The study area was a small lowland river valley (the Łęg river) located in the south-east of Poland. The object of investigation was the macrophytes of 10 river lakes with corresponding active river channel stretches of the same length as the cut-offs. The aim was to check the difference in species diversity between cut-off and active river channels. The second aim was to test the following hypothesis: vegetation of river lake has been shaped under the influence of contiguous river stretch which has left a measurable mark in species abundance and composition. To test this, we checked whether a cut-off channel’s flora is more similar to flora of the contiguous river stretch, than to flora of a farther river stretch. During the course of the study it was found that the average species richness was approximately two times higher in the cut-off channels than in the river stretches. The number of the species exclusive for the river lakes was nine times higher in comparison with the river’s exclusives (not found in the cut-offs). The Shannon diversity index definitely spoke in favor of the river lakes. These results clearly show the significance of river lakes in maintaining biodiversity of aquatic ecosystems in a small river valley. We did not confirm our hypothesis concerning floristic relation between an active channel and its cut-offs. The floristic similarity between a given cut-off channel and the contiguous active river channel stretch is not stronger than the similarity between this lake and more distant river stretches. The cause of such a state of affairs may be high natural dynamics of investigated habitats and anthropogenic transformation of the river valley.

The aim of this study is to present the updated list of rare and threatened vascular plant species in the Lublin province. The threatened species categories are presented according to the IUCN criteria. The regional list contains 408 species (that makes up 25% of the Lublin Region flora); 56 of which are considered critically endangered, 81 represent endangered species, 51 belong to the category of vulnerable species, and 49 are near threatened species, respectively. The presence of 37 species has not been confirmed and therefore they are listed as regionally extinct (RE). Of the total number of species, 134 are rare, but because the data about those species is insufficient, they have been classified as DD category (data deficient). The species of the genera: Alchemilla, Callitriche, Hieracium, Oenothera, Rosa, Rubus and Taraxacum have not been evaluated (NE category). Other species – those whose occurrence is doubtful and all neophytes were defined as not applicable (NA). The endangered taxa occur mostly in dry grasslands, deciduous forests, wetlands, and calcareous weed communities of cereal crops.

Macrophytes and selected physical-chemical water properties were studied in 17 post-mining lakes of Jeziórko Sulphur Mine – one of the largest borehole sulphur mines in the world. Artificial lakes were constructed in subsidence depressions during the reclamation process of mining fields. They were characterized by high mineralization – conductivity ranged from 723 to 2295 μS/cm. The reaction was near neutral, or more frequently, slightly alkaline. Concentrations of phosphorus and organic matter were low. In the group of hydrophytes, Ceratophyllum demersum, Myriophyllum spicatum, Najas marina, Utricularia vulgaris, Eleocharis acicularis, Potamogeton pectinatus, Potamogeton natans and Potamogeton pusillus were frequently dominant. In several lakes, large macroscopic algae dominated – charophytes forming large, dense Chara meadows. In marsh communities, Phragmites australis was the most expansive. Reclamation process had a positive effect on diversity, some of the species found in the study area are regionally rare, including one species which is threatened in Poland (Najas minor). No alien species were recorded.

A novel approach to a trigger mode in the Gas Electron Multiplier (GEM) detector readout system is presented. The system is already installed at WEST tokamak. The article briefly describes the architecture of the GEM detector and the measurement system. Currently the system can work in two trigger modes: Global Trigger and Local Trigger. All trigger processing blocks are parts of the Charge Signal Sequencer module which is responsible for transferring data to the PC. Therefore, the article presents structure of the Sequencer with details about basic blocks, theirs functionality and output data configuration. The Sequencer with the trigger algorithms is implemented in an FPGA chip from Xilinx. Global Trigger, which is a default mode for the system, is not efficient and has limitations due to storing much data without any information. Local trigger which is under tests, removes data redundancy and is constructed to send only valid data, but the rest of the software, especially on the PC side, is still under development. Therefore authors propose the trigger mode which combines functionality of two existing modes. The proposed trigger, called Zero Suppression Trigger, is compatible with the existing interfaces of the PC software, but is also capable to verify and filter incoming signals and transfer only recognized events. The results of the implementation and simulation are presented.

