Ulcerative colitis (UC) is a inflammatory disease of large bowel. The amount of people suffering from UC increases from year to year. Pathogenesis of this affection is still not entirely clear. Mechanisms of proliferation and apoptosis in colonocytes in the course of the disease are defectedThe aim of the study was to assess the rate of proliferation and intensity of apoptosis in colonocytes in patients with diagnose UC.Material and methods. Colon pathological samples taken from patients with diagnosed ulceraive colitis were examined. Patients were in both clinical and endoscopic remission and were treated with mesalazin. They were patient of Department of General and Colorectal Surgery. To estimate proliferation index dye with monoclonal antibody against Ki67. To determine apoptosis level immunohistochemistry with antybody against Bax was used.Results. Average Ki-67 in the test group was 42,13%, the largest value amounted to 57% and the lowest of 33%. Average value of Bax was 1.47 and ranged between 0-3. High index of bax appear not only in the bottom of the crypt, but also at their outlet.Conclusions. In ulecerative colitis genetic and immunological disturbances occur despite treatment. Mesalazine acting only on certain routes associated with the UC holds the remission, without, however "the molecular remission". Thus, it appears that the results of our research are another proof of the necessary caution in weaning support treatment.

Opis:
Microscopic colitis (MC) is a common cause of unexplainable, chronic diarrhea. The disease is characterized by the presence of clinical symptoms, a normal colonoscopy and typical histopathological changes upon microscopic examination. The aim of the study was to present a case of an atypical histological form of MC. A 56 year-old man presented with chronic, watery diarrhea. Gastrointestinal infection had been excluded. The colon appeared almost normal on the colonoscopy. Inconsistent histological findings were observed. The pathology of randomly taken colon biopsies showed collagenous colitis (CC; thickness of collagen bands > 10 μm, < 20 IELs). Six months later during a follow-up colonoscopy, colonic specimens revealed typical findings of lymphocytic colitis (LC), namely, no thickened subepithelial collagen bands were identified. The authors analyzed the risk factors, diagnosis, treatment response, clinical course and the atypical histological outcomes.
Mikroskopowe zapalenie jelita grubego to częsta przyczyna przewlekłej biegunki niewiadomego pochodzenia. Choroba charakteryzuje się obecnością objawów klinicznych, makroskopowo prawidłowym wyglądem błony śluzowej jelita grubego w kolonoskopii i obecnością typowych zmian w badaniu histopatologicznym. Celem pracy jest przedstawienie nietypowego obrazu histologicznego mikroskopowego zapalenia jelita grubego. Autorzy prezentują przypadek 56-letniego mężczyzny z uporczywą, wodnistą biegunką, z pochopnie rozpoznaną niewydolnością zewnątrzwydzielniczą trzustki. Po wykluczeniu infekcyjnego tła biegunki wykonano kolonoskopię, ukazując makroskopowo niemal prawidłowy obraz jelita grubego. Otrzymano niespójne wyniki badań histopatologicznych. W badaniu mikroskopowym wycinków z okrężnicy potwierdzono rozpoznanie kolagenowego mikroskopowego zapalenia jelita grubego (w preparacie grubość kolagenu podnabłonkowego wynosiła > 10 μm, liczba limfocytów śródnabłonkowych < 20 w przeliczeniu na 100 komórek nabłonka). Tymczasem podczas kontrolnej kolonoskopii w wycinkach z jelita grubego ujawniono cechy zapalenia limfocytowego, w badanych próbkach nie wykazano obecności złogów podnabłonkowego kolagenu. Autorzy analizują czynniki ryzyka zachorowania, odpowiedź na zastosowane leczenie, przebieg kliniczny choroby oraz rozbieżne wyniki badań histopatologicznych.

Introduction. Infliximab is a monoclonal antibody that acts against tumor necrosis factor TNF-α. The drug is used in the treatment of autoimmune diseases. Aim. This article reviewed the efficacy and safety of infliximab for the treatment in severe ulcerative colitis. This review included studies that evaluated the clinical use of infliximab. Material and methods. This meta-analysis was performed according to systematic literature search of three major bibliographic databases (Scopus, PubMed, and Cochran). Results. Infliximab has been approved by the US Food and Drug Administration (FDA) as a medicine to treat Leśniowski and Crohn’s disease, ulcerative colitis, psoriasis, psoriatic arthritis, ankylosing spondylitis, and rheumatoid arthritis. However, further trials are required to compare other parameters of efficacy such as the clinical response with infliximab. Conclusion. In patients suffering from Crohn’s disease or ulcerative colitis under infliximab maintenance therapy, sustained good trough levels are associated with: better response and remission rates, more mucosal healing and less loss of response.

