Background: Microbiological contamination of water in dental unit waterlines (DUWL) creates a risk of cross-infections, and is a source of biological risk factors in the work environment of a dentist. The aim of the study was to evaluate dentists' knowledge on DUWL microbiological contamination and the scope of activities/procedures they undertake to monitor it. Material and Methods: The questionnaire survey was conducted in 2010 among 107 Polish dentists using dental units in everyday clinical practice. Results: It has been found that in their daily practice, dentists do not follow procedures leading to reduction or elimination of microbiological contamination of dental unit reservoir water. They are not aware of microbiological contamination of DUWL that supply working handpieces with water. They are unaware of the principles of dealing with dental water and water supply systems or the health risk posed by microbiological contamination of unit water for a dental team and patients. Conclusions: It is necessary to provide dentists with information on microbiological contamination of water in dental units, on the correct procedures of handling water and waterlines that supply working handpieces with water. Wstęp: Mikrobiologiczne skażenie wody w instalacji wodnej unitów dentystycznych stwarza zagrożenie zakażeniami krzyżowymi, stanowiąc źródło szkodliwych czynników biologicznych w środowisku pracy stomatologa. Celem badań była ocena wiedzy dentystów na temat skażenia mikrobiologicznego wody z unitów dentystycznych oraz zakresu czynności/procedur, które powinny być podejmowane w celu monitorowania tego skażenia. Materiał i metody: Badanie ankietowe przeprowadzono w 2010 r. wśród 107 polskich stomatologów używających unitów dentystycznych w praktyce klinicznej. Wyniki: W codziennej pracy dentyści nie stosują procedur minimalizujących/eliminujących mikrobiologiczne skażenie wody w zbiornikach unitów dentystycznych. Nie mają oni wiedzy na temat skażenia instalacji wodnej dostarczającej wodę do końcówek roboczych, nie znają zasad postępowania z wodą z unitów ani z instalacją wodną, nie są też świadomi zagrożeń zdrowotnych stwarzanych przez mikrobiologiczne skażenie wody w unitach dla zespołu stomatologicznego i pacjentów. Wnioski: Konieczne jest dostarczenie dentystom informacji na temat problemu mikrobiologicznego skażenia wody w unitach dentystycznych oraz procedur prawidłowego postępowania z wodą w instalacjach wodnych unitów. Since the refugee crisis of 2015, the European Union's institutions and EU Member States' governments have strengthened policies to manage better migration flows and protect the EU's external borders. In the external dimension, the Union implemented a wide variety of economic, political, and deterrence measures to regain control over migratory flows. Although development cooperation was declared one of the important tools for addressing root causes of migration, the externalization of migration management to neighboring transit countries became the main pillar of the anti-crisis strategy. Although this policy enabled to reduce essentially the number of irregular arrivals in Europe, it cannot be considered as a long-term solution. To be better prepared for migration challenges of the future, the EU should rethink its development cooperation with the origin and transit countries and include both forced and economic migrants in its comprehensive response. Aid can be a useful tool for the EU if it is used to manage rather than to stop migration.
Od czasu kryzysu uchodźczego z 2015 r. instytucje UE i rządy państw członkowskich wzmocniły działania na rzecz lepszego zarządzania migracją oraz ochrony zewnętrznych granic UE. W wymiarze zewnętrznym Unia zastosowała wiele różnych środków gospodarczych i politycznych, a także rozwiązań o charakterze odstraszającym, aby odzyskać kontrolę nad przepływami migracyjnymi. Chociaż współpraca na rzecz rozwoju została uznana za jedno z ważnych narzędzi eliminowania pierwotnych przyczyn migracji, to eksternalizacja zarządzania migracją do krajów tranzytowych stała się głównym filarem strategii antykryzysowej. Choć polityka ta pozwoliła zasadniczo zmniejszyć liczbę nielegalnych przekroczeń europejskiej granicy, nie można jej traktować jako rozwiązania długoterminowego. Aby lepiej się przygotować na wyzwania migracyjne w przyszłości, UE powinna przemyśleć współpracę na rzecz rozwoju z krajami pochodzenia i tranzytu oraz włączyć do swojej strategii zarówno działania wobec migrantów przymusowych, jak i ekonomicznych. Pomoc rozwojowa może być przydatnym narzędziem dla UE, jeżeli będzie wykorzystywana raczej do zarządzania niż do blokowania migracji.

