Workplace safety performance is computed using frequency rate (FR) and severity rate (SR). Only work time lost due to occupational incidents that need to be reported is counted. FR and SR are the 2 most important safety performance indicators that are applied universally; however, calculations differ from country to country. All injuries and time lost should be considered while calculating safety performance. The extent of severity does not matter as every incident is counted. So, a new factor has to be defined; it should be based on the hours or days lost due to each occupational incident, irrespective of its severity. The new safety performance factor is defined as the average human-hour unit lost due to occupational accidents/incidents, including fatalities, first-aid incidents, bruises and cuts. The formula is simple and easy to apply.

In comparison, railways are the safest mode of land transport, not only regarding pure safety performance indicators but also considering external costs to society and to a member state. As for Polish railways, the year 2016 was the safest since 2008 even despite the growth of the railway market. There are still elements that need to be taken care of, such as continuous improvement of safety culture so that there can be a consistency in safety statistics, and improvement of total safety performance. The other key aspect is the analysis of human factor with the involvement of professionals in the area of human performance since it seems that current activities are insufficient. Authors would like to summarise this short article regarding railway safety performance by saying that undoubtedly, railways are constantly the mass transport mode to choose regarding the safety of travel.

The use of roundabouts is well recognized for sustaining an efficient and safe intersection. However, the safety results may vary based on the prevailing conditions. Therefore, this study assesses the safety performance of roundabouts in Jordan. This study developed a predictive model by collecting and analyzing all accident records of 12 major urban roundabouts in the country over 3 years. For developing the model, this study employed an accident frequency analysis. The model calculated the rate of accidents and incorporated the geometric and operational characteristics of roundabouts. This was followed by ranking the safety performance of the roundabouts. It was concluded that driver behavior of violating the traffic rules, lack of clear lane markings in the circulating area and inadequate signage at the roundabouts entries are the main causes of roundabout accidents. The research recommends including the developed predictive model in future traffic control and planning studies, for identifying hazardous locations, or for prioritizing roundabout improvements based on safety performance.

Safety performance indicators are the parameters used for monitoring and assessing safety performance. Such factors are determined based on available safety databases, collected on government level (in Poland Civil Aviation Authority) or by aircraft operators. Aircraft system failure during different flight phases can cause an accident or an incident. Polish Civil Aviation Authority between other data bases manages two important ones called: European Coordination Centre for Aviation Incident Reporting Systems (ECCAIRS) and Aircraft Continuing Airworthiness Monitoring (ACAM).General Aviation (GA) operates mainly aircraft with MTOM<5700 kg powered by the single piston engine. At present, reliability of GA aircraft systems in Poland is unknown. Increasing size of this fleet in Poland requires taking necessary measures in order to establish safety risks and safety performance targets for GA fleet. The authors have performed processing of the data included in available databases analysing airframe failures based on criteria like: phases of flight, ATA chapters concerning aircraft systems and the category of occurrence. The goal of this article is to present method of the current reliability of GA aircraft systems assessment. The results of this analysis can support the decisions of supervisory authorities in the areas where security threats are most important also can help production organizations in identification of the aircraft systems, which required design changes.

Introduction. The majority of industrial accidents occur because of human errors. Human error has different causes, however, in all cases cognitive abilities and limitations of human play an important role. Occupational cognitive failures are cognitively-based human errors that occur at work. The aim of this study was to examine the relationship between occupational cognitive failures and safety consequences. Method. Personnel of a large industrial company in Iran filled out an occupational cognitive failure questionnaire (OCFQ) and answered questions on accidents. Univariate and multiple logistic regression analysis were used to determine the relationship between cognitive failures and safety consequences. Results. According to developed regression models, personnel with a high rate of cognitive failure, in comparison to low rate, have a high risk of minor injury involvement (OR 5.1, 95% CI [2.62, 10.3]); similar results were for major injury and near miss. Discussion. The results of this study revealed usefulness of the OCFQ as a tool of predicting safetyrelated consequences and planning preventive actions.

Under eccentric load, a single column pier bridge often overturns. In order to study the anti overturning performance of a single column pier bridge, taking the accident bridge in Wuxi as an example, a finite element model is established based on ABAQUS. According to the model simulation results, the relationship between the ultimate rotational load and overturning load of the accident bridge is obtained, and the ratio of the latter to the former is 1.75. Based on the model, the stress state, displacement state, and support state of the accident bridge under dead load, highway class I vehicle load, and accident vehicle load are obtained. Whether the strength and stability of the accident bridge under each load meet the service requirements is analyzed. In order to explore the differences among China, United States, and Japan specifications, the lateral stability of accident bridges is checked. It is found that the safety of the United States and Japan specifications is conservative, but the utilization rate of bridge traffic capacity is low. The safety of China specifications is slightly lower, but it can maximize the bridge’s traffic capacity and judge the ultimate overturning state of the bridge more accurately. The research results can provide technical references for the design and application of a single-column pier bridge.

