Opis:
The health of homeless people is a serious public health and social problem, which is enhanced by the lack of interest of homeless people to care about their own health. Homeless people are more likely to succumb to various diseases. It is a consequence of their lifestyle, lack of hygiene, diet, drinking too much alcohol, not attending regular medical preventive examinations, mandatory vaccinations, and so on. Homeless people seek medical help late when their illness is unbearable, or when they be- come victims of an accident or a violent crime. The submitted paper in the theoretical and practical part explains the issue of the health status of homeless people. The aim of the paper is to identify the presence of diseases among homeless people and the mutual relationship between their health status and reintegration into the mainstream society. The research part of the paper presents the results of a survey conducted within the Streetwork project in Trnava through qualitative research. The research file consists of 10 homeless people aged 22 to 52 living in the city of Trnava. The group of participants includes 6 women and 4 men. The results of the survey showed the unfavorable health status of ho- meless people. The conclusion of the paper classifies the recommendations for practice directed to streamline health care of homeless people.

In the Second Republic of Poland health protection was insufficiently developed. There were very few doctors, nurses and hospitals. Only people who were insured or the wealthy had access to medical treatment. The majority of citizens, especially rural resi-dents, could not use health care. The health policy of the state was passive for many years. Changes appeared only in the mid-thirties. The aim of health policy was a public health service for all citizens. The authorities declared their support for this idea on a number of occasions and the parliament accepted the law of public health in June 1939. It was the basis for the construction of common access to healthcare in Poland. To achieve this aim, health centres were built and the compulsory medical practice for the doctors was introduced in villages. The beginning of the Second World War prevented the enforcement of this Act. However, the attempt to create a public health service made in the Second Republic of Poland was a proof of progress which has been made in state health policy.

Health inequities are defined as systematic differences in health that can be avoided by appropriate policy intervention, and for this reason are considered unfair and unjust. Health inequities are not solely related to access to health care services; they are caused by the unequal distribution of these determinants of health, including power, income, goods and services, poor and unequal living conditions, and the differences in healthdamaging behaviours that these wider determinants produce. They are defined as systematic differences in health that can be avoided by appropriate policy intervention and that are therefore deemed to be unfair and unjust. To be able to devise effective action, we first need to understand the causes of these inequities in health. Health inequities are not solely related to access to health care services; there are many determinants related to living and working conditions, as well as the overall macro-policies prevailing in a country or region. The differences in social and economic development are reflected in health inequities that can be seen both between and within countries. Furthermore, evidence shows that even in the more affluent countries health inequities are seen in all parts of Europe. In the WHO European Region the gap in life expectancy between countries is 17 years for men and 12 years for women. Inequities in health are caused by the unequal distribution of these determinants of health, including power, income, goods and services, poor and unequal living conditions, and the differences in health-damaging behaviours that these wider determinants produce. The experiences of various countries indicates that in order to narrow the health inequities countries have to improve living conditions including the provision of comprehensive welfare systems, and high-quality education and health services. The Strategy Health 2020 developed and approved by the WHO European Region countries is focusing on reducing inequities in health, which are key strategic objectives of endorsed by the 53 Member States. It emphasizes the need to strengthen population-based prevention on the social determinants of health. Also, in 2009 the European Commission developed European Union (EU) Health Strategy Programme titled “Solidarity in health: reducing health inequalities in the European Union”.

Longevity can be perceived as increasing life expectancy in a population or as the long lifespan of an individual. Longer lifespans and decreasing fertility rates in many countries will cause societies to rapidly grow old. This paper is an attempt to define the determinants of the ‘out-of-pocket’ medical expenditures of the elderly. In this research statistical tests and micro-econometric modelling were carried out on a sample of the Polish population. The results of the analysis indicate that the elderly with their elevated needs for medical assistance spent more on the health care, than the rest of population (in the absolute value and in relation to income) and that their expenditures are constantly increasing. The main limitation to households’ health care expenditures is the income, which suggests that there may be an unrealized demand for medical services and products, especially medical drugs that cannot be obtained entirely free of charge. This highlights a new direction for the development of Poland’s health care policy; otherwise, longevity may be a luxury that not everyone can afford.

Opis:
Purpose: the main objective of the article is to support building sustainable development of spa towns by protecting their unique resources. Design/methodology: the theses presented in the article were verified using: critical analysis of literature, document analysis, descriptive statistical analysis. Findings: on the basis of the article it can be concluded that demographic change is an important challenge for numerous areas of public policy, such as family, health care, labour market, social security, education and active leisure. The identified challenges and areas of their impact are part of a broader spectrum of reflection that should focus on a holistic approach to active policies, which are a key determinant of human capital – in which, as forecasts indicate, by 2050 people over sixty-five years of age may constitute 20% of the total population of Europe. Originality: the article enriches knowledge and develops discussion on demographic changes and the resulting great contemporary challenge for environment, institutions and people creating the future of the studied areas, which may turn out to be maintaining the professional activity of the ageing population while building modern active social and labour market policies – corresponding to the economic structure of the studied areas.

