According to recent society guidelines, upper gastrointestinal bleed initial approach includes assessment of hemodynamic status, fluid resuscitation if necessary, transfusion strategy to target hemoglobin above 7 (g/dL), use of intravenous proton pump inhibitor and generally upper endoscopy within 24 hours. We present a case of a 26-year-old woman who sought treatment after one episode of hematemesis and pre-syncope. She had a similar presentation three months earlier and received interventional radiology-guided mesenteric angiography and the use of multiple coils to embolize a 1.5-cm deep punched-out duodenal ulcer. Migration of the coil was noted on endoscopy within the previously described ulcer. Coil migration is expected to occur in up to 3% of cases of endovascular embolization. However, migration into the duodenum is uncommon and could have actually been a contributing factor to the current bleed.

Introduction and aim. Anemia remains a leading contributor to years lived with disability (YLDs), being responsible for 50.3 million (5.82%) YLDs worldwide and 19.3 million (12.03%) YLDs in India, respectively. Results of the National Family Health Survey 2019-2021 (NFHS-5) suggest a high burden of anemia in India among women of reproductive age and children aged 6-59 months at the national level (57%, 67.1%), and in the state of Bihar, India (63.5%, 69.4%). Iron deficiency is the leading cause, accounting for more than half the cases. Anemia bodes harmful implications for both the mother and child, with long-lasting consequences for the latter. Anemia control programs have yielded little benefit despite efforts stretching over five decades. This narrative review aims to highlight the burden of anemia and the probable factors behind it among under-5 children and women of reproductive age in the Indian state of Bihar. Material and methods. The paper is a narrative review. The following databases were used to search and select literature: PubMed, Web of Science, Scopus, and Google Scholar. In addition, the websites of relevant government departments and national health programs were searched for pertinent material. Analysis of the literature. A multitude of reasons seem to be behind the unabated high prevalence in Bihar: low socioeconomic status, gender disparities, traditional customs and practices, food insecurity, lack of diverse diets, poor consumption, and no adherence to iron and folic acid (IFA) supplements, groundwater contamination with arsenic and fluoride, and supply chain mismanagement, all playing roles of varying degree. Conclusion. An all-encompassing approach and not merely the provision of IFA supplements are necessary to unravel the intricate web of factors that lead to anemia.

INTRODUCTION: Anemia is a risk factor for cerebrovascular events. The aim of this study was to evaluate the prevalence of anemia among patients who underwent a first-ever stroke and its impact on the neurological and functional state in the acute phase of stroke. MATERIAL AND METHODS: The prospective study encompassed 109 patients (53 women) aged 72.8 ± 11.12 after stroke. We analyzed their neurological state on the first day after stroke by NIHSS and their functional status on the 14th day after the onset of stroke by the modified Rankin scale among patients with anemia and without anemia. RESULTS: Anemia was found in 34 patients, in 15 women and 19 men; 8 patients aged ≤ 65 years. The neurological deficit according to NIHSS, functional status at the level of 3–5 by mRS and death did not differ significantly between the analyzed groups. An independent predictor of a worse functional status on the 14th day after stroke or death in Group 1 included the neurological state on the 1st day following stroke. CONCLUSIONS: Anemia appeared in approximately 1/3 patients suffering a stroke. Slight or moderate anemia in the acute period of stroke did not significantly influence the neurological or functional status in the acute phase of stroke. The neurological state on the first day is an independent factor of a poor prognosis in patients with anemia in short-term follow-up.
WSTĘP: Anemia jest czynnikiem ryzyka chorób sercowo-naczyniowych. Celem badania była ocena potencjalnego wpływu niedokrwistości na stan neurologiczny pacjentów w pierwszej dobie oraz funkcjonalny w 14 dobie od wystąpienia niedokrwiennego udaru mózgu. MATERIAŁ I METODY: Do prospektywnego badania włączono 109 pacjentów (w tym 53 kobiety) w wieku 72,8 ± 11,12 w pierwszej dobie pierwszego w życiu udaru mózgu. Porównano częstość wybranych chorób i parametrów biochemicznych, stan neurologiczny (wg NIHSS) w pierwszej oraz stan funkcjonowania (wg mRankin) w 14 dobie od wystąpienia udaru mózgu u pacjentów z anemią oraz prawidłowym stężeniem hemoglobiny. WYNIKI: Anemię stwierdzono u 34 pacjentów (15 kobiet oraz 19 mężczyzn) oraz 8 pacjentów w wieku ≤ 65 r.ż. Częstość lekkiego i umiarkowanego/ciężkiego deficytu neurologicznego w pierwszej dobie nie różniła się znamiennie między pacjentami z anemią i bez anemii. Częstość stanu funkcjonalnego na poziomie 3–5 Rankin w 14 dobie oraz zgonu (do 14 dni od zachorowania) nie różniła się znamiennie między tymi pacjentami. Stan neurologiczny pacjentów z anemią w pierwszej dobie udaru mózgu okazał się niezależnym czynnikiem gorszego rokowania odnośnie do stanu funkcjonalnego w 14 dobie udaru mózgu oraz zgonu do 14 doby od zachorowania. WNIOSKI: Niedokrwistość występuje u ok. 1/3 pacjentów z ostrym niedokrwiennym udarem mózgu. Lekka oraz umiarkowana niedokrwistość nie wykazuje istotnego negatywnego wpływu na stan neurologiczny oraz funkcjonalny pacjentów w ostrym okresie udaru. Stan neurologiczny w pierwszej dobie udaru niedokrwiennego jest niezależnym czynnikiem złego rokowania w obserwacji krótkoterminowej u pacjentów z anemią.

