Polish culture is strongly linked to both nuclear family and family networks, which are believed to safeguard stability and sense of security for individuals and communities. The migrations of young Polish men and women, which often scope to entire families, significantly alter the fundamentals of the above guarantees. The migration of Polish youth changes social expectations, possibilities of a family existing in an unchanged form, as well as provisions of care to those who need it, primarily children and elderly members of the kinship structure. For families affected by temporary migrations and experiencing increasingly settlement-oriented mobility, being “on the move” becomes “a way of life,” in which periods of “togetherness” are intertwined with much longer phases of separation. The practices and strategies employed by migrants in the hopes of overcoming the aforementioned challenges require thorough analysis. Therefore, a main goal of our article is to focus on multi-dimensional consequences of migration that pertain to the changes of ties and relationships in families, as well as the organization of child and elderly care.

Opis:
Both expenditure on healthcare and the functioning of the entire healthcare system in Poland stir up considerable controversy and are often discussed in the media. Hospital debts, the low quality of services, and the low availability of specialist medical services form the basis for the discussion of the effectiveness of the healthcare system. Statistical data are also bleak. Total health expenditure in Poland in 2019 amounted to 6.3% of GDP (estimated expenditure), whereas the average for health expenditure in the OECD countries was 8.8%. Therefore, Poland is below the average, and is placed last but four in the ranking (stat.oecd.org). The aim of this article is the presentation of public expenditure on healthcare in Poland from 2010 to 2020. In order to achieve this, the following research methods were used: a critical analysis of the literature, an analysis of statistical data, and - to make the research more transparent and the research results clearer - a tabular method was used. Also, widely accepted measurements were used, such as absolute values in domestic and international currencies, values per capita, and values in relation to the Gross Domestic Product (GDP).oduct (GDP).

Introduction and aim. The incidence of cancer is increasing on a daily basis. One of the methods used for treatment is radiotherapy. Owing to interventions during the radiotherapy process, the patient may experience care dependency. In this study, the aim was to investigate care dependence and related factors in radiation oncology patients. Material and methods. This was a descriptive and cross-sectional study. Data were collected between September 2020 and September 2021. In the collection of data, a sociodemographic information form and a Care Dependency Scale were used. The sample consisted of 52 people. Results. Number of participants was 52, mean age was 60.25±11.715, mean care dependency score (initial) 66.19±18.966, mean care addiction score (final) 66.27±22.795. Conclusion. The care dependency of patients hospitalized in the radiation oncology clinic is moderate. The care dependency of these patients decreased partially during their stay in the clinic. The patient’s inability to walk, speak and the presence of a companion affected the patient’s condition. By evaluating the care dependency levels of the patients, the awareness of the nurses about their patients can be increased. In addition, it may be appropriate to consider the care dependency levels of the patients for the nurse workforce planning to work in the oncology clinic.

Introduction and objective. Measuring the level of patient satisfaction is a useful tool in delivering quality care that is responsive to consumer preferences. Various socio-demographic factors might be considered as potential predictors of patient satisfaction. The aim of the study was to assess whether place of residence (rural/urban) affects patient satisfaction with hospital health care. Materials and method. Data were obtained using face-to-face questionnaire, administered in five large, urban hospitals in Podlaskie Province, north-eastern Poland, during 2014–2018. The study sample comprised of 1,624 participants (585 rural, 1039 urban) who assessed satisfaction with 28 hospital health care items. The means and standard deviations were presented to compare 28 satisfaction items between the rural and the urban samples. Regression analysis was used to determine whether location difference (rural vs urban) affected patient satisfaction with various domains of hospital health care. Results. The mean results of 28 satisfaction items on the 1–5 scale were similar among the rural and the urban samples, and generally skewed towards positive experiences. In the univariate analysis, significant associations between place of residence and patient satisfaction were identified with regard to three components of inpatient care: 1) hospital settings and staff care, 2) doctors’ professional skills, and 3) hospitalization outcomes. After adjusting for socio-demographic variables, the association remained significant only with respect to satisfaction with hospitalization outcomes (b = 0.121; SE = 0.055; p = 0.028). Conclusions. Some evidence for differences in patient satisfaction by place of residence was found. Study findings may be helpful in implementing care quality improvement programmes.

