Przedstawiono retrospektywną analizę parametrów widzenia oraz zgłaszanych dolegliwości pooperacyjnych u 428 pacjentów (606 oczu) poddanych zabiegowi usunięcia zaćmy lub refrakcyjnej wymianie soczewki z implantacją soczewek dwuogniskowych z grupy Oculentis®. Wszczepienia jedno- lub obuocznego dokonano u pacjentów normowzrocznych, krótkowzrocznych, nadwzrocznych oraz u pacjentów ze stwierdzonym astygmatyzmem rogówkowym. Największą grupę stanowiły osoby, którym wszczepiono soczewki sferyczne z addycją do bliży o wartości 3,0 Dsph, w dalszej kolejności – z addycją 1,5 Dsph oraz soczewki toryczne. Osobną grupę poddaną analizie stanowili pacjenci, u których wykonano zabieg witrektomii tylnej, oraz pacjenci, u których przedoperacyjnie stwierdzono zmiany zwyrodnieniowe plamki typu starczego (AMD). Głównymi kryteriami kwalifikacyjnymi do zabiegu były chęć pozbycia się korekcji okularowej oraz możliwość uzyskania dobrej ostrości wzroku do dali i bliży. Praktycznie wszyscy pacjenci uzyskali satysfakcjonującą ostrość wzroku oraz jakość widzenia przy zachowaniu określonych kryteriów kwalifikacyjnych. Jedno z kryteriów zakłada, iż u pacjentów z większą średnicą źrenicy, powyżej 5,8 mm, wszczepiano soczewkę z mniejszą addycją do bliży, a astygmatyzm rogówkowy korygowano nawet przy wartościach poniżej 1 Dcyl.
A retrospective analysis of parameters of vision and postoperative complaints in 428 patients (606 eyes) undergoing cataract surgery or clear lens extraction with ontime bifocal lens implantation from Oculentis®. An artificial lens implantation has been used in one or in both eyes in patients without preoperative eye defect. Next groups have included patients with preoperative hyperopia, myopia and with corneal astigmatism. The biggest number of patients have got spherical lenses with addition +3,0 Dsph to near distances, subsequently with addition +1,5 Dsph and toric lenses. Another two groups who were analyzed were whose patients who needed posterior vitrectomy and who have been preoperatively diagnosed with Age Related Macular Changes, dry and exudative changes. The main inclusion criteria was a need to become free of glasses to far and near distances. Almost all of patients have gained satisfactory visual acuity and quality of vision while maintaining the appropriate inclusion criteria. One of the criteria assumes that patients with bigger size of the pupil have implanted lenses with less addition to near distances and corneal astigmatism was corrected even if the value was below 1 dioptria.

The paper deals with issues connected with the administration of the educational system in the time of Warsaw Duchy. The Author discusses the organizational structure of offices and institutions responsible for education both on the central and local tiers. Warsaw Duchy, formally sovereign, was in fact in the French zone of interest. Attempts to create an autonomous ministry of education by the educational officials were marred by the Constitution of Warsaw Duchy, instituted by Napoleon. It was modelled upon French law, under which educational matters were decided by the Minister of Interior. Consequently, the Chamber of Public Education and later the Administration of National Education, were to struggle for the greatest autonomy possible within the Ministry of Interior. The latter part discusses the situation of local school authorities. Both for economic and practical reasons the institutions of the Prussian Partition era were retained, such as administrative chambers in departments and executive commissions in poviats. Characterised are the competences of school supervision bodies on the department, poviat and local level. Finally, the vital question of educational funds is raised without which any educational reforms would not have been possible. The paper is ordered chronologically, covering the whole period of the existence of Warsaw Duchy. The Author uses modern sources and source documents thanks to which the article draws a complete picture of the educational authorities of the time, focusing on the historical and legal dimensions.

