INTRODUCTION: For many months now, the entire world experiences effects of the COVID-19 pandemic, and current data indicate that this condition will continue even longer. About 5% of patients who become infected with the SARSCoV-2 coronavirus may develop a critical form of severe respiratory failure, due to which they will undergo intubation and mechanical ventilation. Prolonged mechanical ventilation will be an indication for an open tracheostomy, burdened with an increased production of aerosol containing virus particles and an increased risk of transmitting the highly contagious pathogen to medical personnel. In this situation, surgical departments with a limited number of operating theaters may be dedicated only to COVID-19 patients. Their use for other procedures will be significantly reduced or even impossible to prevent transmission of infections and exposure to other people. Taking into account the well-being of patients and staff and limiting the transmission of the virus to other departments, other safe solutions are being sought to perform a tracheostomy. An alternative procedure may be to perform a tracheostomy directly at the bedside (point-of-care) of a patient hospitalized in an intensive care unit (ICU). PURPOSE: The purpose of the study is to present an effective performance of safe, open tracheostomy in the ICU. METHODS: The bedside tracheotomy as an alternative to the procedure performed in the setting of an operating theater has been presented. A case study was based on the analysis of the procedure performed in a 42-year-old patient who was installed an open tracheotomy tube on the twenty-ninth day of intubation. CONCLUSIONS: In order to limit the transmission of the SARS-CoV-2 coronavirus outside the intensive care unit, it is recommended to perform a tracheotomy in ICU, in patients requiring mechanical ventilation, as the method is effective and safe.

Schizophrenia is a serious mental disorder and has a huge economic burden related to direct and indirect costs. Direct costs include such expenditure as hospitalization. Indirect costs, however, result mostly from the loss of productivity. Although patients are frequently interested in job opportunities, unemployment is widespread among them. Thus, we evaluated opinions of medical universities students’ concerning situation on the labor market among those in the remission stage of schizophrenia. The study was carried out between October 2015 and October 2016 in Poland and other European Union countries using a self designed questionnaire. The study tool consisting of five questions was distributed to students by direct contact and via the Internet. Of a 5200 distributed sheets, we received 1036 questionnaires and 942 were finally analyzed. In the students’ opinion, patients are generally interested in finding a job but they are afraid coming back to work. Although unemployment is common among patients with schizophrenia, the number of patients capable of working is significantly higher than the number of those actually employed. According to results of the study, getting a job might be effectively supported by non-pharmacological interventions. Considering the noticeably higher number of patients capable of working compared to those employed, development of employment possibilities should be a priority for mental health care decision makers. It should lead to reduction of the patients’ concerns related to looking for a job. Moreover, comprehensive treatment could reduce the economic burden of schizophrenia.

Immunization is a very effective health intervention. Moreover, the global vaccines market has been growing rapidly and costs of full children immunization are dramatically higher nowadays than 30 years ago. High vaccine prices may limit affordability of vaccination which is why we attempted an evaluation of availability and affordability of 7 and non-reimbursed vaccines mostly used in children in Poland. The study was conducted between October 2016 and October 2017 using a specially designed anonymous questionnaire comprising three closed-ended questions. The study tool was distributed by direct contact or via the Internet. Eventually, answers from 505 pharmacies from the Greater Poland region and 10 primary care clinics were included. 5 out of 7 vaccines were available in all types of facilities. There were some issues, however, with availability of Bexsero® and Nimenrix®. Considering prices, the highest difference (of more than 100%) was found for Infanrix hexa® and the lowest (38.5%), for Infanrix IPV+HIB®. In 88.17% of pharmacies included, patients were informed about a thermo-insulating package. 48.39% of respondents indicated that such package is free of charge, while in other pharmacies an average price for the package was EUR 0.48. Although availability and affordability of medicines are crucial objectives of the public health policy, it seems that access to vaccines in Poland might be an area for improvement. Thus, prices of non-reimbursed vaccines could be regulated in Poland nationwide. Moreover, to provide trustworthy information concerning vaccination, healthcare decision makers should consider social education about immunization as an important issue.

The presented studies focus on changes in groundwater levels and chemistry, and the identification of important factors influencing these changes on short- and long-term scales in urban areas. The results may be useful for rational and sustainable groundwater planning and management in cities. The studies concerned three aquifers: (1) the shallow Quaternary aquifer, (2) the deep Quaternary aquifer, and (3) the Oligocene aquifer in the capital city of Warsaw (Poland). The spatial variability of groundwater recharge was determined and its changes in time were characterized. The characteristics of groundwater levels were based on long-term monitoring series. The results indicate that urban development has caused overall reduction in infiltration recharge (from 54 to 51 mm/year), which is particularly clear in the city suburbs and in its centre, where land development has significantly densified during the last 30 years. Studies of groundwater levels indicate variable long-term trends. However, for the shallowest aquifer, the trends indicate a gradual decrease of the groundwater levels. In the case of the much deeper Oligocene aquifer, groundwater table rise is observed since the 1970s (averagely c. 20 m), which is related with excessive pumping. Based on the studied results, the groundwater chemistry in the subsurface aquifer indicates strong anthropogenic influence, which is reflected in multi-ionic hydrogeochemical types and the occurrence of chemical tracers typical of human activity. The Oligocene aquifer is characterized by a chemical composition indicating the influence of geogenic factors.
