Opis:
Malignant melanoma is highly aggressive type of malignant neoplasm. The skin of the head and neck region is common localisation for appearance of this tumour due to exposition to the sun light. However, some number of cases concern development of the malignant melanoma on the mucous membranes. Most common signs are epistaxis and nasal obstruction. Late diagnosis effects with poor clinical prognosis. In our case, despite complete surgical resection and adjuvant therapy, outcome of the treatment was unsatisfying, because of metastasis and local recurrence.

Malignant skin neoplasms are one of the most common human malignancies. The incidence of nonmelanoma skin cancers and malignant melanoma is constantly increasing. The current therapies, especially for malignant melanoma, have relatively low success rates. Therefore, there is an urgent need to develop new remedies that are both safe and effective. Natural substances have always been an important source for the discovery of new therapies. In turn, a number of studies have indicated that some phytocheicals could have an anti-tumour effect. In vitro and in vivo testing of malignant skin neoplasm models revealed different anti-tumour actions, including antioxidation, carcinogen inactivation, anti-proliferation, cell cycle arrest, induction of apoptosis, inhibition of angiogenesis, or a combination of them. The aim of this paper is to describe anti-tumour compounds derived from natural sources that might be used in the therapy of malignant skin neoplasm. The phytochemicals discussed below include carotenoids, terpenoids and flavonoids.

We reported a 57 year old woman treated for 2 years because of pustulosis plantaris (periodic exacerbations and remissions). In July 2013 she reported to the Outpatient Surgery because of a tumor in the left groin area. The biopsy revealed: metastases melanomatosis in lymphonodus. The patients was in October at the surgical removal of lymph nodes from left groin area (lymphadenectomy groin - ilio - obturatorii right). Full diagnostic was carried out and searching of of primary foci to no results. In the patient revealed the presence of BRAF and she was qualified to study of drug vemurafenib.

Two water soluble porphyrins: meso-tetra-4-N-methylpyridyl-porphyrin iodide (P1) and 5,10-di-(4-acetamidophenyl)-15,20-di-(4-N-methylpyridyl) porphyrin (P2) were synthesised and evaluated in respect to their photochemical and photophysical properties as well as biological activity. Cytotoxic and phototoxic effects were evaluated in human malignant melanoma Me45 line using clonogenic assay, cytological study of micronuclei, apoptosis and necrosis frequency and inhibition of growth of megacolonies. Both porphyrins were characterised by high UV and low visible light absorptions. Dark toxicity measured on the basis of the clonogenic assay and inhibition of megacolony growth area indicated that P1 was non-toxic at concentrations up to 50 μg/ml (42.14 μM) and P2 at concentrations up to 20 μg/ml (16.86 μM). The photodynamic effect induced by red light above 630 nm indicated that both porphyrins were able to inhibit growth of melanoma megacolonies at non-toxic concentrations. Cytologic examination showed that the predominant mode of cell death was necrosis.

Opis:
Segmentation of the border of the human pigmented lesions has a direct impact on the diagnosis of malignant melanoma. In this work, we examine performance of (i) morphological segmentation of a pigmented lesion by region growing with the adaptive threshold and density-based DBSCAN clustering algorithm, and (ii) morphological segmentation of the pigmented lesion border by region growing of the lesion and the background skin. Research tasks (i) and (ii) are evaluated by a human expert and tested on two data sets, A and B, of different origins, resolution, and image quality. The preprocessing step consists of removing the black frame around the lesion and reducing noise and artifacts. The halo is removed by cutting out the dark circular region and filling it with an average skin color. Noise is reduced by a family of Gaussian filters 3×3−7×7 to improve the contrast and smooth out possible distortions. Some other filters are also tested. Artifacts like dark thick hair or ruler/ink markers are removed from the images by using the DullRazor closing images for all RGB colors for a hair brightness threshold below a value of 25 or, alternatively, by the BTH transform. For the segmentation, JFIF luminance representation is used. In the analysis (i), out of each dermoscopy image, a lesion segmentation mask is produced. For the region growing we get a sensitivity of 0.92/0.85, a precision of 0.98/0.91, and a border error of 0.08/0.15 for data sets A/B, respectively. For the density-based DBSCAN algorithm, we get a sensitivity of 0.91/0.89, a precision of 0.95/0.93, and a border error of 0.09/0.12 for data sets A/B, respectively. In the analysis (ii), out of each dermoscopy image, a series of lesion, background, and border segmentation images are derived. We get a sensitivity of about 0.89, a specificity of 0.94 and an accuracy of 0.91 for data set A, and a sensitivity of about 0.85, specificity of 0.91 and an accuracy of 0.89 for data set B. Our analyses show that the improved methods of region growing and density-based clustering performed after proper preprocessing may be good tools for the computer-aided melanoma diagnosis.

