INTRODUCTION: Poland’s healthcare systems is based on insurance and universal and equal access. The first component of the system is primary healthcare. At night and on holidays, the medical help can be obtained by means of After Hours Medical Services. Private paid visits can also be arranged. In emergencies, and cases of risk to health or life, emergency medical team can be called at 112 or 999, or a patient can go to the hospital emergency department (ED) on their own. There are also pharmacies where selected over-the-counter medicines can be bought. MATERIALS AND METHODS: The study was conducted in the first quarter of 2023 using a proprietary questionnaire with 5 sociodemographic questions and 20 case descriptions for which the participants indicated the type of medical help which they would use. The expert panel prepared a set of cases (accidents and illnesses), taking into account patient’s age and gender, time of day and working days and holidays when the incident occurred. The group of participants was selected using the snowball sampling method. The results were subjected to statistical analysis using nonparametric tests, the Spearman’s rho test and the Wilcoxon test. The used significance level was p<0.05. RESULTS: The study comprised 233 participants (166 females and 66 males). The mean age was 25 years (SD ± 9.50). A significant correlation was showed between the decision made and the gender in case of stroke occurring on a weekday (p=0.042); stroke symptoms on a holiday (p=0.024); head injury in a child (p=0,024); chest pain in an adult (p=0.009), and also the place of residence in case of an infection in a child (p=0.008). The decision was also influenced by education in case of chest pain in an adult (p=0.048); stroke (p=0.046) and sudden weakness (p=0.009). Also performing a medical profession correlated with the decision in case of arterial hypertension (p=0.028) and chest pain (p=0.010) in adults. The type of day (working/holiday) and the time of day were also analysed, revealing statistical differences depending on the type and time of day for infection in a child (p<0.000 in each comparison); working day and holiday for abdominal pain (p=0.000), stroke (p=0.023), diarrhea (p<0.000) and weakness (p<0.000), and also depending on the time of day in arterial hypertension (p<0.000); however, no differences were found in case of chest pain depending on the time of day(p=0.073). CONCLUSIONS: Emergency medical teams are called both in urgent cases, such as stroke or chest pain, and also in cases that do not require an immediate responses, such as diarrhea or weakness. Solutions need to be sought which would provide information what healthcare system units should be used in specific situations. The EMTs are called more often during the day on weekdays than at night or on holidays. The ambulance service responds most often in emergency cases such as convulsions, stroke or chest pain, but a significant number of calls is related to cases of weakness or arterial hypertension where the medical help should be sought in other healthcare units.

Aim: The increasing staff shortages in Polish surgery incline towards posing the following questions: What are the reasons for the reluctance of Polish doctors in training and residents to specialize in this field of medicine? What measures could be pursued to raise the interest of students and trainees in specializing in general surgery? The aim of this paper is to attempt to answer these questions. Material and methods: From August 5 to December 30, 2019, an online survey of doctors in training and resident doctors was conducted. The study covered 2,498 people. Results: The key reason indicated by young doctors of both sexes was the disproportion between the level of remuneration and professional responsibility. Other most commonly reported reasons include: high overtime, physical and mental strain at work, and fear of not being allowed by senior colleagues to perform surgery. Conclusions: In view of the aging Polish surgical staff and a decreasing number of young doctors interested in this field of medicine, it is necessary to introduce changes into their mode of education.

ObjectivesIncreased life expectancy results in greater challenges posed to healthcare. Concurrently, a shortage of healthcare workforce, including nurses, has been observed. Thus, an urgent need exists to implement improvements in healthcare services based on sufficient evidence. The aim of the study was to evaluate the influence of the relative number of nurses/midwives on life expectancy, and the influence of selected economic variables: gross domestic product (GDP), health expenditure as a percentage of GDP, and health expenditure per capita, on this number. The aim of the study was to evaluate the influence of the relative number of nurses/midwives on life expectancy, and the influence of select economic variables: GDP, health expenditure as a percentage of GDP, and health expenditure per capita on this number.Material and MethodsA retrospective analysis based on data from 46 countries was performed. Correlations between the relative number of nurses/midwives and life expectancy as well as economic variables were evaluated. To trace the differences between the countries with different relative numbers of nurses/midwives, the countries were divided into groups as follows – group 1: <5 nurses and midwives/1000 nurses inhabitants, group 2: 5–10 nurses and midwives/1000 inhabitants, and group 3: >10 nurses and midwives/1000 inhabitants.ResultsCorrelations were found between the relative number of nurses/midwives and life expectancy (p < 0.001, r = 0.68), and economic variables (p < 0.001, r = 0.82; p < 0.001, r = 0.62, and p < 0.001, r = 0.8, respectively). Life expectancy was higher in group 3 vs. groups 1 and 2 (p < 0.001 and p = 0.036, respectively), and in group 2 vs. group 1 (p = 0.006). Economic variables were higher in group 3 vs. group 1 (p < 0.001 for all) and group 2 (p = 0.016, p = 0.025, p = 0.022, respectively), and in group 2 vs. group 1 (p = 002, p = 0.024, p = 0.002, respectively).ConclusionsThe relative number of nurses/midwives correlates with life expectancy and relies on the country’s income and level of healthcare system financing.