Safety performance index is a tool with the potential to grasp the intangible domain of aviation safety, based on quantification of meaningful aviation safety system properties. The tool itself was developed in the form of Aerospace Performance Factor and is already available for the aviation industry. However, the tool turned out to be rather unsuccessful as its potential was not fully recognised by the industry. This paper introduces performed analysis on the potential and it outlines new features, utilising time-series analysis, which can improve both the recognition of the index by the industry as well as the motivations to further research and develop methodologies to evaluate overall aviation safety performance using its quantified system properties. This paper discusses not only the features but also their embedding into the existing approach for the development of aviation safety, highlighting possible deficiencies to overcome and relating the scientific work already performed in the domain. Various types of appropriate time-series methodologies are addressed and key specifications of their use with respect to the discussed issue concerning safety performance index are stated.

Starting from consideration that urban intersections are sites with promise for safety and operational improvements, the paper describes the steps taken to develop a crash predictive model for estimating the safety performance of urban unsignalized intersections located in Palermo, Italy. The focus is on unsignalized four-legged one-way intersections widespread in Italian downtowns. The sample considered in the study consist of 92 intersections in Palermo, Italy. For the study were collected crashes occurred in the sites during the years 2006-2012, geometric design and functional characteristics and traffic flow. Results showed that data were overdispersed and NB1 distributed. In order to account for the correlation within responses Generalized Estimating Equations (GEE) were used under different working correlation matrices.

Alongside the increase in air traffic and number of aircraft, the number of reported aviation events has also been increasing. We processed the data included in the European Coordination Centre for Aviation Incident Reporting Systems (RCCAIRS), analyzing small and large aircraft reliability and the safety of their operations, covering events according to ICAO aviation occurrence categories. Airframe systems are the largest contributor to the total number of reported events which occurred in Polish registered aircraft in the years 2008-2020. A detailed study of airframe systems reliability was carried out in order to assess the real reason for the failures. Airframe systems faults were assigned to specific ATA chapters and then to each of their sections. The results of this analysis may support the decisions of supervisory authorities in the areas where security threats are most important. They can also help aircraft operators with identification of the airframe units which require special attention. Identification of significant parts due to the frequency of malfunctions of particular system components may support designers. In short, these results are valuable in terms of further developments in statistical tools facilitating New Product Introduction (NPI).

System zarządzania bhp składa się z zestawu oddziałujących pomiędzy sobą procesów, a jego cel to zapobieganie wypadkom przy pracy i chorobom zawodowym oraz zapewnianie bezpieczeństwa w miejscu pracy. Przeprowadzono szereg badań, w ramach których dowiedziono, że realizacja wybranych pojedynczych procesów w sferze zarządzania bhp wywiera pozytywny wpływ na bezpieczeństwo zatrudnionych. Podjęto również wiele prób udowodnienia, że systemy zarządzania bhp są skuteczne, tj. spełniają swój cel. W badaniach tych stosowano różne miary skuteczności odnoszące się zarówno do całego systemu, jak i do poszczególnych procesów. Część badaczy posługiwała się wskaźnikami wypadków przy pracy, badała i analizowała zmiany w ocenie fizycznego i psychospołecznego środowiska pracy oraz zmiany w ocenie samopoczucia pracowników, a także wskaźniki w zakresie konkurencyjności i wyników ekonomicznych przedsiębiorstw. Inni opierali się w swoich badaniach na wynikach pomiarów poszczególnych procesów. Niemniej dotychczas przeprowadzone badania nie dały jednoznacznej odpowiedzi na pytanie, czy systemy zarządzania bhp wdrożone w przedsiębiorstwach są skuteczne ani jakie czynniki o tej skuteczności decydują. Bez wątpienia zapewnianie skuteczności systemów zarządzania bhp wymaga wdrażania, utrzymywania i doskonalenia w przedsiębiorstwach zestawu podstawowych procesów. Jednak istnieją odmienne poglądy na temat tego, które z tych procesów pełnią w systemach kluczową rolę. Według różnych naukowców i praktyków, skuteczność systemów zależy przede wszystkim od wiarygodnej i szczegółowej oceny ryzyka, od otwartej komunikacji i rzeczywistego zaangażowania pracowników, od wysokiej jakości szkoleń w zakresie bezpieczeństwa, od wnikliwego audytu i przemyślanych działań prewencyjnych lub od dojrzałego przywództwa w zakresie bhp. Proponuje się, aby - celem rozstrzygnięcia wątpliwości w tym zakresie - zastosować metodę rozmytych map kognitywnych. Wskazanie procesów w systemie, które w największym stopniu decydują o jego skuteczności ułatwi określenie priorytetów w obszarze doskonalenia systemów i może znacząco i pozytywnie wpłynąć na bezpieczeństwo zatrudnionych w przedsiębiorstwach, które wdrożyły systemy.
The Occupational Safety and Health Management System (OSH MS) consists of a set of processes that interact with each other, and its goals are to prevent injury and/or ill health to workers and to provide safe and healthy workplaces. A number of studies have been conducted, which proved that the selected occupational health and safety management processes have positive impact on the safety of employees. A the same time a lot of effort has been made to prove that OSH MSs are effective, i.e. meet their goals. In their studies, researchers applied different measures of effectiveness, relating to the entire system or individual processes: they used occupational injury rates, examined and analyzed changes in the assessment of the physical and psychosocial work environment and changes in the assessment of employees well-being, or took into account competitiveness and economic performance indicators. In some studies safety performance was measured by application of performance indicators for individual processes. Nevertheless, the research carried out so far did not give a definite answer to the question whether the occupational health and safety management systems implemented in enterprises were effective and which factors determined this effectiveness. Undoubtedly, ensuring the effectiveness of OHS MSs in enterprises requires the implementation, maintenance and improvement of a set of basic processes. However, there are different views about processes playing a key role in the systems. According to various scientists and practitioners, the effectiveness of systems depends primarily on a reliable and detailed risk assessment, on open communication and real employee involvement, on high quality safety training, on comprehensive audit and thorough preventive actions or on mature OSH leadership. In order to resolve this issue it is suggested to apply the Fuzzy Cognitive Map approach. Identification of processes that affect the system effectiveness most will facilitate the continual improvement of OSH MSs and can significantly increase employee safety in enterprises.

