Opis:
Małopolska Vistula River Gorge is located in the middle stretch of the Vistula valley between Zawichost and Puławy. Its great portion is supposed to be turned into a Geopark safely approachable by tourists along walking trails leading to planned geosites. While general criteria of geosite selection are oriented on promoting expected interesting attributes of the planned Geopark or, in a special case, on recognizing possible strong geological hazards such as volcanic, earthquake and tsunami phenomena, the inner problems of tourist safety are not commonly recognized. The Vistula Gorge and its close vicinity of Geopark is dedicated to cultural heritage, geological history, landscape and local folklore. The inner hazards of disasters are created here by steep, slippery paths, collapsible loessial canyons, rock-falls from weathered rock-walls, and high waters of the nearby river channel running alongside or flooding tourist footpaths. Recognition and preventing measures against such hazard require determination of the current engineering-geological conditions by testing rock and soil properties along the gorge, and by checking the likelihood of natural disturbance of the terrain safety. The vulnerability of exposed rocks to processes of natural erosion, steady weathering progress, turning steep clayey bluffs into slippery ground, and to an impact of long-term anthropogenic factors had to be recognized, checked, and tested for a final assessment of suitability of the selected place to serving as a Geopark. The special prognostic procedure of engineering-geological hazards was recommended, applicable prior to the Geopark planning stage. The procedure encompasses a thorough assessment of possible safe accessibility to particular geosites in the area designated to become a Geopark. The suitability of the selected area to serving as a Geopark was, thus, analyzed methodically with regard to a detailed engineering-geological and geomechanical evaluation of possible hazard factors within the whole terrain. Proven engineering geological methodologies were employed for the hazard assessment procedure. Each of the factors was subjected individually to close examination, with results recorded into a GIS integrated informative layer, and envisioned upon digital thematic maps. These maps remain to be used for the preparation of regional suitability maps of specific engineering-geological conditions.

The aim of the article is to develop a method for the analysis of tram dynamics related to safety during operation. To achieve this, a mathematical model of the vehicle represented by a multibody simulation MBS system is used. Models of tram with a classic and innovative drive, based on a system of independently rotating wheels on crank axles are analyzed. A new configuration of an innovative drive control of the considered vehicle with the use of braking of independent wheels is proposed. A new geometry of test track is presented. During numerical investigation the values of ‘Y’ leading forces of tram wheels with the considered innovative drive proved to be lower than in the corresponding vehicle with standard wheelsets. It has been demonstrated that the active control systems are of key importance and should be applied in such innovative tram drives.

Maintaining, repairing and installing machines and devices are some of the basic processes being run within the frameworks of broadly comprehended technological process management in manufacturing companies. The purpose of this paper is to identify the level of accident occurrence risk, to determine trends in frequency indicators of total, fatal, major and minor accidents as well as to analyse basic actions being performed at the time of accident and basic reasons for accidents while maintaining, repairing and installing machines and devices.
Konserwacja, naprawa i instalowanie maszyn i urządzeń są jednymi z podstawowych procesów realizowanych w ramach szeroko rozumianego zarządzania technologią w przedsiębiorstwach produkcyjnych. Celem tego artykułu jest zidentyfikowanie poziomu ryzyka wypadkowego, określenie kształtowania się wskaźników częstości wypadków ogółem, śmiertelnych, ciężkich i lekkich a także przeanalizowanie podstawowych czynności wykonywanych w chwili wypadku oraz podstawowych przyczyn wypadków przy wykonywaniu konserwacji, napraw i instalowaniu maszyn i urządzeń.

