We present case report of 62-year-old man who underwent total laryngectomy, bilateral neck dissection and radiotherapy due to laryngeal cancer. After 11 months he was reoperated because of a local recurrence. The second procedure was complicated with postoperative wound dehiscence in tracheostomy site which was successfully treated with negative pressure wound therapy. We indicate the difficulties and point technical solutions for negative pressure dressing around the airway.

The aim of the studywas to evaluate the efficacy of different microvascular techniques in breast reconstruction with the analysis of postoperative complications. The additional goal of the study is to analyze the quality of life of patients after microvascular breast reconstruction in comparison to the control group of patients who underwent only mastectomy without any reconstructive procedures. Also the algorithm of breast reconstruction is presented as the result of own experiences. Material and methods.Clinical material contain 2 groups of patients - women after surgical treatment in Department of Oncological and Reconstructive Surgery, Cancer Center in Gliwice in the year 2004-2009 where in 53 cases immediate and in 26 delayed breast microvascular reconstruction were performed. In all cases the diagnosis of cancer was proved by histopathological biopsy before the treatment. The type of radical resection (mastectomy) depended on histopathological type of cancer and its localization. The reconstruction - immediate vs delayed was carefully planned together with oncological treatment of the cases. Everywhere this plan was established based on carefully examinations of inferior epigastric vessels and theirs perforators. The choice between immediate and delayed microvascular reconstruction was based on prognosis and predictive factors. The QOL was analyzed due to own questionnaire when functional, aesthetics and social effects were evaluated. ResultsFree flap survival rate for all types of free flap was 95%. In cases where classic TRAM was used the rate was 85%, in cases where muscle sparring TRAM was chosen the survival rate was 100% and in remaining cases of DIEP reconstructions the rate was 89%. Generally the complications after microvascular reconstruction occurred in 13 cases (16%). In 9 cases the problems with flaps perfusion were notified. Total flap necrosis was observed in 2 TRAM and in 2 DIEP cases. In all those cases salvage surgery was administered in which the microanastomoses were explored and repaired. In 5 cases the cause of the complications was venous thrombosis, in 2 cases the vascular pedicle was kinked, and in remaining 1 the arterial thrombosis was found. The second type of complication was fat necrosis (<25% of flap volume) which was observed in 5 cases between 1 and 4 months after surgery, and it request minor plastic surgery. Donor site complications were noted in 4 cases. In two of those hernia in cicatrices was diagnosed (both were classic TRAM’s), in remaining 2 in which also fully muscle TRAM was classic the weakness of abdominal wall was observed. In group were msTRAM and DIEP were used no donor site complications occurred.

Opis:
Reconstructive techniques for extended posttraumatic or post-resectional nose defects are well described. Small defects can usually be closed using local flaps with a good esthetic result. Extended resections in the mentioned area require more complex reconstruction methods, including microvascular techniques. The Authors' presented a case of whole nose and partial cheek simultaneous reconstruction using three free flaps. Two auricular free flaps and one radial forearm free flap were used to cover the defect. Meticulous preoperative planning and harvesting of flaps enabled to achieve good esthetic and functional results.