System ochrony zdrowia (OZ) odgrywa szczególną rolę tak dla społeczeństwa, jak gospodarki. Niezależnie od stopnia rozwoju i poziomu wydatków na zdrowie, krajowe systemy jego ochrony borykają się z problemem niedoboru środków. W Polsce owe zjawisko dotyczy także istotnych braków w innych obszarach, w tym przede wszystkim kadrowych. W świetle nieuchronnych i koniecznych zmian stwarza to poważne ryzyko zarówno dla bezpieczeństwa zdrowotnego Polaków, jak i niepowodzenia wdrażanych rozwiązań mających na celu modernizację obecnie działającego systemu. Badanie zrealizowane przez pracowników Szkoły Głównej Handlowej w Warszawie uwydatniło możliwość rozwiązania tego problemu poprzez wykorzystanie i rozwój kompetencji przywódczych liderów OZ. Artykuł stanowi punkt wyjścia do dyskusji o roli kadry zarządzającej w procesie zmian systemowych OZ.
Health plays a special role both for societies and economies. Regardless of the level of development of health systems and respective expenditures, they are struggling with shortage of resources. In case of Poland, this phenomenon also concerns significant deficiencies in other areas, including personnel. In the light of inevitable and necessary changes, this poses a serious risk both to the health security of Poles and successful implementation of modernization plans. Research carried out by employees of the Warsaw School of Economics highlighted the possibility of solving this problem through the use and development of leadership competences. This article is a starting point for a discussion on the role of the leadership in the process of changes in healthcare system.

The design of neonatal intensive care units (NICU) influences both patient safety and clinical outcomes as well as the acoustic conditions. In NICU exposure to sound pressure levels above the recommended can affect both neonates and healthcare staff. This study aimed to evaluate the sound pressure levels and to assess noise perception of professionals in a NICU before and after structural modifications and layout redesign. The measurements were performed with a sound level meter. A questionnaire was given to staff before and after the intervention. The opinion of healthcare staff regarding noise in NICU was better after the intervention, when compared with the responses previously given. The results showed that noise levels were excessive in the NICU (before and after), exceeding the international recommendations, with the levels ranging between 46.6 dBA to 57.8 dBA before and 52.0 dBA to 54.0 dBA after intervention. Overall, there is a need for more research in order to verify the effectiveness of some actions and strategies to reduce the impact of noise in NICU.

RESEARCH OBJECTIVE: The purpose of the paper is to analyse the impact ageism may have on providing healthcare to elderly people. THE RESEARCH PROBLEM AND METHODS: The article investigates the consequences of ageism in medical settings, where the elderly are particularly vulnerable to discrimination, distinguishing ageism from appropriate practice variation as well as giving suggestions to improve the situation. The paper analyses research published worldwide on selected issues connected with ageism with special attention to medical professionals’ attitudes and ageist behaviours of the elderly’s environment in a situation of their disability or illness. THE PROCESS OF ARGUMENTATION: Firstly, the relation of ageism to other forms of stereotyping is outlined. Then, the threats of positive ageism are elaborated. The next section is devoted to self-stereotyping of the elderly and its impact on their functioning. Then, the paper discusses medical professionals’ attitudes and ways to measure them, as well as educational interventions that may alter them. Finally, the article refers effective ways of adjusting health promotion messages to the needs of older adults. RESEARCH RESULTS: Ageism bases on mechanisms similar to sexism and racism, but there are some remarkable differences. Positive ageism may be compassionate, however, might be harmful when intruding into an elderly person’s decisions. Self-stereotyping may be described with a model of a vicious cycle. Contact with the elderly is found to be useful in changing medical staff’s attitudes. Family-centred messages are most effective when it comes to health promotion among older adults. CONCLUSIONS, INNOVATIONS AND RECOMMENDATIONS: Studies on ageism prove that negative convictions about the elderly are harmful and affect their functioning. Inclusion of the elderly should be applied in social contexts as well as in science, as the elderly are often missed out in research samples.

The article pertains to the subject of health policy. The paper examines in detail one of the best health systems in the world, which operates in Switzerland. The aim of the article is a detailed presentation of the functioning of the health system in Switzerland, according to selected criteria. The article was developed based on the study of literature (both national and international positions). The health model adopted and implemented in Switzerland has its advantages and disadvantages. The success of the health system in Switzerland is determined by, among other factors, the operation of many competing companies offering health insurance, broad and at the same time socially acceptable patient co-payment and the development of large-scale private health insurance and high "saturation" of medical personnel within the health system. Concrete proposals to im-prove the situation in the Polish health care system were indicated, based on the rich experience of the Swiss health system. The solutions adopted in the Swiss health care system should provide inspiration for all decision-makers in Poland.

