Introduction and aim. Chronic hepatitis B infection (CHB) affects millions of people around the world. Many clinicians find it challenging to choose therapeutic agents due to the mutations that occur in the hepatitis B virus (HBV) that cause drug resistance. Thus, the aim of this study was to determine the HBV resistance rates against the currently recommended first-line therapies in the region of our country where HBV prevalence is high. Material and methods. A total of 96 patients (56 men and 40 women) with HBV infection were enrolled in the study. The serum samples collected from those were analyzed with real-time polymerase chain reaction analysis followed by pyrosequencing (PyroStar HBV Drug Resistance Test, Altona Diagnostics, Germany) for drug resistance mutations associated with lamivudine, adefovir, telbivudine, entecavir, and tenofovir. Results. HBV drug-resistance mutations were investigated in 80 treatment-naïve and 16 treatment-experienced patients (6 entecavir, 4 PEGylated-interferon, 4 tenofovir, 2 lamivudine). None of the HBV-DNA samples had mutations cause to drug resistance were detected in any codons regions that were analyzed. Conclusion. Antiviral resistance poses serious obstacles for clinicians in the treatment of CHB. Determining whether antiviral resistance exists in HBV is critical to choose the appropriate treatment agent.

Background: There are few data on the efficacy and safety of pegylated interferon treatment in adolescents with chronic hepatitis B. Aim: We conducted a pilot study in 13 adolescents with chronic hepatitis B treated with peginterferon alfa-2a at 100 µg/m2 once weekly for 48 weeks. Methods: HBV DNA was assessed by qPCR method. Results: After four weeks of treatment six adolescents had undetectable HBV DNA (<12 IU/mL). Seven adolescents - including five HBV negatives at week 4 - had undetectable HBV DNA (<55 IU/mL) at week 24, and seven adolescents - including all HBV DNA negatives at week 4 - had undetectable HBV DNA at week 48 of treatment (<55 IU/mL). Five adolescents had undetectable HBV DNA (<55 IU/mL) after 24 weeks of follow-up (sustained viral response). HBeAg seroconversion was achieved in one patient. HBsAg loss was documented at the end of therapy in two of the six adolescents HBV DNA negative at week 4 of treatment. Three adolescents withdrew from the treatment (two because of adverse events, one because of withdrawal of parental consent). Leukopenia was reported in seven adolescents and three individuals experienced thrombocytopenia. Except for one patient who discontinued treatment due to leukopenia, no dose modifications for adverse events or laboratory abnormalities were required. Conclusion: This pilot study shows that 48 weeks of treatment with peginterferon alfa-2a can result in sustained HBV DNA suppression, HBeAg seroconversion and HBsAg loss in adolescents with CHB. Larger and longer trials are now required to better define the magnitude of the benefit in this group of patients.

ObjectivesSince 1991 hepatitis B vaccination has been mandatory for all newborns in Italy. The aim of the study was to verify the long-term seroprevalence and the efficacy of hepatitis B vaccination in medical students of the University of Siena.Material and MethodsA cross-sectional observational study was conducted on a population of 850 medical students of the University of Siena (322 males and 528 females, mean age: 23 years) by obtaining from the medical reports the serological analysis data for the total anti-hepatitis B antibodies (HBsAb) and information on hepatitis B vaccination (number of vaccine doses, age at the first vaccination, time since the final vaccination dose, country of origin). Raw odds ratios (ORs) and 95% confidence intervals (CIs) were initially calculated to evaluate the association between 2 variables. The adjusted ORs were then calculated using a multivariate logistic regression model to study the association between the variables and the possible confounding factors.ResultsOverall, 593 students (69.76%) were immunized against hepatitis B, while 257 (30.24%) had HBsAb antibody titer <10 mIU/ml. From the OR calculation, an inverse correlation emerged between seropositivity to hepatitis B and age, and between seropositivity to hepatitis B and the age at the first vaccination dose. There was also a correlation between seropositivity and the number of vaccination doses received. By performing the multivariate logistic analysis, correlations with these variables were confirmed.ConclusionsA significant part of the studied population was not immunized against hepatitis B virus, despite the fact that vaccination had been carried out as prescribed by law. The results of the study reaffirm the importance of health surveillance in subjects at biological risk such as medical students.

