Opis:
A series of N,O-acylated chitosan derivatives were emulsified with different fatty acids. Hydrophobically modified chitosan derivatives were expected to exhibit self-assembly behaviour resulting in micelle formation. Several parameters of the oil-in-water emulsification process were investigated: mixing method, speed and duration, volume oil phase and addition of modifiers. Their influence on micellar Z-average diameter, size distribution and Zeta potential was analysed based on dynamic light scattering measurements. There were various relations between the hydrodynamic behaviour of chitosan derivatives, their chemical structure and the process parameters. Additionally, the obtained micelles could serve as active compound carriers because they encapsulated two model substances, namely ibuprofen and α-tocopherol.

We present an overview of current diagnostic and therapeutic guidelines on Ménière’s disease. Over the last five years, several guidelines and studies on this subject have been published, including The International consensus (ICON) on treatment of Méniere’s disease of 2018, The European Position Statement on Diagnosis, and Treatment of Ménière’s Disease, The Diagnostic and therapeutic strategy in Menière’s disease. Guidelines of the French Otorhinolaryngology-Head and Neck Surgery Society (SFORL) of 2017 . Consequently, also the “American Academy of Otorhinolaryngology, Head and Neck Surgery” (AAO-HNS) published updated recommendations on the subject in April 2020. On the basis of these studies and a literature review, the main treatment methods used in Ménière’s disease and an assessment of their effectiveness were presented. The current recommendations, as well as own experience of the Clinic of Otorhinolaryngology, Head and Neck Surgery of the Medical University of Warsaw, gave grounds for proposing a therapeutic scheme.

Introduction: It is estimated that hearing impediment affects approximately 12% of the population. Some forms of hearing disorders can be successfully treated conservatively, while in others surgery plays a key role. Often, therapeutic methods are exhausted and the patient faces the decision about the need for acoustic organ prosthesis. Depending on the causes of anomalies, apparatus for air conduction, bone conduction or cochlear implants are used. A cochlear implant replaces the damaged sensory cells of the inner ear. It constitutes an electronic prosthesis of the acoustic organ, composed of two parts. The internal part (implanted in the inner ear) consists of an implant and electrodes, which conduct electrical signals to the cochlea. The external part consists of a microphone, a speech processor, and a transmitter. Care report: A 70-year-old patient with deep hearing loss in the right ear and deafness of the left ear. During the interview, the patient denied mechanical injuries of the head and neck, acoustic trauma, inflammation or otorrhea. Based on the conducted tests, qualification otolaryngology, audiology, and otoneurology, radiology, surdologopedy and psychology were performed for cochlear implant insertion. The patient was qualified for cochlear implantation in the left ear under general anesthesia. The surgical team was composed of dr hab. Maciej Wiatr, prof. dr hab. Kazimierz Niemczyk, prof. dr hab. Jacek Składzień, MD Agnieszka Wiatr with anesthetic monitoring under the supervision of MD Renata Witkowska. The patient was discharged home on day 7 after surgery in good general and local condition. Simultaneous rehabilitation was started at the Center for Daily Rehabilitation of Hearing and Speech at the Specialist Center of Diagnosis and Rehabilitation for Children and Adolescents with Hearing Disabilities of the Polish Association of the Deaf in Cracow.

Ménière’s disease (MD) is characterised by intermittent episodes of vertigo with fluctuating sensorineural hearing loss, tinnitus and aural fullness. Despite numerous studies, the etiology of this disorder remains poorly understood and thus diagnostic criteria are mainly clinical. Development and progress in magnetic resonance imaging (MRI) techniques, in 2007, has enabled visualisation of endolymphatic hydrops, which was the milestone achievement. We present two cases of patients with clinically confirmed MD with a set of audiological, otoneurological examinations and MRI performed. Structures of the inner ear were visualized using contrast-enhanced MR imaging after a 4-hour delayed intravenous gadolinium administration. The cochlea and vestibule were evaluated separately and classified using Barath grading scale. Endolymphatic hydrops in the clinically affected ears were confirmed using MRI. Due to disabling and intractable MD, a decision about vestibular neurectomy was made.

Ménière’s disease is characterized by sudden episodes of vertigo accompanied by tinnitus and/or feeling of fullness in the ear as well as fluctuating sensorineural hearing loss. Despite numerous studies, the etiology of this disease remains unknown. However, the enlargement of the inner ear’s endolymphatic spaces, referred to as endolymphatic hydrops, is considered the underlying condition. Thanks to recent advances in magnetic resonance (MR) technology, it is now possible to obtain in vivo imaging of endolymphatic hydrops in patients presenting with Ménière’s disease symptoms. Visualization of the inner ear fluid compartments is achieved after gadolinium contrast is administered into the tympanic cavity or via the intravenous route. Evaluation of endolymphatic hydrops is possible as the contrast agent selectively penetrates the perilymph, and endolymph is visualized as contrast defects. The currently used radiological hydrops grading systems include qualitative, semi-quantitative, and volumetric scales. The methods are subject to ongoing modifications to increase their sensitivity and specificity. Numerous studies describe correlations between clinical symptoms and audiological and otoneurological examination results with the endolymphatic hydrops grade. MRI is also applicable in patients’ diagnostics with an incomplete or atypical course of the Ménière’s disease. In the course of the treatment, follow-up MRI scans enable assessing individual treatment modalities’ efficacy in terms of the severity of lesions and the further course of the disease within the inner ear.

