    Opis:
    The aim of the study is to present the problems arising during general anesthesia in patients with cerebrovascular diseases, taking into account the appropriate perioperative preparation. When planning the anesthetic procedure strategies aimed at the prevention of cerebral ischemia or hemorrhage into the brain must be taken into account. Material and research methods: a descriptive analysis was applied on the basis of the literature collected from the years 1992 to 2013. Conclusions: Preoperative evaluation of patients with cerebrovascular disease requires a thorough neurological examination, assessment of cardiovascular complications, including the risk of cerebrovascular, respiratory, renal, and endocrine system complications. During general anesthesia one should take care of haemodynamic stability and proper ventilation parameters. The choice of anesthetic agents so that during the induction of anesthesia, throughout anesthesia, during the recovery from the anesthesia there is no risk of hemodynamic instability and an appropriate level of blood perfusion in the brain is maintained. In the postoperative period the neurological status of the patient should be assessed, in order to ensure that he does not demonstrate symptoms of delirium and that he does not experience a recurrence of previous neurological deficits. Each surgery and anesthesia should be discussed with the operator in terms of the risks and benefits of surgery. Scheduled surgery in patients with acute cerebrovascular incidents need to be postponed until their performance is reasonably safe for the patient.

    This article contains the current state of knowledge on modern methods of treating patients with severe cranio-cerebral injuries, hospitalized in intensive care due to respiratory failure. Severe traumatic brain injury (TBI) can be divided into primary and secondary. Primary brain damage takes place in mechanism of direct injury most often due to an impact. Secondary damage is the result of subsequent disorders of physiological conditions such as ischemia and hypoxia of the brain areas covered by the primary injury. Treatment of patients with severe cranio-cerebral injuries is focused on maintaining proper ventilation, hemodynamic stability and optimal values of intracranial pressure. Maintaining adequate blood flow through brain vessels is necessary to prevent the development of ischemic changes. Respiratory failure develops in these patients by direct injury to the brain in the form of tachypnea, bradypnoea, shortness of breath, sleep apnea, or secondarily to trauma as pneumonia, acute respiratory distress syndrome (ARDS), or pulmonary edema. Mechanical ventilation in patients with TBI seeks to maintain an oxygen partial pressure in the arterial blood of more than 11 kPa and the partial pressure of carbon dioxide in the range of 4.5 to 5.0 kPa. The use of appropriate therapy is to prevent the secondary damage to the brain or reduce complications associated with secondary damage.

    Back pain is a common health problem. Natural degenerative processes reduce its strength, causing symptoms of sciatica, numbness, paraesthesia with muscle weakness and sensory surface, leading to muscle atrophy, inclusive. Degeneration of the intervertebral disc and hernia formation in the first place causing damage to the spinal cord and nerves and spinal facet joints and ligaments become a source of pain spondylogenic. In the case of advanced degenerative changes of the axial movement of the camera conservative treatment is sometimes ineffective, and the chronic nature of the disease and its neurological complications diminish the quality of life and often lead to disability. In order to reduce the time the patient recovers minimally invasive procedures are used, the effectiveness of which can sometimes vary depending on the location of the source of the pain. PURPOSE OF THE WORK is to present the possible neurological complications resulting from chronic spinal pain syndromes. MATERIAL AND METHODS: Data were from the PubMed database of the last 20 years, presented in a descriptive analysis. CONCLUSIONS: lower back pain make a huge therapeutic difficulties.

    The aim of this article is to review current reports pertaining the effects of treating patients with diagnosed degenerative disc disease in the lumbosacral section of the spine. Materials and methods: the method of descriptive analysis was used. The research was conducted on the basis of materials from MEDLINE and EMBASE databases from the last ten years. The cases of patients with sciatic neuritis caused by degenerative disc disease undergoing conservative treatment, and similar group of patients treated surgically, were analysed. Conclusions: Conservative treatment lowers the intensity of pain, but does not improve the functional abilities of the spine. Surgical treatment gives good and quick therapeutic results, but its long-term effects do not seem better than in the case of conservative treatment. Moreover, surgical treatment may result in serious complications. In rare cases the condition results in a severe impairment of motor functions in legs and sphincter paralysis. Such patients should undergo immediate surgical treatment.