Czerniak złośliwy jest jednym z najszybciej rozwijających się nowotworów skóry, a zachorowalność na niego stale wzrasta. Podstawowym badaniem nieinwazyjnym pozwalającym na jego rozpoznanie jest dermatoskopia. Celem badania dermatoskopowego jest diagnostyka różnicowa zmian barwnikowych z podziałem na zmiany melanocytowe i niemelanocytowe. Badanie pozwala odróżnić zmiany melanocytowe, wymagające wycięcia chirurgicznego oraz badania histopatologicznego, od zmian łagodnych. Szybki rozwój elektroniki i informatyki pozwolił na wyodrębnienie dwóch nowych dziedzin w dermatoskopii (foto- i wideodermatoskopii), które umożliwiają cyfrowy zapis zdjęć. Komputerowa analiza obrazów dermatoskopowych polega na ocenie poszczególnych zmian, określaniu ich zaawansowania i wyznaczaniu podstawowych parametrów diagnostycznych (określanie barwy, ilości barw, rozmiaru, symetrii oraz struktur różnicujących). Badania komputerowe wykazują dużą skuteczność, jednak nie są zalecane jako jedyny sposób oceny zmian. Obecne systemy nie ograniczają niepotrzebnych zabiegów chirurgicznych, co uznawane jest za ich podstawową wadę. Wymagany jest dalszy rozwój aplikacji oraz opracowanie nowych, nowatorskich rozwiązań, aby dermatoskopia wspomagana komputerowo stała się wiodącą metodą diagnostyczną.
Skin melanoma is one of the most malignant tumours and increasing melanoma incidence rate has been observed worldwide in the last several years. Due to high skin cancer incidence, dermatologic oncology has become a quickly developing branch of medicine. Dermoscopy is the most common and non-invasive method to diagnose skin cancer. The aim of dermoscopy is to dif erentiate malignant melanoma from other lesions of the skin (hemangiomas and nevi) as well as preliminary staging and malignancy assessment. It is possible to distinguish malignant tumours, requiring surgical removal followed by histopathological examination, from benign changes. The rapid development of electronics and information technologies enabled to create two new areas of dermoscopy (photo- and videodermoscopy) that use digital imaging for storing the data. The aim of computer systems in dermoscopy is to analyse each image and to evaluate each change, identifying them and determining the advancement of basic diagnostic parameters (determination of colour, border, size, asymmetry and dif erential structures). Recent studies of software systems show high ei ciency, but it is still not recommended that the software systems are the only one to evaluate the changes. Rapid development of medical equipment and computer systems for medical applications gives hope for better and faster diagnoses of malignant melanoma and that dermoscopy image processing will become a leading diagnostic method.

Skin cancers are one of the most common cancers in the Caucasian population. A constantly increasing number of nonmelanoma skin cancers and malignant melanomas is observed. The incidence of skin cancers is associated mainly with exposure to sunlight. Therefore, agricultural workers who work in open spaces are a particularly vulnerable group. Currently, studies on the pathogenesis of skin cancer focus on the molecular basis associated with ultraviolet radiation. This study is an attempt to summarize the current state of knowledge on this issue. There have been demonstrated mutations in different classes of genes associated with carcinogenesis, including protooncogenes, tumour suppressor genes, genes that control apoptosis, genes encoding transcription factors and DNA repair genes in patients with skin cancers. Mutations in the latter result in reducing the effectiveness of DNA repair and fixation of mutations. All changes at the gene level lead to structural changes, quantitative and dysfunction of proteins encoded by these genes. All these factors contribute to the process of carcinogenesis. Due to increasing number of skin cancers, it seems important to increase knowledge of the molecular basis of skin cancers. This knowledge could be crucial for predicting the course of the disease, and for the development of new therapeutic strategies.