Ulcerative colitis (UC) is a kind of inflammatory bowel disease involving rectal mucosa, or the rectum with colon, resulting in ulcerations in some cases. It is incurable and varies with periods of exacerbation and remission. The disease affects mainly population of highly-developed European or North American nations. It can relatively rarely be observed in South America, Asia and Africa. It is estimated that morbidity and incidence in those regions is about 10 times lower than in Europe and North America [1,2]. The incidence of UC in Europe is approximately 10 new cases per 100,000 people annually [3]. In Poland, epidemiology assessment is difficult due to a lack of reliable statistics. In Lower Silesia, 840 hospitalizations for UC on average are registered annually [4,5]. In UC pathogenesis, genetic, environmental and immunologic factors play an important role. UC can occur at any age, however, the peak incidence is observed between ages 20 and 40 and over 65 [6]. UC is incurable and accompanies the patient through the rest of his/her life. It varies with periods of exacerbation and remission, which impair everyday activities and lower patient’s quality of life. As a consequence, the patient generally feels bad, which is not only caused by gastrointestinal symptoms but also by emotional and psychological issues [7]. Nowadays, a clear tendency can be observed in medicine to include health and disease-related issues in biopsychosocial context, including social and psychological factors affecting the course of the disease. The assessment of such factors brings up a broader perspective on patient’s health problems [8]. Adjustment to the disease is important in many chronic diseases, which demand a change of the patient’s attitude and taking aspects such as acceptance of one’s disease into consideration [9]. The term ‘acceptance’ denotes adopting an opinion, judgment, belief, behavior, favorable attitude or to give a consent [10]. Accepting one’s disease means adjusting to limitations posed by it and accepting the associated disability. The consequences include accepting the limitations, not being self-sufficient, dependence on others and low self-esteem [11]. In a chronic and difficult therapeutic process, medical staff play the key role, in addition to other patient’s support. Bonding with other patients is an important regulating factor in the emotional sphere, while psychological counseling can turn out beneficial in accepting one’s new difficult life situation. The aim of the study was to evaluate the acceptance of the disease in patients with ulcerative colitis with respect to selected socio-demographic and clinical variables.

Introduction. Ulcerative colitis is a chronic, inflammatory disease of the mucous membrane of the large intestine manifesting itself through diarrhoea with blood, mucous and pus. It progresses with periods of relapses and remissions. The treatment is a long-term process which should aim at improving the patient’s clinical condition and quality of life, as well as minimising the disease-related anxiety and fears. Objective. The aim of the study was recognition of the determinants of the level of anxiety and fears in a group of patients with ulcerative colitis. Materials and method. The prospective study comprised 102 patients with diagnosed ulcerative colitis. The data were collected by means of the following tools: Clinical Disease Activity Index, Rating Form of IBD Patients Concerns (RFIPC), and a questionnaire by the authors. Statistical parametric and non-parametric tests were used to analyse the data, depending on the type of scale and nature of the variable distribution. Results. Most of the patients (64%) were in the active phase of the disease. A high level of fear and anxiety occurred in 73% of the patients and concerned the impact of the disease, intimate life, complications and stigmatisation. The highest levels of fears and anxiety were observed in the field of complications and the impact of the disease on the patients’ lives. Conclusions. The disease activity and high levels of anxiety and fears influenced the psychosocial functioning of the patients with ulcerative colitis.

Opis:
An ST-segment elevation myocardial infarction represents a time-sensitive cardiac pathology with utmost importance placed upon timely coronary angiography with percutaneous coronary intervention. While emphasis is placed on atherosclerotic or thrombotic coronary occlusion, it is important to recognize other etiologies which may present in a similar fashion. This case demonstrates a 71-year-old female patient with prior coronary artery disease and stenting who presented with acute abdominal pain and elevated cardiac biomarkers as well as ST-segment elevation on initial EKG. Coronary angiography revealed only mild to moderate coronary lesions and patent stents while echocardiography was essential unchanged from prior evaluation. Computed tomography of the abdomen would show findings suggestive of infectious colitis and empiric antibiotics led to full resolution of symptoms. While no definitive cause for her cardiac manifestations was discovered, the authors propose coronary vasospasm or myo-pericarditis as likely etiologies in response to an overwhelming inflammatory state. The case underscores the importance of formulating a comprehensive differential diagnosis during the initial workup of a ST-segment elevation myocardial infarction.

