Opis:
Sword is one of the most important identifiers of elite character of the burial in the Przeworsk Culture. The way of wearing double-edged sword is determined by scabbard slide and strap that passes through the slide. The side on which the sword was worn is also incontrovertible. More complicated dilemma can be observed while working on the interpretation of one-edged swords and their suspension system. In some cases there are preserved scabbard clasps and mounts still attached to the blade. This could happen when grave inventory was burnt together with sword hidden in its carrying case. By analysing the placement of the suspension rings with attached strap, we can get answers to such questions as: which side of the body was worn particular sword, what was its inclination towards the body and how high it was suspended. The sole interpretation of research results could give us incomplete view of the past. Running some tests with reconstructed artefacts could significantly influence and change final conclusions regarding analysed archaeological material.

Artykuł przedstawia wyniki ankiet przeprowadzonych przez autorów, których przedmiotem była archeologia eksperymentalna. Do grupy rzemieślników zajmujących się rekonstrukcją skierowano pytania dotyczące trudności, z którymi spotykają się podczas swojej pracy odtwórczej. Co jest największym problemem? Skąd rzemieślnicy czerpią informacje dotyczące technologii wytwórstwa? Jak ich wyroby są przyjmowane przez odbiorców? Ramy chronologiczne odtwórstwa zawężono do jednego okresu historycznego (czasu występowania kultury przeworskiej, a więc okresu przedrzymskiego, okresu wpływów rzymskich i okresu wędrówek ludów). Pozwoli to uczytelnić powiązania między niewielką grupą rzemieślników zajmujących się rękodzielnictwem metodami starożytnymi a światem naukowym, zdobywaniem wiedzy historycznej i archeologicznej oraz inspiracji etnograficznych w celu rozwiązania problemów technik rzemieślniczych. Badaniami zostały objęte osoby zajmujące się różnymi rzemiosłami, od obróbki metalu po garncarstwo, co pozwala spojrzeć na owo zagadnienie z szerszej perspektywy.
The paper presents the results of the survey carried out by the authors of this paper, the subject of which was experimental archaeology. We asked a group of artisans that deal with historical reconstruction a set of questions about the difficulties they encounter during their reconstruction work. What is their biggest problem? Where do the craftsmen derive information about the ancient technology from? How are their products received by recipients? The chronological framework of the reconstruction industry was narrowed down to one historical period (the time of the Przeworsk culture, that is the Pre-Roman Iron Age, the Roman Iron Age and Migration Period). This clearly showed some connections between a small group of craftsmen using ancient methods in their work and the academic world, and how important it is to acquire historical and archaeological knowledge and to find ethnographic inspirations in order to solve the problems of craft techniques. The survey covered artisans practising various crafts, from metalwork to pottery, which added a broader perspective to the issues studied.

Soya meal is the main high protein source in feeding pigs and poultry. Taking into account the annual value of domestic import, limited soya seed producers (USA, Argentina, Brasil) and China as the main soya seed importer (66 percent of the world import), a use of alternative protein sources (grain legumes, rape meal, DDGS) seems to be justified. An additional reason for a so called protein security in Poland are the feed regulations prohibiting a usage of GM products in feeding. Polish Government launched research projects for two periods (2011–2015 and 2016–2020) to solve main problems related to increased production and usage of domestic protein sources in animals feeding. Main strategies and research results of four areas – grain legume genetics and breeding, cropping technologies, pigs and poultry feeding and economic aspects of production, market infrastructure and turnover of domestic protein crops – are presented in the paper. Satisfactory is an increase of acreage under grain legumes in Poland – from 100 thousand ha in 2011 to 300 thousand ha in 2019. At present, given the availability of domestic grain legumes and rape meal it is possible to decrease soya meal imports up to 60–50 percent. Over the past decade, a strong increase of poultry meat production (about 120 percent) with a rather small increase of soya meal imports (about 18 percent) is also as optimistic tendency. A lasting solution may be achieved by setting a national/ European indicator target that would put the mandatory share of domestic protein sources in feed mixes at 10–20%, and creation of Polish feed companies, competitive to foreign, both large and mobile feed mixing plants, using the farmer’s raw materials.