The validation of the measurements quality after on-site diagnostic system installation is necessary in order to provide reliable data and output results. This topic is often neglected or not discussed in detail regarding measurement systems. In the paper recently installed system for soft X-ray measurements is described in introduction. The system is based on multichannel GEM detector and the data is collected and sent in special format to PC unit for further postprocessing. The unique feature of the system is the ability to compute final data based on raw data only. The raw data is selected upon algorithms by FPGA units. The FPGAs are connected to the analog frontend of the system and able to register all of the signals and collect the useful data. The interface used for data streaming is PCIe Gen2 x4 for each FPGA, therefore high throughput of the system is ensured. The paper then discusses the properties of the installation environment of the system and basic functionality mode. New features are described, both in theoretical and practical approach. New modes correspond to the data quality monitoring features implemented for the system, that provide extra information to the postprocessing stage and final algorithms. In the article is described also additional mode to perform hardware simulation of signals in a tokamak-like environment using FPGAs. The summary describes the implemented features of the data quality monitoring features and additional modes of the system.

The paper presents improvements of the developed system for hot plasma radiation measurement in the soft Xray range based on a Gas Electron Multiplier (GEM) detector. Scope of work consists of a new solution for handling hardware time-synchronization with tokamak systems needed for better synchronization with other diagnostics and measurement quality. The paper describes the support of new modes of triggering on PC-side. There are communication and data path overview in the system. The new API is described, which provide separate channels for data and control and is more robust than the earlier solution. Work concentrates on stability and usability improvements of the implemented device providing better usage for end-user.

In the paper, we try to find a new, intuitive solution to the Fitch paradox. We claim that traditional expression of Knowability Principle (p → ◊Kp) is based on erroneous understanding of knowability as de dicto modality. Instead, we propose to understand knowability as de re modality. In the paper we present the minimal logic of knowability in which Knowability Principle is valid, but Fitch Paradox does not hold anymore. We characterize the logic semantically as well as by an axiomatic and tableaux procedure approach.
W artykule staramy się znaleźć nowe, intuicyjne rozwiązanie paradoksu Fitcha. Twierdzimy, że tradycyjne wyrażenie zasady poznawalności (p → ◊Kp) opiera się na błędnym rozumieniu poznawalności jako modalności de dicto. Zamiast tego proponujemy rozumieć poznawalność jako modalność de re. W artykule przedstawiamy minimalną logikę poznawalności, w której zasada poznawalności jest ważna, ale paradoks Fitcha już nie obowiązuje. Logikę charakteryzujemy semantycznie, a także poprzez podejście aksjomatyczne i tabelaryczne.

Resection of the gall-bladder is still the most common surgical procedure performed at general surgery departments. The laparoscopic method used in the majority of cases offers considerable benefits but at the same time is associated with an increased rate of bile duct complications. So far, a slim female aged 25-50 years was a typical patient with a iatrogenic bile duct injury. The aim of the study was to identify the age of patients with iatrogenic bile duct injuries as well as the clinical course observed in recent years. Material and methods. Gender and age structure of patients admitted to the Department of General, Transplant and Liver Surgery between the beginning of 2011 and June 2014 and treated for iatrogenic bile duct injuries, complications of laparoscopic cholecystectomy, were analysed. The patients were referred to the department as a reference centre. Results. In the group of 186 patients, females predominated (69.4%) and the mean age was 52 years. A considerable increase in the mean age of patients treated in 2014 as compared with previous years was seen. This was related to an increased rate of bile duct injuries in patients aged over 70 years, who accounted for about 25% of the group. In previous years, bile duct injuries in patients of such an advanced age happened considerably less frequently. Conclusions. A iatrogenic bile duct injury in an elderly person may prove a fatal complication. A repair surgery, i.e. the biliary-enteric anastomosis, is a major and burdensome procedure, particularly in the case of patients aged over 70 years. Special caution during laparoscopic cholecystectomy is advised in this population, and the slightest doubts should lead to conversion.

Hemangiomas are the most common benign primary hepatic neoplasms, often being incidentally discovered. In most of the cases they are small and asymptomatic. It is widely accepted that clinical intervention is indicated only for symptomatic hemangiomas. We present a case of an asymptomatic giant hemangioma managed by enucleation due to its atypical localization. The hemangioma, originally located in segment 5, was now described in Computer Tomography (CT) Imaging as separating the gallbladder from the liver parenchyma. A careful evaluation of images revealed proximity to the portal vein (PV), right hepatic artery (RHA), right hepatic duct (RHD) and right branch of the portal vein (RBPV). Thus, in the case of an emergent operation, surgical maneuvers in the area of the altered hepatic anatomy and proximity to the hemangioma itself, would in fact increase the risk endangering the patient’s life. After patient’s consent, a surgical enucleation en block with the gall-bladder was performed. It is of great importance that specifically selected, asymptomatic patients diagnosed with a giant hemangioma, with the above mentioned or similar localization should be considered for surgical treatment.

