Opis:
The second edition of the Independent Farmers' Opinion Poll (IFOP 2018) resulted in a database of 600 subjective and reliable opinions used for the analysis of the ergonomics and safety of farm tractors. The poll was conducted in all regions of Poland and resulted in Polish farmers’ opinions of several dozen parameters of tractors, which are basic machines on each farm. John Deere won the second edition of the Independent Farmers' Opinion Poll as users of these tractors considered them the most ergonomic and safest during long-term operation. Like in the first edition (650 questionnaires), the users were the most critical about the insufficient number of compartments in the tractor cab.
Po zakończeniu II edycji Niezależnego Badania Opinii Rolników (NBOR 2018) otrzymano bazę 600 subiektywnych i rzetelnych opinii do analizy o ergonomii i bezpieczeństwie w użytkowaniu ciągników rolniczych. Badanie swym zasięgiem objęło cały kraj i pozwoliło poznać opinie polskich rolników o kilkudziesięciu parametrach podstawowej maszyny w każdym gospodarstwie. Zwycięzcą II edycji według użytkowników zostały ciągniki firmy John Deere, które uznano za najbardziej ergonomiczne i bezpieczne podczas wieloletniej eksploatacji. Podobnie jak w I edycji (650 ankiet) użytkownicy najwięcej zastrzeżeń mieli do niewystarczającej liczby schowków w kabinie ciągnika.

Introduction. Introduction. The aim of the study was to evaluate the neck torsion test in objective examinations of patients with vertigo and/or hearing loss. Material and methods. The study was conducted in 100 patients, including 54 women and 46 men aged 17–79 years, who were divided into two groups: I – 50 patients, including 30 women and 20 men aged 17–79 years (mean age 49.92 years) with dizziness and/or hearing impairments, and confirmed asymmetry of intracranial vessels, II – 50 patients – control group, including 24 women and 26 men aged 20–71 years without dizziness and/or hearing disorders and without disturbance in the construction of intracranial vessels. For each patient, the following tests were carried out: subjective, objective otorhinolaryngological, Doppler ultrasound specifying diameter of vertebral and carotid arteries and the velocity of blood flow in these vessels, audiological diagnostics, including the examination of latency of waves I, III, V of the auditory evoked potentials of the brain stem, otoneurological diagnostics with used the neck torsion test. Results. It appears from the analysis of the material presented that the application of the neck torsion test in the Doppler ultrasound results in the fact that the difference in the mean systolic velocity of blood flow in vertebral artery is higher on the side opposite to the turning of the neck, and the increase in the average diastolic blood flow velocity in the vertebral artery on the side of the test being performed and its reduction on the opposite side in the study group, when compared to the control group. The value of the wave I, II, V latency in the ABR test during the neck torsion test is extended more in the study group than in the controls, on the side of the performed test. The performed neck torsion test in the VNG test increases the occurrence of both, square waves and nystagmus (much higher in the study group than in the controls). Conclussion. The application of the neck torsion test in the Doppler ultrasound, ABR and VNG test in patients with vertigo and/or hearing loss means that these tests become functional, thereby increasing their diagnostic value and may be used to monitor the rehabilitation of inner ear disorders.

Introduction: The aim of the study was to assess the effect of nasal mucosa irritants on the occurrence of chronic rhinosinusitis without/and with nasal polyps. Material and methods: The study involved 100 adult participants, including 39 women and 61 men, aged 21–68, diagnosed and treated at the Department of Otolaryngology, ENT Oncology, Audiology and Phoniatrics at the University Clinical Hospital WAM in Łódź. Based on the otorhinolaryngological and imaging (CT) tests they were divided into two groups: I – 50 patients, including 23 women and 27 men, aged 21–64 – with chronic rhinosinusitis without nasal polyps, II – 50 patients, including 16 women and 34 men, aged 22–68 – with chronic rhinosinusitis with nasal polyps. The control group consisted of 50 people (group III), including 25 women and 25 men, aged 18–30, students of the Faculty of Military Medicine at the Medical University of Lodz. All respondents completed a prepared questionnaire consisting of 17 questions addressed in the form of an anonymous interview among patients treated in the Department of Otolaryngology, ENT Oncology, Audiology and Phoniatrics. Results: The conducted surveys indicate the impact of the following factors in pathogenesis of chronic rhinosinusitis without/ with nasal polyps: exogenous factors (viruses, bacteria, fungi, drugs, injuries, toxic substances, environmental pollution), general endogenous factors (allergy, hypersensitivity to acetylsalicylic acid and its derivatives, hormonal disorders, supraesophageal reflux disease, granulation disease, immunity disorders, local endogenous factors. Conclusions: In the examined material, patients with chronic rhinosinusitis without/and nasal polyps in most cases are in the age range 51–60 years and over 60 years, they most often live in large cities over 250 thousand inhabitants, suffer from allergic rhinorhinitis in 38.0% in group I and 36.0% in group II, rapid temperature changes and dry air have a negative impact on comfort of breathing. The conducted surveys confirm that the cause of chronic rhinosinusitis with polyps is multifactorial, but a significant factor affecting typical tissue remodeling in this disease is long-term breathing of polluted atmospheric air.

