The goal of upbringing is affirmation of the student, self-development within the system of values that is recognized by a group and eventually interiorized. A correctly shaped hierarchy of values, which fulfills a regulatory and motivating role, provides favorable conditions for maturation of a young person, gaining autonomy and at the same time integration with society. In the article the results of the research conducted to identify the system of values of youth with hearing impairments are discussed. The position of responsibility, whose essence consists in regulating one’s free conduct, was identified within the hierarchy of values. Responsibility is an instrumental value that is relatively highly valued by youth (rank 3 for hearing youth and rank 4 for youth with hearing impairment), which is a rewarding fact.

Introduction: Thanks to the Polish Universal Neonatal Hearing Screening Program (PUNHSP), all newborns in Poland undergo a free, screening hearing examination. Between 2006 and 2015, the average number of tested children per year was 373,477. According to the analysis of The Central Database (CDB), only 55.8% of the children attended the detailed hearing examinations at the second level of the Program. Aim: The aim of this study is to analyse the dates concerning the attendance of the children at the diagnostic level of PUNHSP in different regions of Poland. Materials and methods: To conduct an analysis of this fact and find out the reasons for low attendance at the second level in 2015, a telephone survey questionnaire was developed for parents who had not registered their babies for further consultation – 3,239 randomly selected parents. Results: The analysis revealed that the number of children examined at the second diagnostic level of the program is in fact much higher than the results of The Central Database show. The actual number is 83.6% as opposed to 55.8%. As a result of the telephone questionnaire some inaccuracies in the input data to the CDB were detected. The main errors in gathering the information for the CDB were incorrect OAE test result and no examination performed. C onclusion: In Poland the worst results (i.e. questionnaire results compared to CDB) for the attendance at the diagnostic level were shown in Pomorskie, Lubelskie, Mazowieckie and Podlaskie regions. In many cases there was a large discrepancy between the reality and the information in the CDB. The improvement of clarity concerning the CDB application is important in order to minimise the possibility of malformation in the CDB.

The object and aim of the undertaken surveys was to assess the modifying impact of a child’s language development (independent variable) on selected areas of their mother’s functioning-the appearance of symptoms of depression, and concentration on the child’s disability (dependent variable). Surveys in a group of the same 30 dyads (mother-child) connected with the Association of Family and Friends of Children with Hearing Impairment in Krosno were conducted in 2007 and repeated after 9 years. Analysis of the test data (n=60) confirmed the hypothesis that the appearance of symptoms of depression in mothers is less frequent and a mother’s perception of a child’s hearing impairment as burdensome decreases alongside the development of a child’s language competence. The article is critical of the results of surveys conducted by other researchers. The authors also analyse other factors, including the Universal Neonatal Hearing Screening Programme, place of residence, quality of specialist treatment and therapy, and the passage of time, which may influence change in the functioning of mothers of children with hearing impairment.

A questionnaire survey was conducted in the residential quarters of Guangzhou, for which 582 elderly people over 60 years old were randomly recruited. The hearing impairment of the participants was evaluated using the Hearing Handicap Inventory for the Elderly (HHIE). The participants’ subjective responses to the acoustical environment of their living place and the impact of the living acoustical environment (LAE) on the participants were investigated. The results show that the participants with a low HHIE score and no hearing impairment evaluated their LAE more favourably, and they considered that the effect of the LAE on their daily life was weak. However, those with a high HHIE score and severe hearing impairment evaluated their LAE poorly, and considered its effect on their daily lives to be significant. For the elderly, the worse the hearing is, the higher their demand for a better LAE. Traffic noise, construction noise, noise from residential quarters, and noise from next door or upstairs neighbours were the main noise sources in the elderly’s living places, and traffic noise, construction noise, and noise from next door and upstairs were the most influential sources. 28.9% of the respondents had trouble hearing what their family said in their living place. The elderly without hearing impairment considered that continuous noise was the main reason that they could not hear what their family said in their living place, while those with hearing impairment believed that their own hearing problem was a contributing factor.

