Opis:
Magnetic resonance spectroscopy is a non-invasive method used to measure concentrations of selected metabolites in brain such as: N-acetylaspartate, creatine, choline, glutamic acid, myo-inositol, lactic acid or γ-aminobutyric acid. The MRS allows the researcher to obtain information about biochemical composition in selected localizations of the examined CNS and is based on the interpretation of spectra of specific chemical compounds. The aim of this study is a literature review of papers from last 5 years involving the use of MRS in multiple sclerosis. Magnetic resonance spectroscopy is a modern, promising metabolomic imaging method enabling the assessment of brain metabolite concentrations and the dynamism of their changes in healthy people and patients suffering from multiple sclerosis at every stage of the disease. MRS is helpful not only in correlating changes in metabolite concentrations at various central nervous system locations with clinical manifestations, but is also an increasingly improving tool for predicting disease progression. Magnetic resonance spectroscopy may also be useful in more specific clinical situations such as differential diagnosis between multiple sclerosis and Devic’s syndrome or between tumefactive demyelinating lesions and gliomas. Especially in the latter case, the development of this technology may in the future result in the possibility of avoiding invasive biopsy in patients during the diagnosis of focal changes in the CNS. One should also not forget about the role that MRS may play in the future in monitoring the course of treatment with modern MS drugs, not only in everyday clinical practice but also at the stage of clinical trials. The development of fast MRS techniques, significantly shortening the acquisition time and 7 T magnetic resonance spectroscopy, precise and repeatable method of quantitative analysis of brain metabolites may be particularly helpful in achieving these goals.

Opis:
Currently, liver disease is widespread and the awareness of these diseases is low. Early symptoms of liver disease do not necessarily indicate problems with this organ and patients are usually informed of their problems when the stage of the disease is already advanced. Invasive biopsies are the clinical diagnostic method most commonly used in the evaluation of liver disease. A biopsy is associated with a high risk of false results and additional complications. Finding new non-invasive imaging methods has led to the discovery of a new method called Magnetic Resonance Elastography (MRE). This technique allows one to evaluate the mechanical properties of tissues and to distinguish between pathological states. Testing using this technique can be performed on a conventional magnetic resonance system by using few additional components and properly prepared software. Studies have shown that there is a strong correlation between MRE-measured liver stiffness and the degree of fibrosis. MRE is also useful in characterizing liver tumors. Studies show that this technique is highly credible in both health volunteers and patients with liver fibrosis. MRE has tremendous diagnostic potential. The described technique is not currently widely used and has the 127-132potential to serve as a safe and accurate alternative in clinical diagnostics in the future.

Opis:
Over the past few decades, magnetic resonance imaging (MRI) has proven to be extremely successful in medical applications. More recently, the biomedical applications of MRI have been gaining more use in the field of clinical pharmacy. In 1977, perfluorocarbon compounds (PFC), which form emulsions that can carry drugs, were analyzed by 19F MRI and emulsified PFC compounds have been investigated as potential blood substitutes since the early 1960s and now a wide variety of PFC compounds are currently available as 19F MRI biomarkers. Molecules with 19F substituents are particularly attractive for use in drug tracking by 19F MRI due to 100% 19F abundance, high 19F MRI sensitivity (0.83 relative to 1H MRI) and an impressively large chemical shift range (400 ppm). Another benefit in the use of 19F MRI is a zero background signal in biological samples due to lack of endogenous fluorine. Therefore, drugs containing fluorine atom have potential for 19F MRI imaging drug delivery to tissue. This article will review recent developments in the use of 19F MRI in imaging drug delivery to tissue and individual cells.

Opis:
In magnetic resonance imaging (MRI), k-space sampling, due to physical restrictions, is very time- -consuming. It cannot be much improved using classical Nyquist-based sampling theory. Recent developments utilize the fact that MR images are sparse in some representations (i.e. wavelet coeffi cients). This new theory, created by Candès and Romberg, called compressed sensing (CS), shows that images with sparse representations can be recovered from randomly undersampled k-space data, by using nonlinear reconstruction algorithms (i.e. l1-norm minimization). Throughout this paper, mathematical preliminaries of CS are outlined, in the form introduced by Candès. We describe the main conditions for measurement matrices and recovery algorithms and present a basic example, showing that while the method really works (reducing the time of MR examination), there are some major problems that need to be taken into consideration.

Imaging and quantification of MAbs distribution by MRI after administration in vivo can improve patient outcomes, although it is not routinely used in the clinic hence the motivation for development of new MAbs imaging methodologies. Improvements in imaging and quantification of antibody distributions can be made by conjugating MAbs to “hotspot” atoms such as 19F as 19F Currently, research investigating the incorporation of 19F to antibody delivery systems has been limited to trastuzumab perfluorocarbon emulsions and this method has not been extended to other clinically approved antibodies. The methodologies of 19F MRI can improve current limitations of antibody immunotherapy such as quantitative visualization of targeted surface antigens expressed on tumor cells.