Aim: The aim of the study was to assess the acoustic and capacity analysis of singing voice using DiagnoScope Specialist software. Material and methods: The study was conducted in 131 adult subjects, including 74 women and 46 men aged 21–51, divided into 3 groups: I – 40 subjects (treatment group) – professional vocalists, II – 40 subjects (treatment group) – semiprofessional vocalists, III – 40 subjects (control group) – students of The Military Medical Faculty at the Medical University of Lodz – nonsingers. The research methodology included: primary medical history, physical examination (otolaryngological), videolaryngoscopic examination, the GRBAS scale for subjective voice evaluation, diagnostic voice acoustic and capacity analysis using DiagnoScope Specialist software, survey on lifestyle patterns which may affect voice quality. R esults: Average value of the fundamental frequency F0 was the highest in professional vocalists group; it was 316.46 Hz in women and 165.09 Hz in men. In semiprofessional vocalists group it was accordingly 260.50 Hz and 149.26 Hz, in nonsingers group it was accordingly 261.23 Hz and 159.27 Hz. The mean value of Jitter parameter in professional vocalists group was 0.30% in women and 0.54% in men, in semiprofessional vocalists group it was accordingly 0.31% and 0.57%, in nonsingers group it was 0.31% and 0.56%. The mean value of Shimmer parameter in professional vocalists group was 3.27% in women and 3.75% in men, in semiprofessional vocalists group it was accordingly 3.46% and 3.77%, in nonsingers group it was 4.33% and 4.39%. The mean value of the NHR index in professional vocalists group was 3.28% in women and 6.00% in men, in semiprofessional vocalists group it was accordingly 3.23% and 6.72%, in nonsingers group it was 3.89% and 6.13%. Conclusions: Values of the parameters which measure the character of the voice, relative period-to-period fundamental frequency perturbations, relative period-to-period amplitude perturbation and level of buzzing together with other methods have diagnostic and predictive value in early detection of voice disorders. Capacity analysis in singing voice showed very low values of the following parameters: phonation time, true phonation time, no phonation coefficient, voice efficiency coefficient and voice capacity.

Objective: The aim of the study was to estimate usability of the piezoelectric knife in larynx surgery. Prove that the piezoelectric staff can be used to do any different shape incision within the larynx cartilages. Material and methods: 35 patients hospitalized in our Department in 2014–2016 were enrolled in our study. 24 patients went vertical partial laryngectomy and 1 patient went horizontal partial laryngectomy because of larynx cancer. 5 patients went partial laryngectomy because of low stage of piriformis recess cancer. Piezoelectric staff was used to do thyroidectomy and resection of thyroid cartilage suspected of carcinomatosis infiltration. The rest 4 patients had done widening of larynx lumen due to larynx stenosis or slenderness. The piezoelectric tool was used to do different incision or resection within the larynx cartilages in case of widening lumen of the larynx. Results: The larynx cartilages, especially thyroid cartilage could be cut in different shapes using piezoelectric tools. The usage of this equipment causes the minimal loss and small destruction of local healthy tissues. Conclusions: The Piezoelectric instrument is useful instrument suit to operate within larynx cartilages. Exchangeable tip available in different shapes enables different resections of cartilages. Using the piezoelectric staff we can remove pathological tissue with minimal local destruction. Our observation shows that larynx operation with a usage of the piezoelectric knife is safe and effective. Current English literature does not describe usage piezoelectric tools in larynx surgery. It is essential to do more observation about that type of operations.

