Opis:
Despite the emerging evidence suggesting a proatherogenic role of C-reactive protein (CRP) in atherosclerosis, the contribution of CRP in pathogenesis of atherosclerosis and atherothrombosis has not been unequivocally defined. The role of CRP in pathophysiology/pathology seems to largely depend on its structure. Two CRP isoforms, the native pentameric and the modified monomeric one, differ substantially in their physiological functions, which is thought to origin from the considerable structural heterogeneity of the CRP molecule. The present review provides an overview of the experimental evidence with relevance to the clinical role(s) of various CRP isoforms. The biological role of the protein, its structure and distribution are discussed with particular emphasis on the diverse properties of the pentameric and monomeric forms of CRP. Some methodological aspects, related to experimental models and techniques of CRP preparation, are also critically reviewed.

Introduction and aim. In our study, our aim was to evaluate the relationship between red cell distribution width (RDW) values and prognosis in geriatric patients with acute pancreatitis. Material and methods. Patients over the age of 65 and diagnosed with acute pancreatitis who applied to the Emergency Department of Ümraniye Training and Research Hospital between 16.07.2021 and 15.05.2022 were included in our retrospective study. RDW levels were recorded using the hospital data system. Results. Our study included 184 patients, 19 (10.3%) of which died. Sixty-five percent of our patients were women. The mean hospital stay was 5 days (from 3 to 9). A statistically significant relationship was also observed between high RDW and mortality (p=0.006). The diagnostic test performance analyses of CRP, and RDW in predicting mortality revealed that they were statistically significant in predicting mortality, with the AUC value being calculated as 0.66 (0.6061–0.7368) for CRP, with a cut-off value of 22; and 0.69 (0.6909–0.7368) for RDW, with a cut-off value of 14.5 (p=0.019, p=0.006, respectively). Conclusion. Hematological parameters can help predict a prognosis in patients with acute pancreatitis. Although RDW is not statistically more significant than CRP, it can be used as a prognostic marker in patients with acute pancreatitis.

Hepatic resections are commonly associated with high morbidity and mortality. Nutrition plays an important role in reducing postoperative complications besides improvement in intensive care and perioperative management. The aim of the study was to evaluate the role of glutamine as an immunonutrient in patients undergoing hepatic resection. Material and methods. The study included 22 patients who underwent hepatectomy. Patients were randomized into two equal groups wherein group A patients received perioperative glutamine whereas group B patients served as controls. Primary outcome measures were level of serum albumin, C‑reactive protein (CRP), liver function tests and absolute neutrophil counts in the postoperative period while secondary outcome included post operative complications. Results. Glutamine decreased the CRP response in liver resection in a statistically significant manner (p=0.028) on the fifth post operative day. This may signify that glutamine decreases the post operative inflammatory response associated with liver resection. Glutamine did not have any significant effect on liver function tests. Postoperative morbidity was less in patients who received glutamine. Conclusion. Glutamine successfully blunted the CRP response in patients who received glutamine postoperatively. Decrease in morbidity following glutamine administration is an attractive area of prospective research and requires further consideration involving larger patient groups.