Liver resection is essential part of colorectal cancer liver metastases (CLM) treatment. Mean 5-year overall survival after resection achieves 30-45%. There are many factors influencing long-term outcomes, and among them the inflammatory response to tumor plays an important role. The aim of the study was evaluation of outcomes and treatment safety of patients with metastatic colorectal cancer to the liver with estimation of prognostic factors. Material and methods. 130 consecutive patients (70 men and 60 women) operated in MSC Institute and Cancer Center in Gliwice from 2001 to 2009 due to colorectal liver metastases were analysed. Age of the patients ranged from 33 to 82 years (median 60 years). 96 (74%) patients underwent potentially radical resection, and in remaining 34 (26%) was performed radiofrequency ablation (RFA) alone or combined with the resection. In the resection group 37 right hepatectomies, 11 left hepatectomies, 28 segmentectomies and 20 metastasectomies were performed. Disease-free survival (DFS) and overall survival (OS) were statistically analysed using the Kaplan-Meier method. Factors determining DFS and OS were analysed using Cox regression model. Results. In the resection group the 3- and 5-years OS was 64,5% and 46,6% respectively, and the 3- and 5-years DFS was 32% and 30,5% respectively. In the RFA group the 3- and 5-years OS was 33% and 9,5%. Statistically significant prognostic factors in the resection group in uni- and multivariate analysis were: grade and nodal involvement of the primary tumor, diameter of metastatic focus, positive and narrow (<1 mm) resection margins, preoperative fibrinogen level, preoperative neutrophil to lymphocyte ratio and leukocyte amount of the peripheral blood. The perioperative mortality rate was 3%. Conclusions. Liver resection due to colorectal liver metastases is a safe and effective method resulting in high survival rates. We confirmed some generally accepted prognostic factors influencing longterm outcomes and shown the impact of inflammatory response. We also confirmed the hypothesis that preoperative plasma fibrinogen level influences outcomes after liver resection due to CLM.

Preoperative preparation of working models of the skull and free bone flaps using the digital print technology and photocured polyacrylic resins may be of a great benefit to the patient, for whom a virtual resection and reconstruction procedure may be planned in detail and performed. The purpose of mid-facial reconstruction using 3D models is to plan a functional mid-facial reconstruction procedure in order to restore supportive function of intraorbital structures and to make placement of dental implants and further prosthetic rehabilitation possible.Maxillary and mid-facial reconstruction using a free fibula flap based on a three-dimensional working model was performed in a patient diagnosed with a squamous cell carcinoma of the left maxillary sinus penetrating to the orbit, the ethmoid complex, and the pterygopalatine fossa. The use of three-dimensional polyacrylic models allowed for detailed preoperative planning and a virtual resection and reconstruction procedure with a highly satisfying functional and cosmetic effect.A procedure based on methods discussed here may be significantly shorter and more precise.

The use of microsurgery for oral reconstruction of cancer patients, has become standard treatment in restoring oral function. The free anterolateral thigh flap is one of the most preferred options in reconstruction after total, subtotal or hemiresection of the tonque due to squamous cell cancer. The aim of the study was to present the reconstructive method using anterolateral thigh free flap with evaluation of quality of live. Material and methods. Clinical material includes 46 consecutive patients with tongue cancer, who underwent complex surgical treatment between 2009 and 2011. There were 36 males and 10 females and the M: F ratio was 3.6: 1. All of them were reconstructed using the anterolateral thigh free flap. The quality of life was evaluated 6 months after completing the treatment, based on postoperative functional and aesthetic status. Results. The overall flap survival rate was 96%. Surgical complications were observed in 8 patients (17%). Donor site was closed primarly in 42 cases and in remaining 4 skin graft was required. In all 46 cases understandable speech and return to unrestricted diet mastication and swallowing were achieved. The mean follow-up period after treatment was 32 months. Analysis of aesthetic effects evaluated in 23 cases and shows generally very good results. According to average transformed scores the QOL can be characterized as excellent for >90, very good for 76-90, good for 51-75, moderate for 25-50 and bad for <25 points. Conclusions. Anterolateral thigh flap, with its versatility in design, long pedicle with a suitable vessel diameter, low donor site morbidity, and very good aesthetic effects, could be the ideal flap for functional tongue reconstruction.

