The assumptions of the own research presented in the article refer to the socio-interactive approach that assumes that learning and proper language acquisition by a child require cognitive activity, proper progress in cognitive development, and active - resulting from social relations - observation of adult speech. The research assumptions also take into account the importance of the autoregulatory function of language and systemic understanding of the concept of family. The aim of the study was to check the presence of the relationship between speech understanding by children with delayed verbal development and the various dimensions of their family functioning and ego-resiliency of parents. The study included a group of 72 well-cognitively functioning Polish children aged 5-7 with a diagnosis of delayed verbal development and their families selected using random and nonprobability sampling. The children were examined using the Polish Picture Vocabulary Test – Comprehension - version A (PPVT-C). Functioning of the families of the examined children was operationalized by the results of FACES IV by D. Olson (the Polish version of the scale was used); while the Ego Resiliency Scale was used to study resilience. The results of the conducted research indicate, among others, that families of children with lower scores on speech understanding present less favorable functioning profiles in the scope of selected dimensions of FACES -IV: Family Communication, Cohesion, Disengaged and Family Satisfaction. However, the relationship between the child's understanding of speech and other demographic variables such as parent's age, parent's gender, parent's education, marital status has not been confirmed. It was recognized that the results of the research will allow in the future to optimize the therapeutic services offered to children with verbal development disorders and their families in their natural environment, as well as to allow the presentation of appropriate strategies to support speech development (especially speech understanding abilities) in children. The creation of a group representing a certain type of language disorder will allow a better adaptation of the training program to the specific difficulties experienced by a child, as well as allow for more effective involvement of parents in preventive measures.

Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disorder caused by mutations of the SMN1 gene. It is characterized by significant phenotype variability. In this study, we analyzed possible phenotype modifiers of the disease - the size of the deletion in the SMA region, the number of SMN2 gene copies, as well as the effect of gender. Among the factors analyzed, two seem to influence the SMA phenotype: the number of SMN2 gene copies and a deletion in the NAIP gene. A higher number of SMN2 copies makes the clinical symptoms more benign, and the NAIP gene deletion is associated with a more severe phenotype. The influence of gender remains unclear. In a group of 1039 patients, 55% of whom were male, the greatest disproportion was in the SMA1 (F/M = 0.78) and SMA3b (F/M = 0.45) forms. In SMA1 a deletion in the NAIP gene was seen twice as frequently in girls compared to boys. In three patients, we observed genotypes atypical for the chronic forms of SMA: two patients with SMA3a and 3b had a deletion of the NAIP gene, and a third patient with SMA2 had one copy of the SMN2 gene.

Opis:
Background Determining the prevalence of alcohol intoxication and the level of intoxication in victims of fatal occupational accidents is necessary to improve work safety. The circumstances of the accident and the time between alcohol consumption and death are important factors. Material and Methods A retrospective review of 18 935 medico-legal autopsy reports and toxicological reports performed in the Department of Forensic Medicine at the Wroclaw Medical University, Poland, in the years 1991–2014. The study protocol included circumstances, time and cause of death, injuries, quantitative testing for the presence of ethyl alcohol, gender and age. Results There were 98 farm-related fatalities. There were 41.8% (N = 41) of victims who had been intoxicated – 95.1% (N = 39) of them were males aged 19–70 years old, 4,9% (N = 2) were females aged 37–65 years old. In 8 cases the blood alcohol concentration (BAC) was 50–150 mg/dl; in 15 cases it was 150–250 mg/dl and in 18 cases it was > 250 mg/dl. In 21 cases, the BAC was determined using alternative material and 76% (N = 16) victims were in the alcohol elimination phase with 19% (N = 4) victims in the alcohol absorption phase. The most common causes of death were traffic accidents, drowning and deaths resulting from being caught in or hit by moving parts of machinery or equipment. Conclusions Alcohol consumption is a crucial risk factor in fatal agricultural accidents. In order to establish the time of alcohol consumption, all victims of agricultural accidents should be tested for alcohol concentration in their blood and urine or vitreous. Improving safety at work requires that the sobriety of employees should be monitored before and during work. Med Pr 2017;68(1):23–30
Wstęp W celu poprawy poziomu bezpieczeństwa pracy w rolnictwie konieczne są gromadzenie i analiza danych dotyczących liczby osób w stanie nietrzeźwości, które ulegają śmiertelnym wypadkom, poziomu alkoholu w ich organizmie, okoliczności śmierci i czasu między konsumpcją alkoholu a zgonem. Materiał i metody Przeprowadzono retrospektywną analizę wyników 18 935 sądowo-lekarskich sekcji zwłok i raportów toksykologicznych wykonanych w Katedrze Medycyny Sądowej na Uniwersytecie Medycznym we Wrocławiu w latach 1991–2014. Protokół badania obejmował okoliczności, czas i przyczynę zgonu, obrażenia, wynik badania ilościowego na zawartość alkoholu, płeć i wiek badanych. Wyniki Stwierdzono 98 wypadków śmiertelnych przy pracy w rolnictwie. Pod wpływem alkoholu było 41,8% (N = 41) ofiar wypadków, w tym 95,1% (N = 39) mężczyzn w wieku 19–70 lat i 4,9% (N = 2) kobiet w wieku 37–65 lat. W przypadku 8 osób stężenie alkoholu we krwi wynosiło 0,5–1,5‰, u 15 osób – 1,5–2,5‰, a u 18 osób – powyżej 2,5‰. U 21 ofiar wypadków poziom alkoholu oznaczono w materiale alternatywnym i ustalono, że 76% (N = 16) z nich znajdowało się w fazie eliminacji alkoholu z organizmu, a 19% (N = 4) w fazie wchłaniania. Najczęstszymi okolicznościami śmierci były wypadki drogowe, utonięcia i urazy związane z pochwyceniem lub uderzeniem przez poruszające się elementy maszyn rolniczych. Wnioski Spożycie alkoholu jest istotnym czynnikiem ryzyka śmiertelnych wypadków w rolnictwie. W celu ustalenia czasu konsumpcji alkoholu przez ofiary wypadków należy oznaczać u nich stężenie alkoholu we krwi i w moczu lub szklistce. Poprawa bezpieczeństwa w miejscu pracy wymaga monitorowania trzeźwości pracowników zarówno przed rozpoczęciem pracy, jak i w jej trakcie. Med. Pr. 2017;68(1):23–30

