Opis:
The present article aims to present a transdiagnostic approach to eating disorders (anorexia nervosa, bulimia nervosa, and eating disorders not otherwise specified) using the example of two relatively new types of therapy: Cognitive Behavior Therapy (CBT), based on Fairburn’s model, and Acceptance and Commitment Therapy (ACT). Theoretical frameworks, psychopathology, proposed change mechanisms, as well as objectives and selected therapeutic techniques are discussed for both approaches. Directions for further research comparing the efficacy of the two approaches, the factors that moderate their outcomes, and the possibilities of integrating the two models are also suggested.

European cities face urban, demographic and climate challenges. According to forecasts, annual extreme phenomena will intensify - including torrential rains. Comprehensive solutions (also those based on nature), climate adaptation strategies, runoff management, incorporation of new design (e.g. sponge cities) are urgently required in order to strengthen urban resilience and to minimise the effects of extreme weather events (droughts, floods or heat islands). The aim of the research was to develop a methodology for activating selected elements of blue-green infrastructure within areas of natural and cultural protection as an adaptive tool of urban planning. Modelling of infiltration possibilities, programmed with SCALGO Live Poland software, was performed as a case study based on a research city - Sandomierz (in Poland). Selected parameters (stormwater surface runoff, chosen runoff areas, land cover) are strongly correlated with urban indicators relating to the vegetation coverage (biologically active area - BAA). Results pointed out urban units, which BAA is lower than 25% (e.g. Old Town Square, courtyards of tenement houses). Modelling was carried out for these units by concentrating on the undeveloped area for which the BAA was increased. The enhancement assumed values in the range of 41-45%. In analysed cases, an improvement (decrease) in runoff volume was obtained, even by 8.69%. Simultaneously, infiltration increased by 19.61%, calculated over entire runoff area. Implementation of solutions based on these results, in the form of appropriate planning provisions, can raise the quality of environment (e.g. improving water infiltration) and life (e.g. more effective air cooling on hot nights).

Empirical evidence suggests that mindfulness, psychological flexibility, and addiction are interrelated in decision making. In our study, we investigated the relationship of the behavioral profile, composed of mindfulness and psychological flexibility, and smoking status on delay and probability discounting. We demonstrated the interaction of the behavioral profile of mindfulness and psychological flexibility (lower or higher) and smoking status on delay discounting. We found that individuals who smoked and displayed higher mindfulness and psychological flexibility devalued rewards at a slower rate, compared to smokers with a lower profile. Importantly, in those with a higher profile, smokers discounted rewards no differently than nonsmokers. Smokers with a lower profile did display, however, increased impulsivity, compared to nonsmokers. These results suggest that behavioral interventions aiming to modify the behavioral profile with regard to mindfulness and psychological flexibility can indeed support the regulation of elevated impulsivity in smokers to equate with that of nonsmokers. In probability discounting, we observed that individuals with a higher profile displayed lower discounting rates, i.e., were less risk-averse, with no other significant main effect or interaction.

Introduction. Oxidative stress induced by the generation of excess reactive oxygen species is one of the cause of atherosclerosis finally leading to coronary heart disease (CHD). Glutathione reductase (GR), a flavoprotein antioxidant enzyme, regenerates glutathione (GSH) from its oxidized form, which is essential for scavenging of hydrogen peroxide by glutathione peroxidase. The aim of this study was to analyze the activity of GR and GSH level in serum of patients with various stages of CHD. Materials and method. Sixty consecutive patients with diagnosis of stable angina (SA, n=20), unstable angina (UA, n=20) and non-fatal myocardial infarction (MI, n=20) together with 20 healthy control subjects were enrolled. Venous blood samples were collected just before the coronary catheterization procedure (in study patients). GR activities and GSH concentration was determined using commercially available colorimetric kits. Results. The highest GR activity and GSH serum level was noticed in UA patients. However, statistical evaluation showed that only glutathione reductase activity was significantly higher in serum obtained from myocardial infarction patients, compared to stable angina pectoris patients (19.52 ± 13.88 vs 11.63 ± 5.45 nmol/min/ml for MI and SA, respectively; p<0.048). There were no significant differences in GSH between the CHD patients and controls. Conclusion. The elevated activity of glutathione reductase in serum of patients with unstable angina pectoris and myocardial infarction suggests the role of antioxidant system acute coronary syndromes.

We examined the relationships between the compositional changes in demographic and socioeconomic factors and the changes in the prevalence of material deprivation in rural and urban areas in Poland. Using the European Union Statistics on Income and Living Conditions (EU-SILC) data for 2019–2020, we applied the Fairlie decomposition approach for a logit model. We found that the important characteristics affecting a gap in material deprivation between rural and urban areas are: equivalized income, the level of education, the type of household, and the presence of disabled or unemployed people in the household. A non-significant effect of the pandemic on the material deprivation gap between rural and urban areas was observed.