Introduction: The aim of the study was to present selected cases with Eagle’s syndrome diagnosed and treated at the Department of Otolaryngology, Laryngological Oncology, Audiology and Phoniatrics, Medical University of Lodz, in the years 2016–2020. Material and method: Five selected clinical cases with Eagle’s syndrome are presented, including three male patients and two female patients, aged 28 to 42 years. Results: The prevalence of Eagle’s syndrome was similar for both female and male patients. The carotid artery syndrome, which is characterized by: visual disturbances, unilateral pain located along cervical vessels, headache and migraine, was more common. Lack of proper diagnosis of Eagle’s syndrome often significantly delays the implementation of proper treatment and thus exposes patients to long-term struggle with pain. Our observations have shown that the time between the onset of symptoms and correct diagnosis in patients averaged about five years. 3D-CT scan is the gold standard for detecting Eagle’s syndrome. An intraoral approach was used in surgical treatment. Although this approach offers shorter treatment time and better cosmetic effect, there is also a greater risk of complications associated with limited visibility of the surgical field and infection. Conclusions: Although styloid syndrome usually occurs bilaterally, these patients reported unilateral symptoms. No correlations were found between the prevalence of Eagle’s syndrome and sex, the length of the styloid process or age, nor side of the body. The best healing effect is obtained by surgical correction/reduction of the elongated styloid process.

Purpose of the study: The aim of the study was to present the treatment outcomes after vertical partial laryngectomy with or without pedunculated sternothyroid muscle flap reconstruction following the resection of neoplasm-infiltrated vocal fold. The procedure was used in a patient with glottic cancer. Oncological outcomes, morphology of neo-vocal fold and the act of swallowing were evaluated. Material and methods: 45 patients with T1-T2 glottic cancer were subjected to vertical partial laryngectomy with 26 patients undergoing a procedure with pedunculated sternothyroid muscle flap reconstruction and the remaining 19 patients undergoing a procedure without such a reconstruction. Two female and 43 male patients aged 35-82 years (mean age of 62.5 years) were enrolled in the study. Local tumor spread and the condition of reconstructed vocal fold were assessed in sequential videofiberoscopy examination conducted each month after surgery whereas the regional spread was assessed in ultrasound scans. Postoperative aspiration was graded according to the Pearson’s scale. Results: Six patients experienced local recurrence while 2 patients experienced regional recurrence of the tumor. The pedunculated sternothyroid muscle flap neo-fold was structurally resemblant of the non-affected vocal fold. Episodic, daily dysphagia was observed in 1 patient while normal act of swallowing with no Pearson’s scale symptoms was observed in the remaining 44 patients. No necrosis of pedunculated flap was observed. Conclusions: Vertical partial laryngectomy with or without pedunculated sternothyroid muscle flap reconstruction is a good method for the treatment of low- or intermediate-stage glottic cancer, especially when endoscopic access to the tumor is limited and when CO2 laser cannot be used. No significant functional disorders were observed in operated larynges.

Introduction: The aim of the work was to evaluate one-time recording of ENG and VNG in healthy subjects selected for the given diagnostic tests with reference to the sensitivity and usefulness of this kind of testing in vertigo diagnostics. Material and methods: The tests covered 50 healthy subjects, including 24 women and 26 men aged 20-22 (the mean age of 20,5 years) who did not report any inconvenience from otorhinolaryngological organs and the conducted interview did not reveal any balance disorder or vertigo. Every patient underwent otoneurological subjective and objective tests, electronystagmography (ENG), including a calibration test, head-tracking test, optokinetic nystagmus test, rotatory chair test, positional tests according to Cawthorne and Rose, a Hallpike caloric test as well as videonystagmography which included a calibration test, spontaneous nystagmus evaluation, head-tracking test, positional tests according to Cawthorne and Rose with a head rotation test and Hallpike caloric test. The above tests were performed separately, 24 hours one by one. One-time recording of ENG and VNG was made 48 hours following the last test while each of the two tests was preceded by an additional calibration test. The final recording included the spontaneous nystagmus evaluation, positional tests according to Cawthrone and Rose together with the head rotation test and Hallpike caloric test. Results: Any negative mutual influence of the conducted tests has not been observed. Both in the one-time and separate recording, the caloric test revealed lower values of the mean velocity of the nystagmus free phase in ENG with relation to VNG. No signs from spontaneous nystagmus, as well as optokinetic and positional nystagmus, could be found in any case. Conclusions: The conducted tests did not show any possibility of mutual exclusion of the given ENG and VNG diagnostic tests one-time recording. However, their use in diagnostics of balance disorders and vertigo requires further studies

