- Introduction: The term “silent sinus syndrome” refers to a rare disease occurring within the maxillary sinus. Pressure changes (underpressure) in the bay affect the reconstruction of its walls causes sinus walls to collapse in the form of, eg. lowering the bottom of the eye socket, which causes the eyeball to move downwards, thus disturbing visual acuity It is the asymmetry of the face and vision problems that are the main ailments of the patient who come to the doctor's office, and the diagnostic and treatment process requires the involvement of many specialists. Aim: The aim of the study is to present a case of a patient with silent maxillary sinus syndrome. Case report: Patient S.K., 45 years old, came to an ophthalmologist with symptoms of visual acuity and double vision disorders, he also complained about a clear asymmetry caused by the collapse of the right eyeball. The craniofacial computed tomography (CT) revealed that the right maxillary sinus was completely filled with thickened mucosa/fluid, with obstruction of the mouth-ductal complex. The volume of this sinus was significantly reduced with the lowering of the lower orbital wall and the collapse of the posterior wall of the sinus, the right-sided endophtalmus, the evolution of the so-called silent bay. Further ENT treatment was recommended. In 2019, the right maxillary sinus was operated using the endoscopic method. The patient endured the procedure well, the postoperative course was uneventful. Conclusions: Silent sinus syndrome is a sporadic disease, the characteristic symptom of which is spontaneous lowering of the orbital floor and the collapse of the eyeball. The primary treatment method is endoscopic surgery of the paranasal sinuses. In the diagnostic and treatment process, the cooperation of many specialists is important – ophthalmologists, endocrinologists, dentists, maxillofacial surgeons and ENT specialists. In the case of unilateral changes in the paranasal sinuses, the development of a neoplastic disease should be excluded.