The maintenance system is a key system that provides continuity and safety of the operation of production systems and that affects the safety of people working in these systems. At the same time, the maintenance system is a set of work processes carried out by people under specific environmental conditions, using specific equipment and within a specific organizational and management structure. The purpose of this article is to identify the main management factors that affect occupational safety and to rank these factors in terms of their effectiveness in ensuring safe maintenance, using the grey systems theory. Based on the literature analysis, 12 key management factors were identified and then subjected to expert assessment. In order to rank the factors, a decision model based on the grey systems theory (GST), i.e. systems with incomplete and uncertain information about structure and behavior, was developed and verified. The use of GST in the area of ensuring safety in maintenance is original. The findings of the article will be very useful for managers in implementing safe maintenance systems in various sectors of the economy.

The technology of production, transportation, and processing of oil and gas involves various hazardous processes. To mitigate the risk that these processes pose, the technological solutions work closely with the automated control and safety systems. The design and organisation of maintenance for the automated safety instrumented systems (SIS) have a significant bearing on the overall safety of operations in this industry. Over the past few decades, many hydrocarbon resources have been discovered in unconventional environments, such as remote, offshore, and arctic locations. Transportation of engineering personnel to these remote locations and back, and thereby, the organisation of the shift work poses additional challenges for the petroleum sector. Under such circumstances, the workforce-related costs play a considerable role in the overall cost of the technological solution and thereby the decisions regarding the workforce organisation should be addressed in the framework of evaluating and choosing the appropriate safety measures. That is why the research presented in this paper aims to address the lifecycle of the technological solution integrating the problems of SIS design, maintenance planning, and employee scheduling into a single decision-making framework to optimise the set of technical and organisational safety measures inherent in the SIS. The performance and maintenance of the SIS are described with a Markov model of device failures, repairs and technological incidents occurrence. The employee scheduling part of the mathematical model utilises the set-covering formulation of maintenance crews taking particular trips. A black-box optimisation algorithm is used to find reasonable solutions to the integrated problem of engineering design and workforce planning. The decisions include the choices of the components and structures for the safety system, the facility overhaul frequencies, the maintenance personnel size, as well as the schedules of trips and shifts for the crews.

Employers are under a legal obligation to provide employees with safe and hygienic conditions of work. These conditions are created by environmental factors that depend on the specifics of the enterprise, production technology and used machinery and equipment. A large number of the risks to which employees are exposed is associated with the use of machinery, equipment and working tools (as confirmed by Statistics Poland). Numerous manufacturing companies increasingly use the practice of involving operators, independently from the maintenance department, in the upkeep and maintenance of machinery and equipment in order to increase efficiency. These activities are undertaken within the scope of Autonomous Maintenance (AM), which is one of the essential elements underpinning the TPM system. The activities performed by operators within AM, such as daily inspections, lubrication or simple repairs, reduce the number of machinery breakdowns. Nonetheless, they could be a potential source of risk for employees. Companies applying the traditional division of tasks entrust the performance of such activities to qualified maintenance staff, so the proper identification of risks that takes into account the specificity of the activities performed by operators and the provision of training in the safe organization of work represent a significant feature of safety improvement. Enterprises may adopt multiple solutions in this regard, including the tools and techniques of the concept of Lean Manufacturing.

Maritime safety involves minimizing error in all aspects of the marine system. Human error has received much importance, being responsible for about 80% of the maritime accident worldwide. Currently, more attention has been focused to reduce human error in marine engine maintenance. On-board marine engine maintenance activities are often complex, where seafarers conduct maintenance activities in various marine environmental (i.e. extreme weather, ship motions, noise, and vibration) and operational (i.e. work overload and stress) conditions. These environmental and operational conditions, in combination with generic human error tendencies, results in innumerable forms of error. There are numerous accidents that happened due to the human error during the maintenance activities of a marine engine. The most severe human error results in accidents due to is a loss of life. Moreover, there are other consequences too such as delaying the productivity of marine operations which results in the financial loss. This study reviews methods that are currently available for identifying, reporting and managing human error in marine engine maintenance. As a basis for this discussion, authors provide an overview of approaches for investigating human error, and a description of marine engine maintenance activities and environmental and operational characteristics.