Background. Tunneled catheters are becoming increasingly used as a permanent dialysis access. Easy way of insertion and good long-term patency make them competitive to fistulas in some groups of patients. Methods. Late complications and survival of 180 tunneled catheters inserted from June 2010 to December 2013 in 171 unselected hemodialysis patients were analyzed. Results. The cumulative time of observation was 2103.5 patient-months and median observation was 9 months (range of 0.5-45 months). Only 19 out of 180 catheters were removed due to complications (12 for infections, 4 due to malfunction and 3 because of mechanical damage). Majority of catheters were removed electively: 27 after maturation of arterio-venous fistula (AVF), 4 after kidney transplant, 5 after transfer to peritoneal dialysis and 3 due to the recovery of renal function. At the end of the observation, 58 catheters were still in use and 64 patients had died with functioning catheter. When censored for elective catheter removal and patient death, 88.2% of catheters survived for 1 year. Catheter survival was significantly better in older patients (over 65 years, in comparison to patients < 65 years, p = 0.046). Conclusions. Nearly 90% of all inserted catheters gave reliable dialysis access as long as it was needed. Among them, over 30% of the inserted catheters were in use at the end of the observation period, and over 30% of patients had died with a functioning catheter. The results of tunneled catheters survival are encouraging and they should be taken into consideration during decision-making on vascular access, especially in the older patients.

Objectives: The aim of the study was to assess physical activity (PA) level in a representative sample of Polish adults.Materials and Methods: A cross-sectional analysis was performed among 2413 randomly selected individuals (51.5% women) aged 18-79 years, who participated in the Nationwide Study of Occurrence of Risk Factors of Cardiovascular Diseases NATPOL 2011 (March-July 2011). The study procedures consisted of a questionnaire as well as of anthropometric, blood pressure and biochemical measurements. Leisure-time, occupational and commuting PA were assessed by the use of a questionnaire interview. Results: About 48.2% of adults do exercise for at least 30 minutes on most days of a week. About 11% of the respondents declare a sedentary lifestyle. About 26.5% of working population report hard physical work, while sedentary work is reported by 47.6% of the employed participants. Active commuting is declared by 27.3% of working/studying population. About 47.2%, 36.6%, and 15.3% spend < 15, 15-30, and > 30 min per day, respectively, on this kind of PA. Conclusions: PA level of more than half of Polish adults is still not satisfactory. Promotion of an active lifestyle should concern mainly leisure-time and commuting PA with paying special attention to substantial differences in various socio-demographic groups.

Background. There is evidence that dyslipidemia is associated with chronic kidney disease (CKD) and it has been implicated in the progression of renal damage. Optimal management of dyslipidemia should therefore lead to renal benefits. A number of experimental models demonstrate a beneficial effect of statins in ameliorating renal damage. However, the exact mechanism by which statins protect against renal damage remains unclear. Methods. In a placebo-controlled, randomized, cross-over study we evaluated the influence of atorvastatin (ATO) 40 mg/day added to the renin-angiotensin-aldosterone systeme (RAAS) blockade on proteinuria and surrogate biomarkers of tubular damage or injury in 14 non-diabetic patients with proteinuria (0.4-1.8 g per 24 h) with normal or declined kidney function (eGFR 55-153 ml/min). In the eight-week run-in period, therapy using angiotensin converting enzyme inhibitors (ACEI) and/or angiotensin II subtype 1 receptor antagonists (ARB) was adjusted to achieve a blood pressure below 130/80 mm Hg. Next, patients were randomly assigned to one of two treatment sequences: ATO/washout/placebo or placebo/washout/ATO. Clinical evaluation and laboratory tests were performed at the randomization point and after each period of the study. The primary end point of this study was a change in proteinuria measured as 24-h urine protein excretion (DPE). Secondary end points included urine N-acetyl-β-d-glucosaminidase (NAG) and α1-microglobulin (α1m) excretion. Results. The ATO therapy significantly reduced urine excretion of α1m (p=0.033) and NAG (p=0.038) as compared to placebo. There were no differences in proteinuria, blood pressure, eGFR and serum creatinine between the ATO and placebo groups. Conclusion. Atorvastatin treatment is safe and improves biomarkers of tubular damage or injury in non-diabetic patients with CKD.

