Polish patients pay immediately for single services and may also privately conclude the contracts for medical insurance or subscription (steady access to complex private medical care). The aim of this paper is to draw the reader's attention to the possibilities a Polish patient has of obtaining complex private medical care. The paper describes the range of services which are guaranteed for all patients insured by National Health Fund (financed from public resources) and contracts of private health insurance and medical subscription.

Theoretical background: Quality management and medical service improvement in district hospitals enables to achieve a number of internal and external benefits. It is a long-term process of conscious transformation of the existing state into a desired and accepted one by the concerned stakeholders. The diagnosis of the medical service organizations’ performance is an important aspect in the context of building a hospital of the future which is focused on quality, development and improvement. Purpose of the article: The article reveals the positions of hospital managers (directors) on the current level of the quality management maturity service improvement of the medical facilities they manage. Research methods: A diagnostic survey was conducted using a paper questionnaire. The form was handed in person and filled in the presence of the author. This provided an inducement for analysis and conclusions. The study was performed in 2022. Main findings: The level of implementation of the quality management system (QMS) principles in the surveyed hospital organizations, despite the variability, uncertainty, complexity and ambiguity of the environment, was evaluated at 3.39. This means that the practice of the QMS is widely established, but not in most areas. Therefore, the scope of the principles cannot be considered as complete. The result is different for the assessed maturity of quality excellence in the selected accreditation standards. The surveyed district hospitals evaluated their maturity level at 3.69. Thus, the respondents considered that the maturity level to be close to 70%. They identify the practice as typical (only some exceptions are deviations from it). The surveyed hospitals also have evidence of continuous improvement after at least one year. Another important aspect concerned maturity research in relation to innovation. The surveyed entities determined this parameter at the level of 3.50, which corresponds to approximately 66%. The highest level of maturity achieved the hospitals in the operational skills in conditions of disruption. They assessed the adaptation abilities and taken actions towards sustainable development at the level of 3.82 (72%). These results prove that the district hospitals in the Wielkopolska voivodeship take steps toward excellence, but their level of excellence in 2022 is not full (as it does not apply to 100% of occurrences, and the practice is also not implemented in entire hospitals, without exceptions).

This review includes information about the number of private health insurance and medical subscriptions in Poland, as well as the level of increases in premium rates, and the scale of health promotion and prevention conducted by Polish employers. The aim of this article is to present the current situation on the supplementary private medical market, which is involved in employees’ healthcare, and the challenges that both employers and medical providers with health insurers are now facing, and also to present the reasons why prevention and health promotion are important factors of healthcare under private health insurance. For the purpose of this review, scientific publications devoted to health promotion at the workplace were used, along with statistical data presented in the studies released by the Polish Insurance Association, the Polish Social Security Institution, the World Health Organization and the Organization for Economic Co-operation and Development. The literature search was carried out using the electronic databases of PubMed. Search terms included medical subject headings and free text words. No year of publication restriction was imposed. The conducted analysis shows that the demand for private medical care is rising. Employers are willing to invest in private medical care for their employees, although the scale is much lower among small and medium-sized entrepreneurs due to fiscal burdens. Given the rising demand and an insufficient number of specialists, access to medical services is deteriorating, and the premium rates and costs are rising. More employers are investing in health promotion at the workplace in order to decrease absenteeism and presenteeism, lower the utilization of medical packages, and improve their attractiveness on the market. Although the interest in private medical care is rising, employers should focus on health promotion and prevention at the workplace, and adapt their actions to the employees’ current needs and health issues. The health care system in Poland or any other country should ensure an equal and unrestricted availability of the health service to its population. This idea, though being noble and harmonious with the country’s constitution, seems to differentiate small administrative regions of the Silesian voivodeship. Therefore the presented paper is an attempt to estimate and analyse conditions of the health care system activity in this region. Those regions were systematized based on a structure of health care system. Also, a homogeneous groups of regions were created. The analysis is based on the cluster analysis.