The extended latissimus dorsi flap (LD) is used in breast reconstruction since the 70's. LD flap is often used in corrective surgery in cases of unsatisfactory cosmetic results after breast-conserving therapy. In our department LD flap has several uses. In addition to free microvascular flaps - which applies in breast reconstructive surgery is routine, there are clinical situations where the use of pedicled LD flap is justified. The main indications for its use are: the inability to apply microvascular flap, the general condition (diabetes, advanced atherosclerosis), smoking, previous surgery of abdominal wall, abdominal obesity, patient preferences. Approximately 30% of patients after breast reconstruction require corrective procedures. Group which uses extended LD flap account for 24 patients. In 16 cases it was used for elective breast reconstruction. In the remaining eight cases it was used in the corrective procedures symmetry and shape of the previously reconstructed breast. Based on our own experience it can be concluded that the LD flap with an alternative to microvascular techniques. Complication rate when using the LD flap is relatively low and includes: seroma and slight motor disability of the shoulder girdle.

Introduction: Central giant cell granuloma (CGCG) is a benign tumor-like lesion of a bone, mainly localized in the mandible. It usually occurs in children and young adults under 30 y/o., predominantly in females. The etiology of the disease remains unknown. Clinically, two types of CGCG have been distinguished – a non-aggressive one, in which the granuloma grows slowly, often asymptomatically, and aggressive type for which the following features are characteristic: increased bone destruction, severe pain, large size, rapid growth, high recurrence rate and complications such as root resorption, tooth displacement or cortical bone perforation. The treatment of CGCG depends on its type. In cases of granulomas of the aggressive type, the following therapeutic procedures have been proposed: intralesional corticosteroid injections, interferon and calcitonin therapy as well as immunotherapy with anti-bone resorptive human monoclonal antibody like denosumab. However, in most cases nonsurgical treatment is insufficient. Local curettage of the lesion also entails a high risk of relapse. Therefore, radical surgical resection, often combined with bone reconstruction, is the recommended way of treatment for aggressive CGCG. Case report: The authors present a case of a 31-year-old female patient treated for central giant cell granuloma of the mandible at the Department of Oncological and Reconstructive Surgery, Maria Sklodowska Curie Memorial Cancer Centre and Institute of Oncology in Gliwice. The resection of CGCG localized in the mandible on the right side together with fibular free flap reconstruction has been performed, with satisfactory aesthetic effect. The immunohistochemical examination indicated a positive stain reaction for CD68 and CD31 and expression of Ki67 marker was 13%. No complications were reported in the postoperative period. The six-month follow up revealed no relapse. Conclusions: The authors claim that radical surgical management should be performed in all patients with CGCG of the aggressive type. Fibular free flap is recommended for reconstruction in large bone defects. This allows tumor-free margins at the resection and satisfactory cosmetic outcome. Quality of life and facial appearance can be improved with dental implantation after a certain period of remission. A regular follow-up is essential as an element of holistic oncological process.
Wstęp: CGCG jest łagodną, guzopodobną zmianą kości, najczęściej lokalizującą się w obrębie żuchwy. Występuje ona głównie u dzieci i młodych dorosłych poniżej 30 r.ż., z przewagą płci żeńskiej. Etiologia schorzenia nie została do końca poznana. Klinicznie wyróżnia się dwie postacie CGCG: (1) nieagresywną, kiedy ziarniniak rośnie powoli, często bezobjawowo, oraz (2) agresywną, charakteryzującą się: nasilonym niszczeniem kości, dolegliwościami bólowymi, większymi rozmiarami guza, jego gwałtownym wzrostem i powikłaniami, takimi jak: resorpcja korzeni czy perforacja blaszki kostnej, oraz wysoką tendencją do nawrotów. Leczenie ziarniniaka zależy od jego postaci. W przypadkach CGCG o agresywnym przebiegu zastosowanie znajduje leczenie zachowawcze pod postacią sterydoterapii w infekcjach miejscowych, aplikacji interferonu, kalcytoniny czy denosumabu. Jednak w większości przypadków ta forma terapii bywa nieskuteczna. Również wyłyżeczkowanie zmiany niesie wysokie ryzyko nawrotu choroby. Stąd najlepszym sposobem postępowania jest radykalne chirurgiczne usunięcie zmiany, często wymagające operacji rekonstrukcyjnej ubytków kostnych. Opis przypadku: W niniejszej pracy autorzy przedstawiają przypadek 31-letniej chorej, leczonej w Klinice Chirurgii Onkologicznej i Rekonstrukcyjnej Centrum Onkologii – Instytutu im. Marii Curie-Skłodowskiej Oddziału w Gliwicach z powodu centralnego ziarniniaka olbrzymiokomórkowego żuchwy. U pacjentki wykonano operację resekcji fragmentu żuchwy po stronie prawej wraz z guzem z następową rekonstrukcją wolnym płatem strzałkowym, uzyskując zadowalający efekt kosmetyczny. Przebieg pooperacyjny był niepowikłany. W badaniu immunohistochemicznym wykazano dodatnią reakcję z przeciwciałami CD68 i CD31, a współczynnik Ki67 wyniósł 13%. W półrocznym okresie obserwacji nie odnotowano wznowy ziarniniaka. Wnioski: Autorzy uważają, że radykalne leczenie chirurgiczne powinno być stosowane u wszystkich pacjentów z agresywną postacią centralnego ziarniniaka olbrzymiokomórkowego. Z uwagi na częste naciekanie kości w tych przypadkach, zalecają rekonstrukcję wolnymi płatami kostnymi z mikrozespoleniem naczyniowym. Pozwala to na uzyskanie radykalności onkologicznej w połączeniu z dobrym efektem kosmetycznym. Jakość życia i estetyka twarzy może być udoskonalona dzięki implantom zębowym, zastosowanym po odpowiednim czasie remisji choroby. W całościowym leczeniu onkologicznym ogromną rolę odgrywa ścisła, regularna kontrola w ośrodku prowadzącym.