Opis:
The aims of the study were to assess whether commonly used anthropometric indices are associated with body fat measures obtained by Bioel ectrical Impedance Analysis (BIA) method, and to determine the best anthropometric predictor of BIA - based body fat percentage (%Fat) and visceral fat rating (VFR) in elderly women. The sample consisted of 24 women aged 80 years and older, the residents of long - term care facilities in Upper Silesia (Poland). All women were subjected to standard anthropometric measurements including the following variables: body weight, body height, waist circumference, hip circumference and neck circumference. On the basis o f these measures Body Mass Index (BMI) as well as waist - to - hip ratio (WHR) were calculated. The subjects’ %Fat and VFR were determined by body composition analyzer TANITA BC 420MA (Japan). Pearson’s correlation coefficients were used to quantify the relati onships between variables. Stepwise multiple regression analysis with backward elimination was performed to identify possible predictors. The basic characteristics of the investigated subjects were as follows (mean±SD): age - 85.5±3.7 years; body weight - 60.4±11.6 kg; body height - 150.6±7.6 cm; BMI - 26.6±4.6 kg/m 2 ; %Fat - 31.3±9.6%; VFR - 10.7±2.5. Both of BIA - based measures significantly, positively correlated with body weight, BMI and circumferences of waist, hip and neck (r values from 0.477 to 0.835) . The multiple regression analysis for %Fat revealed that the body weight was the only variable statistically significant (r 2 =0.414; p<0.001; SEE =7.503%), and for VFR the significant β coefficients were obtained for BMI and neck circumference ( 0.625±0.133 and 0.341±0.133, respectively) ( r 2 =0.754; p<0.001; SEE =1.313). Among popular anthropometric indices of body composition in the oldest old group o f women, body weight seems to be the best predictor of body fat percentage, and VFR could be predicted by BMI along with a neck circumference.

From an economic point of view, protected areas do not bring in high profits, in comparison to other possible ways of land usage. National parks and nature reserves are costly in their creation, as well as in their subsequent upkeep and management. In developing countries, their operation can be the source of many political and economic problems. Usually, local communities neighboring protected areas perceive the existence of a protected zone negatively, although they are often able to avail themselves of park resources in various ways. Besides being given the opportunity to rationally make use of certain park resources, community members may be employed as park attendants, which is a practice that is widespread. A part of the profits obtained from the developing tourist industry may be allocated towards local development programs, e.g. the building of a new hospital or school. Some national parks, while lacking the adequate infrastructure necessary for tourism, grapple with a lack of funds. The lack of earning opportunities in connection with the development of tourism results in the local community's over-exploitation of park resources. This often manifests itself in an increased incidence of poaching in the protected area.

Opis:
Liver transplantation is a well-established treatment of patients with end-stage liver disease and selected liver tumors. Remarkable progress has been made over the last years concerning nearly all of its aspects. The aim of this study was to evaluate the evolution of long-term outcomes after liver transplantations performed in the Department of General, Transplant and Liver Surgery (Medical University of Warsaw). Material and methods. Data of 1500 liver transplantations performed between 1989 and 2014 were retrospectively analyzed. Transplantations were divided into 3 groups: group 1 including first 500 operations, group 2 including subsequent 500, and group 3 comprising the most recent 500. Five year overall and graft survival were set as outcome measures. Results. Increased number of transplantations performed at the site was associated with increased age of the recipients (p<0.001) and donors (p<0.001), increased rate of male recipients (p<0.001), and increased rate of piggyback operations (p<0.001), and decreased MELD (p<0.001), as well as decreased blood (p=0.006) and plasma (p<0.001) transfusions. Overall survival was 71.6% at 5 years in group 1, 74.5% at 5 years in group 2, and 85% at 2.9 years in group 3 (p=0.008). Improvement of overall survival was particularly observed for primary transplantations (p=0.004). Increased graft survival rates did not reach the level of significance (p=0.136). Conclusions. Long-term outcomes after liver transplantations performed in the Department of General, Transplant and Liver Surgery are comparable to those achieved in the largest transplant centers worldwide and are continuously improving despite increasing recipient age and wider utilization of organs procured from older donors.

The aim of the study was to analyze indications and results of the first one thousand liver transplantations at Chair and Clinic of General, Transplantation and Liver Surgery, Medical University of Warsaw.Material and methods. Data from 1000 transplantations (944 patients) performed at Chair and Clinic of General, Transplantation and Liver Surgery between 1994 and 2011 were analyzed retrospectively. These included 943 first transplantations and 55 retransplantations and 2 re-retransplantations. Frequency of particular indications for first transplantation and retransplantations was established. Perioperative mortality was defined as death within 30 days after the transplantation. Kaplan-Meier survival analysis was used to estimate 5-year patient and graft survival.Results. The most common indications for first transplantation included: liver failure caused by hepatitis C infection (27.8%) and hepatitis B infection (18%) and alcoholic liver disease (17.7%). Early (< 6 months) and late (> 6 months) retransplantations were dominated by hepatic artery thrombosis (54.3%) and recurrence of the underlying disease (45%). Perioperative mortality rate was 8.9% for first transplantations and 34.5% for retransplantations. Five-year patient and graft survival rate was 74.3% and 71%, respectively, after first transplantations and 54.7% and 52.9%, respectively, after retransplantations.Conclusions. Development of liver transplantation program provided more than 1000 transplantations and excellent long-term results. Liver failure caused by hepatitis C and B infections remains the most common cause of liver transplantation and structure of other indications is consistent with European data.
