Introduction: Breast cancer is the most common malignant tumor in women in the Polish region. Surgery is a basic method of breast cancer treatment. Surgery often carries a lot of unwanted changes as follows: limitation of mobility in the shoulder joint on the operated side, secondary lymphoedema, post mastectomy pain syndrome (PMPS), reduction of muscle strength or disorders in body posture. Therefore, the implementation of physiotherapeutic activities that are designed to prevent and eliminate postoperative complications seems very important. The main aim of this work was to present physiotherapeutic management in women after mastectomy based on the analysis of available literature. The physiotherapeutic process can be divided into three periods: early hospital, early ambulatory and the late ambulatory period. In the first period, active slow exercises, self-support of the upper limb on the operated side and breathing exercises on the thoracic track are used to prevent circulatory disorders, pulmonary complications, and edema. The early ambulatory period includes corrective exercises, general improvement exercises, stretching and learning of automatic massage of the upper limb of the operated side. The last period should be enriched by recreational methods of physical activity such as swimming, cycling or Nordic walking to maintain physical fitness, proper mobility of the shoulder girdle and improve the patient's psychophysical state. It is very important the patient regular continues the rehabilitation program after curing of breast cancer as well. In the case of secondary lymphoedema of the upper limb, comprehensive rehabilitation physiotherapy is used, consisting of manual lymphatic drainage, healing exercises, compression therapy, and skin care. Conclusions: Physiotherapy in women after breast cancer surgery is a complex and long-term process. Physiotherapeutic methods are effective in treating complications after surgery of breast cancer surgery. It is necessary to constantly update the physio-therapy knowledge in women after breast cancer surgery.

Rehabilitative modalities such as incentive spirometry and physiotherapy interventions, e.g. aerobic exercises, have been shown to produce positive outcomes in patients with sickle cell disease (SCD). Hemispheric CVA and other complications arising from SCD are amenable to physiotherapy. There have been few studies on the effectiveness of physiotherapy in the management of individuals with SCD. The present study attempts to assess the SCD-related knowledge, attitude and level of participation of physiotherapists in the Nigerian healthcare system. It also examines a possible association between the foregoing and practice settings. The study was carried out among physiotherapists in South-West Nigeria registered on professional e-platforms. The respondents filled in an adapted electronic version of a questionnaire. The IBM SPSS 20 was used for statistical analysis, and a chi-square test was used to determine an association between the levels of knowledge, practice settings, and participation among the respondents (p < 0.05). The study findings revealed that only 19% of respondents had received a specific training related to the care of SCD patients, although 65% had been involved in the treatment of at least one patient with SCD. Only 29.5% of respondents had “Good Knowledge” of SCD pathophysiology. A statistically significant association (χ2 = 75.357, p = 0.012) was found between respondents’ level of knowledge and their practice settings, i.e. a teaching hospital mostly. The physiotherapists from the Nigerian South-West,mostly those working in teaching hospitals, reported an average level of knowledge of SCD care. Since the management of sickle cell disease is of multidisciplinary character and requires an all hands-on deck approach, controlled trials should be undertaken by physiotherapy researchers regarding the effects of modalities and interventions on pro-inflammatory bio-molecules.