CBCT is a relatively new diagnostic imaging method. It was first used in 1982 at the Mayo Clinic Biodynamics Research Laboratory [1], and has been commercially used in dentistry since 2001. It differs from multidetector computed tomography in terms of implementation technique and some parameters. A low dose of radiation is its greatest advantage, thanks to which many specialists, including ENT doctors and reconstructive surgeons, are more willing to use it. In otorhinolaryngology, CBCT is applied in pre- and perioperative diagnostics in both rhinology and otology. In reconstructive surgery, CBCT facilitates precise planning of the flap and intraoperatively allows a perfect match of the reconstructed tissue elements. In the article, the authors present current information on CBCT in ENT obtained on the basis of a review of Polish and foreign literature and share their own experience in its application in reconstructive surgery within the craniofacial region.

Introduction: The aim of the study was to prove that a combination of visual surgical planning (VSP) and cone beam computed tomography (CBCT) is an optimal technique in fibular free flap reconstructions after complex tumor resections in the head and neck region and that it leads to better functional and aesthetic outcomes. Material and method: Six patients (3 females, 3 males) with head and neck tumors were included in the study. The region concerned midface in 2 cases and mandible in 4 patients. On the basis of computed tomography of the head, fibular free flap (FFF) reconstruction was planned with the VSP technique. The 3D-printed models were prepared. At the beginning of the operation and a few minutes after the reconstruction, an xCAT CBCT by XORAN was performed. Minor corrections of the angles of the reconstructed bony parts were made where needed. The time of the operation was assessed for each case. Functional and cosmetic results were evaluated in a 1-year follow-up. Results: The mean time of operation was 6 hours and 48 minutes, which was approximately 1hour and 40 minutes less than standard reconstructive surgery. Functional recovery was achieved in all patients. Aesthetic result was unsatisfactory for 2 patients due to insufficient soft tissue masses of FFF. Conclusions: The authors claim that intraoperative CBCT imaging, regardless of the cost, improves the accuracy of aesthetic outcome of reconstructive surgeries based on VSP, especially in the region of the midface and the mandible. Further studies on a higher number of subjects are required.