Liver is the most common location of the colorectal cancer metastases occurrence. Liver resection is the only curative method of treatment. Unfortunately it is feasible only in 25% of patients with colorectal liver metastases, often because of the extensiveness of the disease. The aim of the study was to evaluate the predictive value of total tumor volume, size and number of colorectal liver metastases in patients treated with right hemihepatectomy. Material and methods. A retrospective analysis was performed in a group of 135 patients with colorectal liver metastases, who were treated with right hemihepatectomy. Total tumor volume was estimated based on the formula (4/3)πr3. Moreover, the study included an analysis of data on the number and size of tumors, radicality of the resection, time between primary tumor resection and liver resection, pre-operative blood serum concentration of carcinoembryonal antigen (CEA) and carcinoma antigen Ca19-9. The predictive value of the factors was evaluated by applying a Cox proportional hazards model and the area under the ROC curve. Results. The univariate analysis has shown the predictive value of size of the largest tumor (p=0.033; HR=1.065 per each cm) on the overall survival, however no predictive value of number of tumors (p=0.997; HR=1.000) and total tumor volume (p=0.212; HR=1.002) was observed. The multivariate analysis did not confirm the predictive value of the size of the largest tumor (p=0.141; HR=1.056). In the analysis of ROC curves, AUROC for the total tumor volume, the size of the largest tumor and the number of tumors were 0.629, 0.608, 0.520, respectively. Conclusions. Total tumor volume, size and number of liver metastases are not independent risk factors for the worse overall survival of patients with colorectal liver metastases treated with liver resection, therefore increased values of these factors should not be a contraindication for surgical treatment

Amputation leading to the loss of a body part is associated not only with significant economic costs, but also serious consequences of medical and socio-psychological nature. It is the ultimate means to save a life or improve its quality. The most difficult challenges faced by amputees include accepting changes regarding their own physiognomy and the resulting life restrictions. The patient subjected to amputation is faced with an extremely difficult adaptation process, during which s/he should strive for a maximum degree of independence. Unfortunately, a large group of patients also struggles with various types of sensations and pain located within the lost limb − i.e., so-called phantom phenomena. This is a special group of phenomena of diverse nature, “located” within the lost limb. The occurrence of phantom limb syndrome in amputee patients is extremely common. This problem affects from 45% to even 98% of patients after amputation of one or both upper and lower limbs. The main purpose of this article is to describe phantom phenomena observed in patients after limb amputation in light of current literature. The definition, historical outline, types of phantom phenomena are presented, as well as hypothetical pathomechanisms, factors influencing the frequency and intensity of phantom phenomena and available treatment methods. The work was based on numerous text sources and the author’s own experience.
Amputacja prowadząca do utraty części ciała wiąże się nie tylko ze znacznymi kosztami ekonomicznymi, ale i poważnymi konsekwencjami natury medycznej i społeczno-psychologicznej. Jest ostatecznym środkiem mającym na celu ochronę życia lub poprawę jego jakości. Do najtrudniejszych wyzwań, z jakimi przychodzi mierzyć się osobom po amputacji należy zaliczyć zaakceptowanie zmian we własnej fizjonomii oraz wynikające z nich, ograniczenia życiowe. Pacjent po amputacji ma przed sobą niezwykle trudny okres adaptacyjny, w trakcie którego powinien dążyć do maksymalnego stopnia samodzielności. Niestety duża ich grupa zmaga się także z różnego rodzaju doznaniami i dolegliwościami bólowymi umiejscowionymi w obrębie utraconej kończyny − tak zwanymi wrażeniami fantomowymi. Jest to szczególna grupa odczuć o zróżnicowanym charakterze „umiejscowionych” w obrębie utraconej kończyny. Występowanie zespołu kończyny fantomowej u pacjentów po amputacjach jest niezwykle częste. Problem ten dotyka od 45% do nawet 98% z nich po amputacji jednej lub obu kończyn górnych i dolnych. Głównym celem artykułu było opisanie wrażeń fantomowych, obserwowanych u chorych po amputacji kończyn w świetle aktualnej literatury. Przedstawiono definicję wrażeń fantomowych i ich rodzaje, rys historyczny, hipotetyczne patomechanizmy, czynniki wpływające na częstość i intensywność wrażeń fantomowych oraz dostępne sposoby leczenia. Praca powstała w oparciu o liczne źródła tekstowe i doświadczenia własne.