The paper presents the results of sentence and logatome speech intelligibility measured in rooms with induction loop for hearing aid users. Two rooms with different acoustic parameters were chosen. Twenty two subjects with mild, moderate and severe hearing impairment using hearing aids took part in the experiment. The intelligibility tests composed of sentences or logatomes were presented to the subjects at fixed measurement points of an enclosure. It was shown that a sentence test is more useful tool for speech intelligibility measurements in a room than logatome test. It was also shown that induction loop is very efficient system at improving speech intelligibility. Additionally, the questionnaire data showed that induction loop, apart from improving speech intelligibility, increased a subject’s general satisfaction with speech perception.

Introduction. The aim of the study was to evaluate the neck torsion test in objective examinations of patients with vertigo and/or hearing loss. Material and methods. The study was conducted in 100 patients, including 54 women and 46 men aged 17–79 years, who were divided into two groups: I – 50 patients, including 30 women and 20 men aged 17–79 years (mean age 49.92 years) with dizziness and/or hearing impairments, and confirmed asymmetry of intracranial vessels, II – 50 patients – the control group, including 24 women and 26 men aged 20–71 years without dizziness and/or hearing disorders and without disturbance in the construction of intracranial vessels. For each patient, the following tests were carried out: subjective, objective otorhinolaryngological, Doppler ultrasound specifying the diameter of vertebral and carotid arteries and the velocity of blood flow in these vessels, audiological diagnostics, including the examination of latency of waves I, III, V of the auditory evoked potentials of the brain stem, otoneurological diagnostics with used the neck torsion test. Results. It appears from the analysis of the material presented that the application of the neck torsion test in the Doppler ultrasound results in the fact that the difference in the mean systolic velocity of blood flow in vertebral artery is higher on the side opposite to the turning of the neck, and the increase in the average diastolic blood flow velocity in the vertebral artery on the side of the test being performed and its reduction on the opposite side in the study group, when compared to the control group. The value of the wave I, II, V latency in the ABR test during the neck torsion test is extended more in the study group than in the controls, on the side of the performed test. The performed neck torsion test in the VNG test increases the occurrence of both, square waves and nystagmus (much higher in the study group than in the controls). Conclusion. The application of the neck torsion test in the Doppler ultrasound, ABR and VNG test in patients with vertigo and/or hearing loss means that these tests become functional, thereby increasing their diagnostic value and may be used to monitor the rehabilitation of inner ear disorders.

Advanced age is often burdened with many deficits that are a consequence of the aging process, unfavorable lifestyle and multi-morbidity. They contribute to increased morbidity and disability of the older people. Laryngological problems often concern seniors, particularly progressing with age hearing impairment, dizziness, balance disorders, epistaxis, nasopharyngeal diseases, larynx, sinuses, and salivary glands disorders. They are not only a health threat, such as head and neck cancer, but they contribute to the deterioration of mobility, falls and injuries, depression, cognitive functions impairment and, consequently, functional disability, loss of independence and a sense of isolation. In this context, taking as an example the most common ailment, which is hearing impairment, laryngological procedures in the form of the use of hearing aids or cochlear implant improves cognitive functions, mood, self-esteem, opportunities for social interaction and everyday functioning, and the quality of life of older people. In turn, in view of the typical for older age polypathology and of significant reduction of the reserves of the organism, effective treatment aimed at improving health, preventing complications of diagnostics and therapy as well as disability with the desire to maintain independence and good quality of life requires the cooperation of different specialists. The knowledge and experience of geriatricians and the comprehensive geriatric assessment used as the diagnostic tool, aimed at identifying deficits typical of seniors’ age may significantly contribute to a more adequate risk and benefit assessment, selection of optimal treatment for a given health situation and identification of high-risk individuals requiring during the treatment period special supervision to reduce the risk of complications and the risk of development of geriatric syndromes such as delirium, cognitive disorders, malnutrition, falls and injuries, functional disability.