Introduction: The pathogenesis of chronic hepatitis B depends on both, the immune response and oxidative stress. Aim of the study: To assess the hepatic expression of miR-122 and the antioxidant genes: HMOX-1, NQO1 and GFER1, in liver biopsy specimens obtained from patients with chronic hepatitis B, with regard to selected clinical and histological parameters, using RT-PCR. Results: The study group comprised 34 HBV-infected patients. Statistically significant associations were found between lower hepatic expression of HMOX-1 and greater severity of liver inflammation (p=0.04). However, significantly higher expression of NQO1 was observed in patients with advanced liver fibrosis (p=0.035). Hepatic expression of miR-122 in HBV patients was not associated with viral load or liver injury. Conclusion: The hepatic expression of HMOX-1and NQO1 may be associated with liver injuries in chronic hepatitis B. However, hepatic expression of miR-122 does not seem to correspond to progression of the liver disease.

Objective. Evaluation of the neutrophil-lymphocyte ratio and mean platelet volume as predictive factors for liver fibrosis and steatosis in HBV patients qualified for antiviral treatment. Materials and method. The study comprised 38 CHB patients who had commenced antiviral treatment, and 20 healthy volunteers who constituted the clinical control group. All patients had their blood count taken and underwent hepatic assessment using transient elastography with CAP (controlled attenuation parameter). Results. It was found that the mean hepatic fibrosis was 8.7 kPa (±8.8) and the mean liver steatosis – 286 db/m (±64). Mean NLR – 2.78(±1.1), whereas in the control group the mean NLR value was 1.64(±0.98). A negative linear correlation (r= -0.34; p=0.035) was found between liver fibrosis and the NLR value in the study group. No correlation was observed between hepatic steatosis and the NLR. Mean MPV – 12.6fl (±3.1), which was considerably higher in the CBH patients than in the control group. A positive correlation (r= 0.79, p= 0.001) was found between MPV and disease severity evaluated with transient elastography. Conclusions. The NLR and the MPV were significantly higher in the CHB patients than in the healthy volunteers. Both the NLR and the MPV can be treated as predictive factors for liver fibrosis in this group of patients.

Among possible causes of chronic hepatitis in adolescents most common are infections, autoimmune disorders and metabolic diseases. Thus, diagnostic procedures should be multidirectional. This study reports diagnosis and treatment difficulties in an 18-year-old male patient with hereditary hemochromatosis (HH), ulcerative colitis (UC), chronic hepatitis B (CHB) and Gilbert syndrome. The presented case illustrates problems in diagnostics related to the presence of numerous disease conditions in one patient. It should be taken into consideration that these diseases coexisting in one patient can mutually affect their symptoms creating specific diagnostic difficulties.

Objectives: Several occupations in developing countries lag behind in ensuring the safety of their workers in occupational settings. Lack of implementation of safety guidelines at workplaces can expose workers to health risks. In Pakistan, barbers are one of the un-regulated occupational groups. Low literacy, increased frequency of direct skin contact and blade/razors use can expose barbers to body fluids including blood of the customers. We conducted this study in order to determine hepatitis B virus (HBV) prevalence among barbers and their knowledge, attitude and practices in a peri-urban district of Sindh. Material and Methods: Three hundred eighty-five barbers from the Sukkur district were interviewed using a structured questionnaire. Blood samples were collected and tested for HBsAg. A scale was built to determine the proportions of responses to knowledge, attitude and practice items. Results: The prevalence of HBV among barbers was 2.1%. The barbers’ knowledge on HBV and its transmission routes was poor. The response to attitude items was good, except that only 35.1% of the participants agreed to have vaccination against HBV. The overall performance on the knowledge and practice scales was poor compared to the attitude scale on which 80% of the barbers performed well. Conclusion: The prevalence of HBV among barbers was lower compared to the available national figures for the prevalence among the general population.