Peru has a diversity of exotic fruits and plants with recognized yields for food and health protection. Nectars are highly consumed food products, made from fruits or vegetables. The objective for the present work was to carry out the technical study of the installation of a watercress nectar processor to reduce anemia in adults; identify the demand, technical requirements for the operation and profitability of the processing facilities. Methodology. The analytical-synthetic method was used; The population of the target market was analyzed, the size of the sample was determined with the coefficient of variation method, (n = 384), the instrument used was the survey; the technical, economic-financial study was carried out. The results were the growth in the demand for nectars of 59.82%, with respect to the economic-financial evaluation, it has been shown that the results are satisfactory for the installation of the plant, with the economic and financial IRR, both rates proving to be attractive. for the project. From the projected income statement for the 2021-2025 period, it can be deduced that for 2025 there was a net profit of S/316,971.39. Other indicators such as the economic and financial NPV, the B/C ratio and the PRI that were satisfactory for the Project, making its execution viable. The conclusion this study was the technical study for the installation of a nectar processing plant to reduce anemia in adults in Lima, which was viable in terms of operation and profitability, taking advantage of its healing and nutritional properties.

Introduction and aim. Pancytopenia is the simultaneous presence of anemia, leucopenia and thrombocytopenia. The aim of this work was to study the etiological spectrum of pancytopenia in the National capital region of India, and evaluate the relationship of pancytopenia with serum vitamin B12 levels. Material and methods. This study is of a prospective and analytical type conducted on patients attending the outpatient and inpatient department of Santosh Medical College and the Saraswathi Institute of Medical Sciences tertiary care centers in NCR. Complete blood counts and peripheral blood smear examination was used for morphological classification and typing of anemia. Bone marrow aspiration and vitamin B12 estimation was performed where required. Results. The maximum number of pancytopenia cases were etiologically attributed to megaloblastic anemia (64.2%) followed by aplastic anemia (19.8%). Malaria was attributed to 6.6% cases of pancytopenia. Iron deficiency anemia and tuberculosis both accounted for 1.9% of cases. each. A history of drug intake and mixed nutritional anemia each contributed to 2.8% of cases. Serum vitamin B12 levels showed a significant relationship with pancytopenic cases. Conclusion. In our study, the main cause of pancytopenia is megaloblastic anemia which responds very well to treatment if diagnosed correctly in time. A detailed hematological assessment along with vitamin B12 levels should be evaluated in all cases of pancytopenia irrespective of the etiological categorization.

