Opis:
The article analyzes foundations associated with risk management. The research part is based on a potential risks identification analysis, impact and probability of occurrence and significance assessment for the company performing multimodal transportation services of intermodal transport units (ITU). For this purpose a risk map analysis method was used. Identification of the twenty-four most significant threats led to the identification of the most important elements of the process of ITU carriage, which proper handling allows to minimize possible losses.
W artykule przeanalizowano zagadnienia związane z podstawami zarządzania ryzykiem. Część badawczą oparto na analizie identyfikacji potencjalnego ryzyka, oceny skutków i prawdopodobieństwa wystąpienia oraz oceny istotności dla przedsiębiorstwa realizującego usługi transportu zintegrowanych jednostek ładunkowych (ZJŁ) w przewozach międzygałęziowych. W tym celu posłużono się metodą mapy ryzyka. Wyodrębnienie dwudziestu czterech najbardziej istotnych zagrożeń pozwoliło na identyfikację najważniejszych elementów procesu przewozu ZJŁ, których prawidłowe wykonanie pozwala na minimalizację możliwych strat.

Opis:
The introduction of increasingly strict rules related to the processing and storage of animal waste, the growing demand for energy and the creation of sustainable animal husbandry have led to an increased interest in the production of clean energy from animal waste. The production of biogas and its subsequent burning on the farm is among the most promising technologies. One of the possibilities for the utilization of biogas is through the use of small aggregates for the combined production of electricity and heat energy based on an internal combustion engine. Analysis of such facilities that have been put into operation show that alternative technologies using biogas as fuel are better than conventional options, both from an economic and an environmental point of view. In this sense, however, the introduction of such a technology into operation is always associated with a number of risks, since investments in new technologies are influenced by technical and economic uncertainty. When planning and preparing the plan for the construction of such a biogas facility, the investment costs, technical support and profitability of the project are essential. Introducing critical economic and technical parameters to inform the farmer of all possible investments, operational and unforeseen risks will allow him to accept the challenges and choose the best solution for his farm. In this publication, an analysis and assessment of the risk has been carried out based on the characteristics of the technology – the possible consequences of the risk are also presented. A risk matrix related to the specifics of the object and the technology is proposed, with the help of which, the type of risk is identified. Based on an analysis of the obtained results, a motivated proposal for reducing the risk is made.
Wprowadzenie coraz ostrzejszych zasad związanych z przetwarzaniem i składowaniem odchodów zwierzęcych, rosnące zapotrzebowanie na energię oraz tworzenie zrównoważonej hodowli zwierząt spowodowały wzrost zainteresowania produkcją czystej energii z odchodów zwierzęcych. Produkcja biogazu i jego późniejsze spalanie w gospodarstwie należy do najbardziej obiecujących technologii. Jedną z możliwości wykorzystania biogazu jest wykorzystanie małych agregatów do skojarzonej produkcji energii elektrycznej i cieplnej w oparciu o silnik spalinowy. Analiza takich obiektów oddanych do użytku pokazuje, że alternatywne technologie wykorzystujące biogaz jako paliwo są lepsze od konwencjonalnych, zarówno z ekonomicznego, jak i środowiskowego punktu widzenia. Jednakże wprowadzenie takich technologii do eksploatacji zawsze wiąże się z szeregiem zagrożeń, ponieważ na inwestycje w nowe technologie wpływa niepewność techniczna i ekonomiczna. Przy planowaniu i przygotowaniu planu budowy takich biogazowni istotne są koszty inwestycji, wsparcie techniczne i opłacalność projektu. Przedstawienie rolnikowi krytycznych parametrów ekonomicznych i technicznych informujących go o wszelkich możliwych zagrożeniach inwestycyjnych, operacyjnych i nieprzewidywalnym ryzyku pozwoli mu podjąć wyzwania i wybrać najlepsze rozwiązanie dla swojego gospodarstwa. W publikacji dokonano analizy i oceny ryzyka w oparciu o charakterystykę technologii oraz przedstawiono możliwe konsekwencje tego ryzyka. Proponowana jest macierz ryzyka związana ze specyfiką obiektu i technologią, za pomocą której identyfikowany jest rodzaj ryzyka. Na podstawie analizy uzyskanych wyników formułowana jest umotywowana propozycja ograniczenia ryzyka.