Opis:
Objective. The aim of this study is to evaluate neurological scales, as well as biochemical and radiological parameters measured on day 10 after ischemic stroke (IS), according to their value as predictors of the long-term outcome. Materials and method. 45 patients were assessed according to the Barthel Index (BI) and National Institute of Health Stroke Scale (NIHSS) on day 10, and according to Modified Rankin Scale (mRS) 3 months after the onset of IS. On day 10 of IS, the serum level of C-reactive protein (CRP), albumin, D-dimers (DD), S100BB and Tau proteins was measured and the volume of ischemic focus assessed with the use of Computed Tomography (CT). The patients were divided into groups with good outcome (GO) and mRS 0–2, and with bad outcome (BO) and mRS 3–6. Results. NIHSS and BI scores (p<0.001), the volume of ischemic focus (p<0.01), CRP (p<0.01) and albumin level (p<0.05), but not DD, S100BB and Tau protein levels evaluated on day 10, correlated with mRS after 3 months since IS onset. Patients from the BO group were observed to have lower BI (p=0.001), higher NIHSS (p<0.01) and CRP levels (p<0.05), and bigger volume of ischemic focus (p<0.05) measured on day 10 of IS. In the GO group, there were more patients with atherosclerotic etiology (p=0.02 x2=7.856). Regression analysis showed that only the BI score assessed on day 10 of IS can predict the outcome after 3 months assessed by mRS (OR=1.102, 95%, CI:1.01–1.203; p=0.001). Conclusions. BI assessed on day 10 has a predictive value for the outcome evaluated by mRS 3 months after the onset of IS.

This work was carried out to investigate the role of Glutathione S-Transferase M1 (GSTM1) null genotype frequency in prognosis of β-thalassemia, and to detect the correlation between GSTM1 null genotype and appearance of cardiac complications in β-thalassemia. Materials and Methods. The studied groups in the present work were divided to three groups (group I: 20 healthy subjects, group II: 56 β-thalassemic patients and group III: 16 β-thalassemic patients with cardiac complications were taken from group II). The measurement of human high sensitive C-reactive protein (hs-CRP) was performed using nephelometry. GSTM1 genotype was detected by Polymerase Chain Reaction (PCR) and cardiac complications were determined by using Echocardiography. Results. A statistically significant increase in hs-CRP and interleukin-6 (IL-6) levels was found in β-thalassemic patients with cardiac complications compared to normal subjects. Results showed no relation between GSTM1 null genotype frequency neither with β-thalassemia nor with cardiac complications appearance, where the interaction between GSTM1 null genotype in β-thalassemic patients with cardiac complications and healthy subjects were insignificant compared to subjects with GSTM1 non-null genotype. Conclusions. GSTM1 null genotype frequency has no role in β-thalassemia or cardiac complications appearance.

This paper describes a coaxial contra-rotating propeller (CCRP) system to calculate the hydrodynamic characteristics and then obtain the optimum operational condition to install on two different large bulk carrier and VLCC. The method is based on boundary element method (BEM) to obtain the hydrodynamic performance of any complicated configuration such as CRP system, and then the optimum propeller data is obtained by the systematical method at the design condition. We prepared a software package code, namely SPD (Ship Propeller Design), which has model mesh generation, solver and numerical output results. The comparison of the propulsive performance was made between the propeller alone and CRP arrangement. Major finding include good agreement between predictions using the numerical code and experimental data for both ships.

This case study applied the Dahn Taekwondo Spondylitis Improvement Program (DTSIP) to a 30-year-old male with ankylosing spondylitis five times a week for 28 weeks. Materials and Methods: A 32-year-old male with ankylosing spondylitis underwent the Dahn Taekwondo Spondylitis Improvement Program (DTSIP), an amalgamated program of standard Western medicine practices and complementary and alternative medicine (CAM) exercises based in the oriental medicine practices of yoga and taekwondo. The subject performed the DTSIP 5 times a week for 90 minutes for 28 weeks. Results: First, blood C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and platelet levels decreased by approximately 29-71%. Second, blood leukocyte counts decreased by about 11%. Third, as a result of Bath Ankylosing Spondylitis Metrology Index (BASMI) measurement, tragus to wall (TWD), lumbar side flexion (LSF), LF (lumbar flexion), and intermalleolar distance (IMD) were improved substantially. The students' cervical vertebrae and thoracic and lumbar vertebrae ranges of motion were additionally improved. The range of motion of the shoulder and hip joint improved. Conclusions: Therefore, the DTSIP seems to have a positive effect on the inflammation, immune functions, and spine and joint range of motion in the subject of this case study. Future studies are required with a wider sample population to validate these results.
