The aim of the study was to evaluate the results of surgical treatment and rehabilitation of patients with trigger thumb and finger. In 40 patients, comprising 30 women and 10 men aged 26 to 64, a total of 42 cases of trigger thumb and finger. In the preoperative period, the severity of changes were studied according to the classification developed by Newport et al. Five patients were classified in the first stage, 28 in the second, 6 in the third, and 1 in the fourth. The mean duration of symptoms was five months. The indication for surgery was a lack of improvement following conservative treatment. All of the patients were treated surgically using the open method by cutting the flexor tendon sheath in part A1. The rehabilitation treatment included exercises to improve the range of mobility of the thumb and fingers and to stretch, grelax, and strengthen muscles. Neuromobilisation and automobilisation exercises were conducted. After 5 months, swelling, pain and restricted mobility of the thumb and fingers subsided in all patients. There were no ‘jumping’ symptoms. Apart from a slight transitory inflammatory reaction in 2 patients there were no complications. In patients with trigger finger, open surgery and competent rehabilitation therapy enables the achievement of very good results, with a low complication rate.

Soft X-ray (SXR) plasma measurements are important type of diagnostics especially in tokamak facilities. Main output products are energy and topology spectra distributed in time. In case of Gas Electron Multiplier (GEM) detectors, used as radiation sensors, large number of analog readout channels must be provided. For this purpose Field-Programmable Gate Arrays (FPGAs) are used. Data quality monitoring (DQM) is important topic in case of asynchronous events registered by the measurement unit. Due to numerous FPGAs used it is necessary to provide advanced trigger synchronization between them. In the paper is proposed an algorithmic approach for the trigger link training and diagnostics. It is a key component in order to provide consistent measurement data for DQM analysis. The contents of the article covers the plasma experiments and related DQM topics. In following chapters explanation of the synchronization problem is described. The proposed algorithm will be used for trigger link synchronization and diagnostics aspects. The simulations are also discussed. Currently, the proposed ideas are at the stage of implementation and tests on real hardware. Simulation were successfully performed. Summary of the work carried out is presented at the end of the paper.

A novel approach to a trigger mode in the Gas Electron Multiplier (GEM) detector readout system is presented. The system is already installed at WEST tokamak. The article briefly describes the architecture of the GEM detector and the measurement system. Currently the system can work in two trigger modes: Global Trigger and Local Trigger. All trigger processing blocks are parts of the Charge Signal Sequencer module which is responsible for transferring data to the PC. Therefore, the article presents structure of the Sequencer with details about basic blocks, theirs functionality and output data configuration. The Sequencer with the trigger algorithms is implemented in an FPGA chip from Xilinx. Global Trigger, which is a default mode for the system, is not efficient and has limitations due to storing much data without any information. Local trigger which is under tests, removes data redundancy and is constructed to send only valid data, but the rest of the software, especially on the PC side, is still under development. Therefore authors propose the trigger mode which combines functionality of two existing modes. The proposed trigger, called Zero Suppression Trigger, is compatible with the existing interfaces of the PC software, but is also capable to verify and filter incoming signals and transfer only recognized events. The results of the implementation and simulation are presented.