Objectives: Hepatitis B (HBV) and C viruses (HCV) are among the most frequent blood borne pathogens. According to WHO, 5% of healthcare workers (in central Europe), are exposed to at least one sharps injury contaminated with HBV per year, 1,7% - contaminated with HCV. Aims: The aims of the study were to determine prevalence of HCV and HBV infections, vaccination efficacy against hepatitis B and usefulness of alanine aminotransferase (ALT) testing in prophylactic examinations in healthcare workers (HCWs). Material and Methods: In a group of 520 healthcare workers, a survey, laboratory and serologic tests such as ALT, HBsAg, anti-HBs, anti-HBcT and anti-HCV were carried out. Results: The study revealed a low rate of workers with presence of HBsAg and anti-HCV (1,2% and 0,8% respectively). Anti-HBcT was found in 99 subjects (19%) without a significant association with experiencing an occupational percutaneous injury. Being vaccinated against HBV was declared by 90% of the subjects. There was no relationship between ALT level rise and positive HBsAg, anti-HCV and anti-HBcT tests. Conclusion: A seroprevalence of HBV and HCV markers in HCWs found in the study is low and similar to the one found in general population. Current or past hepatitis B infections were independent of needle stick injuries. Vaccination against HBV coverage, although found to be high, should improve to 100%. Occupational prophylactic medical examinations found performing ALT test (obligatory in Poland for HCWs) not helpful. It seems that determination of anti-HBcT and anti-HCV status would be essential in pre-employment medical examinations.

Viral hepatitis is caused by viruses, the most important of which are hepatitis A virus (HAV), hepatitis B virus (HBV) and hepatitis C virus (HCV). New discoveries and new therapeutic options result in better and better results of treatment. However, the public's awareness of the occurrence of these diseases and the detection of infections as soon as possible is still important, what is associated with better outcomes. In order to assess the basic knowledge about viral hepatitis among Polish society, an online survey spread throughout Poland and covering mainly young people was created. 2205 responses were collected and analyzed. On the basis of the results obtained, it can be concluded that Polish youth have little knowledge about viral hepatitis, what is associated with less awareness of the possibility of infection.

Wstęp. Szczepienia są jedną z metod profilaktyki przeciw wielu chorobom zakaźnym. Podczas analizy różnic w realizacji podstawowych szczepień u noworodków w poszczególnych krajach Unii Europejskiej wyłania się istotny problem braku ujednolicenia tego rodzaju programów profilaktycznych. W Polsce na rodziców nałożony jest obowiązek szczepień noworodków. W pierwszej dobie życia dziecka obowiązkowe są dwa szczepienia: przeciw gruźlicy i wirusowemu zapaleniu wątroby typu B. Obecnie w Polsce zwiększa się liczba rodziców, którzy odmawiają zaszczepienia swoich dzieci. Cel pracy. Porównanie skali zjawiska odmów szczepień noworodkowych w szpitalu klinicznym i powiatowym oraz charakterystyka matek odmawiających zaszczepienia oraz noworodków. Materiał i metody. Badanie obserwacyjne, retrospektywne opierające się na analizie dokumentacji medycznej z roku 2017. Badaniem objęto dwa szpitale: kliniczny i powiatowy w województwie mazowieckim. Analizie poddano 7118 rekordów dotyczących żywych urodzeń ze szpitala klinicznego i 621 ze szpitala powiatowego. Wyniki i wnioski. Zdecydowana większość dzieci zostaje zaszczepiona w pierwszej dobie po porodzie, w badanych grupach odsetek dzieci nieszczepionych był niższy niż 8%. Istotnie częściej odmawiali szczepień rodzice, których dzieci przychodziły na świat w szpitalu klinicznym. W szpitalu powiatowym liczba odmów była znikoma. Najczęściej szczepienia noworodków odmawiały kobiety w przedziale wiekowym 32–35 lat, wieloródki. W wielu przypadkach kobiety miały obciążony wywiad położniczy, co mogło mieć wpływ na ich decyzję dotyczącą zaszczepienia dziecka. Większość noworodków (75%), które nie zostały zaszczepione z powodu braku zgody rodziców, przyszły na świat drogą naturalną, a dzieci rodziły się w stanie dobrym. Opóźnienie szczepień wynikające z decyzji lekarzy dotyczyło przede wszystkim dzieci z masą urodzeniową poniżej 2000 g, gdyż to wyklucza szczepienie przeciw gruźlicy.
Background. Vaccinations are one of the methods of prevention against many infectious diseases. During the analysis of differences in the implementation of basic vaccinations in newborns in individual countries of the European Union, there emerges the significant problem of the lack of unification of such types of prevention programs. In Poland, parents are obliged to vaccinate newborns. In the first day of the child’s life, two vaccinations are obligatory: against tuberculosis and hepatitis B. Currently, the number of parents who refuse to vaccinate their children is increasing in Poland. Objectives. Comparison of the scale of the refusal of neonatal vaccinations in a clinical and poviat hospital as well as the characteristics of mothers refusing vaccination and newborns. Material and methods. An observational and retrospective study based on the analysis of medical records from 2017. The study covered two hospitals: clinical and poviat in the Mazowieckie voivodship. The analysis covered 7118 records regarding live births from a clinical hospital and 621 from a district hospital. Results and conclusions. The vast majority of children are vaccinated on the first day after delivery. In the examined groups the percentage of unvaccinated children was lower than 8%. Parents whose children are born in a clinical hospital more often refused vaccination. In the district hospital, the number of refusals was negligible. Mothers of unvaccinated children were most often multiparous and some of them experienced obstetric loss. Most newborns (75%) who were not vaccinated were vaginally and in good condition. The delay in vaccinations resulting from doctors’ decisions concerned mainly children with birthweight below 2000 g, as this excludes vaccination against tuberculosis.