Polish United Workers Party. The event led to a gradual centralization of the social and political life which resulted in the imposition of unification of the eleven combatant organizations that were operating in the People’s Poland. In consequence, a combatant organization called Society of Fighters for Freedom and Democracy was created. In the time of the so-called “cult of personality”, combatants, those originating form the Home Army in particular, were persecuted by the security authorities of the Polish People’s Republic. They were not admitted to the Society of Fighters for Freedom and Democracy. If it happened that a soldier of the Home Army was a member of the Society he was sidelined by the influential activists of the Polish United Worker’s Party who belonged to the Society and originated from the People’s Army and the People’s Guard. The socio-political crisis that took place in 1956 considerably contributed to the change of the combatants’ situation in the political system of the People’s Republic of Poland. Many former soldiers of the Home Army left prisons, general liberalization of the political system took place. The soldiers of the Home Army started to demand political rehabilitation. The episode in the Lublin branch of the Society of Fighters for Freedom and Democracy can serve as an example of their rebirth after 1956. When the Lublin Society was reactivated on the basis of the former soldiers of the Home Army, the activists of the left-wing provenience were expelled from its voivodship boards. The 1960s were called the “Moczar’s period.” It was a period in the history of the combatants and the Society of Fighters for Freedom and Democracy in Poland when one of the most important fractions of the Polish United Worker’s Party (so-called “partisans”) started using combatants for the perty’s own political goals. Mieczysław Moczar was aware that he would not gather real support in the party. As the president of the Society of Fighters for Freedom and Democracy and the chairman of the Leading Council he gained the favour of combatant circles by, among others, increasing the number of benefits. It made easier for Moczar to fight for the leadership in the Central Committee of the Polish United Workers’ Party.

Polish October 1956 and the Lublin branch of ZBOWiD (Society of Fighters for Freedom and Democracy) The Lublin branch of ZBoWiD was established simultaneously with the veterans’ organisation ZBoWiD, in 1949. In the Stalinist period the activities of the Lublin branch boiled down to propaganda. Its functions were limited by members from PZPR (the Polish United Workers’ Party, PUWP), striving to eliminate the Society, as e.g. Grzegorz Wajskop. After 1956 the organisation accomplished a strong comeback, based upon former AK (Home Army) soldiers. The catalyst of the organisation’s renewal after 1956 was the spontaneous voivodeship (provincial) convention staged by WW2 partisans of the Lublin region on 11 th November, 1956. As a result of elections held in individual local branches throughout the voivodeship, their managements were joined by mainly soldiers, former members of such underground organisations, as BCh (Peasant Battalions) and AK. They dominated the key positions in ZBoWiD, removing the representative of extreme leftist underground organisations (GL/AL, People’s Guard/People’s Army). At conventions numerously attended by partisans of the Lublin region, the Stalinist period (symbolised by Lavrentiy Beria) and the persecutions of those years were discussed, and harshly criticised by participants of the convention. AK/BCh soldiers maintained their influence in the Society until 1958. Subsequently to new elections to the Society’s authorities, strictly associated with the approaching 2 nd Congress of ZBoWiD (in 1959), AK and BCh members were pushed out of the Society’s authorities, former GL/AL soldiers (now members of PZPR) being selected to replace them. AK/BCh member again suffered marginalisation and discrimination from 1959 on, performing marginal, meaningless functions. The article presents how the events of one of the so-called Polish months impacted the functions and rebirth of the Society in the post-Stalinist period; it is also an attempt to outline the political position achieved by AK soldiers in ZBoWiD after 1956.