Introduction. One of the criteria of implant therapy success is marginal bone loss. The objective of the study was to assess the correlation between peri-implant marginal bone loss and implant-abutment connection systems used in the same patient, as well as other specific characteristics of implant treatment. The initial research hypothesis assumed that there was no difference in marginal bone loss around implants with different implant-abutment connection systems placed in the same patient. Materials and method. Marginal bone loss was assessed around implants with two different types of implant-abutment connection: with conical (Type I) and with internal hexagonal (Type II) in the same patient. The study included 28 patients aged 37–66 years. Results. Marginal bone loss around Type I implants was 0.112 mm/month before loading with prosthetic restorations, and 0.010 mm/month after loading, while for Type II implants it reached, respectively, 0.123 mm/month and 0.030 mm/month. Marginal bone loss after loading with prosthetic restorations was 11 times lower for Type I implants and 4 times lower for Type II implants. Evaluation of marginal bone loss in the studied patient groups was made on the basis of orthopantomographic radiographs. Conclusions. Implants with conical implant-abutment connection are significantly more favourable to osseointegration than those with internal hexagonal connection. As marginal bone loss is faster before loading implants with prosthetic restorations than after loading, it is advisable to consider early loading if the necessary clinical conditions are met.

Fungi, mainly yeast-like fungi of the Candida genus, and especially the species Candida albicans, occupy an important place among microorganisms that colonize the oral cavity. The aim of the paper was to assess the susceptibility of different strains to some antifungal drugs. The study material consisted of swab samples taken from the buccal mucosa of 376 healthy individuals of both genders, from neonate to18 years. C. albicans strains were isolated in 79 persons. To evaluate drug resistance phenotypes of the isolated C. albicans strains FungitestTM (Bio-Rad) was used. The study showed that in different age groups the percentage of the tested C. albicans isolates susceptible to all the tested azole drugs varied from 76% to 95%. Drug resistant strains were found only in the eldest age group (4.8%). Phenotypic diversity (drug resistance types) of the studied isolates indicates a wide range of adaptation possibilities enabling colonization of various ecological niches in the oral cavity ecosystem. The presence of C. albicans strains intermediate or resistant to azole drugs among the tested isolates may be due to overuse of that drug group in general therapy and in the oral cavity treatment.

Introduction. Among the microorganisms colonizing the oral cavity ontocenosis, an important role is played by fungi: mainly yeast-like fungi of the Candida genus, and especially by the species C. albicans. Objective. The aim of this study was to evaluate the frequency of oral colonization by Candida species in healthy individuals of different age groups, and to identify the species of the isolates. Materials and method. The fungi were isolated from the oral cavities of 654 healthy individuals of both genders, representing different age groups, and the species of the isolates were determined. Initial identification of the yeast-like fungi was made based on the macroscopic appearance of the colonies on Sabouraud’s medium, and the growth of coloured colonies on ChromAgar Candida medium. API 20 C AUX microtest (bioMerieux) was used to identify the most frequently found species of Candida spp. Results. The frequency of Candida isolation from the oral cavity ontocenosis in healthy individuals of different age groups was assessed as 30.6%. The oral cavity ontocenosis was colonized mainly by the yeast-like fungi of C. albicans species: they were found in 24.5% of the tested population, while the frequency of oral cavity colonization by non-albicans Candida spp. was 6.1%. Seven species of non-albicans Candida spp. were identified: C. glabrata, C. inconspicua, C. famata, C. tropicalis, C. parapsilosis, C. lusitaniae, and C. kefyr. Conclusions. In the Polish population, 30.6% of the oral cavity ontocenosis is colonized by Candida yeast-like fungi, while C. albicans is the fungus that colonizes oral cavity ontocenosis most frequently. The frequency of oral cavity ontocenosis colonization with Candida spp (including C. albicans) increases with age.

Fungal infections are an important medical problem in patients from different risk groups. The majority of these infections are caused by Candida spp., with over 50 % due to C. albicans. The purpose of the study was to evaluate in vitro chlorhexidine effect on C. albicans colonizing the mouth and throat isolated from 5 population groups. The study material included the reference strains of C. albicans ATCC 2091 and C. albicans ATCC 10231, routinely used for evaluation of antimicrobials, and 120 clinical isolates of C. albicans from: hospitalized cancer patients (30 isolates), chronic HCV patients (31 isolates), immunocompromised patients (28 isolates), healthy school-age children (31 isolates), elderly people – aged 65 years or older (30 isolates). Chlorhexidine inhibited the growth of C. albicans at the concentrations of 0.625-5 µg/ml (in particular, 2.5 µg/ml solution was effective against strains from immunocompromised patients and 5 µg/ml – against the remaining isolates). The yeasts were also killed by 2.5-20 µg/ml chlorhexidine solutions. The concentration of 5 µg/ml was particularly active against the strains isolated from the elderly, immunocompromised and lung cancer patients, while 10 µg/ml inhibited the growth of the strains from the remaining two groups. Moreover, C. albicans isolates from hepatitis C patients and healthy children comparing to strains from the elderly were less sensitive to chlorhexidine fungicidal effect and these differences were statistically significant. According to our studies, the fungicidal effect of chlorhexidine seems to depend on the origin of the tested oral C. albicans strains from various patient groups.