Artykuł poświęcony został procesom kształtowania się systemu ochrony zdrowia w Polsce w pierwszych latach po zakończeniu II wojny światowej. Ramy czasowe artykułu obejmują okres od wyzwolenia części ziem polskich i przejęcia władzy przez Polski Komitet Wyzwolenia Narodowego do roku 1950, gdy zakończone zostały zasadnicze przemiany w organizacji systemu ochrony zdrowia w Polsce. From the Second Polish Republic to the People’s Poland. The evolution of the health care system in Poland in the years 1944–1950First years after the Second World War were very difficult for the health care in Poland. The end of German occupation brought a wide range of challenges. They resulted from war damage, a significant loss of medical staff and a large scale of health risks. One of important goals of the health policy was to create the public health service available to all citizens. Initially, the restoration of the health care system was based on the model created in the interwar period. With time, along with political changes taking place in Poland, the transformation of the system began to adapt it to the centrally planned economy. The main part of this process ended in 1950. The new system was compatible with the Soviet model and was based on central and directive management which included all elements of the so-called “social health service”. The private medical practice was pushed to the margin and insurance health service was taken over by the state. The system built after the war, however, was still not widespread. Most of the rural population, which represented nearly half of the country`s population, were deprived of equal access to health care.

Opis:
The article focuses on the process of shaping social policy in the European Economic Community as well as the factors conditioning it in the fields of family support, education and public healthcare. It covers the period from the inception of the EEC to the signing of the Treaty on European Union in 1992 in Maastricht. The characteristic feature of Community social policy in mentioned-above areas was monitoring global trends in social issues and determining the EEC desired direction of aspirations along with defining general standards in the area of social policy. It was concluded that the further harmonious development of the Community would not be possible without making arrangements to solve problems common to all the Member States, such as unfavorable demographic projections, the need for adjusting the methods of education and training to the changing trends in the labor market or the epidemiological threats to the health of European citizens.

The aim of this article is to show the main areas of the activity of the Provisional Government and its bodies related to social policy and to present the most important legal and organizational decisions concerning social policy, made in the first half of 1945. During the existence of the Provisional Government there was a necessary reconstruction of social policy institutions, which involved the restoration of the traditional functions they had carried out before the war. The Provisional Government provided a strong impulse to the unification of the applicable legal basis of social policy. At that time, important legal changes were also made. They aimed at winning the support of the society and they heralded the political transformations which were to take place in the second half of the 40s. As time showed, the fundamental organizational changes concerning the structures that managed respective sections of social policy, and which were brought about by the Government, took the same directions. The creation of the Ministry of Health and assigning the responsibility of taking care of children and youngsters to the Ministry of Education were important steps towards the centralization, nationalization and planning in both spheres which were under the influence of the state and which were reomved from the competences of the department of labour and social welfare. That way, not only did the Provisional Government play an important role in the restitution of Polish social policy and its institutions (which had been initiated in the period of the Polish Committee of National Liberation) but also in the creation of the future solutions related to Stalinization.
Celem niniejszego artykułu jest ukazanie podstawowych kierunków aktywności Rządu Tymczasowego i jego organów w ramach polityki społecznej oraz przedst awienie najważniejszych spośród podejmowanych w pierwszym półroczu roku 1945 decyzji prawnych i organizacyjnych z nią związanych. W okresie funkcjonowania Rządu Tymczasowego następowała niezbędna odbudowa instytucji polityki społec znej połączona z przywracaniem im tradycyjnych pełnionych przed wojną funkcji. Rząd Tymczasowy dał silny impuls do unifikacji obowiązujących w kraju podstaw prawnych polityki społecznej. Pojawiły się także w tym czasie istotne zmiany prawne. Były one nastawione na uzyskanie poparcia społecznego, ale stanowiły też z apowiedź przyszłych przeobrażeń systemowych, mających nastąpić w II połowie lat czterdziestych. W tym samym kierunku szły, jak się miało okazać, zasadnicze zmiany organizacyjne struktur zarządzających poszczególnymi działami polityki społecznej. Wyodrębnienie Ministerstwa Zdrowia oraz przekazanie Ministerstwu Oświaty zadań w zakresie opieki nad dziećmi i młodzieżą oznaczało ważny krok ku centralizacji, etatyzacji i planowaniu w obu wyodrębnionych z kompetencji resortu pracy i opieki społecznej sferach oddziaływania państwa. W ten sposób Rząd Tymczasowy odegrał istotną rolę nie tylko w zainicjowanej jeszcze w okresie Polskiego Komitetu Wyzwolenia Narodowego restytucji polskiej polityki społecznej i jej instytucji, ale także w kreowaniu przyszłych związanych ze stalinizacją rozwiązań.
