Opis:
The current limitations of surgical treatment, as well as restrictions on professional and sport activities related to COVID-19 pandemic require seeking therapeutic solutions for the vast population of patients with chronic venous insufficiency (CVI, chronic venous insufficiency). To understand the principles of pharmacotherapy of this disease, the authors used data related to epidemiology and pathophysiology of CVI. They provided the latest data on venoactive drugs and recommendations that should be followed and summarized the literature based on the EBM. The article should provide the answer to the question of how to deal with patients with varicose veins or, more broadly, chronic venous insufficiency during the pandemic.

Opis:
Penetrating neck traumas are traumas that pass through the platysma muscle, and they account for 5% to 10% of all neck traumas. Penetrating head traumas can damage vital structures such as the airways, esophagus, major nerves and vessels. Thus, patients with penetrating neck traumas should be treated in hospital, and doctors should be careful when deciding on the best treatment. Currently, the need for surgery can be established based on imaging tests, clinical status of the patient, medical history, physical examination, and laboratory tests. Exploratory surgery is not necessary particularly because it is associated with complications. We present two patients with penetrating neck traumas who were treated in our center: a 22-year-old patient with a foreign body that, for two years, mimicked a tumor, and a 45-year-old patient with a stab wound requiring immediate treatment. In the first patient, finding the foreign body earlier would have allowed an immediate removal, which would have prevented subsequent surgery and reduced the risks of leaving the foreign body in the neck. In the second patient, immediate contrast-enhanced computed tomography (CT) of the neck showed a foreign body, its size and position, which helped to plan surgery. Conclusions: In hemodynamically stable patients with penetrating neck traumas, doctors should choose the best treatment based on imaging, medical history, and physical examination.

Opis:
The final success and outcomes of patients treatment undergoing Functional Endoscopic Sinus Surgery depends on the correct preparation for surgery, perioperative and postoperative care. Proceedings before FESS in case of chronic rhinosinusitis arise from „European position paper on rhinosinusitis and nasal polyps 2012” and includes pretreatment with local nasal and systemic steroids, sinus irrigation, as well as in some cases antibiotic therapy. Reduction of intra- and post-operative bleeding is possible by intraoperative use of gauze local vasoconstricting inserts, reverse Trendelenburg position, reduction of blood pressure and application of tranexamic acid. After the surgery the use of nasal packing is often recommended. Moreover postoperative care has been largely transferred to the patient and therefore we are highlighting how important is education of patient, providing accurate information and training in the appropriate use of prescribed preparations for na-posal irrigation and nasal steroids. The use of steroids was proved to reduce the risk of recurrence of the disease and it is reasonable when we are sure that they have an opportunity to reach physically to whole mucous membrane of the nose. The effectiveness of nasal irrigation can be enhanced by use of the containers enabling the appropriate application (the exact adherence to the nasal vestibule , at an angle 45° ) and generating sufficient pressure ( rinse of the entire nasal cavity –120 mbar) and the use of appropriate fluid composition accelerates healing and reduce pain. Use of in vitro studies and computational fluid dynamics simulations allow to predict the effectiveness of rinsing different paranasal sinuses.

juvenile idiopathic arthritis
Opis:
Background: Juvenile idiopathic arthritis (JIA) is a chronic, heterogenous inflammatory disease of unclear pathogenesis. JIA is hypothesized to be linked to a defective immune regulation. Anti-inflammatory cytokines belong to the best known regulatory factors. T-regulatory cells are a crucial cellular component of immune tolerance. One of their functions is synthesis of interleukin 10 (IL-10) and transforming growth factor beta1 (TGF-β1). The aim of this study was to determine the proportion of T-regulatory cells (CD4+CD25highFOXP3+) in peripheral blood, and serum levels of TGF-β1 and IL-10 in patients with JIA. Methods: The study included 25 patients with newly diagnosed JIA: oligoarthritis (n=17) and polyarthritis (n=8). The control group was comprised of 17 healthy children. CD4+CD25highFOXP3+ T cells in peripheral blood were quantified by means of three-color flow cytometry. Serum concentrations of TGF-β1 and IL-10 were estimated with ELISA. Results: The proportion of peripheral CD4+CD25highFOXP3+ cells in patients with JIA was significantly higher than in the controls (p=0.04). The two groups did not differ significantly in terms of their TGF-β1 and IL-10 concentrations. Conclusions: At the time of diagnosis, children with JIA presented with an elevated proportion of T-regulatory cells (CD4+CD25highFOXP3+) in peripheral blood. Anti-inflammatory cytokines, IL-10 and TGF-β1, are not upregulated in the serum of patients with JIA, and therefore should not be considered as biomarkers of this condition.

Opis:
Background The SARS-CoV-2 virus, causing acute respiratory disease, is responsible for the COVID-19 pandemic, which began in early 2020. In addition to symptoms typical of respiratory tract infections, the virus causes a number of non-specific, often long-lasting effects that hinder the daily functioning of individuals. The aim of the study was a subjective assessment of life quality and health perception among recovered COVID-19 patients. Material and Methods The study included 337 subjects who had been infected with SARS-CoV-2 confirmed by a positive RT-PCR test. The study participants were of legal age. The convalescents completed a questionnaire that contained 26 questions about gender, height, body weight, blood type, general and specific symptoms, comorbidities, hospital stay and duration of specific symptoms, the severity of which was assessed on the Visual Analogue Scale (VAS). The subjects determined whether the COVID-19 infection influenced their health perception and life quality. Results According to 46% of the respondents, COVID-19 had an impact on their quality of life and health. The chance for patients to notice the negative effects of COVID-19 on their current health status and life quality increased with each subsequent symptom of the disease by 49%, with each day of its occurrence by 3%, and with each VAS point of the severity of all infection symptoms by 30%. Conclusions The study shows that COVID-19 disease affects life quality and overall health perception after recovery. Significant impact of COVID-19 on the quality of life should be a signal to create mental support and rehabilitation programs for convalescents to minimize discomfort and shorten the duration of absenteeism from work.

The article presents the results of emission tests and vehicle operation indicators fueled with diesel oil. The tests were carried out for a passenger vehicle equipped with a diesel engine meeting Euro 3 emissions standard, moving in urban traffic. The measurements were carried out using modern PEMS (Portable Emission Measurement System) enabling the emission of gaseous components from exhaust systems of the tested object. On the basis of the conducted tests, the load characteristics were determined using the torque values obtained along with the engine speeds. The measurement route included two cycles: urban driving and fast acceleration. The aim of the study was to assess the impact of modifications to the control maps on CO, CO2, PM and NOx exhaust gas emissions under real operating conditions.

Opis:
CK2 is a pleiotropic constitutively active serine/threonine protein kinase composed of two catalytic α- and two regulatory β-subunits, whose regulation is still not well understood. It seems to play an essential role in regulation of many cellular processes. Four active forms of CK2, composed of αα'ββ', α2ββ', α'2ββ', and a free α'-subunit were isolated from wild-type yeast and strains containing a single deletion of the catalytic subunit. Each species exhibits properties typical for CK2, but they differ in substrate specificity and sensitivity to inhibitors. This suggests that each CK2 isomer may regulate different process or may differ in the way of its regulation.