Objectives This paper assesses the effect of neurophysiological rehabilitation in children with postural defects on the depth of thoracic kyphosis, lateral spinal deviation and rotation of spinal motor segments. Material and Methods A total of 201 patients aged 8–15 years old with a postural defect diagnosed by medical examination were enrolled. The analyzed parameters were determined using the DIERS system before the first therapeutic session and after 4 weeks of therapy. The angle of thoracic kyphosis, lateral deviation of the spine and spinal rotation were assessed. The therapy employed techniques associated with the proprioceptive neuromuscular facilitation (PNF) and Vojta’s approaches. The results were analyzed separately for both sexes and for patients rehabilitated solely with Vojta’s techniques vs. patients rehabilitated according to combined Vojta’s and PNF techniques. The χ² test was used for statistical analyses, at p < 0.05. Results There was improvement in the angle of thoracic kyphosis, ranging from 0.14 (among boys with kyphosis < 42°) to 5.47 (among girls with kyphosis ≥ 42°), spinal rotation, from 0.37 (among boys with kyphosis ≥ 42°) to 4.33 (among patients with kyphosis ≥ 42° rehabilitated solely according to Vojta’s method), and lateral deviations, ranging from 1.32 mm (among boys with kyphosis < 42°) to 2.99 mm (among patients with kyphosis ≥ 42° rehabilitated solely according to Vojta’s method). Conclusions Neurophysiological rehabilitation of patients with postural defects produced positive effects by improving the angle of thoracic kyphosis, spinal rotation and lateral deviation of the spine. Children with reduced thoracic kyphosis achieved less improvement in the kyphosis angle, lateral spinal deviation and spinal rotation than children with kyphosis ≥ 42°. Konsekwencje braku aktywności fizycznej oraz siedzącego trybu życia coraz większego kręgu ludzi stanowią obciążenie systemu zdrowia publicznego w skali światowej. Niedostateczna ilość ruchu sprzyja powstawaniu wielu chorób, uciążliwych dolegliwości, obniżeniu jakości i długości życia. Trening marszowy bazuje na naturalnym ruchu, jakim jest chód. Przeprowadzono wiele badań wskazujących na korzyści płynące z treningu nordic walkingu w poszczególnych jednostkach chorobowych. Aktywność fizyczna ma działanie terapeutyczne wówczas, gdy forma ćwiczeń ruchowych zostanie dobrana stosownie do konkretnego przypadku. Poleca się wcześniejsze wykonanie odpowiednich badań oraz ustalenie właściwej intensywności treningu. W artykule dokonano przeglądu publikacji naukowych dotyczących wpływu nordic walkingu na zdrowie, opublikowanych w recenzowanych czasopismach, z wykorzystaniem baz PubMed, NCBI, PMC, ScienceDirect, Oxford Academic Journals i MDPI.
A lack of physical activity and a sedentary lifestyle are a burden to the public health system throughout the world. An insufficient amount of exercise contributes to the development of conditions, troublesome ailments, and a reduction in the quality and length of life. Walking training is based on the natural movement of walking. There have been many studies showing the benefits of Nordic Walking training for individual conditions. Physical activity has a therapeutic effect if the form of physical exercise is matched properly to the specific case. It is recommended that appropriate tests be carried out in advance to determine the appropriate intensity of training. Scientific articles published in peer-reviewed journals were reviewed. The online databases PubMed, NCBI, PMC, ScienceDirect, Oxford Academic Journals and MDPI include research and review papers written in Polish and English.

Rehabilitacja małych dzieci musi być dostosowana do ich potrzeb oraz możliwości, a także powinna w sposób wszechstronny uwzględniać ich odrębność strukturalną i czynnościową wynikającą z wieku. Leczenie dysfunkcji kończyn dolnych przy minimalnej świadomości dziecka podczas zabawy podnosi atrakcyjność rehabilitacji, zwiększając zainteresowanie małego pacjenta oraz umożliwia dłuższe wykonywanie różnych czynności nie powodując znudzenia. Koncepcję rowerku wykonano w programie Autodesk Inventor.
This paper presents the concept of a stationary, interactive rehabilitation bicycle for children below the age of three. The rehabilitation of small children has to be adjusted to their needs and possibilities and it should comprehensively take into account children’s structural and functional diversity related to their age. The treatment of dysfunction of lower limbs while the child is barely aware of it during play, increases the attractiveness of rehabilitation and thus child’s engagement and allows for longer exercises that do not cause boredom. The concept of the bicycle has been prepared with the use of Inventor software.