Introduction. Inflammatory bowel disease (IBD) is group of global range inflammatory conditions. There has been a regular increase in the number of IBD cases. Patients exclude whole food groups from their diet fearing the emergence of disease symptoms or due to learning from unreliable sources. One of the elements of treatment considering inflammatory bowel diseases is nutritional therapy. The duration of the above-mentioned depends on the prevalence of such symptoms as fever, bowel move-ments, length of the functioning gastrointestinal tract, stoma and intestinal fistula presence. Nutritional therapy is an essential element of successful treatment alongside pharmacological, surgical, and biological therapy, as well as other methods. Crohn's disease and ulcerative colitis considered as chronic diseases, lead towards physical and biopsychosocial disability, being responsible for the reduction in the quality of life. The aim of the study was to determine the quality of life after surgical procedures in case of patients diagnosed with Crohn's disease and ulcerative colitis, subjected to natural and parenteral nutrition. Material and methods. The study group comprised 52 patients from the Department of Gastroen-terology, Military Medical Institute, and Department of Surgery and Clinical Nutrition, Clinical Hospital in Warsaw. The study was performed between October, 2011 and April, 2012. The World Health Organization Quality of Life Instrument - Bref (WHOQOL-BREF) questionnaire was used to deter-mine the patients’ quality of life. conclusions. A lower quality of life was observed in case of patients subjected to parenteral nutrition, poor education, disease symptoms exacerbation, in the majority-rural inhabitants. The quality of life does not depend on gender, type of disease, family status, and additional medical care.

We present a report on ultrasound findings in extragenital endometriosis and a literature review accompanied by illustrations. Intestinal endometriosis should be considered in female patients of reproductive age who present with constipation, gastrointestinal bleeding, nausea, vomiting, cramp-like abdominal pain, diarrhoea and pelvic pain. Although definitive preoperative diagnosis of endometriosis is difficult, clinical suspicion and appropriate imaging might prevent extensive surgical procedures with higher morbidity. Contrast-enhanced ultrasound is an efficient non-invasive imaging method without any radiation exposure that supports the early diagnosis of intestinal endometriosis and may help assess the vascularization of endometriotic lesions within the distinct layers of the intestinal wall.
Autorzy przedstawiają sprawozdanie dotyczące badań ultrasonograficznych przeprowadzonych u pacjentek z endometriozą zlokalizowaną poza narządami rozrodczymi wraz z przeglądem piśmiennictwa poparte przykładami klinicznymi. U pacjentek w wieku rozrodczym, u których występują zaparcia, krwawienie z przewodu pokarmowego, nudności, wymioty, bolesne skurcze brzucha, biegunka i ból w obrębie miednicy mniejszej należy rozważyć endometriozę jelitową. Choć postawienie ostacznego rozpoznania przedoperacyjnego jest trudne, wysunięcie klinicznego podejrzenia oraz odpowiednie badania obrazowe mogą pozwolić na uniknięcie rozległego zabiegu chirurgicznego obarczonego wyższą częstością powikłań. Badanie ultrasonograficzne z użyciem środka kontrastującego to skuteczna i nieinwazyjna metoda obrazowania, która nie wiąże się z narażeniem pacjentki na promieniowanie oraz która może zasugerować wczesne rozpoznanie endometriozy jelitowej i pomóc w ocenie unaczynienia zmian endometrialnych w obrębie poszczególnych warstw ściany jelita.

Purpose: Recent literature data indicate a key role of apoptosis in the pathogenesis of inflammatory bowel disease. The aim of the study was to evaluate the expression of Bax, Bid, Bcl-2 and Bcl-xl in non-dysplastic and dysplastic epithelium in inflamated mucosa of patients with ulcerative colitis. Methods: The study consists of 18 patients with diagnosed ulcerative colitis. The expression of proteins was determined immunohistochemically. Results: Lack of Bax expression in normal epithelium of the inflamed intestinal mucosa (94.4%) and a weak expression of this protein were found in dysplastic glandular cells (67%). The Bax expression of dysplastic epithelium correlates with reduced severity of chronic inflammation (p<0.005). Bid expression in non-dysplastic glands was found in 67% of cases vs. 16% in dysplastic epithelium that was associated with the occurrence of epithelial erosions or ulcers (p<0.05). Moderate cytoplasmic expression of Bcl-xl was noted in 27.7% of patients in normal epithelium and in 66.1% within dysplastic lesions. Bcl-xl expression in dysplastic glandular cells correlated with the presence of neutrophils in the lamina propria (p <0.05). Conclusions: The immunohistochemical expressions of Bax, Bcl-2 and Bcl-xl increase and Bid protein expression decreases in dysplastic glandular tubes as compared to non-dysplastic intestinal epithelium in inflamed mucosa, which may suggest an imbalance of controlled cell death in ulcerative colitis.