Healthcare facilities are increasingly drawing on modern management methods and tools to raise effectiveness of performed therapeutic processes, and to optimize their costs, duration and resources. This article aims at demonstrating the possibilities and the advantages resulting from using business process management tools in practice of healthcare centers. The presented process models described the independent decisions that the medical personnel made in their day-to-day operations and - from the process modelling phase - mechanisms of controlling and obtaining knowledge on the actual performance of a selected therapy. In the subsequent stages, by using process mining tools, it will be possible to recreate selected therapeutic processes and use them as knowledge sources during similar cases. The main aim of this research set at the beginning - searching for the „ideal” therapeutic process, was abandoned during the implementation. After the first failed attempts to create a detailed model of a process, the objective of the project was changed to modeling processes in a way to enable their adaptation to the needs of a specific performance, while still retaining the possibility of obtaining practical and useful knowledge.

Cancer is a deadly disease that has gained a reputation as a global health concern. Further, lung cancer has been widely reported as the most deadly cancer type globally, while colon cancer comes second. Meanwhile, early detection is one of the primary ways to prevent lung and colon cancer fatalities. To aid the early detection of lung and colon cancer, we propose a computer-aided diagnostic approach that employs a Deep Learning (DL) architecture to enhance the detection of these cancer types from Computed Tomography (CT) images of suspected body parts. Our experimental dataset (LC25000) contains 25 000 CT images of benign and malignant lung and colon cancer tissues. We used weights from a pre-trained DL architecture for computer vision, EfficientNet, to build and train a lung and colon cancer detection model. EfficientNet is a Convolutional Neural Network architecture that scales all input dimensions such as depth, width, and resolution at the same time. Our research findings showed detection accuracies of 99.63%, 99.50%, and 99.72% for training, validation, and test sets, respectively.

The article focuses on the advantages of the implementation of a quality management system. It shows the difficulties associated with meeting the expectations of the modern competitive market and the patients’ expectations, as well as presents solutions to improve the organization’s management systems, in the form of mechanisms that already exist, for example the ISO standard. Implementation of the quality management system has a positive impact on the health care unit, introducing order in managing system and improving quality. Thanks to the implemented quality system, it is possible to provide services at a much higher level, which can be seen in the analysis of satisfaction of patients, managers and medical staff. The article analyzes the importance of a quality system in the health care sector, as well as the quality management according to the newest trends in that sector.

The study analyses the number of midwives in the counties of Eastern and Western Galicia in census years 1869, 1880, 1890, 1900 and 1910, and the ratios per cadastral commune, 10,000 square kilometers, 10,000 civilians and 10,000 women in a county. The analysis was based on Austrian and Galician statistical reports. The results of the study confirmed that Eastern Galicia outnumbered Western Galicia in terms of midwives. However, it was in Western Galicia where the rate of growth was higher, and the effects of the 1910 collapse more moderate. This could have been due to an amendment to the 1897 midwifery regulations. The position of individual counties changed, depending on the year and the specific measure. In Eastern Galicia, Lisko county ranked the worst and Horodenka, Śniatyń, Tarnopol, Trembowla, and Brzozów counties ranked the best. In Western Galicia, Limanowa county ranked the worst, while Brzesko, Kraków, Łańcut, Wadowice, Przeworsk, and Podgórze counties ranked the best. There is a strong positive correlation between the rank and natural conditions (water, soil, climate), type of crops, agricultural development and processing, transport system (road and rail), population, and stimulating function of large urban centers. Favorable conditions were correlated with higher values in measures of the number of midwives.

Demographic research of the world population shows that societies are ageing. The ongoing changes in the population structure will require appropriate quantitative and qualitative adjustments in health services to meet the needs of society. Simulation methods turn out to be helpful in these kinds of analyses. In this paper, the authors present a case study on using discrete event simulation (DES) to support decision-making in the field of hospital bed management in the light of demographic changes. The case study was elaborated for one of the Polish district hospitals. A DES model was built to simulate admissions to two hospital wards: paediatric and geriatric. A series of experiments were carried out as based on real data extracted from the hospital database and forecasted demographic trends elaborated by the Central Statistical Office of Poland (CSO). The influence of demographic changes on hospital admissions in the chosen age-gender cohorts was explored, examining different variants of hospital bed availability. The results of the experiments show that demographic trends significantly influence healthcare admission and bed utilisation. The reduction in the number of admissions to the paediatric ward by about 6% results in a change in average bed utilisation from 57.90% to 54.06%. With about 12% more admissions to the geriatric ward, the change is from 68.88% to 75.59%.