Radiotherapy is, next to surgery, the basic method of treatment in oncology. The intensive searching of the predictive factors for radiotherapy effectiveness, are still continuing. The mechanism of radiotherapy action depends on the cellular oxygen presence. The latest studies evidence, that the molecular oxygen, through the mechanism of fixation, leads to destruction of the DNA damage, caused by the ionizing radiation, repair. Number of studies has demonstrated, that the tumour hypoxia evaluation correlates with radiotherapy effectiveness. However, the universal method assessing the tumour hypoxia does not exist. Actually used methods, as Eppendorf’s electrode, the molecular markers of hypoxia evaluation, or PET studies with oxydo-reductive radiopharmaceutical agents, bring conflicting results. So, a lot of doubts about the quantitative and qualitative character of relationship between hypoxia and sensitivity for radiotherapy still exist. Moreover, the analysis is impeded by fact, that the hypoxia has significant negative prognostic influence on the treatment results with radiotherapy, through the genome and proteomic changes induction. The paper presents the actual status of knowledge about the influence of tumour hypoxia on radiotherapy effectiveness.
Radioterapia jest obok chirurgii podstawową metodą leczenia nowotworów. Trwają obecnie intensywne poszukiwania czynników predykcyjnych dla skuteczności radioterapii. Mechanizm działania radioterapii zależny jest od obecności tlenu w komórce. Najnowsze doświadczenia dowodzą, że tlen cząsteczkowy poprzez mechanizm fiksacji zaburza naprawę uszkodzeń wywołanych w DNA promieniowaniem jonizującym. Liczne badania wykazały, że ocena niedotlenienia guza koreluje z wynikiem radioterapii. Brak jednak uniwersalnej metody oceniającej hipoksję. Aktualnie stosowane metody, jak pomiar elektrodą Eppendorfa, ocena markerów molekularnych hipoksji czy badania PET z użyciem oksydoredukcyjnych radiofarmaceutyków dostarczają sprzecznych wyników. Istnieje więc wiele wątpliwości co do ilościowego i jakościowego charakteru zależności między niedotlenieniem a wrażliwością na radioterapię. Ponadto analizę utrudnia fakt, że niedotlenienie wywiera znaczny negatywny prognostyczny wpływ na wyniki leczenia radioterapią poprzez indukcję zmian genomowych i proteomicznych. W pracy przedstawiono aktualny stan wiedzy na temat wpływu niedotlenienia guza na skuteczność radioterapii.

BackgroundFemales are more likely than males to suffer from anemia. There have been many studies investigating the nutritional qualities of traditional plants. The of this study was to assess the effect of fenugreek seeds on hemoglobin and PCV values in 20-30 years old females.Material and methodsBaseline hematological tests were obtained from 10 females aged 20-30 years. Supplementation of the standard amount of fenugreek was given to the population. After 48 days the same hematological parameters were analyzed as a post-test. The results of the pre- and post-test analysis were compared by using a paired-sample t-test. Relevant details were also collected and analyzed.ResultsThe mean value of hemoglobin before supplementation (Hb1) and hemoglobin after supplementation (Hb2) was 12.3200 and 11.3300 respectively and the mean value of packed cell volume before (PCV1) and after supplementation (PCV2) was 36.7970 and 35.4700, respectively. This shows that there is a mild decrease in hemoglobin and PCV values after fenugreek supplementation, but not at a statistically significant level.ConclusionsWithin the limitations of our pilot study, we conclude that fenugreek has no significant effect on Hemoglobin and PCV values and can’t be used as an exact supplement to cure anemia.

Introduction. Severe Acute Respiratory Syndrome-2, possesses varying degrees of susceptibility and lethality worldwide and WHO declared this as a pandemic of this century. Aim. In this background, the aim of this present narrative is to provide a complementary overview of how low iron stores and mild anemia offers protection from infectious diseases like COVID-19 by restricting the viral replication and also to suggest some potential adjuvant therapeutic interventions. Material and methods. Therefore, we performed a literature search reviewing pertinent articles and documents. PubMed, Google Scholar, Chemrxiv, MedRxiv, BioRxiv, Preprints and ResearchGate were investigated. Analysis of the literature. Recent studies reported drastic systemic events taking place that contribute to the severe clinical outcomes such as decreased hemoglobin indicating anemia, hypoxia, altered iron metabolism, hypercoagulability, oxidative stress, cytokine storm, hyper-ferritinemia and thus Multi Organ Failure, reportedly hailed as the hallmark of the COVID-19 hyper-inflammatory state. Interestingly it is globally observed that, countries with higher Socio-economic status (SES) have considerably lower prevalence of Iron Deficiency Anemia (IDA) but higher Case Fatality Rate (CFR) rate due to COVID-19 while, low SES countries characterized by the higher prevalence of IDA, are less affected to COVID-19 infection and found to have less CFR, which is almost half to that of the higher SES counterpart. Conclusion. Present review presumed that,low iron stores and mild anemia may play a beneficial role in some cases by offering protection from infectious diseases as low iron restricts the viral replication.Thus, suggested iron chelation or iron sequestration as an alternative beneficial adjuvant in treating COVID-19 infection.