The present study discusses the issue of the supreme school authorities of the last decades of the 18th century in the First Polish Republic. The rules according to which the Commission of National Education (KEN) functioned as well as its political position are characterized. The main subject of the work is showing in what way the KEN was independent of the authorities of the state, owing to which we may talk about the institution as the first Ministry of Education. The origin of the Commission is discussed against the background of education reforms in selected European countries like Prussia, Austria and France. However, these countries did not establish so independent education departments as it was the case with Poland. Apart from this institutions are discussed that were parts of the KEN, and without which it would be impossible to effect the reforms, namely, the Society for Elementary Books and the office of General Inspectors. The former one was responsible for curricula and handbooks, but it often had to take over part of the duties of the very KEN. The inspectors were a kind of a link between the central education authorities and schools all over Poland; they also supervised effecting the reforms and passed the postulates formulated by the education system employees to the Commission. In the conclusion the effects of the KEN's work and its influence on the society harassed by the partitions are summarized. The whole allows the reader to have a closer look at the functioning of the schooling system of that time from the historical-legal perspective.

61-letnia kobieta została poddana zabiegowi trabekulektomii z powodu postępującej jaskry w lewym oku. 10 lat po zabiegu stwierdzono znaczące upośledzenie widzenia w tym oku. Badanie przedmiotowe wykazało hipotonię oraz postępującą wadę astygmatyzmu nadwzrocznego, będącą konsekwencją zbyt niskiego ciśnienia wewnątrzgałkowego. Dodatkowo stwierdzono obecność olbrzymiego wielokomorowego pęcherzyka filtracyjnego zniekształcającego gałkę oczną. Pacjentka została zakwalifikowana do chirurgicznej plastyki spojówki oraz torebki Tenona obejmującej okolicę przetoki twardówki. Po zabiegu uzyskano stopniową normalizację ciśnienia wewnątrzgałkowego, praktyczne ustąpienie wady astygmatyzmu nadwzrocznego oraz związaną z tym poprawę ostrości wzroku.
61-year-old women has undergone trabeculectomy in her left eye, because of progressive glaucomatous neuropathy. Ten years after the surgery a serious impairment of vision was observed. An examination has revealed hypotony and, as a consequence of this, progressive astigmatic hyperopy in this eye. Additionally, a large multicystic filtration bleb has been detected which caused deformation of the eyeball. The patient was qualified to plastic surgery of the conjunctiva and Tenon’s capsule. A gradual intraocular pressure normalization with practical relief of the eye defect and an improvement of visual acuity were obtained.

Infrastruktura tunelowa charakteryzuje się bardzo dużymi konsekwencjami potencjalnej awarii. Konieczne jest zatem właściwe kontrolowanie stanu jej deformacji poprzez pomiary wybranych wielkości fizycznych, np. odkształceń, temperatur i przemieszczeń, umożliwiających finalnie ocenę bezpieczeństwa budowy i eksploatacji danego tunelu. W artykule omówiono możliwości zastosowania punktowych pomiarów czujnikami strunowymi oraz geometrycznie ciągłych pomiarów światłowodowych DFOS. Współcześnie coraz częściej wykorzystuje się zalety obu tych technologii w celu stworzenia hybrydowych systemów monitorowania.
Very high consequences of potential failure characterise tunnel infrastructure. Thus, it is necessary to properly control its deformation state by measuring selected physical quantities, e.g. strains and displacements, enabling the safety assessment of a given tunnel during its construction or operation. The paper discusses the possibilities of spot measurements by vibrating wire gauges and geometrically continuous measurements by distributed fibre optic sensors (DFOS). Today, the advantages of both technologies are increasingly being combined to create hybrid monitoring systems.

Aggression is the growing problem in society. Therefore it is important to identify potential factors preventing aggression. In recent years scientists have observed decreased concentrations of omega-3 acids in people prone to aggression. This discovery suggested that the key to reducing aggression could be omega-3 acids supplementation. The aim of this study is to describe whether omega-3 acids consumption may result in reduction of aggressive behavior. Standard up-to-date criteria were followed for review of the literature data. A search for English-language articles in PubMed database was performed. Papers published between 2014 and 2017 were reviewed. In randomized study scientist discovered that omega-3 acids supplementation results in 42-68% reduction of parental complaints about aggressive behaviors in children. The improvement of behavior persisted for six months after cessation of treatment. Scientists in 2015 found out that in the prison population low levels of omega-3 acids in the blood correlate with more aggressive behavior. Patients in treatment group were characterized by more than 30% reduction in aggressive behaviors comparing to placebo group. In another review study we can notice the same conclusions. Aggression against others and impulsivity were lower in the group receiving omega-3 acids. A greater susceptibility to aggression has been observed among patients who have lower levels of omega-3 acids in their blood. Scientists have demonstrated positive omega-3 effects on the reduction of aggressive behavior. Omega-3 supplementation is safe and well tolerated and it can be used in combination with other methods of aggression treatment.