Objectives. The aim of the study was to analyze the relationship between selected socio-demographic factors and tooth loss in patients using removable dentures. Materials and method. The study comprised 321 individuals, all of whom were residents of the Lublin province in southeastern Poland. The study investigated the influence of socio-demographic factors, such as gender, age and education on the severity of tooth loss. To analyze the structure of tooth loss, the classifications by Galasińska-Lansberger and by Eichner were used. The results were analyzed statistically. Results. Edentulous gaps and lateral edentulism in the maxilla were significantly more frequent in women, while mixed tooth loss was more frequent in men, while the percentages of men and women with maxillary edentulism were similar. There were no statistically significant gender-specific differences concerning mandibular edentulism. The number of individuals who reported total tooth loss in both the maxilla and the mandible increased with age, the differences being statistically significant. Individuals with primary education only were significantly more likely to report maxillary edentulism, while a high percentage of those with secondary or higher education reported edentulous gaps in the maxilla. There was no statistically significant relationship between education and the type of tooth loss in the mandible. Conclusions. There is a strong relationship between age and education level on the one hand, and the severity of tooth loss on the other. However, gender exerts no significant influence on tooth loss, which was confirmed using 2 classifications of edentulism.

Introduction: Ischaemia-reperfusion injury (IRI) is a factor leading to the damages of the transplanted kidney, what affects mainly the proximal tubules. Early monitoring of tubule damage can be an efficient tool to predict the allograft dysfunction. Present in proximal tubules, N-acetyl-beta-glucosaminidase (NAG) is a lysosomal enzyme whose excretion rises as a result of IRI or acute rejection. The aim of this study was to monitor the NAG urine activity to evaluate the early proximal tubule damage, and to try to predict the long-term function of the transplanted kidney. Material and methods: The study enrolled 87 Caucasian renal transplant recipients (61.7% males, 38.3% females, mean age 45.56±14.34 years). Urine samples were collected for NAG and creatinine analysis on the 1st day after transplantation, and then in the 3rd and 12th month. Protocol biopsies were performed in the 3rd and 12th month. Results: A significant positive correlation between NAG urine activity in the 3rd month after transplantation and creatinine concentration on the 14th (p=0.004) and 30th day (p=0.05), in the 3rd month (p=0.009) and after the 1st (p=0.005) and 2nd year (p=0.003) was observed. A statistically significantly higher urinary NAG activity in samples collected in the first 3 days and in the 3rd month after transplantation among patients with DGF (p=0.006 and p=0.03 respectively) was found. There was a significant positive correlation between NAG urine activity in the 3rd month and the grade of tubular atrophy in specimens collected in the 3rd (p=0.03) and 12th (p=0.04) month. Conclusions: Monitoring of NAG urine activity is useful in the evaluation of early proximal tubule damage and predicting the long-term function of the transplanted kidneys.

In bioprocesses lipases are typically used in immobilized form, irrespective of type of reaction systems, to ensure an even distribution of catalysts in water restricted media and/or to facilitate separation and reuse. In these studies we report on the selection of appropriate enzyme-carrier preparation for hydrolysis reaction in aqueous and biphasic systems and transesterification in organic solvent. For this Candida rugosa lipase was bound by adsorption or covalent attachment onto various carriers to give 24 preparations. Selection of proper preparation was based on reactivity, thermal stability (4 h at 60°C), possibility of drying and operational stability in 17-23 successive batch processes of 4-nitrophenyl palmitate hydrolysis in water. Activity of preparations varied from 20 to 5100 U∙mL-1 but the most stable preparations were those of moderate activity: bound by adsorption or covalent attachment to NH2-Kieselgel or acrylic carrier (retained activity over 90%). Selected preparations were used for hydrolysis of ethyl (1-butyryloxyethyl)-phenylphosphinate in biphasic system, and, after drying, in ethyl (1-hydroxyethyl)-phenyl-phosphinate transesterification. In this study operational stability was the principal criterion of selection. In water system, lipase covalently bound to NH2-Kieselgel was the best - preserved 50% of initial activity in consecutive batch processes. In biphasic system and lipase covalently bound to acrylic and NH2-Kieselgel the values were 90 or 77%, respectively, whereas in organic solvent, when lipase was immobilized on NH2-Kieselgel by adsorption, it was 50%. Thus, NH2-Kieselgel appears to be an universal matrix for investigated lipase immobilization and can be used in all reaction systems.