Over ninety percent of End Stage Renal Disease (ESRD) patients suffer from anemia due to insufficient endogenous production of human erythropoietin. Until the advent of Recombinant Human Erythropoietin (r-HuEPO) over 30 years ago, patients with ESRD were treated mainly with multiple blood transfusions. The high cost of r-HuEPO in addition to the narrow margin between an effective do-sage and toxicity in drug administration calls for optimal dosage strategy capable of minimizing cost and toxicity while at the same time achieving the desired do-sage outcome. It is well known from control theory that a controller can be de-signed for any plant provided there is readily available a valid model for such a plant. We present Robust Identification procedure, a dimensionality reduction technique capable of capturing the inherent dynamics of anemia patients; conse-quently producing individualized model suitable for robust control synthesis and any other controller design methodologies.

Background: Low birth weight is an alarming problem in developing countries and has severe future complications. Aim of the study: Our study aimed to compare the risk factors among mothers with low and normal birth weight babies. Material and methods: A cross-sectional study was carried out on 1000 mothers with normal and low birth weight babies (500 per group) over two years. Maternal parameters including age, hemoglobin levels, gravida, maternal weight gain, height, pregnancy-induced hypertension, etc., were collected along with anthropometric data of the child. We compared risk factors among the low and normal birth weight babies using the chi-square test, determining statistical significance at p<0.05, and high statistical significance at p<0.01. Results: Highly statistically significant associations were observed between low birth weight and seven variables: maternal age (p=0.0074), maternal height (p<0.0001), weight (p<0.0001), weight gain (p<0.0001), hemoglobin (p<0.0001), severe anemia (p<0.0001), and pregnancy-induced hypertension (p<0.0001). Conclusions: Our study observed significant modifiable risk factors like weight gain, weight, hemoglobin, and anemia among mothers with low birth weight babies. If we focus on raising awareness surrounding these risk factors, there may be an improvement in the birth weight of babies in this population.

Despite over 40 years of research on the human immunodeficiency virus type 1 (HIV-1) vaccine, we still lack a considerable progress. Equine infectious anemia virus (EIAV) is a lentivirus in the Retroviridae family, akin to HIV-1 in genome structure and antigenicity. EIA is an important infectious disease in equids, characterized by anemia, persistent infection, and repeated fevers. The EIAV attenuated vaccine in China is the only lentiviral vaccine used on a large scale. Elucidating the mechanism of waning and induction of protective immunity from this attenuated vaccine strain will provide a critical theoretical basis and reference point for vaccine research, particularly in the development of lentivirus vaccines, with far-reaching scientific value and social significance. In this paper, we summarize the information related to EIAV integration site selection, particularly for the Chinese EIAV attenuated vaccine strains on the equine genome. This may improve our mechanistic understanding of EIAV virulence reduction at the host genome level. The obtained data may help elucidate the biological characteristics of EIAV, particularly the Chinese attenuated EIAV vaccine strain, and provide valuable information regarding retroviral infections, particularly lentiviral infection and associated therapeutic vectors.

Glycophorins A from erythrocyte membranes of two patients with congenital dyserythropoietic anemia type I and type II (CDA type I and II) were analyzed for carbohydrate molar composition employing a modification of the recently published method that allowed simultaneous determination of carbohydrates and protein in electrophoretic bands of glycoproteins separated by sodium dodecyl sulfate- polyacrylamide gel electrophoresis (Zdebska & Kościelak, 1999, Anal. Biochem., 275, 171-179). The modification involved a preliminary extraction of erythrocyte membranes with aqueous phenol, subsequent electrophoresis and analysis of the extracted glycophorins rather than electrophoresis and analysis of the glycophorin from intact erythrocyte membranes. The results showed a large deficit of N-acetylgalactosamine, galactose, and sialic acid residues in glycophorin A from patients with CDA type I and type II amounting to about 45% and 55 %, respectively. The results strongly suggest that glycophorin A in these patients is partly unglycosylated with respect to O-linked glycans. In addition, glycophorin A from erythrocytes of a patient with CDA II but not CDA I exhibited a significant deficit of mannose and N-acetylglucosamine suggesting that its N-glycosylation site was also partly unglycosylated.