Despite multidirectional activities in the sphere of politics, health care and limiting direct interpersonal contacts to a minimum, the Covid-19 pandemic has covered the whole world and the number of new cases is rising exponentially. This demonstrates the considerable severity of the situation. The doctors and other health professionals are and will be at the forefront of the fight against the pandemic, being at the same time the group most at risk of infection. SARS-CoV-2 virus infection applies to patients of all ages, with a median age of 49–59 years. The most common complaints in patients with Covid-19 include fever, caught, dyspnea, general malaise and muscle aches. In the course of Covid-19 may occur pneumonia, which in about 20% of cases is moderately to severe and associated with respiratory distress. Important for otorhinolaryngologists are reports of smell and taste disorders. Post-infective loss of smell can make up about 30% of all patients with Covid-19. The confirmation of SARS-CoV-2 infection in patients with symptomatic or epidemiological suspicion of Covid-19 are genetic tests RT-PCR. Biological material is usually taken from the nose or nasopharynx. The management of Covid-19 is symptomatic because there is currently no specific cure drug. There is a particularly high risk of transmitting infection from a patient to the physicians, who are treating diseases of the respiratory tract or perform any interventions on the upper and lower respiratory tract. Low-symptomatic first stages of the disease with high viral load mean that during a pandemic, scheduled examinations and procedures should be limited. During the examination of patients or their treatment, adequate full PPE protection is indicated, reducing the risk of accidental infection of the treatment team. For the treatment of patients with confirmed SARS-CoV-2 infection or requiring immediate treatment without the possibility of determining epidemiological factors Covid-19, full protection in PPE is obligatory.

We present an overview of current diagnostic and therapeutic guidelines on Ménière’s disease. Over the last five years, four studies on this subject have been published. In 2015, five Societies including the Classification Committee of the Barany Society, Japan Society for Equilibrium Research, EAONO - European Academy of Otology and Neurotology, AAO-HNS - American Academy of Otorhinolaryngology - Head Neck Surgery, and Korean Balance Society have issued updated guidelines on the diagnosis and treatment of Ménière’s disease. Other multi-center publications have since been published as well such as the International consensus (ICON) on treatment of Ménière’s disease 2018, European Position Statement on Diagnosis, and Treatment of Meniere’s Disease, Diagnostic and therapeutic strategy in Ménière’s disease, Guidelines of the French Otorhinolaryngology - Head and Neck Surgery Society (SFORL) 2017. The above-listed recommendations include the most recent diagnostic advancements (e.g. inner ear MRI) and new electrophysiological tests, but also discuss other commonly used treatment modalities, result reporting and they take into account the quality of life of the patients following treatment. We present the main clinical challenges in managing Ménière’s disease, principles of diagnosis as well as the treatment introduced at our center. Finally, we discuss fundamental updates contained in the published papers.

The paper presents an investigation on the feasibility of recovery of the highly valuable silicon carbide (SiC) from the slurry waste generated from silicon wafer production in the photovoltaic and semiconductor industry. Compared to the other techniques of recycling, a facile and low-cost method of waste treatment via heat drying followed by low-energy mixing in a shaker mixer was proposed. As the result of the treatment, the slurry waste was converted into a powdered form with dominant content of SiC. Separated SiC material was characterized by scanning electron microscopy, energy-dispersive X-ray spectroscopy, X-ray powder diffraction, and sieve analysis. In addition, analyses of the bulk density, moisture content and melting test were carried out. As was confirmed by the physicochemical analyses, the dominant sieve fraction was in the range of 0.1-0.06 mm, the purity level was a minimum 99% mass of SiC, the moisture content - 0.3%, the bulk density - 1.3 g/cm3. The physicochemical characteristics of the material were crucial for understanding the material performance, assessment of the material quality and determining the perspective directions of the industrial application. The studies revealed that the material exhibited a high application potential as abrasive, especially in abrasive grinding and waterjet cutting.

Opis:
Chitosan derivatives with a series of fatty acids (FA) have been developed using simultaneous N- and O-acylation reaction by the combination of two ways of conducting the reaction, i.e. a carbodiimide catalysis and ionic amino group protection. The chemical structure of chitosan derivatives as well as the characterization of the FA substitution degree were done by the IR spectra analysis. The correlation between the substitution of the chitosan functional groups as well as the saturation of FA and the changes of structural and thermal properties of the derivatives has been presented.

Opis:
Patients with head and neck cancers have an increased risk of second malignant neoplasm occurrence (25% in long-term follow-up, 4% yearly). Most patients with a primary tumor can be treated with organ-preserving strategies including radiotherapy (RT) or chemoradiotherapy (CRT). Very often after radical surgical excision there are indications for adjuvant radiotherapy. However, if recurrence or second malignancy will develop after definitive primary RT, there are limitation to use therapeutic dose of radiation again for the same area. Salvage surgery is regarded as the only curative option in those cases. In our study we analysed 3 patients, who presented with head and neck second cancer in the area of hypopharynx, within 10-25 years after primary radiotherapy due to oropharyngeal and laryngeal cancer. A total pharyngectomy with larynx preservation and bilateral neck dissection were performed in the first case and the radial forearm free flap (RFFF) was used for reconstruction of the pharynx. Second patient, due to perilaryngeal invasion in radiological images, underwent total pharyngolaryngectomy with selective bilateral cervical lymph nodes dissection and digestive track was restored also with RFFF. The third patient had previously undergone laryngectomy and after total pharyngectomy the free autologous jejunal flap (FAJF) was used for reconstruction. The main goal of reconstruction after salvage surgery due to hypopharyngeal cancer is to reconstruct the digestive tract. The other important aspect is to optimize the anatomy for voice rehabilitation and swallowing. Unfortunately, radiation and chemotherapy impair wound healing, which indicates increased risk of postoperative complications and makes salvage surgery results unpredictable.
