Opis:
Chronic rhinosinusitis with nasal polyps (CRSwNP) is a disease with still not enough known pathogenesis despite the development of genetics, immunological and microbiological research. The number of patients with CRS has been constantly growing. The coexistence of CRS, bronchial asthma and aspirin intolerance (aspirin triad) is an adverse prognostic factor with higher risk of recurrences. The aim of study was to compare the severity of CRSwNP depending of coexistence of bronchial asthma and/or aspirin intolerance. The research was performed in the group of 204 patients operated 2009-2013 with 5 years follow-up. Higher nasal polyps growth in groups of patients with aspirin triad and CRSwNP and bronchial asthma in endoscopic examination (p=0,0005 and p=0,0030 respectively) and CT-scan according to Lund-Mackay point scale (p<0,0001 and p=0,0009) was showed. Also, these patients presented increased severity of nasal symptoms before surgical treatment according to VAS scale (p=0,0126 for CRSwNP with bronchial asthma; p=0,0390 for aspirin triad). Similarly, 6 months after surgery the same groups of patients presented higher severity of the disease symptoms (p<0,0001 for aspirin triad’ patients; p=0,0174 for CRSwNP and bronchial asthma’ patients) . Patients with aspirin triad had also statistically more surgeries in past (p=0,001), what proves that recurrences in this group are very likely to be observed in spite of the use of proper conservative treatment. No such differences have been shown in the group of patients with CRSwNP and isolated aspirin intolerance (without bronchial asthma). Allergy to inhaled allergens, hypersensitivity to aspirin are factors significantly worsening the course of CRSwNP. It would be advisable to consider, despite a lack of history of aspirin intolerance, a hypersensititvity to aspirin test in patients with particularly severe CRSwNP, especially those associated with bronchial asthma. It also seems reasonable to carry out such a test on every patient with newly diagnosed CRSwNP and bronchial asthma in order to be able to plan further treatment in this group of patients accordingly including biological treatment with antimonoclonal therapy against interleukin 4, 5 or13.

Thyroid surgery is the most commonly performed procedure in the field of endocrine surgery. Studies are still ongoing on the development of a single algorithm for diagnosis and care of patients at risk of postoperative hypoparathyroidism. The aim of the study was to determine the biochemical marker that would allow the most accurate diagnosis of patient groups at risk of developing hypoparathyroidism and to identify risk factors for this disorder. Material and methods. The prospective study included 142 consecutive patients undergoing total thyroidectomy for benign goiter from January 1st 2014 to December 31st 2015. Serum intact parathyroid hormone (iPTH), total calcium (Ca), phosphate (P), and magnesium (Mg) levels have been measured preoperatively and at 1, 6, 24, and 48 h postoperatively. Results. Clinical symptoms of hypoparathyroidism developed in 25 (17.6%) of 142 patients. The best diagnostic accuracy for hypoparathyroidism based on ROC curves was obtained for iPTH at 6h (AUC 0.942; 95% CI: 0.866-1.000, p<0.001) and its percentage change from baseline ΔiPTH at 6h (AUC 0.930; 95% CI: 0.858-1.000, p<0.001). In an multivariate analysis, the preoperative Ca level higher by 0.1 mmol/l, and iPTH level higher by 0.1 pmol/l were associated with a lower risk of hypoparathyroidism, by 68% (p=0.012) and 61% (p=0.007), respectively. A 1% decline in iPTH from baseline increased the risk of hypoparathyroidism by 15% (p<0.001). Conclusions. The most reliable markers indicating a high risk of postoperative hypoparathyroidism are the decline in ΔiPTH at 6h by > 65% or iPTH level at 6h <1.57 pmol /l. A postoperative decline in iPTH levels is an independent risk factor for the development of hypoparathyroidism. Preoperative higher concentrations of Ca and iPTH are protective factors for the development of this disorder.

Introduction: One of the most recent methods used in imaging of the larynx is NBI. NBI allows to detect specific patterns of pathological angiogenesis suggestive of premalignant or neoplastic lesions. Aim: The aim of the study was to compare imaging of laryngeal lesions in WLE and NBI in relation to histopathological examination. Material and methods: 333 patients with laryngeal lesions underwent endoscopic evaluation in WLE and NBI. Sensitivity, specificity, PPV, NPV for both methods were calculated. The diagnostic value for WLE and NBI was evaluated for two assumptions: a positive result indicates (1) severe dysplasia and cancer, or (2) only cancer. Results: Sensitivity, specificity, PPV, NPV for the first assumption were, respectively for white light compared to NBI: 95.4% vs 98.5%; 84.2% vs 98.5%; 79.6% vs 97.7% and 96.6% vs 99.0%. The values for the second assumption were: 97.4% vs 100%; 79.3% vs 93.5%; 72.6% vs 89.4% and 98.2% vs 100.0%. Higher sensitivity was observed for the second assumption, while higher specificity was recorded for the first assumption. Specificity was significantly higher for NBI than for WLE (p < 0.001). Conclusions: NBI allows to detect and differentiate laryngeal lesions, which are invisible in WLE. Endoscopic examination, especially in NBI mode, is non-invasive, repeatable and remains a useful tool in the daily practice and diagnosis of patients with pathological lesions in the larynx.

Opis:
A novel diagnostic method based on recognition of qualitative and quantitative composition of healthy and tumor tissue samples by library of molecular receptors was presented. Molecular receptors were formed by self-organization of N-lipidated peptides attached in the regular fashion via aminophenylamino-1,3,5-triazine linker to the surface of cellulose plate. For samples testing, the library was cloned into multiple, identical copies and for each experiment the new clone was used. The binding process was monitored by staining the discs with Brilliant Black and quantitative color measurement was performed in 256 grade gray scale. Substantial differences in the composition of healthy and tumor samples were observed in most cases. The highly individual chemical fingerprints were found to be reliant on the cancer type. For malignant papillary thyroid cancer statistical analysis identified two receptors most useful for diagnostic purposes.