    Background. Due to the serious course of most severe vascular injuries of the brain, there is a necessity to look for new diagnostic methods which are the least invasive for the patient. Objectives: the evaluation of the usefulness of thermographic camera in stroke diagnostics. Material and methods. The research included 38 patients hospitalised due to cerebrovascular accident in the Intensive Therapy Ward and Neurology Ward. The patients were diagnosed according to EBM (evidence based medicine) by means of CT, then a series of images were taken by means of thermographic camera. The examinations were conducted on the first day of the stroke, preceded by head tomography, and next on the fourth day after the stroke. Results. The examinations showed statistically significant (p<0.05) decrease in the temperature in the brain stroke area in relation to the healthy part both on the first and the fourth day. The difference in the average temperature changes between the cerebral infarction and intracerebral hemorrhage on the first and the fourth day was not statistically significant (P>0.05). The difference in the average temperature changes in the brain stroke area (irrespective of its type) between the first and the fourth day was statistically significant (p<0.05). The difference in the average temperature changes between the smaller and the bigger focal areas in CT on the first and the fourth day was not statistically significant (P>0.05). No significant correlation was observed between the average temperature changes and the clinical state of patients evaluated according to ESS (Epworth Sleepiness Scale) during the first day (p.0.05). Conclusion. Thermographic camera is a useful tool in brain stroke diagnostics, however it does not differentiate between hemorrhagic and ischemic strokes.
    Wstęp. W związku z ciężkim przebiegiem większości ostrych naczyniowych uszkodzeń mózgu istnieje konieczność poszukiwania nowych metod diagnostycznych jak najmniej inwazyjnych dla chorego. Cel pracy: ocena przydatności kamery termowizyjnej w diagnostyce udarów mózgu. Materiał i metody. Badaniem objęto 38 pacjentów hospitalizowanych w Oddziale Intensywnej Terapii i Oddziale Neurologii z powodu udarów mózgu. Pacjentów diagnozowano zgodnie z zasadami EBM przy użyciu TK, a następnie wykonywano serię zdjęć przy pomocy kamery termowizyjnej FLIR E 30. Badania były wykonywane w I dobie udaru mózgu po diagnostyce tomograficznej głowy, a następnie w IV dobie po udarze. Wyniki. Badania wykazały istotne statystycznie (p<0.05) obniżenie temperatury okolicy objętej udarem mózgu w odniesieniu do strony zdrowej zarówno w I jak i w IV dobie pomiaru. Różnica średnich zmian temperatur pomiędzy zawałem mózgu i krwotokiem śródmózgowym w I i IV dobie nie była istotna statystycznie (P>0.05). Różnica średnich zmian temperatur okolic objętych udarem (niezależnie od jego rodzaju) pomiędzy I i IV dobą badania była istotna statystycznie (p<0.05). Różnica średnich zmian temperatur pomiędzy mniejszym i większym ogniskiem w TK w I i IV dobie nie była istotna statystycznie (P>0.05). Nie obserwowano istotnej korelacji średnich zmian temperatur ze stanem klinicznym chorych ocenianych według skali ESS w I dobie (p>0.05). Wnioski. Kamera termowizyjna jest przydatnym narzędziem w diagnostyce udarów mózgu, jednak nie różnicuje udarów na krwotoczne i niedokrwienne.

    Wprowadzenie. Choroba Parkinsona jest nieuleczalną, zwyrodnieniową chorobą układu nerwowego, prowadzącą do postępującej niesprawności, uzależnienia od pomocy innych oraz obniżonej jakości życia. Cel pracy. Celem niniejszej pracy jest przedstawienie najważniejszych kwestii dotyczących kinezyterapii chorych na chorobę Parkinsona na podstawie piśmiennictwa. Skrócony opis stanu wiedzy. Zastosowanie kinezyterapii jest nieodłącznym elementem leczenia i odgrywa bardzo ważną rolę w procesie usprawniania i przystosowania pacjenta do życia z chorobą. Ćwiczenia nie tylko mają istotny wpływ na występujące objawy w przebiegu choroby, ale też na ogólny stan zdrowia. Choroba przebiega u każdego pacjenta w odmienny sposób, dlatego ważny jest indywidualny dobór terapii, w zależności od występujących objawów ruchowych, jak i pozaruchowych, a także ogólnego stanu zdrowia pacjenta. Ćwiczenia zalecane u chorych na chorobę Parkinsona to ćwiczenia oddechowe, chodu, ćwiczenia wzmacniające, równoważne, koordynacyjne, rozciągające, relaksacyjne, muzykoterapia, taniec oraz gry i zabawy ruchowe. Regularne wykonywanie ćwiczeń, zaangażowanie pacjenta oraz realizowanie programu ćwiczeń, również w warunkach domowych, jest warunkiem efektywności kinezyterapii. Podsumowanie. Kinezyterapia odgrywa bardzo ważną rolę w usprawnianiu pacjenta z chorobą Parkinsona, zapobieganiu trwałej niepełnosprawności oraz utrzymaniu jak najdłuższej samodzielności i dobrej jakości życia.
    Introduction. Parkinson’s disease is an incurable, degenerative disease of the nervous system, leading to progressive disability, loss of independence and reduced quality of life. Objective. The aim of this study is presentation of the most important issues concerning kinesitherapy in Parkinson’s disease. Short description of state of knowledge. Kinesitherapy is an inseparable part of the treatment and plays a very important role in the process of increasing efficiency and adaptation of the patient to live with the disease. Exercises not only have a significant impact on the common symptoms in the course of the disease, but also on the general state of health. The course of the disease is different in each patient; therefore, an individual selection of therapy is important. It depends on motor and non-motor symptoms, and the general state of health. Exercises recommended in patients with Parkinson’s disease are: breathing exercises, gait learning, strengthening, coordination, relaxation exercises, music therapy, dancing and movement games. Regular exercises, patient’s active involvement and the implementation of an exercise programme in the home environment is a prerequisite for the effectiveness of kinesitherapy. Summary. Kinesitherapy plays an important role in the process of increasing the efficiency of a patient, preventing disability and maintaining independence and good quality of life as long as possible.