Opis:
Researchers address the generalization problem of deep image processing networks mainly through extensive use of data augmentation techniques such as random flips, rotations, and deformations. A data augmentation technique called mixup, which constructs virtual training samples from convex combinations of inputs, was recently proposed for deep classification networks. The algorithm contributed to increased performance on classification in a variety of datasets, but so far has not been evaluated for image segmentation tasks. In this paper, we tested whether the mixup algorithm can improve the generalization performance of deep segmentation networks for medical image data. We trained a standard U-net architecture to segment the prostate in 100 T2-weighted 3D magnetic resonance images from prostate cancer patients, and compared the results with and without mixup in terms of Dice similarity coefficient and mean surface distance from a reference segmentation made by an experienced radiologist. Our results suggest that mixup offers a statistically significant boost in performance compared to non-mixup training, leading to up to 1.9% increase in Dice and a 10.9% decrease in surface distance. The mixup algorithm may thus offer an important aid for medical image segmentation applications, which are typically limited by severe data scarcity.

Lung cancer is one of the most common types of cancer diagnosed, and the development of methods to image diseased lung tissue by MRI is of utmost importance. Contrast-Enhanced Magnetic Resonance Imaging (CE-MRI) was used to noninvasively evaluate spin-spin relaxation time, T1, of lung cell cultures infused with various clinical gadolinium-based contrast media for imaging. In this study we used a clinical 1.5 Tesla scanner and the contrast agents: Omniscan, MultiHance, Gadovist and ProHance. A significant five-fold reduction of T1 relaxation time was obtained.

Introduction: This retrospective study analyzes radiological findings, therapeutic management and outcomes of patients with intracranial extension of JNA. The routes of intracranial spread, incidence of intracranial disease and influence on therapeutic approach are discussed. Material and methods: An evaluation on the records of 62 patients with JNA was performed and 10 patients with intracranial tumors were included in the study. All patients were males aged 10 to 19 years. R esults: According to Andrews’ classification 8 patients presented with stage IIIb, 1 patient stage IVa and another patient stage IVb tumor. Intracranial invasion was extradural in 8 cases and intradural in 2 patient. Surgery was performed in 9 cases and the most common was combined approach: infratemporal fossa and sublabial transantral. One patient was referred for radiotherapy. Follow-up ranged from 8 to 26 years. There was extracranial recurrence in 2 (22%) of 9 operated patients. C onclusions: The superior orbital fissure is the most frequent route of intracranial spread in patients with extensive involvement of the infratemporal fossa. Due to high risk of recurrence and potential serious complications advanced cases of JNA should be managed by experienced multidisciplinary team, preferably in tertiary referral centers, with an access to modern diagnostic and therapeutic modalities.

Introduction: Cerebral cavernous malformations (CCMs) are one of the most common vascular malformations of the central nervous system. Symptoms of CCMs are not typical; the disease can be asymptomatic or be manifested by a wide range of neurological symptoms. Purpose: To evaluate chosen epidemiologic and clinical issues as well as advanced imaging diagnostics of CCMs in computed tomography and magnetic resonance imaging. Materials and methods: The study was based on retrospective analysis of CT and MRI examinations from the 5 years period. The analysis covered 61 persons, 29 males, and 32 females. The CCMs were diagnosed based on MRI examination in 43 patients and CT in 13 patients. Results: The rate of CCMs occurrence in own material was 0.2%. Single lesions were present in 90.2%, while multiple in 9.8% of cases. Supratentorial CCMs were observed in 77% of cases whereas subtentorial in 23%. Mean size of CCMs in the supra- and subtentorial area equaled 10.6±6.3 and 15.1±5.8 mm, respectively (p<0.05). Clinical symptoms occurred in 65.8% of patients, most frequently in patients with CCMs above 5 mm or with subtentorial lesions. All CCMs were hyperdense in CT images, with calcifications in 13.1%. In MRI, malformations showed diverse intensity of the central part with peripheral low-intensity rim of hemosiderine deposits in T2-weighted images. Conclusions: The clinical symptoms occur in most cases of CCMs. These patients require periodic follow-up MRI examinations, specifically those with haemorrhagic incidents or epileptic seizures, with large size or subtentorial CCMs.

Introduction and Objective. Locally advanced rectal cancers witha high risk of recurrence need multimodal treatment Rusing neoadiuvant preoperative chemoradiotherapy or preoperative radiotherapy. Both rectal cancer surgery and the additional chemoradiotherapy or radiotherapy may cause late sequelae. The aim of the study is to present a clinical situation in which a pelvic recurrence of rectal cancer has to be distinguished from therapy side effects. Insufficiency fracture (IF) is one of the therapy side-effects. It can cause pain and decrease mobility and it is a well-known late complication to pelvic radiotherapy, but can be misinterpreted as a local recurrence. Conclusions. It is extremely important to distinguish IF from metastases, which may require biopsies and initiation of potentially toxic treatments such as chemotherapy. MRI and CT scans are complementary modalities to make an accurate diagnosis of IF.