Opis:
Monitoring and structural health assessment are the primary requirements for performance evaluation of damaged bridges. This paper highlights the case-study of a damaged Reinforced Concrete (RC) bridge structure by considering the outcomes of destructive testing, Non-Destructive Testing (NDT) evaluations, static and 3D non-linear analysis methods. Finite element (FE) modelling of this structure is being done using the material properties extracted by the in-situ testing. Analysis is carried out to evaluate the bridge damage based on the data recorded after the static linear (AXIS VM software) and 3D non-linear analysis (ATENA 3D software). Extensive concrete cracking and high value of crack width are found to be the major problems, leading to lowering the performance of the bridge. As a solution, this paper proposes a proper Structural Health Monitoring (SHM) system, that will extend the life cycle of the bridge with minimal repair costs and reduced risk of failure. This system is based on the installation of three different types of sensors: Liquid Levelling sensors (LLS) for measurement of vertical displacement, Distributed Fiber Optic Sensors (DFOS) for crack monitoring, and Weigh in Motion (WIM) devices for monitoring of moving loads on bridge.

On the basis of our earlier studies in the group of 7,8-disubstituted derivatives of 1,3-dimetyl-3,7-dihydro-purine-2,6-dione, some new derivatives of theophylline were synthesized and tested for their electrocardiographic, antiarrhythmic and hypotensive activity as well as for α1-adrenoreceptor affinities. The performed preliminary tests indicated that new compounds did not significantly affect the normal ECG in vivo. As the result of present studies it may be concluded that all compounds did not possess hypotensive and arrhythmic activity. The results of the binding assays on α1-adrenergic receptors showed, that these derivatives display no affinity for α1-adrenergic receptors. Lack of antiarrhythmic and antihypertensive activity may result from the absence of affinity for the α1-adrenergic receptor. Performed chemical modifications lead to the loss of pharmacological activity previously observed among other derivatives in this group. Generally in comparison with the previously reported derivatives, replacing the phenoxyethylpiperazine by other moiety, changed the antiarrhythmic and hypotensive activity.

Opis:
Introduction and aim. Granulomatosis with polyangiitis (GPA), formerly known as Wegener’s granulomatosis, presents a formidable challenge in the realm of autoimmune diseases. Granulomatosis, characterized by vasculitis and granuloma formation, exhibits diverse clinical manifestations. The rarity of GPA is evident, with an estimated incidence between 0.4 and 11.9 cases per 1 million person-years. The aim of this report is to show the complex diagnostic challenges inherent in GPA, demonstrating the diagnostic process from initial symptoms. Description of the case. This case report unfolds the diagnostic journey of a 52-year-old Caucasian male. The presented case, initially suspected as a respiratory infection, led to a comprehensive investigation owing to persistent symptoms, abnormal blood counts, and elevated inflammatory markers. This narrative aims to depict the patient’s diagnostic journey. Key diagnostic tools include ANCA testing, imaging studies, and tissue biopsy. Pulmonary nodules, lymphangitic changes, and renal involvement culminating in a GPA diagnosis confirmed by positive ANCA and anti-PR3 antibodies. The successful management of this case involved a tailored therapeutic regimen, including cyclophosphamide and methylprednisolone, addressing both vasculitic and renal components. Conclusion. This case contributes to the understanding of atypical presentations of GPA, emphasizing the importance of a holistic and dynamic diagnostic approach.