Background: Ambiguous genitalia pose significant medical and ethical problems. Information regarding the neonate’s sex is important for upbringing since it has a significant impact on gender identity. Parents should be informed of ambiguous genitalia by obstetric personnel in accordance with medical ethics, in an appropriate, clear, and professional manner. Objective: The authors investigated which medical terms referring to ambiguous genitalia are considered the most appropriate according to the judgement of obstetrics students. Design: Students of obstetrics were presented with diagrams of various genital malformations associated with sexual ambiguity. They were provided with a list of medical terms for each condition and asked to grade them with 0–5 points based on the appropriateness of the term. Setting: Poznan University of Medical Sciences and Vocational State School in Kalisz. Population: 209 medical students specializing in obstetrics. Methods: Students of obstetrics were shown pictures of neonates with genital malformations. They were then asked to select the most appropriate term for ambiguous genitals from the options listed in the ambiguous genitals questionnaire by grading each term with 0–5 points. Main outcome measures: The appropriateness of the medical terms for ambiguous genitalia was assessed on a numerical scale. Results: The analysis revealed which terms referring to ambiguous genitalia were deemed the most appropriate by students. Conclusions: When a child is born with genital abnormalities, interdisciplinary diagnostic procedures must be implemented and counseling for family members should be provided. Moreover, the nomenclature for such anomalies should be reviewed. A proper diagnosis of disorders of sexual development is also important from the oncological point of view – a patient with a “Y” in the karyotype requires prophylactic gonadectomy as part of germinal cancer prevention – for example germinal tumors (dysgerminoma).
Wstęp: Nietypowe ukształtowanie narządów płciowych stanowi istotny problem zarówno z medycznego, jak i etycznego punktu widzenia. Informacje dotyczące płci noworodka są bardzo ważne w procesie wychowania i w sposób istotny wpływają na tożsamość płciową dziecka. O występowaniu nietypowo ukształtowanych narządów płciowych należy informować z zachowaniem zasad etyki lekarskiej, w odpowiedni, przystępny i profesjonalny sposób. Cel: Na podstawie opinii studentów położnictwa autorzy pracy określili, które terminy medyczne odnoszące się do nietypowo ukształtowanych narządów płciowych są najodpowiedniejsze. Opis badania: Studentom położnictwa pokazano zdjęcia dzieci z różnymi malformacjami narządów płciowych. Podano im także listę terminów medycznych odpowiadających każdej wadzie. Następnie poproszono o ocenę adekwatności terminów w skali od 0 do 5. Ośrodek: Uniwersytet Medyczny w Poznaniu i Państwowa Wyższa Szkoła Zawodowa w Kaliszu. Populacja: 209 studentów medycyny ze specjalizacją „położnictwo”. Metody: Uczestnikom pokazano zdjęcia noworodków z wadami narządów płciowych. Poproszono o wybór najbardziej adekwatnego terminu z listy nazw zawartych w kwestionariuszu – przez przydzielenie każdemu określeniu od 0 do 5 punktów. Główny miernik wyników: Adekwatność terminów medycznych opisujących nietypowo ukształtowane narządy płciowe oceniono na skali liczbowej. Wyniki: Analiza wykazała, które terminy odnoszące się do nietypowo ukształtowanych narządów płciowych zostały przez studentów uznane za najodpowiedniejsze. Wnioski: W przypadku urodzenia się dziecka z nieprawidłowościami w budowie narządów płciowych należy zapewnić rodzicom pomoc psychologiczną i rozpocząć wielospecjalistyczną procedurę diagnostyczną. Powinno się także ocenić terminologię związaną z takimi wadami. Odpowiednia diagnoza zaburzeń rozwoju płci jest istotna również z onkologicznego punktu widzenia – pacjentki z „Y” w kariotypie należy poddać usunięciu gonad w ramach profilaktyki raka germinalnego, np. guzów germinalnych (dysgerminoma).