Objectives: The aim of the study was to assess the function of semicircular canal in videonystagmography head impulse test (VHIT) in the patients with vertigo and balance disorders. Material and Methods: The study was performed in 135 patients (86 women and 49 men) aged 22–79 years, who were divided into 2 groups: I (study group) – 73 patients with vertigo of peripheral, central or mixed origin, II (control group) – 62 patients without vertigo (healthy individuals). The function of canal was determined on the basis of GAIN and expressed as DG/RH×100% (where DG is deviation of gaze and RH is rotation of head). Results: In the study group the semicircular canal injuries were found in 37 (50.69%) patients, including 24 (32.87%) patients with 1 injury and 13 (17.8%) patients with 2 or more injuries in semicircular canal. The injured anterior semicircular canal was reported 13 times; the lateral – 9 times and the posterior – 31 times. Conclusions: In the study group, in the VHIT, injuries in semicircular canals were reported in peripheral vertigo, mixed vertigo with non-compensated and compensated function of the labyrinth in 50.68% cases, whereas in the caloric test dysfunction of the labyrinth was found in 58.49% cases.

Introduction. The aim of this study was to analyse the occurrence of inverted papillomas of the nose and paranasal sinuses in patients that underwent endoscopic sinus surgery in our department. Material and methods. Between 2006 and 2016, 3,574 patients underwent surgery due to paranasal sinus diseases. Patients were qualified for surgery based on medical history, computed tomography, and laboratory tests. Data were gathered from medical files, and they included age, sex, and histopathological diagnosis. Results. Among 3,574 patients that underwent surgery due to chronic inflammatory changes, on histopathology, inverted papillomas were diagnosed in 80 patients, including 31 women (38.75%) and 49 men (61.25%). Most patients were aged 60-70 years (women, 12.5%; men, 15%) or 50-60 years (women, 5%; men, 21.25%). Between 2006 and 2016, the number of surgeries ranged from 264 (7.38%) in 2013 to 355 (9.93%) in 2016, and the number of inverted papillomas ranged from 4 in 2007 and 2015 (1.23%) to 12 in 2014 (3.87%). Over the last 4 years of the study period, the incidence of inverted papillomas increased. Conclusions. Among 3,574 patients operated on due to chronic inflammatory changes, on histopathology, inverted papillomas were diagnosed in 80 cases (2.23%); thus, all patients qualified for endoscopic surgery due to inflammatory or hypertrophic changes should undergo rhino-fiberoscopy. Recurrence of inverted papillomas was observed in 17.50%, typically in patients with nasal polyps that co-occurred with inverted papillomas. We regard rhino-fiberoscopy as the most valuable method for detecting tumour recurrence in patients after surgery for inverted papillomas ranged from 4 in 2007 and 2015 (1.23%) to 12 in 2014 (3.87%). Introduction. The aim of this work was to describe acoustic neuromas as the first signs of inner ear functional disorders. Material and methods. The study covered 3,456 audiological and otoneurological patients, who were treated in the Department of Otolaryngology, Laryngological Oncology, Audiology and Phoniatrics Military Medical Academy University Teaching Hospital in Lodz within the period of 2011–2016. Among the studied subjects, an acoustic neuroma on the vestibulocochlear nerve was diagnosed in 13 cases (5,16%), including 9 women and 4 men. Each patient underwent a medical interview and an objective examination with static and dynamic tests, pure tone audiometry and speech audiometry tests, impedance audiometry tests, characteristics of tinnitus, videonystagmography and auditory brainstem evoked potentials (ABR) in crack. In each case of an incorrect ABR recording, contrast-enhanced MRI was performed. Study results. The conducted tests showed an incorrect ABR recording in 252 patients (7,29%, including 54,37% women and 45,63% men). In the performed contrast-enhanced MRI, 13 patients (5,16%), including 3,57% women and 1,59% men, had an image typical to neuroma of the vestibulocochlear nerve. Each one of the studied patients showed signs and symptoms typical to an acoustic neuroma such as hypoacusia and balance disorders, tinnitus in 12 subjects, headaches in the temporal and occipital area in 4 subjects, trigeminal neuralgia symptoms in 2 subjects and vision problems like scotoma and blurred vision reported by 1 patient. Conclusion. Each case of sensorineural hearing loss, particularly unilateral sensorineural hearing loss, requires expanded hearing tests and auditory brainstem evoked potentials tests. An incorrect brainstem evoked potentials recording such as: an elongated latency of the 5th wave, an elongated interlatency of 1–2-3 and 1–2–3–4 waves, requires a contract-enhanced magnetic resonance imaging to diagnose cerebellopontine angle tumours.