Introduction: Myofascial pain syndrome is an acute and chronic painful musculoskeletal condition that involves muscle and surrounding connective tissue. Trigger point injection is a common treatment for this condition providing long-term relief. The procedure is generally safe; however some side effects have been reported including pain, nerve injury, bleeding, infection, and pneumothorax. Objective: To report a case of a patient with myofascial pain syndrome successfully treated by Ultra Sound-guided infiltration of a myofascial trigger point in the rhomboid major muscle. Case description: A 39-year-old presented with cervical and dorsal pain of 4 months of evolution. She had physical and occupational therapy, with partial improvement of cervical pain but persistence of dorsal pain. No abnormal finding was noted on the neurological examination. On palpation, the patient had a myofascial trigger point in the left rhomboid major muscle. Given the persistence of the myofascial trigger point after physical therapy, it was considered the patient might benefit from Ultra Sound-guided infiltration. No adverse events were reported. At the end of the procedure, the patient reported a 70% reduction in pain. The patient returned for a follow-up visit one month after the procedure, reporting pain relief of 80%. Conclusions: The use of an Ultra Sound-guided technique for trigger point injection decreases the risk of iatrogenic complications. The blind method may result in poor localization of the point. Further studies are required to develop Ultra Sound based criteria to determine its clinical use.
Wstęp: Mięśniowo-powięziowy zespół bólowy jest ostrym, przewlekłym i bolesnym stanem układu mięśniowo-szkieletowego, który obejmuje mięśnie oraz otacząjącą je tkankę łączną. Zastrzyk do punktu spustowego jest powszechnym sposobem leczenia tego schorzenia, dającym długotrwałą ulgę. Procedura jest generalnie bezpieczna; jednakże zgłaszano pewne objawy niepożądane takie jak ból, uszkodzenie nerwu, krwawienie, infekcję, oraz odmę opłucnową. Cel: Opis przypadku pacjentki z mięśniowo-powięziowym zespołem bólowym leczonej skutecznie za pomocą infiltracji pod kontrolą USG mięśniowo-powięziowego punktu spustowego w mięśniu równoległobocznym większym. Opis przypadku: 39-letnia pacjentka, z występującym od 4 miesięcy bólem szyjnym i grzbietowym. Po fizjoterapii oraz terapii zajęciowej, ból szyi częściowo złagodniał ale ból grzbietu utrzymywał się. W badaniu neurologicznym nie stwierdzono żadnych nieprawidłowości. Podczas badania palpacyjnego, w lewym mięśniu równoległobocznym większym pacjentki wykryto mięśniowo-powięziowy punkt spustowy . Biorąc pod uwagę utrzymywanie się mięśniowo-powięziowego punktu spustowego po fizjoterapii, uznano, że pacjentce mogłaby pomóc infiltracja po kontrolą ultrasonografii. Nie zgłoszono żadnych niepożą danych reakcji. Pod koniec zabiegu, pacjentka stwierdziła zmniejszenie nasilenia bólu o 70%. Miesiąc po zabiegu, pacjentka przyszła na wizytę kontrolną zgłaszając złagodzenie bólu o 80%. Wnioski: Zastosowanie techniki z użyciem ultrasonografii do iniekcji punktu spustowego zmniejsza ryzyko wystąpienia powikłań jatrogennych. Metoda “ślepa”, tj. bez zastosowania USG, może skutkować nieodpowiednią lokalizacją punktu. Potrzebne są dalsze badania w celu opracowania kryteriów opartych na ultrasonografii aby móc określić jej kliniczne zastosowanie.

This paper argues that copular sentences without an overt copular predicate do project a VP with a phonologically null head, hence so-called “verbless” copular sentences are illusory. Data from Standard Arabic, Spanish, Maltese, Russian, Jamaican Creole, Finnish and Hungarian copular sentences are used to support this claim. It is also claimed here that variation between the habitual property vs. ad hoc property interpretations (traditionally called the individual level vs. stage level distinction) of non-verbal predicates found in copular sentences is closely related to the choice of the copula in multiple BE-system languages. Whilst the current accounts explain this variation by introducing an abstract aspectual operator or an incorporated abstract preposition in the functional layer of the copular predicate, the present proposal derives these interpretive differences from the presence or absence of an OPalt alternative state operator, which can bind the temporal variable of non-verbal predicates in two ways. Negation and temporal adverbials show scope ambiguity in copular sentences. They either take scope over the whole proposition or only over the non-verbal predicate. Such interpretive differences are demonstrated in Russian and Hungarian in Section 4 of this paper, however, they are taken to be valid cross-linguistically. These amibiguities cannot be explained under the “verbless copular sentence” account but fall out naturally from the “zero copula” analysis. The “alternative state” approach can be extended to dream narratives and other non-veridical contexts, which serve as alternative triggers. The existing analyses have nothing to say about such contexts.

