The objective of the study was to perform a comparative analysis of genetic similarity, with the use of pulsed field gel electrophoresis (PFGE), of Clostridium perfringens isolates originating from patients with gas gangrene and from the hospital environment. The study encompassed two patients with a clinical and microbiological diagnosis of gas gangrene, who were hospitalized in one of the hospitals of the Małopolska province in the time period between 31st March 2012 and 18th May 2012. Clostridium perfringens isolates genotyping indicated that the isolates originating from the two studied patients did not display genetic similarity and represented two different PFGE types, which corresponded to two different clones (clone A and B). Whereas the strains isolated from the hospital environment were genetically identical with the strain coming from the second patient and represented one PFGE type, which corresponded to one clone (clone A). As a result of the study, it is possible to conclude that both patients developed endogenous infection. Even so, the examination of the hospital environment indicates the possibility of the appearance of exogenous infections. It prompts recommending and following the exact regulations of sanitary regime in the ward and the operating theater if a patient is diagnosed with gas gangrene.

Background The objective of the study has been to evaluate the pathogenic bacteria contamination of touch surfaces in hospital wards. Material and Methods Samples were taken from frequently touched surfaces in the hospital environment in 13 units of various types. Culturing was carried out on solid blood agar and in growth broth (tryptic soy broth – TSB). Species identification was performed using the analytical profile index (API) biochemical testing and confirmed with matrix assisted laser desorption ionization time-flight mass spectrometry (MALDI-TOF-MS) system. Results The total of 161 samples were taken for the study. Fifty-two of them, after 24 h of culture on a solid medium, demonstrated bacterial growth and further 60 samples had growth after prior multiplication in TSB. Overall, 69.6% of samples exhibited growth of 19 bacterial species. Pathogenic species – representing indicator organisms of efficiency of hospital cleaning – was demonstrated by 21.4% of samples. Among them Acinetobacter spp., Enterocococci spp. and Staphylococcus aureus were identified. Coagulase-negative staphylococci (CNS) were predominant. The proportion of various groups of bacteria significantly varied in respective hospitals, and in various types of wards. Disturbing observation is a large proportion of resistance of isolated CNS strains as a potential reservoir of resistance genes. Conclusions The results show that touch surfaces in hospital units are contaminated by both potentially pathogenic and pathogenic bacterial species. In connection with the reported, also in Poland, frequent omission or incorrect execution of hand hygiene by hospital staff, and probably patients, touch surfaces still constitute important reservoir of pathogenic bacteria. Improving hand hygiene compliance of health-care workers with recommendations is necessary for increasing biological safety of hospital environment. Med Pr 2017;68(3):459–467

In the study we tested drug sensitivity to 3 carbapenems (doripenem, imipenem and meropenem) of Gram-negative clinical isolates from Southern Poland. Material and methods. 89 strains were examined: 42 from Pseudomonas genus, 16 Acinetobacter baumannii strains and 31 Enterobacteriaceae strains. Etests were used according to the producers instructions, MIC values were interpreted using EUCAST criteria. Results. Highest in vitro activity against Pseudomonas spp. was shown for doripenem, then meropenem and the lowest for imipenem (MIC values were definitely lower for doripenem; differences were statistically significant); A. baumannii strains showed similar sensitivity to doripenem, meropenem and imipenem (differences non-significant); all Enterobacteriaceae strains showed sensitivity to the tested antimicrobials. Conclusions. As a conclusion-doripenem, which has high in vitro activity (almost the same as imipenem and meropenem) as well as beneficial pharmacologic properties, may be an alternative solution in the treatment of multiresistant Gram-negative bacteria, especially in patients in severe status who require restrictive antibiotic regimens.

The study aim was evaluation of the usefulness of several thermostable DNA polymerases in real time PCR conducted in the presence of the heme. Our study had the advantage of testing several different polymerases, one of which proved to be the least sensitive to heme activity. We also found that there is no need of supplementing the reaction mixture with protective substances like BSA. Selection of the appropriate polymerase can increase the efficiency of the PCR reaction which is very important for diagnosis of sepsis and for other analyses performed on DNA template isolated from the blood.

Background The objective of the study is self-assessment of Polish infection control nurses (ICNs) in terms of the structure of professional tasks and autonomy of decision-making. Material and Methods A questionnaire survey was filled out by 208 ICNs (around 21% of all Polish ICNs) in 15 provinces located in Poland. The research encompassed ICNs surveillance healthcare-associated infections (HAIs) in 2014. Results The work time that ICNs devote to professional tasks and decision autonomy on the scale of 1–100% was as follows: 34% (67% of decision autonomy) was dedicated to detecting and registering HAIs, 12% (71%) – internal control, 10% (58%) – devising and implementing infection prevention practices, 10% (68%) – staff trainings, 8% (65%) – identification and study of outbreaks, 7% (58%) – promoting hand hygiene, 6% (51%) – consults with infected patients, 4% (57%) – consults on decontamination, 4% (54%) – consults on maintaining cleanliness, 3% (51%) – isolation and application of personal protective measures, 2% – other tasks. Infection prevention and control nurses estimated, on average, that their autonomy of decisions concerning the professional tasks performed amounted to 60%. Conclusions Infection control nurses in Poland have difficulty in achieving balance between tasks they perform and the authority they exercise. The ICN professional task structure is dominated by duties associated with monitoring hospital infections, however, the greatest decision autonomy is visible regarding internal control. Decision-making concentrated on internal control may hinder building a positive image of an ICN. We should strive to firmly establish professional tasks and rights of ICNs in legislation concerning performing the duties of a nurse and midwife. Med Pr 2018;69(6):605–612