Introduction: The perforation of gloves during surgical procedures is quite common. A cheap and quite effective method of reducing the risk of blood-borne infections is wearing two pairs of gloves. Unfortunately, some surgeons are reluctant to it, and they report decreased dexterity and sensation. The aim of the study was to evaluate surgeons’ double-gloving practices to determine the factors related to compliance. Material and methods: An anonymous, 21-question survey in Polish was sent by post to 41 surgical departments. The questions concerned: demographic data, type of surgical gloves used, allergy to latex, number of surgeries performed, frequency of using double gloves and negative impressions from using them and finally, the frequency of needlestick injuries during surgical procedures. Results: We received 179 questionnaires back. More than 62% of the surgeons believe that double gloves provide better protection than a single pair, 24% do not believe in this, and 14% have no opinion. Only 0.6% of respondents always use double gloves during surgery, 19% double glove in at least 25% of cases and 68% do it occasionally. 13% of the surgeons declared that they had never worn double gloves. During high-risk procedures, 86% of respondents wear double gloves. About half of respondents (50.3%) report discomfort while wearing double gloves; 45% – decreased dexterity; about 30% complain of numbness and tingling; and 64% – decreased sensation. Conclusion: Due to the high number of surgical glove perforations and relatively high prevalence of needlestick injuries, it is necessary to use methods that reduce the risk of transmission of pathogens. The habit of using a double pair of gloves should be implemented especially among young surgeons starting to train in their specialities. Consequently, the period of initial discomfort will be combined with the acquisition of surgical skills, which will allow for gradual acclimatization.

Introduction: The nose is the central and probably the most important organ of the face. In view of the three-dimensional shape and variety of tissues, reconstructive surgery after tumor resection in this anatomical region requires the surgeon’s knowledge of anatomy. Materials and Method: In the years 2010–2019, 48 patients were treated in the Oncological and Reconstructive Surgery Clinic for extended nasal tumors, which required the use of free microvascular flaps after resection for functional and aesthetic supply of anatomical structures of the nose. Results: In 48 patients, a total of 92 free microvascular flaps were used for nasal reconstruction including: radial forearm free flap in 24 patients, radial forearm free flap with radial bone in 14 patients, auricular free flap in 16 patients, radial forearm free flap in combination with auricle free flap in 7 patients, double auricular free flap in 6 patients, radial forearm free flap in combination with double auricular free flap in 4 patients. Total necrosis of the free flap was noted in 4 cases, partial in 6 patients. Conclusions: The presented surgical techniques using microvascular free flaps constitute a recognized method of treatment and should be used in everyday surgeon practice. The results demonstrated in this article allow to obtain optimal functional and aesthetic effects.

Vitamin D3 (1,25(OH)2D3 (1,25-dihydroxyvitamin D3)) is a hormone playing a crucial role in numerous biological processes in the human body, including induction and control of cell proliferation and differentiation. Numerous data relate the vitamin D3 level with various types of cancer. It has been suggested that SNPs in the vitamin D3 receptor (VDR) gene might influence both the risk of cancer occurrence and cancer progression. The aim of this study was to search for genetic correlations between individual SNPs in the VDR gene and the risk of oral cavity carcinoma. Two SNPs were selected based on the literature and our previous results. Seventy-three patients with squamous cell carcinoma of the head and neck and one hundred control subjects were investigated. Two SNPs in the VDR gene were genotyped in minisequencing reactions followed by capillary electrophoresis. Hardy-Weinberg equilibrium (HWE), the χ2 test and logistic regression were used for statistical analysis. The SNP rs2238135 in the VDR gene displayed statistical differences in frequency between the tested groups (p=0,0007). Furthermore, the G/C genotype of the rs2238135 in the VDR gene was characterized by a 3.16 fold increased risk of oral cavity carcinoma. The obtained results provide evidence for a genetic association between rs2238135 in the VDR gene and the occurrence and risk of oral cavity cancer.