Research background: This research focused on identifying attributes of tourism services which are guided by a responsible vision and which seek to achieve consumer satisfaction with products that respect sustainability principles. Responsible consumer choices were defined as those formed by an orientation toward sustainable local food and drink, health-related services, and entertainment. Purpose of the article: This research had two aims. The first was to create and validate a measurement scale assessing tourists' motivations with regard to three responsible tourism service dimensions. The second was to evaluate how tourists' responsible choices affect their satisfaction. Methods: The methodology included exploratory factor analysis, confirmatory factor analysis, and structural equation modeling to test the hypothesis developed based on a literature review. The convenience sample was made up of tourism service users. Findings & value added: The results include a broad measurement tool that can be applied in other fields of research to detect which variables influence consumer satisfaction. The proposed model incorporates significant determining factors, namely, key aspects affecting tourism service selection by clients focused on sustainability and responsible consumption. Based on a market orientation (MO) perspective, the findings contribute to the existing literature on stakeholder theory (ST) and dynamic capability theory (DCT). The value added comprises a better understanding of responsible tourism consumers' choices based on a three-part theoretical framework (i.e., MO, ST, and DCT).

In undeveloped and developing countries, maternal mortality is an important problem in public health. High maternal deaths observed in some regions of Turkey are related to inequities in reaching health services (low transport facilities). To prevent maternal mortality, pregnant women should have access to health services for professional care before, during, and after childbirth. In Turkey, most maternal deaths are related to socio-economic factors, equity issues, health services and transportation facilities. Especially, transportation facilities to health centers play an important role in preventing maternal deaths. Thus, to prevent the negative effect of the winter season and adverse transportation facilities on maternal mortality, the Turkey Ministry of Health (MoH), aiming to reduce the maternal mortality ratio (MMR) in rural areas, initiated the “Guest Motherhood Project (GMP)” to cover all of its urban and rural areas. In this project, health services for pregnant women in risky regions were restructured. In the scope of the project, access problems to health centers were eliminated and rapid access to rural areas and challenging territories of Turkey were provided. Current maternal mortality statistics show that GMP is yielding good results and high quality delivery services at all health centers in every region of the country despite the regional disparities.

Background. Within the medical facilities provided by state healthcare services, a universally applied technique for patient diagnosis and treatment relies on ionising radiation; for example in radiotherapy and X-ray (ie. examination). Human exposure to such radiation is not however entirely free of associated health risks. Objectives. To determine and estimate the numbers and types of X-ray based medical procedures that are performed in general and dental radiography, mammography and computer tomography on patients from the Mazovian province in Poland, which included children, women and men subjects. Material and Methods. The numbers of patient subjects undergoing X-rays was estimated by surveying the patient intake in X-ray testing rooms within the healthcare facilities of the Mazovian province. Questionnaires were either dispatched by mail to such healthcare centres or were completed by the X-ray operating staff during the testing of quality control. Results so obtained from the latter, were compared to entries from the X-ray rooms’ register Results. During 2009, the number of X-rays performed were 7612046 equivalent to 1460 examinations per 1000 inhabitants. The majority were done on women ie. 3847961 (50.55%), followed by 3193781 (41.96%) on men and 570 304 (7.49%) for children. Conclusions. Results indicated that the predominating medical procedure used of this type, was for making general diagnoses; especially through using chest radiography. Others included, in descending order; dental X-ray (mainly intra-oral examination), computer tomography (mainly CT head examinations) and mammography procedures. It was also found that the annual numbers of having X-rays has increased compared to previous years.
Wprowadzenie. W placówkach służby zdrowia powszechnie wykorzystywane jest promieniowanie jonizujące w celach medycznych zarówno w diagnostyce jak i terapii. Promieniowanie to nie jest jednak całkowicie obojętne dla zdrowia. Cel. Celem niniejszych badań było określenie i ocena liczby i rodzaju wykonywanych na terenie województwa mazowieckiego procedur medycznych z zakresu diagnostyki ogólnej, stomatologii, mammografii i tomografii komputerowej, osobno dla dzieci, kobiet i mężczyzn. Materiał i metody. Szacowanie liczby badań rentgenowskich przeprowadzono metodę ankietową. Zbierano dane liczbowe z placówek służby zdrowia przy wykorzystaniu opracowanej w tym celu ankiety. Ankiety przekazywano listownie do wylosowanych placówek służby zdrowia na terenie województwa mazowieckiego lub proszono o ich wypełnienie w trakcie przeprowadzania specjalistycznych testów parametrów fizycznych medycznych aparatów rentgenowskich. W tym drugim przypadku weryfikowano jednocześnie zgodność danych w wypełnianych ankietach w stosunku do zapisów w prowadzonych rejestrach badań pacjentów w pracowniach rentgenowskiej. Wyniki. Oszacowano, iż w 2009 roku na obszarze objętym niniejszymi badaniami wykonano łącznie 7612046 diagnostycznych badań rentgenowskich, co daje 1460 badań na 1000 mieszkańców. Najwięcej badań przeprowadzono u kobiet 3847961 (50.55 %), w dalszej kolejności u mężczyzn 3193781 (41.96 %) i dzieci 570304 (7.49 %). Wnioski. W wyniku przeprowadzonych analiz stwierdzono, iż największy udział w strukturze wykonywanych badań rtg miały procedury medyczne z zakresu diagnostyki ogólnej, w tym przede wszystkim badania klatki piersiowej. Na drugim miejscu znajdują się badania rentgenowskie stomatologiczne, głównie wewnątrzustne (punktowe). W dalszej kolejności pod względem liczby wykonywanych badań rtg uplasowały się procedury medyczne z zakresu tomografii komputerowej, wśród których dominowały badania tomograficzne CT głowy oraz procedury medyczne związane z badaniami mammograficznymi. Zaobserwowano wzrost liczby wykonywanych rentgenowskich procedur medycznych w odniesieniu do danych z wcześniej prowadzonych tego typu badań na terenie Polski.