The publication presents a UAV pneumatic launcher designed and built in ITWL. The basic element of the pneumatic launcher is the launch mechanism. The starting mechanism determines the energy parameters that will be reached on the launch pad. The work of the launch engines used in propulsion systems has been reviewed and their advantages and disadvantages are discussed when used in pneumatic launchers. A detailed description of the author’s solution of the pneumatic launching mechanism from the launcher is presented. In order to determine guidelines for the design of pneumatic starter systems, a description of the cycle of operation of the complex pneumatic system was analysed. The description of the pneumatic system’s work cycle was based on the parameters of the flow of the compressed air C and b (C – the sound conductivity of the pneumatic element, b – the critical pressure of the pneumatic element). A set of equations describing the pressure change in the filled and emptied valve compartment and the equation of motion are presented. The equations of a mathematical model describing the pneumatic component and the replacement value of flow parameters for the pneumatic system of series pneumatic components are presented. For the mathematical model, a calculation algorithm is presented which takes into account the initial conditions and the boundary conditions of the various periods of the pneumatic system cycle. Examples of calculation results for a specific pneumatic trigger mechanism are shown. The results obtained were compared with the results of the pneumatic starter station tests and the mathematical model of the pneumatic starter was evaluated.

This research study focuses on the problem of populistic propaganda online. In particular, this research study provides three case studies gathered in a Facebook Group of the Italian populistic movement Movimento 5 Stelle. On the one hand, the three case studies provide three powerful counterexamples to the thesis that online media are purposeful aggregator of people. In fact, this research study finds that online media are the perfect environment for populism to thrive. For online media seem to foster the aggregation of people into groups whose main common denominator is the total refusal of anything that opposes the groups’ views. On this basis, this paper provides evidence that online media may impoverish democratic confrontation. On the other hand, this paper finds that the one of the causes of the rapid rise of populistic movements in Western countries might also be related to the problem of cognitive biases. Indeed, the case studies presented in the paper posit the existence of something that is addressed as the trigger effect, i.e. agents’ tendency to react impulsively to any kind of content that fits agent’s views about current events. Specifically, this research study finds that the activation of the trigger effect might be a direct consequence of the activation of the narrow framing bias and of the anchoring heuristic in presence of fake news.

Spatial characteristic of triggering radius of the damaged probe for the CNC machine tools has been presented. In such case a clear distortion of the probe performance characteristic and a significant increase in probe errors have been observed. Then, to such a damaged probe, a variable speed correction method of systematic errors was used. By setting proper measurement speeds, varying for different measurement directions, errors of the probe can be significantly reduced.

A coordinate measuring machine (CMM) as an automation technology is playing the key role in the modern industry to improve the measurement accuracy. Accurate probing that is computer controlled is the current trend for the next generation of coordinate metrology. However, the CMM probing system is limited by its dynamic root errors that may markedly affect its response characteristics. In this paper, dynamic response errors of CMM measurements have been analyzed. The adopted probe stylus sizes throughout the course of measurements are found to cause some waviness errors during CMM operations due to each of the prescribed angle of the probe tip contact point with the specimen surface and the radius of the stylus tip. Variations in the geometry of the stylus have their consequent effects on its inherent intrinsic dynamic characteristics that in turn would cause relevant systematic root errors in the resulted measurements. Unforeseeable geometrical errors of a CMM using a ductile touch-trigger probing system have been characterized theoretically. These results are analyzed in order to investigate the effect of the dynamic root errors in the light of six probe stylus tip of the situation into account when assessing the accuracy of the CMM measurements. Analytical approaches have been applied on a developed two dimensional model (2DM) of stylus tip to demonstrate the capability of such approaches of emphasizing the root error concept using the strategy of CMM ductile trigger type of probe.

Myofascial pain syndrome is sensory, motor, autonomic disorder caused by the occurrence of trigger points (TrP). In literature the possibilities of deactivating TrPs through exposure to magnetic field (MF) can be noticed. The aim of current research is an attempt at answering the question whether MF changes TrP activity. Tests were carried out on 22 volunteers, who were divided into three groups. First was exposed to static MF using MagneticUnit discs, second was exposed to slow changing MF using Viofor-JPS. The third was exposed to magnetoledotherapy, also using Viofor-JPS.

