    The aim of the present study was the evaluation of habitual physical activity in adult members of health promoting associations in compliance with selected health recommendations. Physical activity was monitored for 7 consecutive days using accelerometer ActiGraph GT3X+. It was observed that the percentages of individuals with sufficient physical activity differ depending on healthoriented recommendation used in the evaluation. The results indicated that despite appropriate weekly volume of physical activity expressed in energy expenditure (on the average, twice as high as the recommended minimum) and the number of steps taken daily, the prevailing majority of participants (60%) demonstrated an inadequate level of it when assessed in the context of recommendation by World Health Organization.

    This article discusses an extremely signifi cant issue of using ultrasonography in the delivery room. The authors discuss the role of ultrasonography in assessing the course of labor. Although the fi rst reports on this subject appeared approximately 20 years ago, the mass use of such examinations has started only recently(2–4). In some of these reports, the 2D and 3D imaging is compared(3) whereas some of them compare a physical anorectal examination with ultrasound assessment of labor(5,6). Nearly all authors agree that ultrasonography in the delivery room is useful and, currently, the more information is available to the obstetrician, the safer the delivery is both for the mother and child. Each article on the use of ultrasonography in the delivery room is therefore valuable. The authors of this paper preset a concise historical review as well as indications for and practical aspects of US examinations during labor, referring to 30 recent publications. One should hope that this article will resolve any doubts of obstetricians and gynecologists concerning the utility of ultrasonography during labor.
    Niniejszy artykuł(1) porusza niezmiernie istotną kwestię wykorzystania badania ultrasonograficznego na sali porodowej. Autorzy omawiają znaczenie badania USG do oceny postępu porodu. Chociaż pierwsze doniesienia na ten temat ukazały się około 20 lat temu, to dopiero w ostatnich kilku latach badania te zaczęły być wykonywane masowo(2–4). Część publikacji na ten temat dotyczy oceny porównawczej opcji 2D i 3D(3); niektóre porównują przedmiotowe badanie proktologiczne z oceną ultrasonograficzną przebiegu porodu(5,6). Prawie wszyscy badacze są zgodni co do celowości wykonywania badania ultrasonograficznego na sali porodowej i obecnie nie ulega już wątpliwości, że im większą liczbą informacji dotyczących przebiegu porodu dysponuje lekarz położnik, tym poród jest bezpieczniejszy dla pacjentki i noworodka. Dlatego też każdy artykuł na temat zastosowania ultrasonografii na sali porodowej jest cenny. Autorzy w zwięzły sposób omówili zarys historyczny, wskazania oraz praktyczny aspekt wykonywania ultrasonografii w trakcie porodu, powołując się na 30 pozycji piśmiennictwa z ostatnich lat. Należy mieć nadzieję, że po przeczytaniu tej pracy większość ginekologów położników nie będzie już miała wątpliwości co do przydatności ultrasonografii na sali porodowej.

    Background: Incidence of morbid obesity is rising worldwide. Current clinical practice guidelines for the pre-transplant evaluation of end-stage kidney disease (ESKD) patients lack clear recommendations on morbid obesity. Material and methods: The aim of this review was to summarize the current guidelines on the role and treatment of obesity in kidney transplant recipients. Eight current national and international clinical practice guidelines were identified in a comprehensive literature search. Results: All guidelines underline early detection of obesity and obesity-related comorbidities in ESKD patients. Only two guidelines explored the role of weight-loss surgery, however due to the lack of sufficient evidence no formal recommendation of surgical procedure was given. Conclusions: Diagnosis and treatment of obesity remains underappreciated in the current guidelines, most of which do not include pharmacological and surgical interventions. High-quality evidence is warranted to assess the role of weight-loss including surgery in ESKD patients and to update the recommendations in future guidelines.

    Chronically hemodialyzed (HD) patients are at a high risk of developing very severe forms of COVID-19 disease. In this article we describe three HD patients (all males, aged 70, 70 and 74 years) vaccinated intramuscularly with a two-dose mRNA BNT162b2 vaccine; BionTech/Pfizer Comirnaty, in whom subsequent breakthrough SARS-CoV-2 infections developed. All patients achieved post-vaccine seroconversion for anti-spike antibodies with IgG titers of 445, 227 and 92.5 AU/mL (cut-off, 13 AU/mL) case 1, 2 and 3 respectively. SARS-CoV-2 infection was diagnosed 44, 28 and 48 days after the second dose of BNT162b2 and confirmed with the polymerase-chain-reaction (PCR) test. Two asymptomatic patients underwent this test because of their direct contact with a person with confirmed COVID-19. The third patient reported only a non-significant drop in oxygen saturation, and was hospitalized (case 3). All these patients were characterized by a low post-vaccination neutralizing antibody titer and a high production of these antibodies after falling ill (795, 845 and 5770). Perhaps this production of antibodies is responsible for the mild course of the disease, and the likely reduction of mortality. These breakthrough cases in no way undermine the importance of the vaccinations, and on the contrary argue for their urgency.