The article addresses the parents’ anxiety during the separation from their child in the first days of stay in a nursery. For a parent, who is the first attachment figure for a child, separation is extremely difficult and stressful. The aim of presented research was to verify whether and to what extent, on the basis of such factors as the level of trait anxiety, gender, age of the parent or the number of children in the family, it is possible to predict the level of parents’ anxiety when entrusting children to nursery care. The study was done using State-Trait Anxiety Inventory [STAI] scale and short questionnaire regarding the age and family conditions. Results indicate that the factors predicting the level of state anxiety of the parents who entrust the care of their children to nurseries, were the parent’s gender and the level of their trait anxiety. Mothers had significantly higher levels of anxiety than fathers, and the level of anxiety as a state was higher in parents who generally experienced higher anxiety measured as trait anxiety.

Introduction: Satisfaction with medical care is defined as the patient’s level of acceptance of healthcare, taking into account his or her needs and expectations. Purpose: Determining the level of satisfaction with care in the early postnatal period. Identification of factors affecting the level of satisfaction with care in the early postnatal period. Materials and methods: The study covered 100 women aged 15-45, patients of the Obstetrics and Gynecology Department. The study employed the diagnostic survey method, using a custom-designed questionnaire. Results: 68% of hospitalized patients rated the conditions in the unit as good and very good. According to 42% of them, both the doctors and the midwives provided the patients with exhaustive information and expressed empathy. 54% of the surveyed women had not been acquainted with the floor plan of the unit, and 75% of the personnel had not revealed their names or functions to the patients. Conclusions: More than a half of the respondents assessed the level of care as good or very good. Assessing the early postnatal care they received, they mostly considered the conditions ensured by the hospital, the level of care from the personnel, both with respect to the women and newborns, and the general atmosphere in the unit.

This paper aims at analysing the evolution of palliative care in the international context and their role in the path of care for the patient and the family. Method: born in 1967 by Cicely Saunder, palliative care were aimed at assisting the terminally ill, accommodating both the needs of the patient and the family. Not only to be cured or healed, but to be taken care of. The paper examines the definitions of palliative care provided by the World Helth Organization. We observe that palliative care is not only an effective and timely response to the clinical, psychological, social and spiritual needs of the sick person and their family in an advanced and terminal stage, but an integrated care to support specialist treatment in the presence of an advanced disease picture; a space for in-depth study for the sick person and the family so that the sick person can consciously and freely choose the available treatment proposals, their limits and their consequences. A treatment path in which the transparency of the proposals is a condition for building a shared consensus with the patient and adequate communication with the family. Palliative care has acquired its own identity, its own role in the path of care for the patient and the family, pursuing the proportionality of therapeutic options and the support of the patient and the family without discrimination, with equity and equality.

Objectives: The main objective of this article was to review the literature about the specific nursing care to adult patients with ECMO, identifying the existence of standardized care plans that optimize the quality of the care and favor the continuity of the care. Methods: A search was carried out in the following databases: CINAHL, CUIDEN PLUS, EMBASE, LILACS, MEDLINE, PUBMED. All reference lists were imported to a bibliographic manager (RefWorks), a tool used to eliminate duplicate documents. The inclusion criteria were: (1) original scientific article, (2) full text published in Spanish or English. The exclusion criteria were: (1) articles published in other languages, (2) articles in which the interventions performed on patients did not involve nursing participation. Findings: A review of 11 articles was carried out, analyzing the role of nurses and the care of patients with ECMO. It is important to highlight the importance of prior technical training and the thorough evaluation of all physiological systems and the ECMO circuit, without forgetting family support. Conclusions: The nurse must provide direct care to the patient with ECMO and to the circuit, carefully review all physiological systems and pay attention to the family members’ care. Therefore, the nurse acquires a primary role in the coordination of care and in the monitoring of the patient with ECMO.

The purpose of this paper is not to provide an overall picture of care ethics, but, rather, to reflect upon the concept of care, which has gained significance in particular scientific contexts. Undoubtedly, the importance of the subject of care represents a challenge on the level of fundamental philosophical positions and a diversified look into the occurring forms of the psychological and social suffering, dependency, and vulnerability. I will shed light on tenets that are considered central to the care ethics and that have led to the setting up of the relationship between care and democracy. By introducing the basic aspects of Carol Gilligan’s and Joan Tronto’s theories, I will emphasize their vocation to be politically relevant. A discussion on the relationship between care and justice on the one hand, and needs and rights on the other, will follow. Can hearing voices of care make democracy more just? How can care ethics provide a concrete support for democracy? On the political level, care ethics builds on the assumptions of the necessary relatedness and equality between individuals who are decisive in the light of the welfare state perspective. Upon closer examination, one may see that the setting up of these relationships is not new to the liberal perspective. Liberalism is historically rooted in the fundamental interconnection between freedom and equality. Virtues such as care and meekness can be formulated in the language of politics, but they still remain unpolitical. On the one hand, engaging in a critical reflection upon such virtues might be an indispensable measuring instrument for distinguishing a good democracy from a bad one. On the other hand, care ethics does not prove to be a real alternative to the ethics of justice. In this regard, care ethics seems to show its feet of clay.

