media, hence it is the most probable thatthe patients will address exactly this institution with any problem they may have (very often without knowing any other way or procedure). The aim of the article is also to present selected strategies assumed by thepatients to allow them to solve the encountered difficulties. Recipients of benefits more and more effectively manage and undertake effective actions aimed at obtaining assistance to the extent they expect. They adopt attitudes proving the assumption of responsibility for their own health (empowered), which is proved even by their Internet activity to the extent of which they search for information on e.g. disease prevention, treatment, application of medicines or good nutrition (however, the conclusions in this regard one cannot generalize interms of the whole population of people enjoying medical benefits). In addition, the article presents also a process of changes with respect to the attitudes towards the value of health and changes occurring in the profile of the modern patient. Presentation of the selected problem area was made on the basis of the results of qualitative analysis of the scope and nature of the complaints submitted with the Patients’ Ombudsman (2009–2013) as well as a selective review and survey of the literature and its analysis.

Opis:
Purpose. The article presents an assessment concerning patient satisfaction with anesthesia as based on the Polish version of the Iowa Satisfaction with Anesthesia Scale. Material and Methods. The study group consisted of 198 patients with maxillofacial injury admitted to the Clinical Ward of Maxillofacial Surgery. The quality of the anesthesiological care was evaluated with the Polish version of Iowa Satisfaction with Anesthesia Scale. Results. It was stated that the level of satisfaction with the anesthesia in patients operated on due to maxillofacial injury used was average. According to the Polish version of the Iowa Satisfaction with Anesthesia Scale, the average score in the studied population was 0.8 on a scale from -3 to +3, SD 2.41. There were differences observed depending on patient age (18-30 years old (p = 0.0001)) and clinical condition. Conclusion. The level of satisfaction with anesthesia in patients with craniofacial trauma is moderately positive, however, in patients with an injury of the upper face and in patients with ASA scale I and II, the same level of satisfaction is higher. Among the analyzed socio-demographical factors only the age determines the level of satisfaction with anesthesia. The level of satisfaction is higher in older patients.

This paper explores a range of health care seeking actions adopted by Polish migrants living in London. It is based on the in-depth interviews with 62 Polish migrants who resided in London in 2007 and 2008. The study reflects experiences of a diverse group of participants encompassing individuals of different ages, family circumstances and employment statuses. It uncovered a number of actions such as avoiding contact with any health services, self-medication, utilising Polish private doctors in London, and accessing public health services in Poland and London. These services and strategies were often used successively, concurrently or interchangeably depending on individual’s assessment of their situation and circumstances at the time of experiencing a particular medical need. The research uncovered a prominence of transnational health care seeking practices as most participants continued to access at least some form of health care in Poland. It also noted the impact of the length of stay, knowledge of English and labour market position on patterns of health care use. Engagement with the British National Health Service (NHS) was characterised initially by lack of knowledge of the system resulting in meandering between different institutions. There was also a sense of cultural unease and mismatch of expectations characterising doctor-patient encounters. However, the pathways of access and attitudes towards the NHS began to change as migrants learned their way through the English health care system. This underlines an importance of taking into account a longitudinal dimension in studies of migrant health care seeking behaviour.