ObjectivesThe main aim of this work is to assess the level of knowledge about diabetes prevention and the consequences of untreated diabetes in the Polish society, as well as to indicate the variables that have a significant impact on that knowledge.Material and MethodsThe analysis is based on the results of the National Multicenter Health Survey – WOBASZ II. The number of subjects surveyed was 6170, including 2760 men and 3410 women, aged 20–74 years. Data on socio-demographic variables, lifestyle and subjective health assessment were collected using the face-to-face technique.ResultsThe results obtained in the WOBASZ II project showed that >85% of the respondents had an unsatisfactory level of knowledge about diabetes prevention methods and approximately to 85% of the respondents – about the consequences of untreated diabetes. Moreover, one-fourth of the respondents were unable to identify a single way of preventing diabetes, and more than one-third could not identify a single disease caused by diabetes. The risk of a lack of knowledge about diabetes prevention and the consequences of untreated diabetes is more common for men, people with a low level of education, not married, non-diabetic, and without diagnosed diabetes in the family.ConclusionsThe presented results indicate that there is an prevailing lack of knowledge about diabetes prevention and the consequences of untreated diabetes in the Polish society. It is associated with several variables like: sex, level of education, age, marital status, subjective health assessment and diabetes diagnosed in the respondent and/or in the respondent’s family.

Opis:
The management of peritoneal surface malignancy is a significant clinical problem in oncology. It was demonstrated that the combination of complete cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (HIPEC) may lead to long-term control of the disease or improved survival in selected patients. The aim of this paper was to present the optimal indications and technical guidelines for performing HIPEC in Poland. The application of this method requires experience of a multidisciplinary team of physicians (gynecologic oncologist, surgeon and clinical oncologist), availability of diagnostic and therapeutic resources (intensive care unit) as well as a dedicated perfusion system. A crucial aspect for obtaining optimal treatment outcomes is the selection of patients. Such a selection takes place both at the beginning of treatment and intraoperatively. The initial selection of patients qualified for HIPEC includes ruling out extraperitoneal spread of cancer and metastases to the liver (single resectable liver metastases in patients with colorectal carcinoma are not contraindications) and lungs. According to current international guidelines, the HIPEC procedure is a standard treatment in patients with ovarian carcinoma that metastasizes to the peritoneum, in colorectal cancer, when PCI <20 and in patients with peritoneal mesothelioma or pseudomyxoma peritonei as well as in patients with gastric cancer.
Leczenie nowotworów złośliwych powierzchni otrzewnej stanowi istotny problem w onkologii. Wykazano, że skojarzenie maksymalnej cytoredukcji chirurgicznej z dootrzewnową chemioterapią w hipertermii (hyperthermic intraperitoneal chemotherapy, HIPEC) może prowadzić do długotrwałej kontroli choroby lub wydłużenia przeżyć u odpowiednio wybranych chorych. Celem niniejszego opracowania są zalecenia dla optymalnego zakresu wskazań i sposobu prowadzenia HIPEC w Polsce. Zastosowanie tej metody wymaga doświadczenia wielospecjalistycznego zespołu lekarskiego (ginekolog onkolog, chirurg, onkolog kliniczny), a także zaplecza diagnostycznego i leczniczego (oddziału intensywnej opieki medycznej) oraz dedykowanego systemu do perfuzji. Kluczową kwestią dla uzyskania optymalnego wyniku leczenia jest odpowiedni dobór chorych. Dobór ten następuje zarówno na etapie planowania leczenia, jak i śródoperacyjnie. Wstępna selekcja pacjentów kwalifikowanych do leczenia z wykorzystaniem HIPEC obejmuje wykluczenie pozaotrzewnowych przerzutów nowotworu oraz przerzutów do wątroby (pojedyncze resekcyjne przerzuty do wątroby u chorych na raka jelita grubego nie są przeciwwskazaniem) i płuc. Zgodnie z istniejącymi zaleceniami międzynarodowymi procedura HIPEC jest standardowo stosowana u chorych z przerzutami do otrzewnej raka jajnika, raka jelita grubego, gdy PCI <20 oraz u chorych na międzybłoniaka lub śluzaka rzekomego otrzewnej i wybranych chorych na raka żołądka.