Rehabilitation of people with disabilities is a process aimed at enabling them to reach and maintain their optimal physical, sensory, intellectual, psychological and social functional levels. WHO, in “World Report on Disability”, defines rehabilitation as a set of measures that assist individuals who experience, or are likely to experience, disability to achieve and maintain optimal functioning in interaction with their environments. Lack of rehabilitation services creates a barrier to full inclusion and participation in all aspects of life. The aim of this article was to present the nature of medical rehabilitation, in particular medical rehabilitation of elderly people. The basic definitions of medical rehabilitation and physiotherapy were also presented. The medical rehabilitation system in Poland, its organization and funding system was shown based on publications and statistical data of the Central Statistical Office (GUS) as well the access to medical rehabilitation services and selected medical procedures was assessed based on available data from National Health Fund (NFZ) and Ministry of Health (MZ).

One of the main priorities of stroke rehabilitation is regaining patients’ independence in basic everyday activities. This paper is aimed at assessing impact of early stroke rehabilitation on hip joint mobility of the affected leg in patients after cerebrovascular accidents. The study included 30 subjects (13 men and 17 women) aged on average ±66.1, with hemiparesis, who were treated at the stroke unit and participated in a 4-week rehabilitation program. Measurements of passive and active motion ranges were conducted with a goniometer in both limbs: healthy and affected ones by stroke. There were statistically significant differences in motion ranges between healthy and affected limbs. Examination I revealed that affected limbs amounted to only 40% of physiological range, while in Examination II, the result reached 73%. The most significant motion limitations were noted in terms of bending, adduction and internal rotation, while the least significant in terms of external rotation and abduction. Exercises used during early stroke rehabilitation of hemiparesis patients considerably increased active motion ranges and maintained passive motion ranges in the hip joint of the affected limb. Patients with right-sided hemiparesis experienced much better improvement in hip joint motion of the affected limb. The 4-week period was insufficient to mobilize patients and help them regain full active mobility in the hip joint of the affected limb. These patients required further physiotherapy, until they fully regained functionality.

Evidence-based medicine is the current undisputed predominant paradigm within medicine and allied health care. Guidelines for standardized reporting of research findings have facilitated critical evaluation of the relevant research literature. In addition, systematic reviews of the literature made available through computerized databases allow even busy clinicians and researchers rapid access to current best evidence. Despite the potential benefits of Evidence-Based Medicine (EBM) to clinical practice, over the years various points of criticism with respect to EBM have been formulated. This article provides a critical appraisal of the EBM paradigm discussing perceptions of EBM as cookbook medicine, inconsistency and contradiction in research findings, a proposed research pyramid not necessarily emphasizing the randomized controlled trial, a conceptual framework more relevant to the clinical and research needs of rehabilitation medicine and physiotherapy, the role of and impact on patients within the EBM paradigm, implementation of EBM, but also the current lack of evidence for increased efficacy of patient management based on EBM. The research base used in the EBM paradigm to support clinical decision-making is still far from complete. Demonstrating scientific evidence for EBM is a difficult task. Yet the EBM movement is of great importance for rehabilitation and physiotherapy to allow for increased transparency of care. The purpose of promoting this paradigm is optimum quality of care with conservation of professional autonomy.

Contemporary rehabilitation, which plays a key role in complex patient care, is precisely described. Culture, ethics and human-ism also play an important role (which can, however, be understood in various ways), including a role in determining the need to achieve the best possible therapeutic effects. Therefore, we can assume that the method of rehabilitation treatment is a cultural value (asset) serving the generally acknowledged idea of caring for each person’s proper functioning in society. The above-mentioned cultural value contains within itself the personality structure of its creator. In practice, the decision about selecting the method of rehabilitation is taken by the person responsible for and managing the patient. The decision, made after analysis of available knowledge, is also based on intuition. Analysis of therapists’ professional development pathways allows us to identify typical professional behaviours. It is important that the whole rehabilitation team conducts itself in a humanitarian way in the treatment process. The sum of positive humanitarian behaviours constitutes the basis for achievement of success in the field of improvement of motor activity. The rehabilitation team, consisting of physicians and physiotherapists, together with psychologists, learning support assistants and occupational therapists creates conditions for physically impaired persons to return to full functioning in society. The process occurs by improving motor efficiency in parallel with increasing physical fitness but also by generating and consolidating psychomotor features that enable patients’ recovery and their full acceptance by society.