Leaving a foreign object in the body of a patient during abdominal surgery: still a current problem

Introduction: Leaving a foreign object (retained surgical item, or RSI) during surgery involving the abdominal cavity and pelvis minor is a relatively frequent, underestimated phenomenon which is dangerous to the health of the patient and the legal security of the medical personnel. These adverse events are easy to avoid through the use of appropriate means of prevention. The aim of the present paper is the collection of epidemiological data and determination of risk factors, symptomatology, health effects, and prevention methods associated with RSIs. Material and methods: Analysis of global scientific publications in the databases PubMed, ClinicalKey, Google Scholar, ScienceDirect, and Scopus related to the subject of RSIs. Results: The frequency of RSI incidents ranges from 1 to 10 in 10,000 surgeries, which results in at least one case in an average multispeciality hospital on a yearly basis. The items most frequently left behind include soft foreign objects, such as swabs and bandages (90%). Risk factors include emergency surgical procedures, high patient BMI, significant loss of blood during surgery, and neglect in counting the material and surgical tools. The postoperative course, although in many cases asymptomatic, may be complicated by inflammation, bleeding, or perforation, leading to the necessity of a second operation and, in 2 to 4% of cases, even ending in death. Imaging tests are effective diagnostic tools. Effective methods of preventing RSIs are based on checklists and systems for counting and monitoring the location of material and tools. Conclusions: The globally occurring problem of RSIs requires education of the operating block personnel regarding risk factors and identification with elimination of adverse events of this type. Diagnostics based on imaging should take into account non-specific complaints resulting from a possible oligosymptomatic course. An RSI should not be regarded as a medical error. Changes in the perception of the phenomenon aim aimed at minimising the legal liability of the staff in the event of leaving a foreign object in a patient’s body.

In order to provide a better characterization of the origin and volume of thermogenic gas generation hydrous pyrolysis (HP) experiments were performed on coals and shales at 330 and 360oC for 72 hours. The maturity range of coals and shales used for HP varies from 0.57 to 0.92% Ro. The maturity increase caused by HP at 330 and 360oC ranges from 1.32 to 1.39% and from 1.71 to 1.83%, respectively. δ13C of CH4, 2H6, C3H6 and n-C4H10 in HP gases versus their reciprocal C-number have a concave relationship, and therefore do not follow a linear trend. δ2H of CH4, 2H6 and C3H6 in HP gases versus their reciprocal H-number show both linear and convex-concave relationships. The growth of CO2 yields during HP was higher for shales than for coals. H2S yields from shales are higher than from coals, which can be connected with catalytic and adsorbed influence of shale matrix. H2 was also generated in notable quantities from water and organic matter of coals and bigger amounts from shales. N2 yields grow with the increase of Ro after 3oC HP and it is more enriched in 15N isotope than after 330C.