Dysfunctions of the organ of hearing are a significant limitation in the performance of occupations that require its full efficiency (vehicle driving, army, police, fire brigades, mining). Hearing impairment is associated with poorer understanding of speech and disturbed sound localization that directly affects the worker’s orientation in space and his/her assessment of distance and location of other workers or, even most importantly, of dangerous machines. Testing sound location abilities is not a standard procedure, even in highly specialized audiological examining rooms. It should be pointed out that the ability to localize sounds which are particularly loud, is not directly associated with the condition of the hearing organ, but is rather considered an auditory function of a higher level. Disturbances in sound localization are mainly associated with structural and functional disturbances of the central nervous system and occur also in patients with normal hearing when tested with standard methods. The article presents different theories explaining the phenomenon of sound localization, such as interaural differences in time, interaural differences in sound intensity, monaural spectrum shape and the anatomical and physiological basis of these processes. It also describes methods of measurement of disturbances in sound localization which are used in Poland and around the world, also by the author of this work. The author analyzed accessible reports on sound localization testing in occupational medicine and the possibilities of using such tests in various occupations requiring full fitness of the organ of hearing.

INTRODUCTION: Lyme borreliosis (LB) is the most common tick-borne human disease. The objective of the study was to carry out preliminary evaluation of the incidence of hearing impairments in LB patients. MATERIAL AND METHODS: Audiometric tests were carried out on 66 patients with diagnosed LB (aged between 18 and 45 years). All the women and men were divided into two major groups: 1 – early, disseminated LB and 2 – late LB; among them subgroups of Lyme neuroborreliosis (LNB) were distinguished. The hearing organ was diagnosed on the basis of tonal audiometry, extended high-frequency tonal audiometry (up to 18 kHz), impedance audiometry and speech audiometry tests. RESULTS: Abnormal results of the audiometric tests were recorded in 66.7% of patients. The statistical analysis indicates a significant role of borrelial infection (p = 0.017). Most of those were in patients suffering from late stages of the disease, both in patients with LB and LNB. CONCLUSIONS: The study revealed the occurrence of hearing impairment of different degrees in more than one half of the patients with LB, especially in the late stages of the disease. The results indicate the necessity for further comprehensive studies, considering hearing and balance disorders in the course of LB, comprising larger groups of patients.
WSTĘP: Borelioza z Lyme (Lyme borreliosis – LB) jest najczęstszą chorobą odkleszczową u ludzi. Celem pracy było przeprowadzenie wstępnej analizy występowania zaburzeń słuchu u chorych na LB. MATERIAŁ I METODY: Testy audiometryczne objęły 66 chorych z rozpoznaną LB, w wieku od 18 do 45 lat. Wszyscy badani (kobiety i mężczyźni) zostali podzieleni na dwie główne grupy: 1 – wczesnej, rozsianej LB i 2 – późnej LB; wśród nich zostały wyodrębnione podgrupy pacjentów z rozpoznaniem neuroboreliozy (Lyme neuroborreliosis – LNB). Diagnostykę narządu słuchu przeprowadzono na podstawie badań audiometrii tonalnej, audiometrii tonalnej rozszerzonej (o wysokiej częstotliwości do 18 kHz), audiometrii impedancyjnej i audiometrii mowy. WYNIKI: Nieprawidłowe wyniki testów audiometrycznych stwierdzono u 66,7% badanych, a analiza statystyczna wskazała na istotny związek z zakażeniem Borrelia burgdorferi (p = 0,017). Większość stanowiły osoby z późną LB, zarówno bez, jak i z LNB. WNIOSKI: Zaburzenia słuchu różnego stopnia występują u ponad połowy chorych na LB, zwłaszcza w późnych stadiach procesu chorobowego. Uzyskane wyniki wskazują na konieczność prowadzenia dalszych wszechstronnych badań uwzględniających uszkodzenie narządu słuchu i równowagi w przebiegu LB, obejmujących większe grupy chorych.