The individual respondents were drawn from the engineering students and faculty members, and employees of the respondent institution. These are the respondents who used the civil engineering laboratory. It utilized validated researcher-made questionnaire, unstructured interview, and documentary analysis as its data gathering instruments. Opis:
The usage, repair and maintenance of machines is one of the areas in which it still records a significant number of accident events. The occurrence of accidents at work generates social and economic costs for the enterprise. Therefore, employers should be take measures to improve safety at the workplace. The publication presents an accident analysis based on a comparison of the following measures: accident rates (i.e. number of accidents, event effects) and ratio analysis (frequency rate indicator and total accident rate indicator). Calculated indicators allowed to determine the accident rate during the usage, repair and maintenance of machines. The analysis covered a five-year period - i.e. 2013-2017. On the basis of the analyzes carried out, the proposal was presented of possible prevention solutions to reduce the number of accidents at work.

The maintenance system is one of the key systems in the industry, because it ensures the continuity of work and the safety of the production systems. The maintenance system includes a set of specific activities carried out by people in various environmental conditions, with the use of appropriate equipment and within a specific organizational and management structure. Activities carried out by maintenance workers are related to the occurrence of various types of physical, chemical, biological or psychosocial risks. These dangers can lead to accidents or occupational diseases. Therefore, the task of managers within maintenance systems is to provide people working in these systems with an appropriate level of safety through the use of properly selected preventive measures. The methods of safety analysis are related to the examination of various systems in order to identify and assess the risks in these systems and to prepare the safety characteristics of these systems, which allows for the correct adjustment of preventive measures to the identified needs. The purpose of this work is to present the basic methods of Safety Analysis in terms of their possible use in the area of industrial maintenance. As part of the work, the hazards at work in maintenance were characterized, the importance of safety analysis for the identification of hazards and accident prevention was discussed, and the key methods of safety analysis were presented in terms of their possible use to improve work safety in an industrial maintenance system. The presented considerations are original. The findings of the article will be very useful for management in implementing safe maintenance systems in industry.

The aim in adoption of maritime equipments’ maintenance strategy is the reduction of risk in order to achieve safer navigation, to enhance environmental friendly behaviors, to increase the safety and positively contribute to the overall operability management of the ship. The use of weak maintenance strategy could result in waste of time, money and resources, and often has no effect on improving or maintaining the availability of equipments. In this article, we propose a model in evaluation of on non-deterministic factors that affect the ships’ safety and a framework for ships management governance. The review of maintenance strategies has proven that a decision support mechanism needs mainly multiple criteria decision on ship equipments maintenance strategy selection. Through Analytic Hierarchy Process methodology and the extended evaluation of results on adoption of Technique for Order Preference by Similarity to Ideal Solution a useful tool to prioritize needs at any level and includes the decisions that should be taken into consideration from the shore office managers and the ship personnel for the safe functionality of the ship is provided.

The issue of concrete airfield pavements maintenance is the process, the efficient performance of which may provide suitable life and air traffic safety related to thereof. Obtaining durable airfield pavement is a result of team cooperation in the following fields: suitable design solutions, applicable work technology and principles of arrangement thereof. However, fundamental link of this process is the high level of engineering culture related to construction maintenance during its whole "lifespan". Each pavement must comply with requirements according to principles of "construction limit states" method expressing work safety, i.e. limit state of deformation and serviceability. Thereby, the "weakest link" within the chain of projects and actions implemented in the course of its construction process decides about safety and lifespan of pavement operation. Durability of such engineering constructions as airfield pavements, is specified by the quantity of performed "flight operations" on these pavements, within the assumed operation period. Considering, among others, principles of limit state method, the required and anticipated pavement operation period can be specified. Life aspect with reference to airfield pavements can be analysed in theory by means of application of modern material solutions. The above-mentioned aspects will be described widely in this work. Modern material solutions include modified composition of concrete intended for construction of particularly difficult sections of such pavements and structural tests were conducted for such composition, which would confirm suitability of assumed material solutions.