Objectives: The study presents data concerning occupational exposures among the staff of 5 hospitals in the Małopolska province in 2008–2012, taking into account the frequency and circumstances of exposure formation, occupational groups of hospital workers, as well as diversification of the reported rates in subsequent years between the hospitals and in each of them. An additional objective of the analysis was to assess the practical usefulness of the reported data for planning and evaluation of the effectiveness of procedures serving to minimize the risk of healthcare workers’ exposure to pathogens transmitted through blood. Material and Methods: Data were derived from occupational exposure registries kept by 5 hospitals of varying sizes and operational profiles from the Małopolska province from the years 2008–2012. Results: Seven hundred and seventy-five cases of exposure were found in a group of 3165 potentially exposed workers in the analyzed period. Most cases were observed in nurses (68%) and these were mainly various types of needlestick injuries (78%). Exposure rates with respect to all workers ranged from 2.6% to 8.3% in individual hospitals, but the differences in their values registered in the hospitals in subsequent years did not bear any statistical significance, in a way similar to the rates calculated separately for each occupational group. Conclusions: There was no upward or downward trend in the number of reported cases of exposure to bloodborne pathogens in the studied period in any of the hospitals. Statistically significant differences in the percentages of exposures were reported between individual hospitals in some years of the analyzed period, which confirms the need for registries in individual units in order to plan and evaluate the effectiveness of preventative measures.

As part of the work, new polymer materials were used in 3D printing, which can be used in three technologies depending on the apparatus used: Fused Filament Fabrication (FFF), Melted and Extruded Manufacturing (MEM), Melt Extrusion Polymers (MEP). As part of the work, the properties of obtained polymer composites were examined. Polylactide (PLA) was used as the matrix, and powdered natural ground fillers were used as fillers: bamboo dust (PB), cork dust (PK) and wood dust (PD). In the first part of the work, filaments were received from the tested composites using the filament preparation line made by METACHEM in Torun for use in 3D printers. Samples for mechanical tests were obtained from the filaments thus received by means of an MEP printer and after granulation by injection molding. In the next part of the work, rheological, mechanical and structural properties of obtained composites were examined. It was found that the addition of natural fillers increased the fluidity of the obtained polymeric materials in the case of composite with addition of PK even by 48.73% compared to unfilled PLA. It was also observed that the composites obtained had lower Charpy impact strength, Rockwell hardness and tensile strength. Observation of the microstructure of the composites using SEM confirmed the even distribution of natural fillers in the polymer matrix, which proves well-chosen parameters of their homogenization in the polymer matrix.
Otrzymano nowe materiały polimerowe do zastosowań w druku 3D, które można wykorzystywać, zależnie od stosowanego aparatu, w trzech technologiach: Fused Filament Fabrication (FFF), Melted and Extruded Manufacturing (MEM), Melt Extrusion Polymers (MEP). Zbadano właściwości wytworzonych kompozytów polimerowych. Jako osnowę zastosowano polilaktyd (PLA), a w charakterze napełniaczy użyto sproszkowanych w postaci pyłu naturalnych cząstek: bambusa (PB), korka (PK) oraz drzewnych (PD). Z badanych kompozytów otrzymano filamenty z wykorzystaniem zaprojektowanej i wykonanej przez METACHEM w Toruniu linii do wytwarzania filamentu stosowanego w drukarkach 3D. Z tak uzyskanych filamentów wykonano próbki do badań mechanicznych za pomocą drukarki MEP oraz, po zgranulowaniu, metodą wtryskiwania do formy. Zbadano właściwości reologiczne, mechaniczne oraz strukturalne otrzymanych kompozytów. Stwierdzono, że dodatek naturalnych napełniaczy spowodował zwiększenie płynności materiałów polimerowych, w wypadku kompozytu z dodatkiem PK nawet o 48.73% w stosunku do płynności nienapełnionego PLA. Stwierdzono, że otrzymane kompozyty wykazywały mniejsze: udarność według Charpy'ego, twardość według Rockwella oraz wytrzymałość przy rozciąganiu. Obserwację mikrostruktury badanych kompozytów za pomocą SEM potwierdziły równomierny rozkład cząstek naturalnych napełniaczy w osnowie polimerowej, co świadczy o dobrze dobranych parametrach procesu ich homogenizacji.