The current spread of SARS-CoV-2 indicates a long-term fight against the widespread and exponential increase in morbidity and mortality across the globe. A variety of non-pharmacological strategies to mitigate and suppress virus transmission have been investigated and introduced. Currently, emerging studies focus mostly on the management of hospital-treated patients in the acute phase of the disease, including the legitimacy of using physiotherapeutic procedures. However, current literature lacks guidelines for rehabilitation related to maintaining continuity and universality of the therapy after the end of the acute phase of the disease and discharge from hospital. The authors suggest implementing an immediate rehabilitation program in post-infection patients as data from previous epidemics of respiratory-related viral diseases shows that COVID-19 survivors should be expected to have impaired lung ventilation function, and reduced exercise tolerance and muscular weakness, and prolonged return to work and participation. It should be assumed that only the introduction of immediate recommendations for the implementation of rehabilitation procedures based on simple and well-known tests, as well as their obligatory regime, can contribute to the reduction of respiratory disability leading, in a short time, to infections recurrence and, in the long run, to a lower quality of life and socioeconomic burden on the population. This article presents a respiratory rehabilitation program for COVID-19 survivors, recommended by the Polish Society of Physiotherapy. This program was approved by the Polish Minister of Health and implemented as a pilot program at the Hospital of the Ministry of the Interior and Administration in Głuchołazy, Poland. Opis:
The number of reports describing the effectiveness of comprehensive rehabilitation in multiple sclerosis (MS) is systematically growing. The latest scientific data reported the essential difference in health state between those patients who underwent rehabilitation and those who didn’t. The advantage of inpatient rehabilitation against outpatient has been proved. The correlation between disability and quality of life has been also noticed, as well as influence on burden of patient’s caregivers. Multiple sclerosis is associated with a variety of symptoms and functional deficits that result in a range of progressive impairments and handicap. Goals of rehabilitation are: to give the management of symptoms and to improve general fitness due to aerobic exercise. First goal can be achieved using facilitation – impairment- based approach, while the second one by use of a new strategy: task- oriented – disability- focused approach. First is the treatment of the main symptoms of MS: fatigue, bladder and bowel disturbances, sexual dysfunction, cognitive and affective disorders, and spasticity. Even though these symptomatic therapies have benefits, their use is limited by possible side effects. Moreover, many common disabling symptoms, such as weakness, are not amenable to drug treatment. However, rehabilitation has been shown to ease the burden of these symptoms by improving self- performance and independence.
For these aims the comprehensive multidisciplinary rehabilitation is necessary. Even though rehabilitation has no direct influence on disease progression, studies to date have shown that this type of intervention improves personal activities and ability to participate in social activities, thereby improving quality of life. Since 1890, when Wilhelm Uhthoff for the first time described that increased body temperature from physical exertion may lead to transient impairment of vision in patients with MS, the Uhthoff phenomenon has been defined the strategy of rehabilitation procedures in MS. Heat therapy and physical exercises were restricted because of fear against flare up of disease, aquatic exercises were limited till 30°C of water temperature. Last years something has been changed. Few reports on randomised controlled trials about progressive exercise program, mainly consisting of resistance training for few months has been published. Findings from those studies suggest overall disability and mobility improvement with the aerobic training. All types of rehabilitation should be tailored to fit patient specific needs. Based on the Uhthoff phenomenon, therapeutic trials with cooling vests are investigated and developed. In this review report of the contemporary trends in comprehensive rehabilitation in MS has been presented.