The article presents results of the work on creation of an application for Android mobile devices to perform hearing screening using pure-tone air conduction audiometry. The application was created in accordance with the procedure defined in the ISO 8253-1: 2010 standard and is to serve the widest possible group of people in a domestic environment, using headphones included with mobile devices. The effectiveness of the application was verified by performing comparative hearing tests on a representative group of patients with different hearing status. Validation was performed using the Interacoustics AD629 clinical audiometer and procedures adopted in the standards. The results of the test comparison were satisfactory. The mean error of the hearing threshold tested using the mobile application, compared to the clinical audiometer was 5.2 dBHL. In addition, the hearing threshold curves for the mobile application are similar in shape to the hearing thresholds for the clinical audiometer and coincide with the auditory experiences of the patients.
Artykuł przedstawia wyniki prac nad stworzeniem aplikacji na urządzenia mobilne z systemem Android w celu wykonania przesiewowych badań słuchu w zakresie progowej audiometrii tonalnej przy przewodnictwie powietrznym. Aplikacja została stworzona zgodnie z procedurą określoną w normie ISO 8253-1:2010 i ma służyć jak najszerszej grupie osób w domowych warunkach badań przy wykorzystaniu słuchawek dedykowanych urządzeniom mobilnym. Działanie aplikacji zostało zweryfikowane poprzez wykonanie porównawczych badań słuchu na reprezentatywnej grupie pacjentów o zróżnicowanym stanie słuchu. Walidację przeprowadzono przy użyciu audiometru klinicznego Interacoustics AD629 i procedur przyjętych w normach. Wyniki badań porównawczych okazały się zadowalające. Średni błąd zbadanego progu słyszenia przy pomocy aplikacji mobilnej, w porównaniu do badań przeprowadzonych przy pomocy audiometru klinicznego, wyniósł 5.2 dBHL. Dodatkowo, krzywe progu słyszenia dla aplikacji mobilnej są zbliżone w kształcie do krzywych progu słyszenia dla audiometru klinicznego i pokrywają się z wrażeniami słuchowymi pacjentów.

Pure-tone audiometry (PTA) and transient-evoked otoacoustic emissions (TEOAEs) were determined in 57 classical orchestral musicians along with a questionnaire inquiry using a modified Amsterdam Inventory for Auditory Disability and Handicap ((m)AIADH). Data on musicians’ working experience and sound pressure levels produced by various groups of instruments were also collected. Measured hearing threshold levels (HTLs) were compared with the theoretical predictions calculated according to ISO 1999:1990. High frequency notched audiograms typical for noise-induced hearing loss were found in 28% of the subjects. PTA and TEOAE consistently showed a tendency toward better hearing in females vs. males, younger vs. older subjects, and lower- vs. higher-exposed to orchestral noise subjects. Audiometric HTLs were better than theoretical predictions in the frequency range of 2000–4000 Hz. The (m)AIADH scores indicated some hearing difficulties in relation to intelligibility in noisy environment in 26% of the players. Our results indicated a need to implement a hearing conservation program for this professional group.

Opis:
Identification of factors that affect postural stability may help to improve diagnostic accuracy and enhance the quality of treatment and rehabilitation. This study sought to assess the relationship between postural stability parameters and anthropometric factors of persons with hearing impairment (HI). Methods: The study included 128 individuals – 42 subjects with HI and 86 without HI (healthy controls). Research methodology included an interview and a medical examination, anthropometric measurements and stabilometric tests on platforms with stable and unstable surfaces. Results: In the group of female study participants with HI, significant correlations were only noted between body height and the Fall Risk Index (FRI). In the group of male subjects with HI, the study revealed significant correlations between FRI and body mass, BMI, % MM (muscle mass percentage) and % FAT (fat percentage). Moreover, moderate correlation was found between COP path with eyes open and body mass, while high correlation was observed between COP path with eyes open and BMI, % MM and % FAT. No significant correlation was noted between FRI and body height in men with HI. Conclusions: The examination of correlations between postural stability and body build of persons with HI did not confirm the effects of body height on postural stability in the examined group of individuals with HI, but revealed a greater influence of somatic parameters (body mass, BMI, % MM, % FAT) on postural stability in hearing-impaired men.

Opis:
The article discusses the importance of effective communication and informational support for parents of children with hearing impairment who are in difficult situations. I present three cases: a young parent of a child with hearing impairment; parents and children with genetic deafness (the “Deaf” family); and a child with multiple disabilities, including hearing impairment. Based on these cases, I discuss general guidelines of how to give effective help to patients in difficult situations, which can occur in the practice of physicians and speech therapists. In the last part of the article, I show a roadmap for effective communication between healthcare professionals and patients with hearing impairment. The roadmap was developed based on the classic Jacob’s theory of speech acts.
