Opis:
The definition of a Stefan suspension of a diffeomorphism is given. If $_g$ is the Stefan suspension of the diffeomorphism g over a Stefan foliation , and G₀ ∈ satisfies the condition $g|G₀ = id_{G₀}$, then we compute the *-holonomy group for the leaf $F₀ ∈ _g$ determined by G₀. A representative element of the *-holonomy along the standard imbedding of S¹ into F₀ is characterized. A corollary for the case when G₀ contains only one point is derived.

Introduction: The SARS-CoV-2 pandemic is one of the biggest healthcare challenges that the medical environment has needed to face since many, many years. Clinicians all over the world present their recommendations for everyday procedures in order to provide safety to the medical staff as well as to patients. The disease leads to ARDS in many cases and some patients will require prolonged intubation; therefore, to avoid the negative aspects of this condition, a number of patients will undergo tracheostomy. Tracheostomy is an aerosol-generating procedure, therefore, when performed on a SARS-CoV-2-positive patient, there is a high risk of contamination both of the medical team and the operating theatre. Aim: We describe a set of guidelines that we believe should minimize those risks. We focus on aspects like: presurgical testing, proper preparation of the operating theatre prior to the patients’ arrival, initial education of the medical staff participating in the surgery, patient’ transport and tracheostomy. We describe the critical points during every step and suggest ways to minimalize the risk of viral transmission.
Wprowadzenie: Pandemia wywołana przez wirusa powodującego ostrą chorobę dróg oddechowych 2 (SARS-CoV-2) w 2020 roku jest jednym z największych wyzwań, z jakimi musi zmierzyć się opieka zdrowotna na całym świecie w ciągu kolejnych kilkudziesięciu lat. Każdego dnia lekarze podejmują próbę dostosowania codziennych procedur, tak aby zapewnić bezpieczeństwo na najwyższym poziomie zarówno sobie, jak i pacjentom. Z uwagi na to, że choroba ta prowadzi do zespołu ostrej niewydolności oddechowej (ARDS), wielu chorych wymaga przedłużonej intubacji. Aby uniknąć licznych powikłań, część chorych na ostrą chorobę dróg oddechowych wywołaną przez wirusa SARS-CoV-2 (COVID-19) wymaga bądź będzie wymagało wykonania tracheotomii w warunkach bloku operacyjnego. Podczas tracheotomii wytwarza się aerozol stwarzający bardzo duże ryzyko zakażenia personelu medycznego oraz skażenia bloku operacyjnego. Cel: W pracy przedstawiamy zbiór wskazówek mających na celu jak najbezpieczniejsze przeprowadzenie tracheotomii. Skupiamy się na aspektach, takich jak: wykonywanie testów wykrywających RNA wirusa przed zabiegiem, odpowiednie przygotowanie sali operacyjnej, dokładne przeszkolenie personelu biorącego udział w każdym etapie zabiegu, transport pacjenta oraz procedura tracheotomii. Przedstawiamy punkty krytyczne podczas wykonywania każdej z tych czynności i staramy się wyjaśnić, jakie kroki należy powziąć, aby zminimalizować ryzyko transmisji wirusa.

Pines are generally fire-resistant trees. There is a shortage of research on the behaviour of Scots pine after surface fire in older stands. The aim of the work was to describe the effect of the surface fire intensity on the mortality of pines of various diameter at breast height (DBH), including older trees. The research was conducted in Peucedano-Pinetum oligotrophic Scots-pine forest in Kampinos National Park (KPN, central Poland) on the area of two adjacent surface fire sites originated in spring 2015 in 60- to 200-year-old stands (site area: 10,92 ha). There were 45 (28 burned and 17 control) permanent plots established after the fire. The share of not burned, superficially burned and completely burnout organic horizon of the soil was determined within all of them. DBH and location of pine trees were measured within all of the plots on the area of 200 m2. For all of the trees for which full information about soil organic horizon damage was mapped, the prevailing type of disturbance in their close neighbourhoods with radii of 1 and 2 m was assessed. The mortality of trees was assessed after each vegetation period up to 2017, basing on the presence of green needles on the trees. The influence of fire intensity on the survival of trees was examined on whole permanent plot level as well as on individual tree level. Strong linear correlation was observed between Scots pine mortality and the share of plots area with damaged organic layer, especially at the end of the third vegetation period after fire. Logistic regression models constructed for individual trees suggest that bigger tree diameter (hence, thicker bark) diminished the odds of mortality only after two vegetation periods from the fire. After the third vegetation period, only the intensity of surface fire in the close neighbourhood of trees influenced (negatively) the chance on survival. The size of trees did no matter in this case. Nearly all of the trees that were located within burnout organic matter areas died. The results did not support the commonly known mechanism of enhancement of bigger Scots pine tree survival after surface fire because of thicker bark responsible for heat protection. Probably, the main cause of observed mortality was not overheating of cambium but it was rather connected to massive fine root loses. Scots pines growing on oligotrophic arid sites modify their root system to explore topsoil layers with higher proportion of shallow roots, growing even in organic litter layer. This corresponds with massive (regardless of size) pine mortality within sites characterised by complete burnout of organic matter layer and very high survival in those ones with only surfacely burned litter layer. The results can improve the assessment of surface fires consequences in managed Scots pine stands growing in oligotrophic conditions.

Inguinal hernia repair and cholecystectomy are amongst the most common surgical procedures performed worldwide. In the recent decades, early disease detection has notably increased due to easily accessible ultrasound. The aim of the study was to assess the safety and the possibility of performing a simultaneous hernia repair and cholecystectomy using the laparoscopic approach. Material and methods. Eight patients (M=100%) with inguinal hernia (3 with bilateral hernia) and cholelithiasis were included in the study. The presence of gallstones was confirmed by imaging. Mean age of the patients was 61.75 years (ranging from 47-72). Simultaneous laparoscopic cholecystectomy and transabdominal pre-peritoneal hernia repair was performed in all patients. Postoperative complications were analyzed to assess the safety and feasibility of the procedure. Results. Mean operating time was 55 minutes (ranging from 30-60) and average length of stay was 3.625 days (ranging from 2-7). In order to perform a cholecystectomy, 1-2 additional trocars were used. No intra-operative complications were observed. At a follow-up visit on postoperative day 7, a small hematoma (10 ml of blood was punctured) in the right groin was noted in one patient. Another patient developed fever postoperatively, treated conservatively with antibiotics. Conclusions. Simultaneous TAPP and cholecystectomy proved to be a safe and feasible procedure. Acceptable operating time and hospital stay, as well as lack of influence on the length of convalescence, may present an interesting alternative to two separate procedures

Full Scale Fatigue Test

This paper presents a general concept of the Full Scale Fatigue Test of the Mi-24 helicopter including the test layout and load distribution, as well as describes the milestones to be achieved. Additionally, some initial work conducted in order to determine both the mass and load distribution in the structure is described. The main goal of the test is to verify the low cycle fatigue life of the helicopter structure (fuselage, tail boom, wings and landing gear). The test will be divided into two main stages at which flight and landing loads will be applied. The authors demonstrate the general test concept, the helicopter’s structure fixture and the arrangement of the hydraulic actuators at both stages in order to achieve representative loads during the test. The proposed concept is based on AFIT’s previous experience in full scale structural testing, available literature and the experience of the test staff.