Duża liczba osób zakażonych wirusem zapalenia wątroby typu B na świecie i w Polsce oraz ograniczona dostępność i skuteczność leczenia przyczynowego wymusza poznanie czynników odpowiedzi na stosowaną terapię. Ma to na celu wybranie jak najlepszego leku oraz możliwość modyfikacji leczenia w trakcie terapii. W pracy omówiono poznane czynniki/znaczniki odpowiedzi na leczenie zakażenia HBV. Czynnikami dobrej odpowiedzi na terapię PEG-IFN wyjściowo są: niska wiremia HBV DNA , obecność HBeAg, wysoka aktywność Al at, genotyp A lub B HBV oraz wysoka aktywność zapalna w badaniu histopatologicznym bioptatu wątroby. Monitorowanie skuteczności leczenia przeciwwirusowego oceną wiremii HBV DNA metodą PCR , ilościowego oznaczania HBsAg, HBeAg i kinetyki spadku tych stężeń umożliwia modyfikację leczenia w trakcie jego trwania. Dalej istnieje potrzeba selekcjonowania nowych czynników pozwalających przewidywać skuteczność prowadzonego leczenia przeciwwirusowego u zakażonych HBV w celu ograniczenia ryzyka rozwoju odległych powikłań, tj. rozwoju marskości wątroby ze wszystkimi następstwami, oraz raka wątrobowokomórkowego
A large number of people infected with hepatitis B virus in Poland and the world as well as limited availability and efficiency of causal therapy necessitate further research on predictors of response to the therapy. It has to provide choosing the best medication and capability to modify the therapy. The paper discusses predictors of responses to the HBV therapy. Factors associated with response to PEG-IFN therapy are: low HBV DNA level, HBeAg presence, high Al at activity and active inflammation in histopathological examination of the liver biopsy. Modification of the therapy is possible by monitoring its effectiveness by using the assessment of HBV DNA levels with PCR method, HBsAg quantification and kinetics of the decrease in their concentrations. There is still a need to discover new factors that will let predict the efficiency of the antiviral therapy in HBV positive patients to decrease the risk of developing the long-term complications such as liver cirrhosis and hepatocellular cancer