The introduction of anions and cations into energetic materials can significantly improve their physicochemical and energetic properties. The K, Zn, Ba, Cu and Pb salts of 1,1,2,2-tetranitraminoethane (TNAE) were synthesized using TNAE as the starting material, and were characterized by IR spectroscopy, NMR spectroscopy and elemental analysis. All of the energetic salts exhibited relatively high thermal stabilities (decomposition temperatures ranging from 195.1 °C to 279.3 °C) and high densities (ranging from 2.1 g·cm–3 to 3.7 g·cm–3). It was demonstrated that the introduction of metal ions conveys better thermal stability than covalent TNAE itself. More importantly, the Zn, Ba and Cu salts exhibited good thermal stability, high density, low sensitivity, and therefore have the potential to be a new class of insensitive, highly energetic explosives for practical applications.

This paper attempts to replicate a safety climate model originally tested in Australia to assess its applicability in a different context: namely, across production workers in 22 medium-sized metal processing organizations in Austria. The model postulates that safety knowledge and safety motivation mediate the relation between safety climate on the one hand and safety compliance and participation on the other. Self-report data from 1075 employees were analyzed using structural equation modeling (SEM). The results of the replication study largely confirmed the original safety climate model. However, in addition to indirect effects, direct links between safety climate and actual safety behavior were found.

As in every industry at risk, the human and organizational factors constitute the main stakes for maritime safety. Furthermore, several events at sea have been used to develop appropriate risk models. The investigation on maritime accidents is, nowadays, a very important tool to identify the problems related to human factor and can support accident prevention and the improvement of maritime safety. Part of this investigation should in future also be near misses. Operation of ships is full of regulations, instructions and guidelines also addressing human factors and safety culture to enhance safety. However, even though the roots of a safety culture have been established, there are still serious barriers to the breakthrough of the safety management. One of the most common deficiencies in the case of maritime transport is the respective monitoring and documentation usually lacking of adequacy and excellence. Nonetheless, the maritime area can be exemplified from other industries where activities are ongoing to foster and enhance safety culture.

The construction industry in India is the second most important contributor to its gross domestic product. However, high rate of accidents and fatalities have tarnished the image of industry in India. Although the industry contributes significantly to the Indian economy, safety management is the primary concern alongside with frequent workplace accidents. The role of safety management is vital to improve safety performance of an organization. The functions of safety management include planning, organizing, staffing, directing, controlling and coordinating safety activities with an aim to minimize accidents/injuries. Safety trainings, employee participation, compliance of safety procedures and motivational schemes are part of safety management which influences the overall safety performance. Several metrics were developed to measure the safety performance of an organization but not a single measure will reflect the overall performance. The present study considered parameters pertaining to the safety management which have influence on the safety performance of a construction organization in India. The parameters are analyzed by formulating a goal programming model. The results of the study suggests that much improvement is needed in the area of safety trainings and the revised targets were established.

Macroergonomics, which emerged historically after sociotechnical systems theory, quality management, and ergonomics, is presented as the basis for a needed integrative methodology. A macroergonomics methodology was presented in some detail to demonstrate how aspects of microergonomics, total quality management (TQM), and sociotechnical systems (STS) can be triangulated in a common approach, in the context of this methodology, quality and safety were presented as 2 of several important performance criteria. To demonstrate aspects of the methodology, 2 case studies were summarized with safety and quality performance results where available. The first case manipulated both personnel and technical factors to achieve a "safety culture" at a nuclear site. The concept of safety culture is defined in INSAG-4 (International Atomic Energy Agency, 1991). as "that assembly of characteristics and attitudes in organizations and individuals which establishes that, as an overriding priority, nuclear plant safety issues receive the attention warranted by their significance." The second case described a tire manufacturing intervention to improve quality (as defined by Sink and Tuttle, 1989) through joint consideration of technical and social factors. It was suggested that macroergonomics can yield greater performance than can be achieved through ergonomic intervention alone. Whereas case studies help to make the case, more rigorous formative and summative research is needed to refine and validate the proposed methodology respectively.
