Opis:
The methods of Computational Intelligence (CI) including a framework of Granular Computing, open promising research avenues in the realm of processing, analysis and interpretation of biomedical signals. Similarly, they augment the existing plethora of "classic" techniques of signal processing. CI comes as a highly synergistic environment in which learning abilities, knowledge representation, and global optimization mechanisms and this essential feature is of paramount interest when processing biomedical signals. We discuss the main technologies of Computational Intelligence (namely, neural networks, fuzzy sets, and evolutionary optimization), identify their focal points and elaborate on possible limitations, and stress an overall synergistic character, which ultimately gives rise to the highly symbiotic CI environment. The direct impact of the CI technology on ECG signal processing and classification is studied with a discussion on the main directions present in the literature. The design of information granules is elaborated on; their design realized on a basis of numeric data as well as pieces of domain knowledge is considered. Examples of the CI-based ECG signal processing problems are presented. We show how the concepts and algorithms of CI augment the existing classification methods used so far in the domain of ECG signal processing. A detailed construction of granular prototypes of ECG signals being more in rapport with the diversity of signals analyzed is discussed as well. While a genuine abundance of biomedical data available nowadays becomes a genuine blessing, it also posses a lot of challenges. The two fundamental and commonly occurring directions in data analysis deal with its supervised or unsupervised pursuits. Our conjecture is that in the area of biomedical data processing and understanding where we encounter a genuine diversity of patterns, problem descriptions and design objectives, this type of dichotomy is neither ideal nor the most productive. In particular, the limitations of such taxonomy become profoundly evident in the context of unsupervised learning. Clustering (being usually regarded as a synonym of unsupervised data analysis) is aimed at determining a structure in a data set by optimizing a given partition criterion. In this sense, a structure emerges (becomes formed) without a direct intervention of the user. While the underlying concept looks appealing, there are numerous sources of domain knowledge that could be effectively incorporated into clustering mechanisms and subsequently help navigate throughout large data spaces. In unsupervised learning, this unified treatment of data and domain knowledge leads to the general concept of what could be coined as knowledge-based clustering. In this study, we discuss the underlying principles of this paradigm and present its various methodological and algorithmic facets. In particular, we elaborate on the main issues of incorporating domain knowledge into the clustering environment such as (a) partial labelling, (b) referential labelling (including proximity and entropy constraints), (c) usage of conditional (navigational) variables, (d) exploitation of external structure. Presented are also concepts of stepwise clustering in which the structure of data is revealed via a series of refinements of existing domain granular information.

This work presents the general idea of granular description of temporal signal, particularly biomedical signal sampled with constant frequency. The main idea of presented method is based on using triangular fuzzy numbers as information granules in temporal and amplitude spaces. The amplitude space contains values of first few derivatives of underlying signal. The construction of data granules is performed using the optimization method according to some objective function, which balances the high coverage ability and the low support of fuzzy numbers. The granules (descriptors) undergo the clustering process, namely fuzzy c-means. The centroids of created clusters form a granular vocabulary and the quality of description is quantitatively assessed by reconstruction criterion. There are presented results of experiments with the electrocardiographic signal, digitally sampled and stored in MIT-BIH database. The method of numerical differentiation of function based on finite set of its values is employed, which incorporates polynomial interpolation. The paper presents results of numerical experiments which show the impact of method parameters, such as temporal window length, degree of polynomial, fuzzification parameter, on the reconstruction ability of presented method.

Opis:
The paper describes implementation of a new technology of digital systems design in case of medical measurement interface of IMPULS system used for acquisition of ECG signal from pacemaker patients. Similarly as in other applications, the processor circuit is a base of digital system, however - regarding large number of performed tasks - it is assisted by additional peripheral logic circuits. At the beginning, there were used standard elements (logic gates, counters, registers, etc.), however a growing complexity of beyond processor operations encouraged designers to apply the programmable circuits. The paper presents the stages of digital system development that has been implemented in medical measurement interface. It is the excellent example of possibility to integrate several standard elements in one integrated circuit.

Opis:
Acceleration and deceleration patterns in fetal heart rate (FHR) are widely used as an assessment of fetal condition. The basis for both visual and automated quantitative analysis is correctly determined fetal heart rate baseline. There are many methods for automated fetal heart rate baseline estimation. Since there are no definitive criteria or "gold standard" to evaluate algorithms for baseline determination, a clinical expert could be considered as a reference. Taking into account that not the shape of baseline itself but the results of quantitative analysis obtained using this baseline are important, we stated that such evaluation should be done only on the basis of comparison of effects - the recognized accelerations and decelerations. However, the first step for such comparison is to check how well expert meets established FIGO definition of accelerations and decelerations patterns in FHR trace. The paper presents a method for evaluation of expert's interpretation of these definitions. Two indices describing the inconsistency among fetal monitoring system and experts have been defined. The results obtained indicate the essential differences in procedure of accelerations/decelerations recognition by the computerized system and human expert.