W artykule przedstawiono logiczno-probabilistyczne podejście do modelowania rozwoju wypadków lotniczych na podstawie analizy czynników ryzyka oraz czynników zapobiegających powstawaniu wypadków lotniczych. Przedstawiona metoda może być elementem proaktywnej strategii zapobiegania powstawaniu wypadkom lotniczym poprzez zarządzanie poziomem bezpieczeństwa lotów.
The paper has been intended to present a probabilistic approach to the a priori modelling of air-accident development, based on the analysis of risk factors and factors conducive to preventing air accidents. Methods of the assessment of air-accident risk are an indispensible component of the pro-active strategy for the safety-oriented management. The presented method may be such a component of the pro-active strategy for preventing air accidents, since it allows of what follows: - development of undertakings such as to prevent (reduce probability) of an air accident, based on the elimination of or reduction in the probability that risk factors occur; - preliminary, competent assessment of the effectiveness of the planned undertakings, based on the cost effectiveness - probability' rate; - selection of optimal variants of controlling (managing) the forecasted air-accident risk. Implementation of the pro-active strategy for safety management gives a chance to reduce accident risk by means of initiating indispensable actions (corrective, preventive, improving ones) to minimise the hazards.

Opis:
The basis for the control work for the quality in manufacturing process is the supervision of the effects of this process and the verification of the correctness of the employees' activities. The continuity of the control activities carried out indicates the need to supervise all production elements of the offered products. The article presents unit guidelines of quality analyzes for the production and assembly of car brake calipers. The presented analyzes constitute the basis of a complete supervision system and are an element repeated in each subsequent production batch. The quantitative analysis of production non-conformities was supplemented with the determination of the causes and effects for these errors, and the analysis of statistical indicators of qualitative ability. The results for one production batch were presented along with the adopted procedure, which are a component of the system of supervision over the results of control processes included in the production process.

Podczas projektowania zaawansowanych systemów optomechatronicznych należy uwzględnić problem zwiększonego ryzyka wynikającego z zastosowania rozwiązań innowacyjnych. Identyfikacja i ocena ryzyka są ważnymi elementami podejścia systemowego w projektowaniu systemów technicznych. W artykule przedstawiono główne obszary ryzyka występujące w procesie projektowania i implementacji systemów optomechatronicznych. Zaprezentowano algorytm oceny ryzyka i modelowy generator macierzy ryzyka. Przedstawiono przykład analizy ryzyka dla systemu automatycznej optycznej inspekcji elementów łożysk tocznych, który został wdrożony w przemyśle motoryzacyjnym.
When designing advanced optomechatronic systems, the problem of the increased technical risk as a result of application of innovative solutions should be considered. The identification and assessment of the risk are the important elements of systematic approach in designing of technical systems. In the article, risk sources in the design and implementation process of innovative optomechatronic systems are presented. The risk assessment algorithm and the model generator of the risk matrix are presented. The case analysis was presented for the automatic optical inspection system of bearing elements which was implemented in the automotive industry.

The article describes the components of the risk assessment process, the analysis and classification of modern approaches to the evaluation of risk events. Justification of the possibility of using the methods of statistical, analytical, expert assessment approaches is based on the division of the methods into the groups by types of risks assessed: technical, investment, bankruptcy, market, credit, operational, in-house risk, the risk of financial assets and portfolios. The application examples of the assessment methods in terms of mining industry were provided.
W artykule przedstawiono elementy procesu oceny ryzyka wraz z analizą i klasyfikacją nowoczesnych podejść do ewaluacji ryzyka w przedsiębiorstwie. Szczególną uwagę zwrócono na możliwość zastosowania metod statystycznych, analitycznych i eksperckich w ocenie ryzyka: technicznego, inwestycyjnego, bankructwa, rynkowego, kredytowego, operacyjnego, wewnętrznego oraz ryzyka związanego z aktywami finansowymi i tworzeniem portfela tych aktywów. W części empirycznej artykułu w postaci studium przypadków zaprezentowano przykłady zastosowania metod oceny ryzyka w branży górniczej.

The focus of the paper is to identify risks in various areas of operation of a selected trade and production company. The risks primarily referred to threats that had direct or indirect impact on the functioning of the organisation, thence the causes of their occurrence and effects are determined. Identification of individual risks is made based on analyses of internal and external context of the enterprise. In this study, over thirty key risks were identified, in the operational and strategic area, which is related to the taking of immediate action. The final stage of the paper includes preparation of control mechanisms allowing for minimisation or elimination of the discovered risks.

The presented paper is concerned with the problems of assessing the risk level for service chain of refrigerated containers in seaports. This issue has been examined with regard to the losses related to the loss of cargo’s qualitative values. The paper identifies the links in the service chain as well as related hazards and events resulting in losses. The term “chain” has been defined for the refrigerated container service chain. Material losses generated by the loss of cargo’s qualitative values have also been examined. The paper suggests and justifies functional and numeric risk measures. With regard to the examined process the study presents the elements of loss modelling methodology. The risk level has been evaluated using selected calculation examples, considering the impact of the probability of events resulting in losses.