Introduction. Breast cancer (BC) makes up nearly 26% of malignant tumours worldwide and is the leading cause of cancerrelated deaths in European women. With approximately 18,000 new cases of BC diagnosed in Polish women annually, breast cancer liver metastasis (BCLM) is respectively an increasing issue. Recent data found in literature indicates improved survival following liver resection with systemic therapy. Objective. The aim of study was to evaluate surgical treatment in patients with isolated BCLM. Materials and method. During 2009–2013, a retrospective study was undertaken and 30 cases analysed. From nearly 2,000 liver resections performed, 11 female patients at the mean age of 59.18 years with BCLM were qualified for surgery. Results. The median time between primary and secondary treatment was 3.5 years (1–7). One patient (9.1%) presented an extrahepatic lesion – bone metastasis. The left lobe, right lobe and both lobes of the liver were affected, respectively, in 3 (27.3%), 4 (36.4%) and 4 (36.4%) patients. 5 patients (45.5%) presented single hepatic lesion, in contrast to the maximum number of lesions which equalled 6 in the right lobe. Average hospitalisation period was 13.27 days and discharge on the 11.3 postoperative day. One-year survival was 72.7% (8 patients); therefore, three-year survival was 36.4% (4 patients). Conclusions. Oncological centres should assess BCLM patients more openly and qualify them for hepatic resection along with adjuvant systemic treatment in order to improve overall survival. This, however, needs to be studied in a multicentre randomized trial.

Liver transplantation is a well-established treatment of patients with end-stage liver disease and selected liver tumors. Remarkable progress has been made over the last years concerning nearly all of its aspects. The aim of this study was to evaluate the evolution of long-term outcomes after liver transplantations performed in the Department of General, Transplant and Liver Surgery (Medical University of Warsaw). Material and methods. Data of 1500 liver transplantations performed between 1989 and 2014 were retrospectively analyzed. Transplantations were divided into 3 groups: group 1 including first 500 operations, group 2 including subsequent 500, and group 3 comprising the most recent 500. Five year overall and graft survival were set as outcome measures. Results. Increased number of transplantations performed at the site was associated with increased age of the recipients (p<0.001) and donors (p<0.001), increased rate of male recipients (p<0.001), and increased rate of piggyback operations (p<0.001), and decreased MELD (p<0.001), as well as decreased blood (p=0.006) and plasma (p<0.001) transfusions. Overall survival was 71.6% at 5 years in group 1, 74.5% at 5 years in group 2, and 85% at 2.9 years in group 3 (p=0.008). Improvement of overall survival was particularly observed for primary transplantations (p=0.004). Increased graft survival rates did not reach the level of significance (p=0.136). Conclusions. Long-term outcomes after liver transplantations performed in the Department of General, Transplant and Liver Surgery are comparable to those achieved in the largest transplant centers worldwide and are continuously improving despite increasing recipient age and wider utilization of organs procured from older donors.

The aim of the study was to analyze indications and results of the first one thousand liver transplantations at Chair and Clinic of General, Transplantation and Liver Surgery, Medical University of Warsaw.Material and methods. Data from 1000 transplantations (944 patients) performed at Chair and Clinic of General, Transplantation and Liver Surgery between 1994 and 2011 were analyzed retrospectively. These included 943 first transplantations and 55 retransplantations and 2 re-retransplantations. Frequency of particular indications for first transplantation and retransplantations was established. Perioperative mortality was defined as death within 30 days after the transplantation. Kaplan-Meier survival analysis was used to estimate 5-year patient and graft survival.Results. The most common indications for first transplantation included: liver failure caused by hepatitis C infection (27.8%) and hepatitis B infection (18%) and alcoholic liver disease (17.7%). Early (< 6 months) and late (> 6 months) retransplantations were dominated by hepatic artery thrombosis (54.3%) and recurrence of the underlying disease (45%). Perioperative mortality rate was 8.9% for first transplantations and 34.5% for retransplantations. Five-year patient and graft survival rate was 74.3% and 71%, respectively, after first transplantations and 54.7% and 52.9%, respectively, after retransplantations.Conclusions. Development of liver transplantation program provided more than 1000 transplantations and excellent long-term results. Liver failure caused by hepatitis C and B infections remains the most common cause of liver transplantation and structure of other indications is consistent with European data.