Introduction: A frequently observed health problem among people working with the voice is vocational failure “phonasthenia”. Determining the level of functional voice activity is important for the quality and effectiveness of interpersonal communication in everyday situations and verbal interaction of people working in occupations in which voice and speech are the basic tools of work. The aim of the study was to evaluate the results of the rehabilitation of voice disorders in selected professional groups. Material and methods: The study participants comprised 14 women between the ages of 31 and 48 (mean age: 39.43) professionally working with voice. All participants were subjected to an examination of the hyoid bone, the mobility of the larynx in relation to the mandible and in relation to the cervical spine were performed, the body posture was assessed, the phonation time was recorded and the breathing track was evaluated. A standardized Voice Handicap Index (VHI) scale was used. Respiratory and phonatory rehabilitation was performed and manual methods were used to lower the tension of the larynxmuscles. The patients were examined twice before physiotherapy and after 2 weeks of individuals exercise. Results: In the first study, all women included in the study had an increased tonus of the neck muscles during phonation and an abnormal airway was observed. After the applied physiotherapy, the muscle tone of the neck during the phonation significantly improved in 6 (42%) patients, in 12 (85%) the change of the rib-diaphragmatic breathing track was registered. The MPT result in the first study was on average 17.4 s, while in the second 21.6 s. The results obtained with the VHI questionnaire before therapy were on average 14.14 points, and in the second study they decreased to 10.78 points on average. Conclusions: 1. The tension of the neck muscles significantly influences the time of phonation. 2. The correction of the respiratory tract in people working with the voice positively affects its functional, emotional and physical aspects in the evaluation of the Voice Handicap Index scale.
Wprowadzenie: Często obserwowanym problemem zdrowotnym wśród osób pracujących głosem jest zawodowa niewydolność głosu (fonastenia). Określenie poziomu jego aktywności funkcjonalnej ma istotne znaczenie dla jakości i skuteczności komunikacji interpersonalnej w sytuacjach codziennych oraz dla interakcji werbalnej osób pracujących w zawodach, w których głos i mowa są podstawowymi narzędziami pracy. Celem badań była ocena wyników rehabilitacji zaburzeń głosu w wybranych grupach zawodowych. Materiał i metody: Materiał badany stanowiło 14 kobiet w wieku 31-48 lat (średnia: 39,43 lat), zawodowo pracujących głosem. U wszystkich wykonano badanie kości gny- kowej, ruchomości krtani względem żuchwy i względem kręgosłupa szyjnego, dokonano oceny postawy ciała, zarejestrowano czas fonacji oraz przeprowadzono ocenę toru oddychania. Wykorzystano standaryzowany kwestionariusz Voice Handicap Indeks (VHI). Przeprowadzono rehabilitację oddechową, fonacyjną oraz zastosowano metody manualne obniżające napięcie mięśni krtani. Pacjentki zostały dwukrotnie zbadane - przed fizjoterapią oraz po 2 tygodniach samodzielnych ćwiczeń.Wyniki: W pierwszym badaniu u wszystkich kobiet odnotowano zwiększony tonus mięśni szyi podczas fonacji oraz zaobserwowano nieprawidłowy tor oddechowy. Po zastosowanej fizjoterapii napięcie mięśni szyi podczas fonacji znacząco uległo poprawie u 6 (42%) pacjentek, u 12 (85%) zarejestrowano zmianę toru oddychania z piersiowo-żebrowego na żebrowo-przeponowy. Wynik MPT w badaniu pierwszym wynosił średnio 17,4 s, natomiast w drugim 21,6 s. Uzyskane wyniki za pomocą kwestionariusza VHI przed terapią wynosiły średnio 14,14 pkt., a w drugim badaniu zmniejszyły się średnio do 10,78 pkt. Wnioski: 1. Napięcie mięśni szyi znacząco wpływa na czas fonacji. 2. Korekcja toru oddechowego u osób pracujących głosem pozytywnie wpływa na jego aspekt funkcjonalny, emocjonalny oraz fizyczny w ocenie kwestionariuszem VHI.