Opis:
Wprowadzenie: Wirusy HBV i HCV należą do najczęstszych przyczyn wirusowego zapalenia wątroby (WZW). Przewlekłe formy zakażenia mogą prowadzić do marskości wątroby oraz raka wątrobowokomórkowego, a w konsekwencji do śmierci. Z literatury przedmiotu wynika, że młode pokolenie nie posiada odpowiedniego zasobu wiedzy na ten temat, a brak świadomości ryzyka zakażenia wirusem HBV i HCV stwarza warunki do niekontrolowanej transmisji tych wirusów. Celem pracy była ocena poziomu wiedzy uczniów szkół ponadgimnazjalnych na temat WZW typu B i typu C. Materiał i metody: Badanie przeprowadzono wśród 250 uczniów szkół ponadgimnazjalnych za pomocą autorskiego kwestionariusza ankiety. Do analizy statystycznej użyto testów: U Manna-Whitneya, Kruskala-Wallisa, korelacji liniowej r Pearsona. Przyjęto istotność statystyczną na poziomie p<0,05. Wyniki: Największy odsetek badanych (54%) cechuje się niskim poziomem wiedzy nt. WZW typu B i C. Uczniowie nie mają świadomości, że WZW jest chorobą zakaźną i wirusy HBV oraz HCV mogą przetrwać poza organizmem człowieka. Nie wiedzą również, czy byli szczepieni przeciw WZW typu B, i błędnie uważają, że istnieje szczepionka przeciw wirusowi HCV. Tylko 4% badanych uczniów wykazało się wysokim poziomem wiedzy na ten temat. Istotnie większą wiedzę mają uczniowie płci żeńskiej (p=0,006) oraz mieszkańcy wsi (p=0,024). Głównym źródłem wiedzy jest Internet (55% odpowiedzi). Wnioski: Ogólny poziom wiedzy uczniów szkół ponadgimnazjalnych nt. WZW typu B i C jest niski. Wiedzę badanych determinuje płeć i miejsce zamieszkania, a jej głównym źródłem jest Internet.
Introduction: HBV and HCV are among the most common causes of viral hepatitis. Chronic forms of infection may lead to liver cirrhosis and hepatocellular carcinoma, and therefore death. From the literature it shows that the young generation does not have the right amount of knowledge on the subject, and lack of awareness of infection with HBV and HCV creates conditions for uncontrolled transmission of these viruses. The purpose of this study was to assess hepatitis B and C knowledge among secondary school students’. Material and methods: The study was conducted among the students enrolled in a upper secondary schools (N=250). In order to gather information a personal questionnaire was used. Statistical analysis was performed with Mann-Whitney U test, the Kruskal-Wallis test, Pearson correlation coeffi cient. P-values less than 0.05 were considered statistically signifi cant. Results: The largest percentage of respondents (54%) was characterized by a low level of knowledge about hepatitis B and C. Students are not aware that hepatitis is an infectious disease and viruses, HBV and HCV can survive outside the human organism. They do not know whether they were vaccinated against hepatitis B and mistakenly believe that there is a vaccine against HCV. Only 4% of the students showed a high level of knowledge on the subject. Indeed, more knowledgeable students are female (p=0.006) and rural residents (p=0.024). The main source of information is the Internet (55% of responses). Conclusions: General knowledge of secondary school students about hepatitis B and C is low. Students’ knowledge is determined by the gender and place of residence, and its main source is the Internet.

Opis:
Objectives Health care workers, including dentists, are at the front line for acquiring blood-borne virus infections. This study aimed to investigate the prevalence of hepatitis B, hepatitis C and human immunodeficiency viruses among Iranian dentists. Material and Methods The survey included 1628 dental health care workers who attended the 51st annual Congress of the Iranian Dental Association. Data on the risk of blood-borne virus transmission during health care and infection control practices were gathered from self-administered questionnaires. Sera were screened serologically by enzyme-linked immunosorbent assay (ELISA). The positive samples were examined by polymerase chain reaction (PRC) followed by direct sequencing. Results Six (0.36%) and 81 (5.0%) were positive for hepatitis B surface antigen (HBsAg) and (anti-hepatitis B virus core antigen (anti-HBc), respectively. Only 1 (0.061%) was positive for anti-hepatitis C virus (anti-HCV). No case was positive for anti-human immunodeficiency virus (anti-HIV). One case was diagnosed as being occult hepatitis B virus (HBV) infection. One thousand five hundred thirty-five (94.3%) of participants had received at least 1 dose of HBV vaccine. One thousand three hundred fifty-nine (88.5%) contained hepatitis B surface antibody (anti-HBs) > 10 IU/ml, of whom 55 (4.0%) were anti-HBc positive, suggesting that they had been infected with HBV in the past. Anti-HBc positive cases had past histories of hepatitis, either their own or their spouses’. Individuals with inadequate anti-HBs levels (< 10 IU/ml) were significantly more prevalent among anti-HBc positive cases (p < 0.001). Conclusions The prevalence of blood-borne viruses among dental HCWs found in this study was lower than past reports from Iranian dentists and general population. The implementation of HBV vaccination together with improvement in infection control procedures has reduced the potential for risk infection among Iranian dentists.