Introduction. The study presents the case report of a Wells Syndrome in a 18-year -ld female. Wells Syndrome is a rare inflammatory skin disorder which seems to present an abnormal eosinophilic response to a number of triggers. Objective. The aim of the study is to discuss several problems related to the diagnosis and treatment of Wells Syndrome. Materials and method. Medical examination, blood tests, abdominal ultrasound and skin biopsy were performed. Results. Medical examination revealed plaques with tense blisters on feet and erythematous lesions on trunk. Blood tests showed raised anti-streptolysin O level. Skin biopsy revealed features of eosinophilic cellulitis. Oral administration of 0.5 mg per day prednisolone and antihistaminics with local therapy resulted in good clinical response. Conclusions. Sometimes several biopsy are required to establish diagnosis. In some cases lower doses of corticosteroids with antihistaminics and local anti-inflammatory treatment can by sufficient in treatment.

The authors present a concept of the temperature control system for a spray booth equipped with a single burner. The control system uses PWM modulation with dynamic trigger time periods. The paper presents the results of temperature control simulations for different conditions of external air temperature as well as the analysis of external air temperature and trigger time period impact on temperature fluctuation amplitudes inside the spray booth. A dynamic trigger period will minimize the number of burner ignitions with acceptable fluctuation amplitudes of temperature. The minimization of the number of burner ignitions will increase the life time of a burner and decrease the fuel consumption used to start a burner.

This paper presents signal synchronization aspects in a large, distributed, multichannel RPC Muon Trigger system in the CMS experiment. The paper is an introduction to normalized structure analysis methods of such systems. The method introduces a general model of the system, presented in a form of a network of distributed, synchronous, pipeline processes. The model is based on a definition of a synchronous data stream and its formal, fundamental properties. Theoretical considerations are supported by a practical application of synchronous streams and processes management. The following processes were modeled and implemented in hardware: window synchronization, derandomization, data concentration and generation of test pulses. There are presented chosen results of the model application in the CMS experiment.

Introduction. Fibromyalgia syndrome (FMS) and myofascial pain syndrome (MFPS) can be ranked among disease entities being difficult to diagnose clinically, manifesting themselves mainly through pain in specific hypersensitivity points. Aim. To present the current state of medical knowledge about pain spots appearing on hypersensitive points of soft tissue in the context of selected disease entities. Summary of the knowledge. MFPS is defined as sensory, motor and autonomic complaints, caused by the occurrence of trigger points (TrP). Yet the FMS is stated during the anamnesis on the basis of generalized pain, and pressure achiness of at least 11 out of 18 tender points (TP) of precisely determined location. Patients with FMS report numerous additional complaints – apart from the above mentioned ones; these are however highly non-specific and are not confirmed during routine medical check-ups. There are also no laboratory tests that can confirm presence of TrP being characteristic to MFPS and differentiating it from other muscles’ disease entities. Such points are identified only with the use of palpation. Unfortunately while examining a patient this way TrP – being symptoms of MFPS – can be quite easily confused with TP – being symptoms of FMS. Patients with MFPS which is developing in consequence of long-lasting global disorder of muscle tension balance and sensitivity of nociceptors as a result of chronically remaining pain, frequently suffer from achiness fulfilling the criteria of generalized pain. Moreover – in effect of static overload of soft tissues (especially of tonic muscles) – there occur hypersensitive palpable areas (points). Stimulating them cause lively reaction of the patient. Described symptoms can suggest a suspicion of FMS – the more so that making a diagnosis of MFPS does not exclude its coexistence. Having this in mind, there is a pretty large group of authors who raise a supposition that the differential diagnosis between TrP and TP should be observed in the quantitative rather than in the qualitative categories, despite the still binding definition and nomenclature. Recapitulation. Looking at the MFPS and at the FMS from the perspective of evolution of knowledge about them and from the point of view of period when scientific researches were conducted and their results published, it must be stated that during last years a considerable progress has been obtained in scope of better understanding of pathogenesis and pathophysiology of pain in specified points of soft tissue hypersensitivity, and the parallel clinical studies – confirming the hypotheses that were made – clearly increased the diagnostic and therapeutic capabilities of clinical practice.
