INTRODUCTION: Central serous chorioretinopathy (CSC) is a disease characterized by idiopathic serous elevation of the sensory layer of the retina in the proximity of the macula. This phenomenon predominantly manifests unilaterally, particularly in young males who exhibit increased susceptibility to stress. The optimal treatment strategy for CSC remains questionable due to a diverse range of symptoms, variable clinical courses, an overall limited understanding of CSC’s pathophysiology and a lack of consensus on classification systems. Recently, there has been growing adoption of micropulse laser therapy in CSC treatment, allowing targeted action on retinal pigment epithelium, without causing damage to nearby photoreceptors. The aim was to evaluate the use of micropulse laser therapy in chronic CSC. DESCRIPTION AND RESULTS: A 38-year-old patient with recurrent CSC in the right eye, previously treated locally with eye drops and oral medication for over a year, underwent evaluation. At the time of diagnosis, the patient exhibited a visual acuity of 0.63, intraocular pressure of 19 mmHg, and a central retinal thickness of 530 µm, as measured by optical coherence tomography (OCT). Fluorescein angiography (FA) confirmed the diagnosis, leading to the qualification for retinal micropulse laser therapy. Following the treatment, a subsequent FA test revealed improvement in the patient’s clinical condition, with a vision acuity of 1.0 and intraocular pressure reduced to 17 mmHg. Additionally, the central retinal thickness measured by OCT decreased to 299 µm. CONCLUSIONS: The use of micropulse laser therapy demonstrates the potential to achieve sustainable clinical effects in patients with recurrent CSC.
WSTĘP: Centralna chorioretinopatia surowicza (central serous chorioretinopathy – CSC) jest schorzeniem charakteryzującym się idiopatycznym surowiczym uniesieniem sensorycznej warstwy siatkówki w okolicy plamki, które najczęściej występuje jednostronnie, głównie u młodych mężczyzn ze zwiększoną podatnością na stres. Definiowanie optymalnego leczenia CSC jest skomplikowane ze względu na szeroki zakres objawów choroby, zmienny przebieg kliniczny, słabo poznaną patofizjologię CSC oraz brak konsensusu co do systemu klasyfikacji. W leczeniu CSC coraz częściej znajduje zastosowanie laser mikropulsowy, który działając selektywnie na nabłonek barwnikowy siatkówki, nie powoduje uszkodzeń fotoreceptorów. Celem pracy była ocena zastosowania terapii laserem mikropulsowym w leczeniu przewlekłego CSC. OPIS I WYNIKI: Pacjent 38-letni leczony od kilkunastu miesięcy z powodu nawracającego CSC w oku prawym, leczony miejscowo kroplami oraz lekami doustnymi. Ostrość wzroku pacjenta w chwili rozpoznania wynosiła 0,63, ciśnienie wewnątrzgałkowe 19 mmHg. Centralna grubość siatkówki w optycznej koherentnej tomografii (optical coherence tomography – OCT) wynosiła 530 µm. U pacjenta wykonano angiografię fluoresceinową (fluorescein angiography – FA) potwierdzającą rozpoznanie oraz zakwalifikowano go do laseroterapii siatkówki z użyciem mikropulsów. Podczas kontroli w badaniu FA uzyskano poprawę stanu klinicznego. Ostrość wzroku pacjenta wynosiła 1,0, ciśnienie wewnątrzgałkowe 17 mmHg. Uzyskano spadek centralnej grubość siatkówki w badaniu OCT do poziomu 299 µm. WNIOSKI: Zastosowanie lasera mikropulsowego umożliwia uzyskanie trwałego efektu klinicznego u pacjenta z przewlekłym nawracającym CSC.

Surgical treatment of acute appendicitis in older patients

Introduction: Acute appendicitis (AA) is the most common indication for emergency surgery and it occurs more often in children and young adults than in patients of advanced age. AA in older patients is a challenging surgical problem because of its atypical presentation. This study was performed to determine whether the age of a patient impacts the outcomes and whether laparoscopic appendectomy (LA) is a feasible method for treatment of patients > 65 years. Material and methods: We performed a retrospective study of 355 patients with AA who were admitted to the Department of General, Minimally Invasive and Elderly Surgery in Olsztyn from 2014 to 2017. The patients were divided into three age groups: 18 to 40, 41 to 65 and >65 years. The histopathological diagnoses were divided into three types: simple AA, phlegmonous AA, and gangrenous AA. Results. LA was performed in 96% of young adults and in 67% of older patients. The patients older than 65 years had higher preoperative white blood cell counts, higher C-reactive protein (CRP) levels and had a longer length of stay (LOS) than younger patients (P=0.05, P=0.03, and P=0.03, respectively). We found a positive correlation among the CRP levels, open appendectomy (OA), and gangrenous appendicitis. Conclusions: Patients older than 65 years more frequently underwent OA than LA, had higher preoperative CRP levels and had a longer LOS than younger patients. Higher CRP levels were associated with a greater risk of gangrenous appendicitis. LA is a safe and feasible treatment method for patients older than 65 years.

Introduction: The prevalence of obesity in Poland and worldwide is constantly rising. High effectiveness of bariatric surgery has been proven in literature. It is recommended that bariatric procedures should be done by highly qualified surgeons with the appropriate, up-to-date medical equipment. Aim: The purpose of the study is to establish Polish recommendations and standards for the use of medical equipment for bariatric surgery centers. Materials and methods: The review of the present recommendations of the worldwide organizations and societies (including EAES, IFSO, SAGES) and guidelines was made. On the basis of current literature and authors’s clinical experience we proposed standardized protocol for bariatric surgical equipment. Conclusions: Relevant equipping of bariatric surgery centers and implementation of standardized perioperative and surgery protocols will result in significant improvements in bariatric treatment. This will ensure patients safety, a shorter length of hospital stay and considerably reduce the risk of morbidity. Moreover, it will contribute to the efficacy of the bariatric and metabolic surgery procedures, in accordance with the highest globally accepted standards.

Oncoplastic and reconstructive techniques are essential tools in the armamentarium of contemporary breast surgeons. The aim of the study was to identify oncoplastic reconstructive patterns in breast cancer centers across Poland. A questionnaire of 18 questions was sent by email to the members of the Polish Society of Surgical Oncology and the Polish Society of Plastic, Reconstructive and Esthetic Surgery via their dedicated websites. The numbers of breast cancer patients operated on in each center ranged from 120 to 904 per year. Breast-conserving surgery (BCS) predominated in all but one center (range 50-70%). Immediate breast reconstructions (IBR) accounted for 6-42% of procedures, The most frequent type of IBR was either a two-stage expander followed by a permanent implant or one-stage implant- based with or without synthetic mesh. The most frequent type of delayed breast reconstruction (DBR) was a two-stage expander followed by implant-based reconstruction. None of the surveyed cancer centers performed free flap reconstruction. Deep inferior epigastric perforator (DIEP) flaps were performed in the plastic surgery department. Reconstructions based on pedicled flaps were performed in cancer centers. Acellular dermal matrices (ADM) and fat transfer were used in selected centers. In the clinical scenario of adjuvant radiotherapy, delayed breast reconstruction was favored. The full range of oncoplastic BCS was performed. Patient-reported outcome measures (PROM) and complications were assessed. Our findings can act as a platform for further improvement in skills, certification, data collection and audit, including patient reported expectation measures. There is also an urgent need to address pan-European inconsistencies in procedural reimbursement.