The article is an attempt at diagnosing training needs of the employees of units operating in health care sector in the Podkarpacie Province. In times of permanent changes affecting each sphere of economy, providers of health care services cannot afford to remain outside this trend. Improving qualifications, adaptability of the offer, influencing its quality, and above all, the awareness of the necessity of these changes, have become an element which is fully integrated also with this sphere of public sector operations. Taking into account the above, the article verifies not only training needs articulated by employees of Health Care Centers (HCC) operating in the Podkarpacie Province, but also the way they are perceived by the managers of these centers, the ability to define training needs and their compatibility with characteristic features of analyzed HCCs. Therefore special emphasis has been placed on demonstrating the variety of diagnosed training needs with reference to such criteria as the size of analyzed centers, the market serviced by them, and their location. These determinants allowed us to conduct a complex analysis of conditions and structure of voiced need for subject training, and as a consequence, contributed to diagnosing the expectations of the health care sector concerning initiatives improving the quality of public services in the health care services area.

Purpose: To assess the degree of independence in physical activities of daily living for patients under long-term in-home nursing care; to evaluate any changes in physical activities of daily living at entry and after 90 days of in-home nursing care. Materials and methods: InterRAI-HC (Residental Assessment Instrument – Home Care) questionnaire was used as a tool according to the protocol and assessed patients at entry and again after 90 days. The participants consisted of 100 consecutive patients who were newly admitted to long-term in-home nursing care.Results: The analysis of total dependence showed that more than 50% of patients were entirely dependent in eight out of ten specified physical activities of daily living (except mobility in bed and food consumption). Intention to treat analysis was performed; the percentages of patients totally dependent in performing eating and mobility in bed actions increased significantly by the 90-day follow-up. Also a comparison of the mean value of independence for analyzed activities did not change significantly over the 90 days, with the only exception being the mean value of independence for dressing up the upper part of the body, which significantly improved after 90 days per protocol analysis. A high mortality rate is noted among these patients (15% by the 90 day follow-up). This probably resulted from the restrictive qualification criteria used in Poland for patients to receive this form of in-home healthcare service.Conclusions: The majority of patients who are under long-term in-home nursing care in Poland are entirely dependent when it comes to the physical activities of daily living.

ObjectivesHealth care services in Poland are delivered by public and private providers. The aims of this study were to assess the attitudes towards private and public health care services in Poland and to identify differences between them, in the opinion of physicians.Material and MethodsA questionnaire-based survey was carried out among physicians attending mandatory courses delivered at the School of Public Health, the Centre of Postgraduate Medical Education in Warsaw, Poland. The questionnaire included 29 questions concerning private and public health care services.ResultsCompleted questionnaires were obtained from 502 physicians (67.7% females), aged 42.1±10.8 years, with a response rate of 77.2%. In the opinion of the surveyed doctors, the major advantages of private health care units, in comparison with public ones, are short waiting times for an appointment (88.2%), an efficient on-site service (78.6%) and convenient appointment times (75.7%). The respondents gave high scores to items such as relations with patients (p < 0.001), superiors (p < 0.001) and colleagues (p = 0.03) when working in private, rather than public, institutions. In the opinion of physicians, public health care institutions guarantee better employment conditions (44.4% vs. 13%; p < 0.001) and security (29.1% vs. 11.1%; p < 0.001) than private ones. The respondents did not observe any differences (p > 0.05) between public and private facilities in terms of the involvement of medical staff and infrastructure. There were significant differences (p < 0.001) in the perception of working conditions in public and private health care institutions depending on the medical education level and the place of primary employment. Conclusions: Among physicians in Poland, private medical institutions are perceived as better organized and granting faster as well as more comprehensive access to health care services when compared to public ones. Closing the gaps between working conditions in public and private units could encourage physicians to practice in the public health care sector.