Introduction: Ulcerative colitis is one of the most interesting nosological entities of gastroenterology and internal medicine. This chronic autoimmune disorder of the digestive system, along with Crohn's disease, is a type of idiopathic inflammatory bowel disease. It mainly affects people during their peak reproductive age, i.e., 15 to 30 years old. It is characterized by diffuse mucosal inflammation of the large intestine, which is limited to the colon and shows alternating periods of exacerbations and remissions. Purpose: To review the world literature regarding the impact of the disease on fertility, the course of pregnancy, and consecutively, the outcome of pregnancy.Materials and methods: An extensive review of the recent national and international literature in electronic databases (Pubmed, Google Scholar) and in scientific journals was accomplished through the use of appropriate keywords. Results: The majority of women will have a chance of conceiving. For about 25% of them, conception will be achieved in the course of the disease and it will progress normally, resulting in a healthy fetal outcome. Infertility only occurs among men while they are taking medicines, whereas among women it occurs after surgery for ileoanal anastomosis. During periods of flare-ups, there is a possibility of spontaneous abortion, premature labor and birth of an infant with low birth weight. The majority of pharmaceutical formulations can be used safely both before and after pregnancy without causing any birth defects.

Awareness of smoking in adolescents with inflammatory bowel disease

Introduction and objectives. Inflammatory bowel disease (IBD) is long life disease that results from an interaction between a polygenetic predisposition and environmental factors, including smoking. The aim of this study was to evaluate beliefs about and awareness of smoking among adolescents with IBD compared to healthy controls. Materials and method. Patients with IBD, Crohn’s disease (CD) and ulcerative colitis (UC), and healthy controls were asked to complete a questionnaire on demographic data and smoking status. The questionnaire also included data on beliefs and awareness of smoking-related health effects, including effects on IBD. Results. A total of 139 IBD patients and 108 controls were enrolled in the study. Of the IBD patients, 17/139 (12.2%) were smokers compared to 18/108 (16.7%) of controls (p=0.3). Patients with IBD were nine times more likely to be everyday smokers than occasional smokers (OR=9.2, 95% CI: 1.9- 45.1, p=0.004). No difference was found between patients with CD and UC in their answers to the question of whether “smoking increases the risk for surgery in your type of IBD” (17/28 (60.7%) vs. 10/29 (34.5%), respectively (p=0.047). More patients with CD than UC were aware of the risks of smoking on their disease: extra-intestinal manifestations and disease exacerbation, OR=11.3 (95% CI: 4.1 – 30.9; p=0.000) and OR=19.3 (95% CI: 6.7 – 55.1; p=0.000), respectively. Conclusions. The data obtained demonstrated that adolescents with CD are much more aware of the role of smoking on CD than are their peers with UC; however, this awareness is still unsatisfactory. Therefore, there is an unmet need to implement better anti-smoking strategies for this group of patients.

Opis:
The aim of presented study was to investigate the influence of Lactobacillus plantarum LS/07 and inulin on the activity of β-glucuronidase enzyme, and counts of coliform and lactobacilli in fresh caecal digesta, cytokine levels (IL-6, IL-8), and trancription nuclear factor kappa beta (NFκB) activities in colon tissue and blood samples of rats with dextran sulphate sodium (DSS) induced acute colitis. The rats were randomly divided into four groups - CG, AC, AC+PRE and AC+PRO. Colitis was induced using of 5% DSS in drinking water for 7d. DSS application increased activity of β-glucuronidase (P < 0.001), increased counts of coliforms, and decreased lactobacilli counts (P < 0.05) in comparison to control group. Serum and tissue levels of IL-6 and IL-8 as well as tissue NFκB activities showed increased expression in acute colitis group. Inulin diet modified counts of microorganims and decreased β-glucuronidase activity, suppressed NFκB activities (P < 0.001) and down regulate synthesis of IL-6 (P < 0.01) in serum and colon tissue and tissue IL-8 (P < 0.05). Lactobacillus plantarum LS/07 decreased β-glucuronidase activity (P < 0.05), levels of IL-6 and IL-8 (P < 0.001). These results were consistent with the addition of histological findings. Our results indicate that dietary intake of Lactobacillus plantarum LS/07 and inulin suppressed expression observed markers, which play an important role in the inflammatory process, which predisposes their use in prevention or treatment of acute colitis.