Opis:
The safety risk management is crucial for aviation industry companies. Each of aviation organizations (i.e. airlines, aerodrome operators, General Aviation entities, etc.) has different specificity and deals with different factors. Numerous studies on safety risk management have been conducted, however authors of presented paper have seen a need to review one of the its’ numerous aspect - the risk assessment from the perspective of aerodrome operator. The variety of risk assessment tools and techniques gives many possibilities, but can also cause disarray if rules or selection criteria for their use have not been developed. The aim of the article is to present the risk assessment tools and techniques, that may be the most beneficial and useful for selected safety aspects of aerodrome functioning. For this purpose, existing risk assessment techniques and tools were collected and briefly reviewed, as well as their usefulness for the aerodrome operator was verified according to the proposed issues related to the safety of aerodrome operations. Analysis have shown that a manual summarizing and reviewing risk assessment tools and techniques could be useful for aerodrome operators.

Purpose: The purpose of the paper is to illustrate the usage of techniques known from chaos theory to analyze the risk Design/methodology/approach: In this case the objects of application are winnings graphs of different poker players. Two types of players are presented; winning players (those with positive expected value) and breaking even players (expected value close to zero). Findings: Charts were analyzed with a fractal dimension calculated with the box method. Originality/value: Relation between fractal dimension and Hurst exponent is shown. Relation between risk in sense of chaos theory and players’ long-term winning is also described. Further applications of chaos theory to analyze the risk in games of chance are also proposed.

W artykule przedstawiono zagrożenia w aspekcie terminowości dostaw oraz wynikające z zagrożeń ryzyko strat finansowych. Opóźnienia dostaw są jednym z najpoważniejszych problemów w sieci powiązań między przedsiębiorstwami, dlatego w artykule przedstawiono możliwość zastosowania metod programowania sieciowego w celu analizy i oceny ryzyka na przykładzie łańcucha dostaw z branży motoryzacyjnej. W pracy podano przykład mapy ryzyka, która jest szczególnie istotna przy wyborze sposobów minimalizacji ryzyka i eliminacji jego skutków.
The aim of this article was application of network programming in evaluation, analyzing and estimating the risk in a supply chain, then presentation the method of mapping the risk as a method of supporting decisions concerning emergency procedures towards the risk. The threats and vulnerabilities, which appear in a supply chain causing delays, were specified in this article. Graphical methods of tasks’ representation in a supply chain were mapped out. Presented in this article example of a supply chain from automotive industry subserved to the calculations and enable to indicate activities in a supply chain, which require shortening their duration. Critical Path in aforementioned example, which was deprived of a slack time, run across the activities of transport undertaken by the suppliers of petroleum, manufacturers and suppliers of polyesters, manufacturers of car seats and other upholstered components, then the production and assembly lines of automotive manufacturer, car dealer and activities of transport to the final customer.

Opis:
The effectiveness of lightning protection on the power and distribution grid is a significant factor, which influences the power distribution reliability and the failure rate of system elements. As part of this article, a mathematical model will be presented, taking into account selected parameters that affect the assessment of the lightning hazard of an overhead line. The proposed model will consider the location of the object near the line and the adjustment of line conductor overhangs. Moreover, the mentioned mathematical model allows for analyzing the impact of considered parameters on the protection level of the power system, and transient overvoltages that occur in this system. The article contains also a detailed description of an effective and fast method to assess the lightning discharge impact on the power system with insufficient data. The introduced model was tested to verify the correctness of its operation by comparison of calculation results and functional data. High convergence of calculated and functional data and uncomplicated model structure ensure a wide range of applications for the proposed solution to easily prevent emergency situations in the power system. Furthermore, the described model gives the opportunity to assess the reduction of the range of selectivity zone associated with the power line, in conjunction with the impact of constructional peculiarities and a near object.

Opis:
The effectiveness of lightning protection on the power and distribution grid is a significant factor, which influences the power distribution reliability and the failure rate of system elements. As part of this article, a mathematical model will be presented, taking into account selected parameters that affect the assessment of the lightning hazard of an overhead line. The proposed model will consider the location of the object near the line and the adjustment of line conductor overhangs. Moreover, the mentioned mathematical model allows for analyzing the impact of considered parameters on the protection level of the power system, and transient overvoltages that occur in this system. The article contains also a detailed description of an effective and fast method to assess the lightning discharge impact on the power system with insufficient data. The introduced model was tested to verify the correctness of its operation by comparison of calculation results and functional data. High convergence of calculated and functional data and uncomplicated model structure ensure a wide range of applications for the proposed solution to easily prevent emergency situations in the power system. Furthermore, the described model gives the opportunity to assess the reduction of the range of selectivity zone associated with the power line, in conjunction with the impact of constructional peculiarities and a near object.

Opis:
The aim of the article is to present a novel, innovative method of monitoring endogenous fire hazard in hard coal mines, based on a system of temperature sensors placed within solid coal. The developed system of monitoring the level of endogenous fire hazard was tested in an active hard coal mine in Poland. The scope and the method of temperature measurements with downhole temperature sensors in solid coal were presented in detail. Results of systematic temperature monitoring fire prevention activities are presented.