The following paper introduces the problem of noninvasive pressure measurements in Polish artificial heart prosthesis. The pressure sensor is proposed and introduced, and preliminary tests are shown. The paper deals with the proposed sensing elements, requirement noninvasive solution and preliminary tests of polyurethane membrane transmission. The paper includes also experimental results of a fiber pressure sensor introducing noninvasive pressure measurements with pressure transmission through a polyurethane tube wall.

The following paper presents researches concerning a noninvasive real-time blood volume measurement system applied in POLVAD prosthesis. The system is based on the acoustic Helmholtz resonator principle. The basis of the measurement method, followed by the preliminary tests of the possibility of incorporating the Helmholtz resonance idea into the POLVAD prosthesis is shown. The paper includes the actual measurement system construction and test results, both static and dynamic obtained at the Foundation for Cardiac Surgery Development in Zabrze, Poland. Conclusions and future plans are presented too.

Opis:
The stroke volume of pulsatile ventricular assist device (VAD) is one of the key clinical indicators to evaluate the adequacy of the process of mechanical heart assistance and ensuring the patient’s safety. In order to evaluate the volume changes in VAD blood chamber, a bioimpedance method was applied, based on volumetric electric conductivity of the blood. Periodic changes in VAD volume during its operation result in a pulsatile impedance waveform; its amplitude in the subperiods of filling and ejection correlates with blood volume in the chamber. The paper presents the method of analysis of the bioimpedance signal recorded in VAD blood chamber with a reference to the classical method of analysing the cardioimpedance signal from the patient’s thorax. The paper also presents the results of determining stroke volume of blood chamber based on an empirical formula that has been developed.

Commonly used noninvasive fetal monitoring is based on fetal heart rate (FHR) variability analysis of the Doppler ultrasound signal coming from the mechanical activity of the fetal heart. Estimation of periodicity of acquired signals using the autocorrelation technique is very important. The determination of cardiac intervals using the Doppler signal is more difficult than in electrocardiography, where the R-waves are evident. We investigated the influence of the autocorrelation window size on the FHR variability analysis. The indices describing the FHR variability calculated for signals obtained using two different autocorrelation techniques with various window lengths were compared with the reference ones obtained from fetal electrocardiogram. The optimal window was a compromise between artifacts resistance and the averaging level of instantaneous variability.

This paper presents some aspects of the on-line remote fetal monitoring system based on the GPRS data transmission service and WAN network. The remote signal recording instrumentation consists of bioelectrical signal recorder and tablet PC computer. The central surveillance system located in hospital has a capability of simultaneous monitoring of up to 24 patients, both remotely and within hospital. The system enables analysis, dynamic presentation and archiving of signals and medical data. The clinical interpretation is immediately available in hospital as well as at patient's home or attending doctor office.

The paper presents an instrumentation set for effective diagnostics of early symptoms of fetal distress in high-risk pregnancy based on bioelectric signals recording. The representative database of records has been established that enabled the complex analysis of signals recorded on a surface of maternal abdomen. As an outcome, the effective algorithms have been developed for processing both the fetal electrocardiogram and the signal of electrical activity of a uterine muscle. This allowed the development of unique diagnostic system based on external biosignal measurement module connected to the standard personal computer. In this system, like in a classical fetal monitor, the fetal heart rate (FHR) signal together with contraction activity signal of uterine muscle and fetal movement become an essential source of information on fetal condition. In addition, there is a possibility for spectral analysis of FHR signal as well as a morphology assessment of fetal ECG signal.

Fetal monitoring system belongs to signal processing system class. The main functions of the system are signal acquisition from bedside monitors, on-line trace analysis and dynamic presentation of incoming data. Collected data set is controlled by centralised application. Relational database management system cannot process samples of high frequency biomedical signals on-line. Therefore, we decide to build our own data management system dedicated to stream processing that support continuous query. This paper describes a method of building a query plan based on proposed algebra. The presented example of application enables implementation of algorithm determining long- and short-term indices for fetal heart rate variability assessment on the basis of declarative query language. Our solution enables to define query based on data streams that make the updated answers currently available.

ITAM completed work on developing a prototype of the system designed to monitor and conduct cardiac rehabilitation at home. In the course of project execution, a number of devices have been made or adapted to work within the system. The primary problem to be solved was to develop a way of communication for these devices, which would allow the transmission of necessary data in a way that would be convenient for the patient and provide the staff supervising the patient with easy access to this data. Ensuring correct operation the system required a variety of cooperating transmission systems to be used. Communication within the apartment utilizes a wireless network based on a Bluetooth link. The parent node of this network is the Base Station, which is also an intermediary for communication between devices within the apartment and the Centre for Monitoring and Communications. The main module carried by the patient is the ECG module that, in addition to its measurement function, serves as a personal area network node based on elements of the ANT standard supporting the Messenger carried by the patient. This module is also tasked with supervising the patient outside the apartment, making communication with the Centre possible via a GSM/GPRS network. The Base Station exchanges information with the Monitoring and Communication Centre located on a server via the Internet. Access for the patient's doctor or support worker to the Monitoring Centre is possible from any computer with access to the Internet by logging onto the Centre's server.