has dramatically increased. The number of visits to EDs can be an important indicator of the quality of primary health care. Objectives. To analyse: 1) the frequency of admissions to EDs, 2) the type and number of medical procedures performed in EDs, and 3) the type and number of services provided by PHC in the Kedzierzyn-Kozle district. Material and methods. A retrospective analysis of the statistical data regarding services provided by PHC in the Kedzierzyn-Kozle district within the National Health Fund (NHF) and data from the hospital emergency department in Kedzierzyn-Kozle (Opole Province, Poland). Results. The results showed an annual increase in the number of hospitalisations (13,815 – 2012, 14,192 – 2013, 15,123 – 2014) and the number of medical procedures performed in the ED (mainly those from I–III categories), as well as the low rate of admission from the ED to ICU (Department of Anaesthesiology and Intensive Care). There was an increase in the overall number of consultations given by a primary health care doctor and the total number of healthcare services provided at night and during holidays. Conclusions. Reducing the number of non-urgent visits to EDs can be achieved by making efforts to meet the needs of patients at the level of primary health care, in particular through better coordination of services provided by EDs and PHC and better motivation of primary care doctors to perform the role of gatekeeper to the health care system

Oczekiwana długość życia w UE jest jedną z najwyższych na świecie. W rezultacie średnia długość życia i udział osób w wieku 65 lat w populacji również należą do najwyższych na świecie. Fakt ten pociąga za sobą konsekwencje związane zarówno z opieką zdrowotną, jak i z siłą roboczą. Wzrost udziału osób starszych w populacji skutkuje zwiększeniem popytu na usługi opieki zdrowotnej i usługi pokrewne. W miarę wzrostu przeciętnej długości życia siły roboczej i wydłużania się czasu zatrudnienia, będzie rosło zapotrzebowanie na podnoszenie kwalifikacji/przekwalifikowanie, jak również na bardziej elastyczne warunki zatrudnienia. Pomimo ogólnie wysokiej oczekiwanej długości życia różnice pomiędzy regionami pozostają stosunkowo duże. Przyczyny są różnorodne, począwszy od rożnych poziomów dochodu, wykształcenia i warunków życiowych, a skończywszy na nierównym dostępie do wysokiej jakości opieki zdrowotnej. Fakty te uzasadniają podejmowanie skutecznych działań w celu niwelacji powyższych różnic, a narzędziem są środki pomocowe UE. Fundusze strukturalne są dostępne na wszystkich poziomach – od Brukseli przez programy operacyjne do programów regionalnych włącznie. Celem referatu było zaprezentowanie możliwych źródeł finansowania oraz pokazanie dobrych praktyk odnośnie do realizacji kluczowych projektów głównie na przykładzie województwa lubuskiego. Obszarem badań były publiczne placówki ochrony zdrowia w województwie ze Szpitalem Wojewódzkim w Zielonej Górze na czele. Końcowa konkluzja brzmi: mimo znacznych kwot z budżetu UE oraz środków krajowych lokowanych w ochronę zdrowia niezadowolenie społeczne ze świadczonych usług jest ciągle duże.
Life expectancy in EU is one of the highest in the world. In result the average length of life and participation of people at the age of 65 in the population is also one of the highest in the world. This fact entails consequences related to both health care and work force. The increase of the participation of older people in the population results in the growth of the demand for healthcare and related services. With the increase of the average length of life of work force and longer periods of employment, there will be higher demand for the need to improve qualifications or retraining as a well as more flexible conditions of employment. Despite generally high life expectancy, the differences between particular regions remain significant. Various reasons can be identified, ranging from varied levels of income, education and living conditions to unequal access to high quality health services. The above facts justify efficient actions aimed at leveling the differences, and EU aid funds seem to be the appropriate tool. Structural funds are available from Brussels at all levels including Operational and Regional Programs. The aim of the paper was to present various potential sources of financing and demonstrate good practice in respect of realization of key projects mainly on the basis of the example of the Lubuskie province. The research covered the institutions of the National Health Service, including the District Hospital in Zielona Góra. In conclusion it can be observed that despite significant financing from the EU budget and national sources, the level of dissatisfaction with the quality of health services provided remains high.