Objectives: Vertigo is a very common symptom mainly caused by the lesion of vestibular system (peripheral or central) and often accompanied by some work-related diseases and occupational intoxications. The aim of this study was to assess the value of electronystagmography (ENG) and videonystagmography (VNG) for diagnosing vertigo of various origin. Materials and Methods: The study included four groups, 25 subjects each, of patients suffering from vestibular disorders of peripheral, central and mixed origin versus healthy controls. All were examined by means of ENG and VNG, using the bithermal caloric test with 30°C and 44°C air irrigations of the ears. The findings (frequency of induced nystagmus FRQ, its slow phase velocity SPV, canal paresis CP, directional preponderance DP, vestibular excitability VE) were analysed and compared. Results: In all patients with vertigo due to vestibular neuritis, barotrauma and kinetosis, significant CP, the important sign of peripheral site of vestibular lesion was identified both in ENG and VNG. None of the patients with central origin disorders showed CP in VNG; in the majority of cases DP was observed. However, in ENG we found CP in 5 patients with central origin disorders. There were no essential differences between ENG and VNG in measurements of FRQ and SPV except for higher values in VNG in controls and patients with mixed vertigo. Conclusions: The results suggest that the VNG should be recommended in preference as the valuable method to assess vertigo and to discriminate between the peripheral and the central vestibular lesions.

The human SUV3 helicase (SUV3, hSUV3, SUPV3L1) is a DNA/RNA unwinding enzyme belonging to the class of DexH-box helicases. It localizes predominantly in the mitochondria, where it forms an RNA-degrading complex called mitochondrial degradosome with exonuclease PNP (polynucleotide phosphorylase). Association of this complex with the polyA polymerase can modulate mitochondrial polyA tails. Silencing of the SUV3 gene was shown to inhibit the cell cycle and to induce apoptosis in human cell lines. However, since small amounts of the SUV3 helicase were found in the cell nuclei, it was not clear whether the observed phenotypes of SUV3 depletion were of mitochondrial or nuclear origin. In order to answer this question we have designed gene constructs able to inhibit the SUV3 activity exclusively in the cell nuclei. The results indicate that the observed growth rate impairment upon SUV3 depletion is due to its nuclear function(s). Unexpectedly, overexpression of the nuclear-targeted wild-type copies of the SUV3 gene resulted in a higher growth rate. In addition, we demonstrate that the SUV3 helicase can be found in the HeLa cell nucleoli, but it is not detectable in the DNA-repair foci. Our results indicate that the nucleolar-associated human SUV3 protein is an important factor in regulation of the cell cycle.

Objectives: The aim of this study was to compare the parameters of vocal acoustic and vocal efficiency analyses in medical students and academic teachers with use of the IRIS and DiagnoScope Specialist software and to evaluate their usefulness in prevention and certification of occupational disease. Material and Methods: The study group comprised 40 women, including students and employees of the Military Medical Faculty, Medical University of Łódź. After informed consent had been obtained from the participant women, the primary medical history was taken, videolaryngoscopic and stroboscopic examinations were performed and diagnostic vocal acoustic analysis was carried out with the use of the IRIS and Diagno- Scope Specialist software. Results: Based on the results of the performed measurements, the statistical analysis evidenced the compatibility between two software programs, IRIS and DiagnoScope Specialist, with the only exception of the F4 formant. The mean values of vocal acoustic parameters in medical students and academic teachers, obtained by means of the IRIS software, can be used as standards for the female population not yet developed by the producer. When using the DiagnoScope Specialist software, some mean values were higher and some lower than the standards specified by the producer. Conclusions: The study evidenced the compatibility between two measurement software programs, IRIS and DiagnoScope Specialist, except for the F4 formant. It should be noted that the later has advantage over the former since the standard values of vocal acoustic parameters have been worked out by the producer. Moreover, they only slightly departed from the values obtained in our study and may be useful in diagnostics of occupational voice disorders.