The article has considered the general principles and capabilities of the medical support of NATO troops, confirmed the expediency of further transformation of tasks and functions of the state in terms of medical support of troops (forces) of the security and defense sector of Ukraine in crises through the approximation to the relevant NATO standards, justified the need to implement the Euro-Atlantic experience of rendering medical care, use the general framework of categories and concepts, principles of the evaluation of capabilities (assets) of medical support which will contribute to the development of integrated medical space for medical defense of troops (forces) and elaboration of individual crisis response model taking into account the basic international requirements. The authors have determined the priority tasks of further cooperation between Ukraine and the North Atlantic Treaty Organization in the context of the reforms of medical logistics. It has been substantiated the need to keep reforming military medicine in conformity with NATO standards and recommendations that will promote an active involvement of medical services of troops (forces) in medical support of national and multi-domestic contingents during crises and adaptation of interoperable medical capabilities of joint missions under the auspices of NATO and the UN. The research has analyzed the mechanisms of interdepartmental coordination on medical support of troops (forces) and proved the feasibility of using the experience of NATO member states in the organization and functioning of the system of medical support of the military contingent with the adaptation of NATO doctrinal documents to the realities of modern Ukraine.

The objective of the study was to investigate the career-choice motives for one’s entering the prison service as an operational officer, and the level of physical fitness. The study was conducted in April 2015, and included 100 officers employed in Biała Podlaska prison. The examined group consisted of 93 males; aged 23-52; mean age 37.2, and 7 females aged 3-43; mean age 37.7 (30-43). 61 (61%) of the males were employed as security guards, while 39 persons (39%), including 7 females, as clerks in the administration section. The study methods included: a physical fitness test consisting of 5 physical performance tests, recommended by the regulation of the Minister of Justice, the International Physical Activity Questionnaire(IPAQ) – long-form, questions designed by the author of the paper. Career-choice motives turned out to be conditioned purely be economic incentives and the professions’ stability as opposed to other professional specialties. The respondents were fit and engaged in numerous physical activates. The factors which significantly determined the level of one’s fitness depended on: age, education, a high self-report on physical fitness, total physical activity and the number of done sports. Originality of the study is based on investigating the condition of physical fitness of prison officers with the use of currently recommended assessment methods in Poland.

The characteristics of medical and sanitary practice and its development during the inter-war period (1919–1939), the role of the state in the fight against infectious and social diseases, the characteristics and the evaluation of the activity of the newly organised Epidemic Hospitals, Health Centres, Hospital Sick-Funds and other institutions providing anti-epidemic measures and health care protection are presented using the example of one area in the eastern provinces of the Second Rzeczpospolita.
Artykuł na przykładzie konkretnego regionu województw wschodnich Drugiej Rzeczypospolitej charakteryzuje rozwój i stan opieki medyczno-sanitarnej w dwudziestoleciu międzywojennym (lata 1919–1939), pokazuje rolę państwa w zwalczaniu chorób zakaźnych i społecznych, opisuje i ocenia działalność nowo powstałych szpitali epidemicznych, ośrodków zdrowia, kas chorych i innych form i ich rolę w zapewnieniu ochrony epidemiologicznej oraz zdrowia obywateli.
